We studied 100 patients with tick-borne meningopolyneuritis (Garin-Bujadoux, Bannwarth), the neurologic component of European erythema chronicum migrans disease. They had intensive radicular pain, asymmetric polyneuritis combined often with uni- or bilateral facial palsy, lymphocytic meningitis without or with only slight meningismus, and a course lasting three to five months. Neurologic abnormalities were preceded by the bite of a tick or an insect in 37 percent of patients or by an erythema in 41 percent. In addition, many patients had extraneural involvement, such as fever or fatigue. The o

Mollaret's meningitis (benign recurrent aseptic meningitis) is a rare syndrome with characteristic features. Patients present with recurrent attacks of meningismus which are sudden in onset and last from one to seven days. During the attacks the spinal fluid shows pleocytosis with lymphocyte predominance. Large endothelial cells with indistinct cytoplasm (Mollaret's cells) are typically present in the CSF. The attacks resolve spontaneously and without sequellae. Symptom-free intervals can be as brief as a few days or as long as several years. Though specific treatment is not available, Colchic

Ventriculitis developed suddenly in a patient with an undiagnosed abscess and was accompanied by obtundation, fever, meningismus, and hemiplegia. Aspiration of a streptococcal thalamic abscess and high doses of intravenous penicillin produced marked improvement. Increasing dysphasia 5 weeks later was accompanied by ventricular dilatation, most marked on the left, with no evidence of recurrent abscess. The left foramen of Monro was demonstrably patent. Shunting relieved the symptoms of aphasia; they recurred with one episode of malfunction of the shunt. This case lends support to the belief tha

The records of 134 patients with the clinical diagnosis of epidemic hemorrhagic fever (EHF) were evaluated. The conditions of 74 patients could be categorized as "serious" based on hemorrhagic complications and magnitude of proteinuria, BP abnormality, and pyrexia. Forty-six of these patients with a serious condition had one or more CNS abnormalities. Common manifestations included confusion, meningismus, and convulsions. Whereas abnormality in fever could not be related to CNS abnormality, BP disturbance, acidosis, and azotemia were significantly more common among patients with CNS disease. F

Severe chronic cerebral vasospasm was reliably induced in dogs by two injections, 2 days apart, of autologous blood into the cisterna magna. Treatment with ibuprofen or high-dose methylprednisolone after the first injection prevented or reduced vasospasm. Both drugs reduced meningismus and accelerated the rate of neurological recovery. Compared with specimens from normal dogs, rings of basilar arteries obtained from untreated dogs contracted weakly in response to 5-hydroxytryptamine, prostaglandin F2 alpha, potassium chloride, and barium chloride. Rings of arteries from dogs who received ibupr

The fluorescence method of Blume et al. was found to be useful for identification and morphological evaluation of lysosomes in cerebrospinal fluid cells. The investigations were carried out in 49 patients with viral and bacterial meningitis or meningismus. It was demonstrated that the CSF cells in most patients with purulent meningitis contained no fluorescent granules in an early stage of the disease before introduction of antibacterial therapy. These granules were found in CSF cells in cases of leptospirosis, viral meningitis and meningismus of various aetiology in acute stage of the disease

A 21-year-old woman with systemic lupus erythematosus (SLE) had meningismus, conjunctivitis, lymphadenopathy, and elevation of liver function values following ibuprofen therapy. All symptoms and laboratory value abnormalities resolved rapidly when the drug therapy was stopped. Two years later, a similar reaction occurred after taking tolmetin sodium. Fever, adenopathy, aseptic meningitis with a polymorphonuclear pleocytosis, and serum transaminase level elevations resolved rapidly after use of the drug was discontinued. The mechanism of this reaction to nonsteroidal antiinflammatory drugs in p

Meningococcemia caused by serogroup W135 occurred in four patients during a period of nine months. Three of the patients had hypogammaglobulinemia with normal hemolytic complement levels; the fourth had normal immunoglobulin levels one year after her illness. None of the patients had classic meningitis. One patient had pneumonia; one appeared to have encephalitis with mild meningismus; and two had sepsis without localizing signs or symptoms. Patients with hypogammaglobulinemia are predisposed to the development of meningococcemia, and the incidence of W135 carriage in the population is increas

The published literature suggests that the technique of epidural "blood patch" injection is both safe and effective for the treatment and prevention of headache after spinal puncture. However, a case is reported in which the complications following a "blood patch" outweighed the disability and discomfort produced by the headache itself. The etiology is believed to have been inadvertent injection of blood into the subarachnoid space.

Using the method of Park et al. the author studied spontaneous and stimulated NBT reduction by neutrophil granulocytes in the cerebrospinal fluid and blood, and by monocytic-reticular cells in the cerebrospinal fluid of the patients with bacterial and viral meningitis and meningismus. The author performed 333 investigations in 74 patients. Significantly higher mean values of the index of spontaneous and stimulated NBT reduction by the granulocytes and cerebrospinal fluid were observed in cases of bacterial meningitis as compared with the granulocytes of the peripheral blood in healthy subjects

Three cases are used to illustrate the computed tomography (CT) findings of tuberculous meningitis in infants and children. The clinical and laboratory findings of these patients are presented, and the differential diagnosis of viral, bacterial and fungal meningitis is reviewed. Tuberculous meningitis should be suspected, even in the infant or child with a negative tuberculin skin test, when a chronically ill patient presents with the acute signs of meningismus, the cerebrospinal fluid analysis demonstrates a low glucose and monocytosis, and cranial CT shows ventricular enlargement with promin

The authors describe side effects of Dimer X radiculography after premedication. The side effects in a study of hundred patients are divided in early complications (during the examination : dizziness, radicular pains and collapse) and in late complications (noted during the first day following the examination : headache, meningismus, myocloni, epileptic seizure, cauda syndrome, increase of radicular pains). After premedication with tiapride the rate of dizziness is decreased, the rate of headache is not diminished, but the headaches are very slight. Nausea and vomiting are suppressed by the pr

The authors present four cases of abscess of the thalamus and basal ganglia and review an additional 10 cases from the literature. These abscesses appear to be metastatic and are usually associated with a cardiac septal defect or a preceding intrathoracic infection. Fever, elevated white blood count, and/or meningismus, in combination with angiography demonstrating an avascular mass, help distinguish abscess from tumor in this location. The authors advocate aspiration rather than excision.

Seven patients with Candida meningitis are reported. These 7, plus 21 previously cited cases, were reviewed. This infection arose by two distinct mechanisms: hematogenous dissemination and direct inoculation. Recent antibiotic therapy, corticosteroid administration and severe underlying diseases were important predisposing factors. Fever, meningismus, elevated CSF pressures and localizing neurologic signs were commonly noted. Organisms were seen on gram-stain of CSF in only 43% of cases. Mortality rate in patients receiving inadequate or no antifungal therapy was high (greater than 90%), while

Pathologic examination in a case of fatal intracerebral hemorrhage from a berry aneurysm showed that the "sentinel" or warning headache in this patient was due to the leakage of blood into the subarachnoid space through a previous small tear in the wall of her saccular aneurysm. Oribital pain, transient, dysphasia, dizziness and, later, meningismus might have prompted the performing of a lumbar puncture to determine the presence of blood in the cerebrospinal fluid. This type of event is the likely pathogenetic mechanism for the premonitory headache that may precede a lethal rupture of a saccul

The complications which occur in a child with acute leukemia depend on the stage of the disease and the therapeutic regiman. Most children will present with some manifestation of marrow failure. An occasional child will have marked leukocytosis and disturbance of organ function due to massive leukemic infiltrates. Metabolic disturbances such as hyperuricemia and hyperphosphatemia-hypocalcemia may develop, expecially after therapy is initiated. The myelosuppression and immunosuppression due to drug toxicity may result in opportunistic infections. Other toxicities which can occur with a chemothe

A study of 170 patients with juvenile rheumatoid arthritis and a review of the literature indicate that this disease can significantly affect the central nervous system. Signs of CNS dysfunction were observed in 13 children. During the acute toxic stages the EEG is abnormal in many cases. Other manifestations of toxic encephalopathy such as irritability, drowsiness, stupor, convulsions and marked meningismus may be evident in severe cases. Meningitis is often suspected but ruled out by the finding of normal CSF. Steroids can rapidly improve the condition of these children. If ;unexplained' sei
