Leptomeningeal metastasis (LM) is a rare complication of esophageal adenocarcinoma, with reported cases typically presenting with meningitic, encephalopathic, or multifocal neurologic symptoms. We report a 70-year-old man with metastatic esophageal adenocarcinoma with signet-ring cell features who developed subacute, painless, bilateral visual loss with bilateral optic disc edema as the initial manifestation of probable LM. Symptoms arose four months into first-line folinic acid, fluorouracil, and oxaliplatin (FOLFOX) plus pembrolizumab while all measurable extracranial disease was responding

Scrub typhus is an important cause of acute febrile illness in Nepal and can rarely present with meningoencephalitis and multiorgan involvement. We report a 32-year-old female farmer from rural Nepal who presented during the monsoon season with high-grade fever, myalgia, vomiting, and an eschar following an unidentified forest bite. She developed meningismus, hypotension, mild acute respiratory distress syndrome, and transaminitis. Cerebrospinal fluid showed lymphocytic pleocytosis and elevated protein, while bacterial culture, acid-fast staining, and GeneXpert were negative. Serum scrub typhu

BACKGROUND: Subdural empyema (SDE) is a neurosurgical emergency with 6-15% mortality. Culture-negative cases pose significant diagnostic and therapeutic challenges.
CASE PRESENTATION: A 51-year-old male with chronic alcohol abuse presented with high fever (39°C), severe headache, meningismus, and altered mental status (GCS 13). CSF analysis revealed protein 28 g/L (60-fold elevated), uncountable cells due to viscous fluid, and Gram-positive cocci. Initial non-contrast CT suggested chronic subdural hematoma, but surgical exploration revealed frontal subdural empyema with frank purulent dischar

Scedosporium apiospermum is an emerging cause of invasive mold infection in immunocompromised hosts, often with central nervous system involvement and limited susceptibility to amphotericin B. We describe a 36-year-old lung transplant recipient who presented with fever, meningismus, and multiple enhancing brain lesions nine months post-transplant. Cerebrospinal fluid studies, including metagenomic next-generation sequencing (mNGS), were negative. Cardiac imaging revealed a pedunculated right ventricular septal mass, and plasma cell-free DNA (cfDNA) testing (Karius) identified S. apiospermum. S

BACKGROUND: Transanal protrusion of a ventriculoperitoneal (VP) shunt catheter is an uncommon manifestation of bowel perforation. Most reports favor shunt removal or externalization because retrograde contamination may cause ventriculitis, meningitis, peritonitis, or sepsis.
OBSERVATIONS: A 69-year-old man with posttuberculous hydrocephalus and bilateral VP shunts presented 20 days after trocar-assisted left distal catheter reinsertion with the catheter protruding 2 cm beyond the anus. He was afebrile and without peritonitis or meningismus. Recent CSF testing showed clear, colorless CSF with

INTRODUCTION: This case series discusses five cases of invasive Nontyphoidal Salmonella (iNTS) infections. These presentations are atypical in healthy children without immunodeficiency or hemoglobinopathy.
METHODS: We present five cases of iNTS in otherwise healthy children that were not previously known to have established risk factors. An abridged literature review of iNTS was performed.
RESULTS: The patients in this case series were older than previously documented reports of iNTS with a mean age of 12.4 years. Each patient was immunocompetent aside from hypogammaglobulinemia in case 5. T

BACKGROUND: Fusobacterium necrophorum is an anaerobic Gram-negative organism classically associated with severe head and neck infections and Lemierre syndrome. Central nervous system involvement is uncommon but may be rapidly progressive and fatal, particularly when associated with sinusitis, intracranial empyema, cerebritis, or brain abscess.
CASE PRESENTATION: We report the case of a previously healthy 18-year-old male who presented after four days of high fever up to 41.5 °C, severe holocranial headache, vomiting, and acute hyperactive delirium. On admission, he had meningismus, markedly e

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

The activities that define survival and reproductive success in animals depend on their physical performance. However, performance is a complex trait, and organisms must balance competing demands of multiple underlying factors every time they undertake an activity. For example, the morphology that increases bite force (i.e. increased head size) - improving fighting ability - should constrain sprinting performance by adding mass to the body. Consequently, trade-offs between fighting and escape performance might be sex specific where sexual dimorphism is present, or pronounced in animals with ex

A 67-year-old woman was admitted to the hospital for 3 months of new-onset headaches and meningismus that progressed to focal neurologic deficits. These first manifested as episodes of flashing lights and black blotches obscuring her vision for minutes. She also began to have visual hallucinations with formed letters and words projected onto nearby walls. She developed gait unsteadiness and difficulty using her phone and reading a clock. She was seen by neuro-ophthalmology and found to have a left homonymous hemianopsia, at which point she was admitted for further evaluation. Physical examinat

This study aimed to identify factors present at the initial visit for the development of sunset glow fundus (SGF) in Vogt-Koyanagi-Harada (VKH) disease. A total of 139 eyes from 70 naïve patients with acute VKH disease who underwent pulse steroid therapy were categorized into "SGF" and "non-SGF" groups based on fundus appearance at 12 months post-therapy. Multivariable analysis revealed that patients with meningismus (p = 0.004), incomplete type (p = 0.035), and probable type (p = 0.002) in older revised diagnostic criteria; ex-smoker (p = 0.016), and smoker (p = 0.010) status; high logarithmi

This report describes an atypical presentation of cryptococcal meningitis (CM) in a 51 year-old HIV-negative male with diabetes, hypertension, and psoriasis. The patient presented with a 10 day history of subacute headache, vomiting, and fever, followed by the abrupt onset of profound bilateral sensorineural hearing loss (SNHL), ataxia, diplopia, anosmia, ageusia, drowsiness, meningismus, absent gag reflex, and bilateral abducens palsy. Brain MRI revealed inflammatory changes involving the sellar diaphragm, thalamus, and pituitary stalk. Cerebrospinal fluid analysis confirmed CM (pleocytosis,

This report presents a rare case of acute bacterial rhinosinusitis with orbital and intracranial complications caused by Capnocytophaga sputigena in a pediatric patient. A 15-year-old male presented with orbital cellulitis, acute sinusitis, and meningismus. Brain imaging showed evidence of early intracranial abscess formation and areas of cerebral infarction. He underwent urgent endoscopic sinus surgery (ESS) and drainage of an orbital subperiosteal abscess. This case underscores the critical role of timely diagnostic imaging, multidisciplinary care, appropriate surgical management, and effect

BACKGROUND: Despite decreased mortality over the past several decades, the prognosis of brain abscesses remains dependent on intracranial location and causative organism. Deep-seated brain abscesses carry a risk of intraventricular rupture, an event with reported mortality near 80%. Coccidioidomycosis from the fungus Coccidioides immitis, endemic to the American Southwest, is growing in incidence but uncommonly produces deep brain abscesses, making management unclear. The authors report the diagnosis and management of a solitary thalamic Coccidioides abscess without meningitis.

History: An 8-year-old previously healthy female presented to the emergency department after new-onset seizure activity. Three days prior, she experienced severe headaches and rash over her left lower extremity. She developed nonbilious, nonbloody emesis on the day prior to admission. The morning of presentation, she was found unresponsive, exhibiting right gaze deviation and clenched jaw concerning for seizure activity. Further history revealed recent Group A streptococcal pharyngitis, which had been treated with a 10-day regimen of amoxicillin. During this time, her mother reported decreased

Cryptococcal meningoencephalitis is a grave and lethal form of an opportunistic infection usually occurring in immunocompromised patients (i.e., those with AIDS, organ transplant recipients, or those on immunosuppressive therapy) caused by Cryptococcus species, namely Cryptococcus neoformans, which has a worldwide distribution, or Cryptococcus gattii, which is predominantly found in tropical, subtropical, and temperate regions. Cryptococcus species are usually found in soil contaminated with bird or animal droppings in the form of cryptococcal spores. The former occurs predominantly in immunoc

Longitudinally extensive myelitis with 15 or more vertebrae in length is extremely rare, with limited evidence regarding clinical features and therapeutic response. We report a case of a 29-year-old male patient with extremely longitudinally extensive myelitis ultimately diagnosed as myelin oligodendrocyte glycoprotein-associated disease (MOGAD). The patient presented with an acute onset of meningismus, limb weakness, sensory disturbance below the C5 level, ataxia, and urinary retention. T2-weighted imaging on MRI showed an extremely longitudinally extensive spinal cord lesion ranging from C2
