Reconstruction with muscle flaps after hip joint heterotopic ossification resection in spinal cord injured patients: a case series.

INTRODUCTION: Heterotopic ossification (HO) is a frequent complication in persons with spinal cord injury (SCI). This condition leads to limitations in movement and to abnormal posture, which can impair sitting and bed positioning and may result in pressure ulcers. Surgery is considered when HO causes severe limitations due to joint obstruction caused by the ossification. However, the defect left after resection is often extensive and may create significant dead spaces that can lead to seroma or hematoma formation if not adequately filled, increasing the risk of infection and prolonged hospita

Movement disorders with autoimmune neuromuscular origin: an overview of Isaacs' syndrome, stiff person syndrome, immune-mediated rippling muscle disease.

Movement disorders associated with neuromuscular diseases are often underrecognized, yet they represent a distinct clinical entity. Abnormalities in areas of the peripheral nervous system, i.e., nerves and muscles, can stem from dysfunction of ion channels and proteins involved in membrane excitability. Hyperexcitability of peripheral motor nerves presents as cramps, stiffness, abnormal posture and gait, as well as changes in motor unit potentials during electromyography studies; hence, it may be classified as a movement problem. Etiologies range from hereditary, immune-mediated, or may be sec

Seasonal moderators of maternal CRP levels during pregnancy.

BACKGROUND: The human immune system is highly susceptible to seasonal changes, however it is unknown whether this holds true during pregnancy. This study assessed whether C-Reactive Protein (CRP) levels are moderated by season of sample collection and/or a person's tendency to have low mood during the fall-winter months (Seasonal Affective Disorder or SAD) in a Canadian pregnancy and birth cohort. As overweight/obesity has been associated with both elevated CRP and fall-winter SAD, the moderating effect of overweight/obesity on these seasonal effects was also examined. METHODS: Maternal plasm

A novel mutation in exon 18 of FGFR1 causing hypoparathyroidism: A case report.

Hypoparathyroidism is a rare endocrine condition characterized by insufficient secretion of parathyroid hormone (PTH), resulting in abnormally low calcium levels (hypocalcemia) and elevated phosphate levels (hyperphosphatemia) in the blood. This report describes a man in his late 30s with a chronic skin condition marked by dryness and desquamation. He occasionally experienced mild perioral numbness. Over the past year, he developed recurrent neuromuscular irritability, including worsening perioral numbness, tingling or numbness in the hands and feet, and muscle spasms consistent with tetany. H

Refractory Rickets: Evaluation and Management.

Refractory rickets refers to a set of diseases which are identified by a lack of response to therapeutic doses used to treat vitamin D deficiency. A child presenting with refractory rickets can pose a diagnostic dilemma as many kidney diseases have been identified as possible causes. Inherited (e.g., distal renal tubular acidosis, Fanconi syndrome, hypophosphatemic rickets, vitamin D-dependent rickets, nephronophthisis) and acquired tubular disorders [e.g., posterior urethral valves, reflux nephropathy leading to chronic kidney disease (CKD)-mineral bone disorder] may be complicated by refract

Autosomal dominant hypocalcemia type 1: status quo of tailored management and future perspectives.

A 28-day-old female infant presented with clonic seizures secondary to hypocalcemia (calcium [Ca], 5.7 mg/dL) and hypoparathyroidism (intact PTH, 7 pg/mL). Despite the initiation of oral alfacalcidol therapy, she experienced recurrent episodes of generalized convulsions or focal tetany, despite calcium lactate administration during febrile episodes. Her younger sister exhibited frequent irritability until day 38 of life due to hypocalcemia (Ca, 7.7 mg/dL) and hypoparathyroidism (intact PTH, 5 pg/mL). She experienced 3 febrile generalized seizures during infancy despite oral alfacalcidol treatm

Case Report: Diagnostic challenge between epileptic seizure and tetany in Albright hereditary osteodystrophy.

BACKGROUND: Albright hereditary osteodystrophy (AHO) is an epigenetically inherited condition resulting in end-organ resistance to parathyroid hormone (PTH) and consequent hypocalcemia, leading to tetany and seizures. AHO-driven calcification of the basal ganglia can often be visualized on computed tomography (CT). CASE DESCRIPTION: We report the case of a 36-year-old patient with AHO presenting with epileptic seizure vs. tetany, with an analysis of non-contrast head CT. An adult patient presented with bilateral clenching of fists, progressing to motor weakness, atonic loss of upright posture

Multiple dyselectrolytemia in a patient with alcohol use disorder (AUD).

Alcohol use disorder (AUD) is a chronic disease characterized by an inability to limit alcohol use despite knowledge of the negative health and social impacts. An umbrella entity, AUD encompasses the terms alcohol abuse, alcohol dependence, alcohol addiction, and alcoholism. In patients with AUD, who often have multiple electrolyte abnormalities, hypomagnesemia or magnesium deficiency can lead to prolonged hypokalemia or hypocalcemia, because magnesium is a coenzyme required for renal K excretion or 1,25-(OH)₂D₃ conversion. We experienced a 61-year-old patient with AUD with severe hypomagnesem

Does exposing all parathyroids during total thyroidectomy with central lymph node dissection protect long-term function? A prospective cohort study.

BACKGROUND: Hypoparathyroidism (HypoPT) is the most common complication after total thyroidectomy (TT) and central lymph node dissection (CLND) for thyroid carcinoma. However, there's still no effective surgical strategy to prevent its incidence. This study evaluated the impact of sufficient intraoperative exposure of the parathyroid glands (PG) on postoperative parathyroid function, the rate of parathyroid autotransplantation, and incidental parathyroidectomy. METHODS: Prospective data from 133 consecutive patients who underwent TT and CLND by a single surgeon at the First Affiliated Hospita

Case Report: Palopegteriparatide as a novel therapeutic option in pediatric autosomal dominant hypocalcemia type 1.

INTRODUCTION: Autosomal dominant hypocalcemia type 1 (ADH1) is a rare genetic disorder caused by gain-of-function variants in the CASR gene, leading to suppressed parathyroid hormone (PTH) secretion, hypocalcemia, hyperphosphatemia, hypomagnesemia and hypercalciuria. Evidence on the use of long-acting PTH analogs in pediatric ADH1 remains scarce. This report describes a boy with a de novo heterozygous activating CASR variant and highlights the therapeutic response to palopegteriparatide. CASE PRESENTATION: A 16-year-old boy presented with hypocalcemia, recurrent tetany, seizures, hypercalciur

Anthropogenic tritium as a continental-scale tracer in river-derived recharge.

BACKGROUND: Environmental hazards like floods, droughts and landslides pose serious public health and wellbeing consequences for populations, especially vulnerable communities in countries with low adaptive capacity. Empirical evidence from peer-reviewed literature and official climate change assessments indicates that, alongside high interannual climate variability, Uganda has experienced observable climate change over the past 50 years. This changing and increasingly variable climate poses challenges to population health in diverse ways, including through the transmission of infectious disea

Case Report: Retrosternal parathyroid lipoadenoma in an obese male patient-diagnostic pitfalls and postoperative biphasic parathyroid hormone dynamics.

INTRODUCTION: Parathyroid lipoadenoma is a rare histologic variant of parathyroid adenoma. Its occurrence in the retrosternal mediastinum poses significant diagnostic and surgical challenges. The clinical significance lies not in its rarity, but in the diagnostic pitfalls it presents, particularly when located in the retrosternal mediastinum. We present a case that underscores the pitfalls in preoperative diagnosis and the intricacies of minimally invasive resection. Sanjad-Sakati syndrome: integrated emergency care, long-term management, and expert perspectives-a narrative review.

BACKGROUND: Sanjad-Sakati syndrome (SSS) is a rare autosomal recessive disorder characterized by congenital hypoparathyroidism, persistent hypocalcemia, growth retardation, and distinctive craniofacial dysmorphism. Despite its rarity, SSS is more prevalent in the Middle East due to high consanguinity rates. Its multisystem involvement-including endocrine, neurologic, respiratory, renal, dental, and immunologic systems-poses significant challenges in pediatric emergency care and long-term management. OBJECTIVE: This review aims to provide a comprehensive, clinically focused overview and expert

Hyperventilation syndrome following epinephrine nasal packing for intubation-related epistaxis: a case report.

BACKGROUND: Epistaxis is the most common complication of nasotracheal intubation. Epinephrine-impregnated nasal gauze packing is usually employed as a hemostatic method for intubation-induced epistaxis. Systemic absorption of the topically administered epinephrine may lead to adverse effects. Currently reported adverse events associated with topical epinephrine administration are predominantly cardiovascular reactions. Here we describe a rare case of hyperventilation syndrome potentially induced by epinephrine-impregnated nasal packing for treatment of epistaxis in a patient who underwent naso

Reframing myofascial pain syndrome: pathological disorder or adaptive protective response?

Myofascial pain syndrome (MPS) is one of the most common causes of chronic pain. It is characterized by trigger points (TrPs) that are located within palpable taut bands (TBs) in muscles and their fascial structures. Traditionally, this clinical condition has been regarded as a dysfunction of the musculoskeletal system, commonly associated with mechanical overload, such as acute strain, repetitive movements, and/or abnormal posture holding. However, the intrinsic mechanisms underlying its development remain a subject of debate. This review proposes a paradigm shift: to consider MPS as a physio

[Cardiorespiratory functional disorders: A transnosologic approach].

Functional disorders affecting the cardiorespiratory system are common. Non-cardiac chest pain affects more than 10% of the population. It can result from gastro-oesophageal reflux or chest wall disorders but is also very often associated with anxiodepressive disorders, particularly panic disorder. Postural orthostatic tachycardia syndrome (POTS), a rarer entity, is characterized by a sharp increase in heart rate without orthostatic hypotension when standing. The pathophysiological mechanisms of POTS include 'partial' autonomic neuropathy, hypovolaemia and noradrenergic dysregulation, but it i
