Cilia are highly conserved organelles that use hundreds of unique proteins to drive extracellular motility and serve as signaling hubs for eukaryotic cells. The Ciliogenesis and Planar Polarity Effector (CPLANE) protein complex controls basal body docking during early ciliogenesis and recruits the intraflagellar transport (IFT) proteins to the base of growing cilia. Leveraging recent advantages in deep learning neural networks and high-performance computing, we predict the structure of JBTS17, the largest CPLANE subunit, and the tertiary structure of the complete CPLANE complex, revealing dire

Ataxias are a heterogeneous group of rare neurological disorders characterized by progressive impairment of motor coordination and balance, caused by a broad range of genetic, immune-mediated, and degenerative mechanisms. Despite significant advances in molecular diagnostics and disease characterization, effective disease-targeted treatments remain limited, with omaveloxolone currently representing the only approved therapy for Friedreich's ataxia. In this review, we provide a comprehensive and up-to-date overview of the clinical drug development pipeline for ataxias, based on a systematic ana

Fragile X-associated tremor ataxia syndrome (FXTAS) is a multi-faceted and progressive neurodegenerative disorder that manifests clinically with action tremor, gait ataxia, parkinsonism, peripheral neuropathy, and cognitive and psychiatric features. The disease is highly heterogeneous and there are no FDA approved medications for FXTAS as a specific indication. As such, treatment is symptomatic and must be tailored to the specific needs of a given patient. This review provides expert opinion-based treatment recommendations for the core and ancillary clinical features of FXTAS. Movement disorde

Spinocerebellar ataxia type 27B (SCA27B) is a recently described autosomal dominant ataxia caused by a pathogenic GAA repeat expansion in the FGF14 gene. The frequency and the distribution of FGF14 repeat alleles remain largely unexplored in underrepresented populations, particularly in Latin America. To investigate the frequency of SCA27B and characterize the allelic distribution of the GAA-FGF14 microsatellite locus in a Peruvian cohort of individuals with ataxia of unknown molecular etiology. We analysed a cohort of affected individuals evaluated at a referral center for neurogenetic disord

Mutations in the mitochondrial DNA polymerase gamma (POLG) gene cause a broad spectrum of neurodegenerative disorders, including the ataxia-neuropathy spectrum (ANS). Substantial phenotypic heterogeneity and overlapping clinical features can delay diagnosis. We report a 38-year-old woman with no significant past medical history who presented with a three-year history of slowly progressive sensory ataxia that worsened in the dark. Examination revealed mild dysarthria, large-fiber sensory loss, and a positive Romberg sign. Extensive metabolic, toxic, autoimmune, and infectious work-up was unreve

UNLABELLED: Microsatellite instability-high (MSI-H) colorectal cancer cells depend on the Werner syndrome helicase (WRN) to resolve cruciform DNA structures that arise from expanded TA-dinucleotide repeats. Loss of WRN induces replication stress and double-strand breaks, a vulnerability that can be recapitulated with the selective WRN inhibitor (WRNi) HRO761 in MSI-H cancer cells. To uncover the mechanisms governing sensitivity to WRN inhibition, we conducted genome-wide CRISPR/Cas9 screens in colorectal cancer cell lines treated with or without HRO761. These screens identified SMARCAL1 as a k

Anti-GQ1b antibody syndrome encompasses related postinfectious neurologic disorders, including Miller Fisher syndrome, Guillain-Barré syndrome, and Bickerstaff brainstem encephalitis. Recognizing overlap among these phenotypes can be difficult in the intensive care unit because profound unresponsiveness and loss of brainstem reflexes may be attributed to sedation or structural brain injury. A middle-aged man developed perioral numbness, blurred vision, ataxia, extremity paresthesias, and facial weakness after an upper respiratory infection. His condition progressed rapidly to respiratory failu

BACKGROUND: Epstein-Barr virus (EBV)-associated central nervous system (CNS) involvement in children is uncommon and may present with nonspecific neurological symptoms. Brain magnetic resonance imaging (MRI) may remain normal, making diagnosis challenging. Cerebrospinal fluid (CSF) targeted next-generation sequencing (tNGS) has emerged as a useful diagnostic tool, although positive EBV results require careful clinical interpretation.
CASE DESCRIPTION: A previously healthy 5-year-old boy presented with fever (38.5 ℃), headache, dizziness, and nausea for 3 days without seizures, altered conscio

Ischemic stroke occurring during direct oral anticoagulant (DOAC) therapy for atrial fibrillation represents a frequent and challenging clinical dilemma due to increasing stroke rates and increasing DOAC prescription rates. Such events are often interpreted as treatment failure, prompting escalation with antiplatelet therapy, particularly in the presence of concomitant intracranial atherosclerotic disease (ICAD), and thereby exposing patients to increased bleeding risk. However, apparent DOAC failure may reflect correctable pharmacokinetic or adherence-related factors. We report a 75-year-old

Cerebellar ataxias are a diverse group of hereditary and acquired disorders that impair balance, gait, posture and ocular movements. With limited treatment options, particularly in hereditary forms, neuromodulation has emerged as a promising therapeutic approach particularly the transcranial direct current stimulation (tDCS). This scoping review aims to discuss the role of tDCS as a potential therapy in reducing ataxia symptoms in both hereditary and acquired forms. A total of 787 studies were found after searching Pubmed, EMBASE, and Cochrane. After removing 273 duplicates, 514 studies remain

Prostate cancer exhibits complex pathophysiology, rendering its management arduous due to heterogeneity and recalcitrance toward conventional therapeutics. The increasing focus on immune checkpoint inhibitors for treatment stems from the limited existing therapeutic approaches and the impressive effectiveness of these inhibitors demonstrated in other malignancies. Denoted as immunologically "cold" with low tumor mutation burden (TMB), prostate cancer manifests diminished immune cell infiltration in tumoral tissue, potentially culminating in inadequate response to immune checkpoint blockade. No

Wilson disease (WD) is a rare autosomal recessive disorder of copper metabolism that usually presents with a wide spectrum of seemingly unrelated symptoms, such as dysarthria, tremors, dystonia, features of advanced liver disease, and psychiatric manifestations. Atypical presentation may lead to delayed diagnosis. We present the case of a 25-year-old man who presented with slurred speech and abdominal distension. Workup revealed the presence of Kayser-Fleischer rings, reduced serum ceruloplasmin levels, elevated 24-hour urinary excretion of copper, and decompensated liver disease. Magnetic res

BACKGROUND: Whipple disease (WD) is a rare, potentially fatal infection caused by Tropheryma whipplei (TW). Central Nervous System involvement (CNS-WD) is a severe complication of WD with a challenging diagnosis, due to limited data integrating neurological features, imaging, cerebrospinal fluid analysis, PCR, and intestinal histopathology. Prompt recognition and treatment are essential to improve patient outcomes.
METHODS: A scoping review of the literature from 2000 to 2025 was conducted, including all CNS-WD case reports and series, to outline its clinical spectrum and diagnostic workup.

Guillain-Barré syndrome (GBS) is an acute immune-mediated polyradiculoneuropathy typically triggered by an antecedent infection. Human immunodeficiency virus (HIV) is an uncommon but recognized association, and GBS may rarely constitute the first clinical manifestation of previously undiagnosed HIV infection. A 36-year-old man presented with subacute facial diplegia, predominantly distal tetraparesis, sensory ataxia, glove-and-stocking paresthesia, and hyporeflexia. Cerebrospinal fluid (CSF) analysis showed albuminocytologic dissociation; electrophysiological studies demonstrated a sensorimoto

The genetic and phenotypic heterogeneity of hereditary ataxias complicates the design of clinical trials since each ataxia progresses at different rates. Consequently, robust biomarkers are urgently needed to monitor therapeutic efficacy and safety of potential therapeutic interventions. Neurofilament light chain has emerged as a leading candidate, serving as a sensitive pharmacodynamic biomarker of neuronal injury and treatment response in neurodegenerative disorders. This review explores the utility of neurofilament light chain across various ataxias, including polyglutamine spinocerebellar

SIGNIFICANCE: Immune checkpoint inhibitors (ICIs) have transformed cancer therapy by reactivating antitumor immunity and improving outcomes across multiple malignancies. Yet, clinical observations and preclinical studies suggest that ICIs may increase long-term cardiovascular disease risk, including hypertension, atherosclerosis, and thrombosis. The underlying mechanisms remain incompletely defined, highlighting the need for focused investigation into the vascular consequences of immune activation.
RECENT ADVANCES: ICI-induced immune activation disrupts vascular and immune homeostasis through
