Unfortunately, there is no general consensus as to how long patients with bacteriuria or urinary tract infections should be monitored and certainly there is no agreement on how long recurrent episodes should be treated beyond ten days to two weeks. The most important points to remember are: 1. Culture the urine both at the time of therapy and during follow-up. The patient should be examined periodically for the presence of bacteruria. The aut horsdescribe the technique and the first results of para-sphincteric anaesthetic infiltrations of the striate sphincter of the urethra in paraplegics. These infiltrations may decrease, or even cause to disappear altogether, dysuria as long as the striate sphincter is alone responsible and that the straite sphincter muscle is not the process of fibrosis. In certain special cases, this simple technique may have a durable effect which makes it possible to avoid definitive procedures such as endoscopic or surgical section.

Urinary tract infections (UTI's) are common in medicine. Symptoms may include fever, chills, frequency, and dysuria. Asymptomatic UTI's are also common, with a prevalence of one percent in school girls and ten percent in pregnant women. Pyuria, dysuria, and frequency may be absent in patients with UTI's or present in patients without UTI's. Therefore, a UTI must be bacteriologically diagnosed as greater than 100,000 organisms/ml, usually of a single organism, in a properly obtained urine specimen. The dipslide is a simple, convenient, inexpensive device for the quantitative diagnosis of a UTI.

The diagnosis and management of 34 men and 13 women with urethral calculi are reviewed. A majority of patients presented with dysuria, weak stream and perineal pain. Diagnosis was made radiographically in only 42 per cent but was confirmed in all patients who underwent cystourethroscopy. Small-bowel infarction resulting from mesenteric venous thrombosis is a rare but life-threatening complication to which young women taking oral contraceptives are liable. The patient characteristically presents with an "acute abdomen" after a variable prodromal period of vague abdominal pain. The correct diagnosis can be made from the macroscopic appearance at laparotomy, provided the surgeon is alert to the condition. An accurate diagnosis is important, as anticoagulation measures must be instituted early to counter the commonly associated thromboembolic phenomena.

The aim of this study was in the first place to elucidate the tolerance of rheumatic patients to prolonged treatment with tolfenamic acid. 91 patients took part in the trial, most of them suffering from rheumatoid arthritis. The daily dose of tolfenamic acid was 600 mg and the trial lasted 6 months. We are reporting on a decade of experinece with cases of renal tuberculosis treated at a large tuberculosis hospital. Most patients were men less than 50 years old. The most frequent symptoms were dysuria, back or flank pain, nocturia and hematuria. Physical examinations were generally normal and hypertension was not seen. Most of all cases of extrapulmonary tuberculosis i.e. 35% are urogenital tuberculosis UGT. 36 patients with flat carcinoma in situ of the bladder have been reviewed. Those with widespread disease usually presented with dysuria or obstructive outflow tract symptoms and radical surgery appeared to give better results than radiotherapy. Urodynamic findings, and in particular measures of outflow obstruction in a consecutive series of 156 women presenting with recurrent increased frequency and dysuria, have been analysed. Poor correlation between these various measures has been demonstrated; at most only 8% of these patients have demonstrable outflow obstruction. More than a quarter of the patients had bladder instability.

The records of 141 consecutive patients who underwent simple or radical cystectomy between 1955 and 1971 were reviewed. Forty female patients suffering for long periods from frequency, urgency, and dysuria without any definite organic cause received nortriptyline chloride for three weeks. In 75 percent there was either considerable improvement or total disappearance of urinary complaints. At the same time the in vitro effects of the drug were tested using the isometric muscle contraction technique. Nortriptyline was found to have anticholinergic properties. The importance of this effect in the clinical study and the possible mode of action of the drug are discussed.
