22q11.2 deletion syndrome (22q11.2DS) is a relatively common disorder in pediatrics, frequently presenting with immune dysfunction, cardiac malformations, and hypoparathyroidism, all stemming from the development of the fourth branchial arch. While intracardiac malformations, such as tetralogy of Fallot, are highly prevalent, aortic abnormalities are also common. We report the case of a male diagnosed with 22q11.2DS at 12 years of age. His medical history included neonatal hypocalcemia, developmental delays, and later-onset tetany due to hypoparathyroidism. Despite a normal echocardiogram, a c

Background: Gitelman syndrome is a rare autosomal recessive renal tubulopathy, characterized by hypomagnesemia, hypokalemia, hypochloremia, and metabolic alkalosis. The syndrome commonly presents with symptoms such as fatigue, muscle cramps, and tetany, impacting patients' quality of life. Although genetic confirmation via identification of mutations in the SLC12A3 gene is ideal, resource constraints often limit access to these tests, especially in low-resource settings. This study aims to analyze the clinical, biochemical, and familial features of Gitelman syndrome in three patients, highligh

Secondary hyperparathyroidism (SHPT) in chronic kidney disease often necessitates parathyroidectomy (PTX), but this definitive treatment can precipitate hungry bone syndrome (HBS)-a profound, prolonged hypocalcemia caused by the rapid skeletal uptake of minerals after surgery. HBS results from the abrupt cessation of parathyroid hormone (PTH)-driven bone resorption while bone formation continues, leading to intensive mineral deposition (mainly calcium) into chronically demineralized bone. Clinically, HBS ranges from asymptomatic biochemical disturbances to life-threatening hypocalcemia with te

Patients with dialysis have a high risk of osteoporosis and fractures. However, the optimal treatment strategies for osteoporosis in dialysis patients remain unclear due to the complexity of chronic kidney disease (CKD)-mineral and bone disorder. This case report describes severe symptomatic hypocalcemia and prolonged heart failure following denosumab treatment in a peritoneal dialysis (PD) patient with secondary hyperparathyroidism and osteoporosis. A 70-year-old woman on PD for dialysis caused by diabetic nephropathy was diagnosed with osteoporosis (T score -2.5) during a routine checkup. De

RATIONALE: Fahr's syndrome is characterized by bilateral intracranial calcifications involving the cerebellum, basal ganglia, thalamus, radial crown, and subcortical white matter. It is a rare neurological disorder often associated with hypoparathyroidism (HypoPT). HypoPT-induced hypocalcemia triggers calcium-phosphate deposition in brain parenchyma, typically manifesting as tetany, seizures, or extrapyramidal symptoms. Diagnostic challenges arise from overlapping symptom features with neurodegenerative and psychiatric disorders. We present a case of Fahr's syndrome secondary to iatrogenic Hyp

Familial Hypomagnesemia with Hypercalciuria and Nephrocalcinosis (FHHNC) is a rare tubulopathy resulting from mutations in the CLDN16 and CLDN19 genes. The affected individuals commonly present with polyuria, polydipsia, excessive urinary magnesium (Mg) and calcium (Ca) wasting, nephrocalcinosis, nephrolithiasis, recurrent urinary tract infections, and renal dysfunction. We herein report a 22-year-old male born to consanguineous parents presenting with recurrent nephrolithiasis since early childhood. He gave a previous history of muscle cramps, tetany, and seizures on multiple occasions with r

In this case report, we present an infant with abnormal body movements in her second month. She was born of a third-degree consanguineous marriage, and the previous sibling, who had similar complaints beginning in the fourth month, expired in the fifth month. On reviewing the video recorded by parents, the abnormal movements were suggestive of tetany. Biochemical investigations showed hypocalcaemia, hypomagnesaemia, hypoparathyroidism, normal urinary excretion of calcium and urinary magnesium losses. Intravenous calcium and magnesium corrections were given, and oral supplements were initiated.

Hypocalcaemia is a common metabolic abnormality in neonates, with distinct causes based on the timing of onset. Early-onset hypocalcaemia (within 72 hours of life) is typically related to transitional physiology, prematurity, perinatal asphyxia or maternal diabetes. In contrast, late-onset hypocalcaemia (after 72 hours) is less common and may result from high phosphate intake, cow's milk-based formulas, vitamin D deficiency or resistance, maternal vitamin D deficiency, hypoparathyroidism (eg, DiGeorge syndrome), magnesium deficiency or activating mutations in the calcium-sensing receptor. Hemilingual atrophy, often secondary to lesions (e.g. neoplasia) affecting the hypoglossal nerve ipsilaterally, is the only unilateral tongue muscle-related neurological disorder reported in dogs. Hemilingual spasm is a descriptive term used in the human medical literature that refers to a disorder of involuntary, unilateral contractions in tongue musculature. We report a 5-year-old Border collie with hemilingual tetany ("hemilingual spasm"). Clinical examination revealed a resting unilateral contraction of the tongue on the right side with a resultant curvature of the tongue of variable degre

Calcineurin inhibitors remain a cornerstone in the treatment of steroid-resistant nephrotic syndrome (SRNS) in pediatric patients. Hypomagnesemia, a clinically relevant yet frequently neglected complication of calcineurin inhibitors like cyclosporine, may exacerbate hypokalemia and induce neuromuscular symptoms such as tetany, challenging patient management. However, serum magnesium is not routinely monitored in many clinical settings. A 46-month-old girl with SRNS had been receiving high-dose cyclosporine for over 1 year. She was admitted with vomiting, diarrhea, generalized edema, and hypovo

Microbes can alter host behavior, immunity, and neurological function at a distance without extension into the brain and spinal cord. Clostridia provides a predicate for such an infection in the periphery by causing "lock jaw" and generalized tetany. This case series presents five patients who showed rigidity or tetany of the forearm. All were diagnosed with vertigo of central origin by video nystagmography (VNG) and posturography. Each had an apparent slow-rolling tetany, most visible in the forearm. Each had a consistent focus of pain within the supinator, diminution of extended wrist rotati

Hypocalcemia is an uncommon electrolyte abnormality. We evaluated a young woman with episodes of recurrent symptomatic hypocalcemia, which had started in her late teens. She was hypertensive and laboratory evaluation revealed elevated parathyroid hormone, elevated phosphate, and decreased renal phosphate excretion along with low ionized calcium levels. Her renin and aldosterone levels were elevated. While the response to treatment with calcium and 1,25(OH)2 dihydroxyvitamin D was modest, her laboratory values and symptoms improved significantly during pregnancy and then recurred post-delivery.

BACKGROUND/OBJECTIVE: Pseudohypoparathyroidism (PHP) has a varied presentation, ranging from biochemically detected hypocalcemia in asymptomatic individuals to treatment-refractory seizures. Features may develop over time. Hypocalcemia with recurrent seizures in adolescents indicates a cause other than a primary central nervous system cause, rather than hypoparathyroidism and pseudohypoparathyroidism being the endocrinological causes.
CASE REPORT: We present a case of an adolescent girl with difficult-to-treat seizures and dystonic body movement whose diagnostic workup confirmed PHP. Phenotyp

UNLABELLED: Essential oils have proven to possess great potential in the field of biomedicine due to their ability to effectively eradicate a diverse range of pathogenic microbes. In this study, the antimicrobial activity of ylang-ylang essential oil (YY-EO) was screened against twelve multidrug-resistant pathogens. The YY-EO was effective up to 536 μg/ml, with the highest inhibition zone in case of S. aureus MMCC21 and Escherichia coli MMCC24. The least effect on both Bacillus cereus MMCC11 and Klebsiella pneumonia MMCC16. The major components of the essential oil were identified using GC-MS

Despite the significant presence of plant-derived tricarboxylic acids in some environments, few studies detail the bacterial metabolism of trans-aconitic acid (Taa) and tricarballylic acid (Tcb). In a soil bacterium, Acinetobacter baylyi ADP1, we discovered interrelated pathways for the consumption of Taa and Tcb. An intricate regulatory scheme tightly controls the transport and catabolism of both compounds and may reflect that they can be toxic inhibitors of the tricarboxylic acid cycle. The genes encoding two similar LysR-type transcriptional regulators, TcuR and TclR, were clustered on the

Hypercalcemia in pregnancy is rare and can pose a great diagnostic challenge due to its asymptomatic presentation. It is associated with maternal complications such as urolithiasis, pancreatitis, renal insufficiency and preeclampsia, fetal complications such as growth restriction and intrauterine fetal demise, and neonatal complications such as neonatal hypocalcemia, tetany and hypoparathyroidism. Prompt diagnosis and treatment of the underlying cause of hypercalcemia is important. Two cases of hypercalcemia in pregnancy were encountered over 12 months. Both presented asymptomatically in the f

Proton pump inhibitors (PPIs) are commonly used for many gastrointestinal issues, such as gastroesophageal reflux disease (GERD), peptic ulcer disease, and Zollinger-Ellison syndrome. Many patients are on life-long daily therapy with this class of medications. The adverse effects of long-term use of PPI have been studied, and over the last two decades, a link between hypomagnesemia and PPI has been established. In addition, other electrolyte derangements can also ensue, such as hypokalemia and hypocalcemia. Losses through the gastrointestinal or renal systems may also be responsible for this e

Celiac crisis (CC) is a rare complication of celiac disease (CD), usually observed in younger children with unrecognized CD or poor compliance with a gluten-free diet (GFD). We present a case of celiac crisis in a 3-year-old girl who was recently diagnosed with celiac disease. She was referred to our clinic with anasarca, tetany, and severe malnutrition, with hypokalemia, hypocalcemia, hypomagnesemia, and hypoalbuminemia. During hospitalization, she presented hypertransaminasemia with positive anti-actin smooth muscle antibodies (SMA). Abdominal ultrasound and liver biopsy were normal, excludi

To develop effective technology that employs electric fields to simultaneously guide valued freshwater fish whilst limiting the range expansion of undesirable invasive species, there is a need to quantify the electrosensitivity of multiple families. This experimental study quantified the electrosensitivity of two carp species that, in UK, are invasive (grass carp, Ctenopharyngodon idella, and common carp, Cyprinus carpio) and compared the values with those previously obtained for adult European eel (Anguilla anguilla), a species of conservation concern in Europe. Electric field strengths (V/cm

Autoimmune polyglandular syndrome 1 (APS1) is an autosomal recessive disorder due to biallelic pathogenic variants in the autoimmune regulator (AIRE) gene that manifests with chronic mucocutaneous candidiasis, primary hypoparathyroidism, and adrenal insufficiency. We report a 39-year-old woman with APS1 who developed partial lipodystrophy during adulthood. She presented with diaper rashes, oral thrush, and tetany during infancy due to candidiasis and hypoparathyroidism. During childhood, she developed hypothyroidism, primary adrenal insufficiency, and ovarian insufficiency. At age 14, she rece
