Biallelic protein truncating EXOSC6 variants cause a neurodevelopmental disorder with cerebellar atrophy, ataxia, and global developmental delay.

The multisubunit RNA exosome complex provides an essential, highly conserved multifunctional 3' to 5' exoribonuclease activity in the eukaryotic nucleus and cytoplasm. Inherited bi-allelic single amino acid variants in the core of the RNA exosome complex have been implicated in causing Mendelian syndromes that affect brain development, collectively termed exosomopathies. The core RNA exosome consists of nine subunits, and pathogenic variants in eight of them (EXOSC1- 5 and EXOSC7-9) have been described in exosomopathies. Here, we describe a patient with cerebellar atrophy, ataxia, and global d

A low dose of dexmedetomidine reduces behavioral activity and visual attention to aggressive faces in dogs.

Dexmedetomidine is widely used for sedation and alleviation of fear and anxiety in dogs, but its effects on cognitive processes are not known. We studied the effect of a low dose of dexmedetomidine on dogs' gazing behavior while they observed images of emotional facial expressions of conspecifics. Eight dogs were administered intramuscular dexmedetomidine (2.5 μg/kg) and saline-placebo in a randomized cross-over design. Images of neutral and aggressive dog faces were shown on a monitor, and dogs' eye movements were recorded with an eye gaze tracker. The dogs' behavior was evaluated using three

Association Between Cortical Cerebral Microinfarct and Motor Performance at One-Year Follow-Up in Patients with Cerebral Small Vessel Disease: An Exploratory Study.

Background/Objectives: Cerebral small vessel disease (CSVD) is an important contributor to motor impairment. Cortical cerebral microinfarct (CCMI) is an emerging imaging marker of CSVD. Although its association with cognitive impairment has been well established, its relationship with motor function remains unclear. We explored associations of CCMI with motor performance at baseline and 1-year follow-up in this secondary analysis of data from the China Imaging-based Biobank of Cerebral Small Vessel Diseases. Methods: CCMI was assessed on baseline brain magnetic resonance imaging. Motor functio

ATXN2 Spectrum Disorders: Genetic Complexity Beyond Dominant Inheritance.

The genetic complexity of ATXN2 challenges conventional paradigms in neurodegeneration. While initially linked to autosomal dominant spinocerebellar ataxia type 2, ATXN2 CAG expansions have emerged as a major risk factor for amyotrophic lateral sclerosis. In a recent article, Saucier et al. describe an Acadian family with a recessive inheritance pattern of ATXN2-related disease, challenging the long-held assumption of a predominantly autosomal dominant, gain-of-function-driven mechanism. Here, we analyze how ATXN2's inheritance mode may shift based on repeat interruptions, somatic mosaicism, a

Optimized Cas9-Enriched Nanopore Sequencing and Analysis Workflow for Clinical Diagnosis of Repeat Expansion Disorders.

Short tandem repeat (STR) expansion is a major genetic mechanism underlying numerous neurogenetic disorders. However, traditional PCR amplification and short-read next-generation sequencing-based methods often fail to detect large-scale, complex expansions and to capture methylation information. Thus, this study aimed to modify an amplification-free nanopore Cas9-targeted sequencing (nCATS) platform to achieve uniform coverage across 56 currently defined STR loci using a single test with genomic DNA from patient-derived blood cells and to develop a dedicated analysis algorithm, STRiker, capabl

Serial assessment of echocardiographic measures in Friedreich ataxia.

BACKGROUND: Cardiomyopathy is the most common cause of death in Friedreich ataxia (FRDA), but the relationship between genetic factors, disease course, echocardiographic measures, and clinical outcomes is not completely defined. METHODS: Data on echocardiographic results and disease status were obtained from records from a large cohort of subjects followed at the Children's Hospital of Philadelphia (CHOP) or other locations. We compared clinical features with long term clinical outcomes and echocardiographic markers of cardiomyopathy using correlation and linear regression. RESULTS: Overall,

Nitrous Oxide-Induced Myelopathy: A Case From French-Speaking Switzerland.

Recreational nitrous oxide (N₂O) use has increased worldwide, driven by its accessibility, low cost, and perception as a harmless "party drug." Chronic or heavy use can cause neurological complications through functional vitamin B₁₂ deficiency. Despite its growing prevalence, such cases remain rarely documented in French-speaking Switzerland. Our patient is a 27-year-old man of Cape Verdean origin who presented with rapidly progressive paresthesia and gait ataxia following heavy N₂O inhalation. Neurological examination revealed profound sensory ataxia and loss of vibration sense in the lower l

Exploring the Safety and Efficacy of Gabapentin Use In Perioperative Analgesia Protocols: An Integrative Review.

Gabapentin is widely used in perioperative analgesic protocols. Many clinical studies supporting the safety and analgesic efficacy of gabapentin for postoperative pain have been published over the last two decades. Several studies, many recently, have questioned its effectiveness and safety for perioperative pain management in early recovery after surgery (ERAS) and non-ERAS protocols leading to the research question: Is the use of gabapentin in perioperative analgesic protocols efficacious and safe for all perioperative patients? An integrative review was performed to identify, analyze, and s

Management of glioblastoma intramedullary spinal cord metastasis with advanced intraoperative techniques: a case series and systematic review.

BACKGROUND: Glioblastoma intramedullary spinal cord metastasis (GISCM) is a rare sequela of high-grade astrocytoma and glioblastoma multiforme (GBM). Discrete intramedullary spinal cord metastases are less common than spinal leptomeningeal spread and may follow a more indolent course. Once identified as GISCM, palliative maximal safe resection of the tumor may be considered to alleviate neurological symptoms. Reports describing the surgical management of these rare lesions, including the use of emerging technologies that may aid in maximal safe resection, are sparse. A further understanding is

Results From a Phase 2, Open-Label Study Evaluating the Safety, Tolerability, and Effect on Ataxia of GLM101 in Three Adult Patients With PMM2-CDG.

Phosphomannomutase 2 congenital disorder of glycosylation (PMM2-CDG) is a rare, autosomal recessive disease caused by PMM2 deficiency, which impairs conversion of mannose-6-phosphate into mannose-1-phosphate (M1P) and disrupts N-linked glycosylation. It typically results in a multisystem disorder in which a prominent cerebellar syndrome, characterized by ataxia among other clinical manifestations, can be assessed using the International Cooperative Ataxia Rating Scale (ICARS). GLM101 is a liposomal M1P substrate replacement therapy in clinical development. We present results conducted both wit

Not a Disease of the Past: A Case Report of Heroin-Induced Toxic Leukoencephalopathy Causing Progressive Neurologic Decline.

Toxic leukoencephalopathy is a rare neurological complication associated with various toxins, including inhaled heroin ("chasing the dragon"). We report a case of a 36-year-old female with a history of heroin dependence, in remission for seven years until a recent relapse, who presented with progressive deterioration in speech clarity, swallowing difficulty, and imbalance. Neuroimaging revealed diffuse leukoencephalopathy involving the cerebellar white matter and internal capsules, suggestive of heroin induced leukoencephalopathy. Laboratory evaluation was unremarkable, and infectious, autoimm

Advanced Presentation of GEMIN5 Neurodevelopmental Disorder in an Elderly Female.

GEMIN5, an RNA-binding protein, recognizes small nuclear RNAs and delivers them to the survival motor neuron complex for assembly of small nuclear ribonucleoproteins, which is vital for the maintenance of motor neurons. Loss-of-function mutations in the GEMIN5 gene are associated with neurodevelopmental disorders. Biallelic GEMIN5 variants cause motor-predominant developmental delay and cerebellar atrophy. Clinically, GEMIN5 variants have been associated with neurodevelopmental disorder with cerebellar atrophy and motor dysfunction syndrome, which is characterized by developmental and cognitiv

From Neonatal Cholestasis to Progressive Neurological Impairment: A Case of Niemann-Pick Disease Type C.

Physiological neonatal jaundice typically resolves within the first two weeks of life. Persistence beyond this period should prompt evaluation for neonatal cholestasis, which may represent the first manifestation of an underlying metabolic disorder. We report a six-year-old boy who presented at two months of age with cholestatic jaundice, hepatosplenomegaly, and elevated transaminases. Initial findings suggested congenital cytomegalovirus infection; however, liver biopsy was nonspecific, and further investigations were negative, leading to a diagnosis of idiopathic giant cell hepatitis. Progress on the mechanism of Piezo1 in mechanical ventilation-induced lung injury.

Ventilator-induced lung injury (VILI) causes 40-45% mortality in acute respiratory distress syndrome (ARDS) despite lung-protective ventilation. Current therapies remain empirical, lacking mechanistic understanding of how cells transduce mechanical forces into pathological responses. The mechanosensitive ion channel Piezo1 has emerged as a critical mechanotransducer, yet exhibits a fundamental paradox: endothelial Piezo1 deletion worsens edema while excessive activation disrupts barriers and triggers inflammatory cell death. Conventional "protective versus injurious" frameworks cannot explain
