Ionizing radiation and Alzheimer's disease: epidemiology, mechanisms, and eCIRP-mediated tau pathology.

As human radiation exposure expands through medical imaging, nuclear industry growth, and the acceleration of crewed space exploration, understanding the molecular mechanisms linking radiation to Alzheimer's disease (AD) is becoming an increasingly urgent priority. Here, we review epidemiological, mechanistic, and therapeutic evidence for a relationship between ionizing radiation and AD, focusing on a neuroinflammatory pathway centered on extracellular cold-inducible RNA-binding protein (eCIRP). Epidemiological support comes from occupational cohorts of nuclear workers and studies linking resi

Ficd loss rescues motor impairments and reverses oligodendrocyte maturation deficits in a mouse model of spinocerebellar ataxia type 3.

Spinocerebellar ataxia type 3 (SCA3) is an inherited, fatal neurodegenerative disease caused by a pathological CAG repeat expansion in the ATXN3 gene, resulting in the selective degeneration of vulnerable neuronal populations. Recent work has identified impairments in oligodendrocyte maturation as a novel and robust feature of SCA3 pathogenesis. Oligodendrocytes synthesize myelin structural components through the endoplasmic reticulum (ER), rendering this organelle essential for white matter integrity. Despite this, the role of ER function in SCA3 remains unclear. In this study, we show that l

Mitochondrial dynamics and repair differences in two mouse models of Friedreich's ataxia.

Friedreich's ataxia (FA) is an inherited disorder caused by reduced expression of the mitochondrial protein frataxin (FXN). Although defined as a neurodegenerative disease, cardiomyopathy is the leading cause of mortality. Cardiac manifestations of FA span from concentric hypertrophic cardiomyopathy with preserved systolic function and impaired diastolic relaxation, resembling heart failure with preserved ejection fraction (HFpEF), to systolic heart failure with reduced ejection fraction (HFrEF). The molecular mechanisms driving progression towards these cardiac phenotypes remain poorly unders

[Atlantoaxial luxation in a neonatal Warmblood foal].

Atlantoaxial luxation represents a rare but clinically relevant differential diagnosis in neonatal foals presenting with neurological abnormalities or dysphagia. This case report describes a newborn filly without apparent ataxia, but presenting with a stiff head-neck posture and milky nasal discharge. Clinical examination revealed a compromised general condition and abnormal swelling in the region of the fossa retromandibularis. Radiographic examination confirmed the diagnosis of an atlantoaxial luxation. Due to the poor prognosis the foal was euthanized. This case highlights the diagnostic ap

Atypical Lhermitte-Duclos Disease Presenting as a Malignant Cerebellar Tumor: NeuN-Positive Neuronal Differentiation as a Diagnostic Clue.

Lhermitte-Duclos disease (LDD), or dysplastic cerebellar gangliocytoma, is a rare benign cerebellar lesion that may present with atypical imaging and histopathological features, resulting in diagnostic uncertainty and potential overtreatment. We report a 32-year-old woman who presented with progressive headache, vomiting, vertigo, and right-sided cerebellar ataxia. Magnetic resonance imaging revealed a large right cerebellar lesion extending into the vermis, middle cerebellar peduncle, and hemipons, with inhomogeneous contrast enhancement and diffusion restriction, closely mimicking a malignan

Three-dimensional Motion Capture Discriminates Extremely Mild Cerebellar Ataxia from Parkinsonism in the Upper Limbs.

Upper-limb motor symptoms in spinocerebellar degeneration (SCD) and Parkinson's disease (PD) frequently overlap, making clinical differentiation challenging in mild cases using the finger-to-nose test (FNT) or the diadochokinesis test (DDKT). This study aimed to identify quantitative kinematic markers that distinguish mild SCD from mild PD using three-dimensional motion capture. Twenty-six individuals per group-mild SCD, mild PD, and normal controls (NCs)--underwent motion analysis during FNT forward, FNT backward, and DDKT. Key parameters including segmental velocity ratios during FNT and rot

When Speech Signals Wilson Disease: A Case Report.

Wilson disease (WD) is a rare autosomal recessive disorder of copper metabolism that usually presents with a wide spectrum of seemingly unrelated symptoms, such as dysarthria, tremors, dystonia, features of advanced liver disease, and psychiatric manifestations. Atypical presentation may lead to delayed diagnosis. We present the case of a 25-year-old man who presented with slurred speech and abdominal distension. Workup revealed the presence of Kayser-Fleischer rings, reduced serum ceruloplasmin levels, elevated 24-hour urinary excretion of copper, and decompensated liver disease. Magnetic res

Neurological Manifestations in Whipple's Disease: A Scoping Review of Clinical Features, Laboratory Findings, and Neuroimaging Characteristics of 111 Cases.

BACKGROUND: Whipple disease (WD) is a rare, potentially fatal infection caused by Tropheryma whipplei (TW). Central Nervous System involvement (CNS-WD) is a severe complication of WD with a challenging diagnosis, due to limited data integrating neurological features, imaging, cerebrospinal fluid analysis, PCR, and intestinal histopathology. Prompt recognition and treatment are essential to improve patient outcomes. METHODS: A scoping review of the literature from 2000 to 2025 was conducted, including all CNS-WD case reports and series, to outline its clinical spectrum and diagnostic workup.

Guillain-Barré Syndrome As the Initial Manifestation of Human Immunodeficiency Virus Infection: A Case Report.

Guillain-Barré syndrome (GBS) is an acute immune-mediated polyradiculoneuropathy typically triggered by an antecedent infection. Human immunodeficiency virus (HIV) is an uncommon but recognized association, and GBS may rarely constitute the first clinical manifestation of previously undiagnosed HIV infection. A 36-year-old man presented with subacute facial diplegia, predominantly distal tetraparesis, sensory ataxia, glove-and-stocking paresthesia, and hyporeflexia. Cerebrospinal fluid (CSF) analysis showed albuminocytologic dissociation; electrophysiological studies demonstrated a sensorimoto

A second-generation "hypoxia in a pill" rescues neurodegenerative phenotypes across distinct mouse models.

A growing body of preclinical research is demonstrating the therapeutic potential of chronic, continuous hypoxia (11% FIO2) for rare and common diseases. However, the chronic delivery of hypoxic gas poses both practical challenges and long-term safety concerns. We previously introduced a small-molecule, "hypoxia-in-a-pill" combining a hemoglobin affinity enhancer (GBT440) to limit oxygen delivery with a HIF-2α inhibitor (PT2399) to prevent detrimental compensatory erythropoiesis. Although this small-molecule regimen extended the lifespan of the Ndufs4 knockout (KO) mouse model of mitochondrial

The role of neurofilament light chain and other fluid biomarkers in cerebellar ataxias.

The genetic and phenotypic heterogeneity of hereditary ataxias complicates the design of clinical trials since each ataxia progresses at different rates. Consequently, robust biomarkers are urgently needed to monitor therapeutic efficacy and safety of potential therapeutic interventions. Neurofilament light chain has emerged as a leading candidate, serving as a sensitive pharmacodynamic biomarker of neuronal injury and treatment response in neurodegenerative disorders. This review explores the utility of neurofilament light chain across various ataxias, including polyglutamine spinocerebellar

Immune Checkpoint Inhibitor-Induced Cardiovascular Disease: The Role of Accelerated Senescence in Vascular Dysfunction.

SIGNIFICANCE: Immune checkpoint inhibitors (ICIs) have transformed cancer therapy by reactivating antitumor immunity and improving outcomes across multiple malignancies. Yet, clinical observations and preclinical studies suggest that ICIs may increase long-term cardiovascular disease risk, including hypertension, atherosclerosis, and thrombosis. The underlying mechanisms remain incompletely defined, highlighting the need for focused investigation into the vascular consequences of immune activation. RECENT ADVANCES: ICI-induced immune activation disrupts vascular and immune homeostasis through

Reduction of interleukin-6 levels in patients with Friedreich's Ataxia treated with omaveloxolone.

BACKGROUND: Friedreich's Ataxia (FRDA) is the most common inherited spinocerebellar ataxia and is caused by a homozygous GAA trinucleotide repeat expansion in the frataxin (FXN) gene, leading to reduced intramitochondrial frataxin expression with a consequent progressive neurodegeneration and multisystem involvement. Neuroinflammatory pathways may contribute to FRDA pathophysiology and IL-6 represents a biologically relevant inflammatory mediator. Recently, omaveloxolone (Omav), an activator of the nuclear factor erythroid 2-related factor 2 (Nrf2) pathway, demonstrated clinically meaningful n

Parkinsonism with Reduced Striatal Uptake on DAT-SPECT in a Patient Harboring a Novel KCND3 Variant.

KCND3 encodes the voltage-gated potassium channel Kv4.3, and pathogenic variants in KCND3 are associated with spinocerebellar ataxia type 19/22 (SCA19/22) and related neurological phenotypes. Here, we report a Japanese patient harboring a novel KCND3 missense variant, classified as a variant of uncertain significance, who presented with a clinical phenotype compatible with SCA19/22, including slowly progressive cerebellar ataxia beginning in childhood, accompanied by asymmetric parkinsonism. Dopamine transporter single-photon emission computed tomography demonstrated reduced striatal uptake. P

Remote Online-Based Application of the Cerebellar Cognitive Affective Syndrome Scale.

The Cerebellar Cognitive Affective Syndrome Scale (CCAS-S) is increasingly used in clinical trials to assess non-motor function in cerebellar disorders, yet its suitability for remote administration has not been evaluated. Using a matched-pairs design, we compared remote, video-based administration of the German CCAS-S (version A) in 22 patients with cerebellar disease and 22 matched controls with at site -testing from a larger, previously tested cohort. Group matching included age, level of education, sex, occupational experience, and ataxia severity. Uncorrected as well as age, education and

KCNA2-Associated Hereditary Spastic Paraplegia: Clinical Characteristics and Rationale for Targeted Treatment.

BACKGROUND: A single recurrent variant, NM_004974.4(KCNA2):c.881G>A:p.(Arg294His), causes hereditary spastic paraplegia (HSP) through a unique variant-specific mechanism. Data from paralogous variants suggest that this may be a druggable mechanism. OBJECTIVE: The objective of this work is to give an overview of the clinical spectrum and course of KCNA2-p.(Arg294His)-associated HSP (HSP-KCNA2) and report treatment with eplerenone, a potassium-sparing diuretic. METHODS: In this retrospective study, we recruited individuals with the variant of interest through international rare disease network
