Robot-Assisted Partial Nephrectomy in a High-Complexity Renal Tumor in a Horseshoe Kidney.

INTRODUCTION: Horseshoe kidney is an uncommon congenital fusion anomaly that can make renal tumor surgery especially challenging because of altered rotation, limited mobility, variable vascular supply, and an unpredictable collecting system (1-7). This video presents a robot-assisted partial nephrectomy for a high-complexity renal tumor in this setting. CASE PRESENTATION: A 33-year-old man, with ECOG 0 and no relevant comorbidities, was diagnosed with a 7.5-cm solid renal mass in the central posterior portion of the left moiety of a horseshoe kidney. The lesion had a RENAL score of 10p. Contr

Extraperitoneal Single-Port Robotic-Assisted Radical Cystectomy with Orthotopic Ne-obladder: Technique and Surgical Considerations.

INTRODUCTION: Radical cystectomy (RC) is the standard therapy for muscle-invasive bladder cancer (MIBC) and refractory high-risk non-muscle-invasive bladder cancer (1-3). Single-port robot-assisted radical cystectomy (RARC) offers notable minimally invasive advantages (4-6), whereas conventional transperitoneal approaches are associated with intestinal and gastrointestinal complications (7). This study evaluated an optimized extraperitoneal single-port RARC with orthotopic neobladder reconstruction for improved surgical safety and clinical outcomes. MATERIALS AND METHODS: A 57-year-old male p

Implementation of an evidence-based app to improve healthcare for female urinary incontinence: identification of implementation strategies using the consolidated framework for implementation research-expert recommendations for implementation change matching tool.

BACKGROUND: Female urinary incontinence (UI) affects one in three women and is frequently managed suboptimally. The app-based treatment URinControl demonstrated cost-effectiveness. However, implementation in Dutch primary care is limited. This study aimed to identify key barriers to implementation and develop strategies to support the uptake of URinControl in primary care settings. METHODS: We conducted a cross-sectional survey among Dutch general practitioners (GPs), community pharmacists, and supporting personnel working in general practice or local pharmacies. Barriers were identified usin

Ginsenoside Rb1 ameliorates stress urinary incontinence by upregulating SIRT1 to preserve mitochondrial homeostasis and MAM integrity.

INTRODUCTION: Stress urinary incontinence (SUI) is a prevalent pelvic floor disorder characterized by impaired connective tissue integrity. Oxidative stress-induced mitochondrial dysfunction has been implicated in pelvic floor degeneration. Ginsenoside Rb1 (Rb1), a bioactive component of ginseng, exhibits antioxidant properties; however, its therapeutic effects and underlying mechanisms in SUI remain unclear. METHODS: A vaginal distension (VD)-induced mouse model of SUI and an H2O2-induced oxidative stress model in L929 cells were established to investigate the effects of Rb1. Urodynamic eval

Urinary Incontinence, Pelvic Floor Care, and Support Preferences Among Parous Women: A Cross-Sectional Online Survey.

INTRODUCTION AND HYPOTHESIS: Urinary incontinence (UI) is common after childbirth, but uptake of care may be limited. We aimed to describe UI burden, pelvic floor muscle training (PFMT), care-seeking, and support preferences among parous women in Japan. METHODS: In March 2026, 1000 parous women aged 20-49 years completed a cross-sectional online survey with equal age-decade quotas. This unweighted, non-probability sample was not nationally representative. Current UI was operationalized as an International Consultation on Incontinence Questionnaire-Urinary Incontinence Short Form (ICIQ-UI SF)

Artificial intelligence-based prediction of postoperative voiding dysfunction following midurethral sling.

PURPOSE OF REVIEW: Midurethral sling (MUS) surgery is the standard procedure for stress urinary incontinence, offering favorable long-term outcomes. Despite its success, postoperative voiding dysfunction - manifesting as urinary retention, elevated post-void residuals (PVRs), slow or weak stream, hesitancy, or the need for prolonged catheterization - remains a clinically significant complication that is difficult to predict using traditional risk stratification. Conventional methods, including history, physical examination, PVR measurement, and selective urodynamics, provide limited individual

Beyond Sepsis Survival: Post-Acute Vulnerability Profiles in Older Home Health Patients.

BACKGROUND: Older sepsis survivors face elevated rehospitalization risk shaped by multimorbidity and post-sepsis vulnerability, yet traditional risk models evaluate these factors in isolation. To identify clinically meaningful profiles of coexisting rehospitalization risk factors among older sepsis survivors receiving home health care (HHC), we used clustering to characterize risk subgroups and examined their association with 30-day rehospitalization. METHODS: A retrospective cohort study of Medicare fee-for-service beneficiaries aged 65 years or older who transitioned to HHC after hospitaliz

Caregiver-implemented standardized-to-individualized behavior-analytic toilet-training package for preschoolers with autism.

Children with autism often face challenges with incontinence and may benefit from more structured toilet training relative to neurotypical children. In the current study, we evaluated a standardized-to-individualized behavior-analytic toilet-training package with caregivers as the interventionists. We used telehealth to train caregivers remotely to support their implementation of the procedures and obtain additional data on implementation and outcomes. Four preschool-aged children with autism and their caregivers participated. Three of four participants met mastery criteria with a standardized

Early apical release versus conventional (non-early apical release) holmium laser enucleation of the prostate: a systematic review and meta-analysis of postoperative urinary incontinence and functional outcomes.

BACKGROUND: Despite proven safety, efficacy, and durability of Holmium laser enucleation of the prostate (HoLEP), postoperative transient stress urine incontinence (SUI) was a concern for years. A technical modification of the original HoLEP, namely early apical release (EAR) has been suggested to reduce postoperative SUI. This study aimed to collate available evidence on the impact of EAR on the post HoLEP SUI and functional outcomes in comparison to conventional (non-EAR) HoLEP. METHODS: Following PRISMA guidelines, a comprehensive literature search was performed until February 2026. The Se

Prophylactic Midurethral Slings at the Time of Pelvic Organ Prolapse Repair.

IMPORTANCE: Prophylactic midurethral sling (MUS) placement can prevent de novo stress urinary incontinence (SUI) after pelvic organ prolapse (POP) repair, but the concern about additional MUS-related adverse events remains. OBJECTIVE: The objective of this study was to compare postoperative outcomes between women with and without preoperative SUI after undergoing concomitant POP repair and MUS placement. STUDY DESIGN: This retrospective cohort study included women undergoing POP repair with concurrent MUS placement between 2017 and 2023. Patients in the prophylactic group did not have preope

Analysis of urinary tract infections as a predictor of obstruction in women with a history of urethral sling surgery.

INTRODUCTION: Midurethral sling (MUS) remains a widely used and effective treatment for stress urinary incontinence, although complications such as voiding dysfunction and urinary tract infections (UTIs) may occur. Prior research has shown that up to 60% of women with chronic lower urinary tract symptoms (LUTS) after MUS surgery meet objective criteria for bladder outlet obstruction (BOO). This exploratory study aimed to assess whether UTI burden is associated with the presence of BOO in women with chronic LUTS following MUS. Secondarily, UTI patterns before and after urethral sling excision w

Urinary incontinence among older adults in Saudi Arabia: prevalence, severity, and associated factors.

BACKGROUND: Urinary incontinence (UI) is a common yet under-recognized condition among older adults, particularly in outpatient settings, where symptoms are often underreported. Despite its significant impact on quality of life, data on the prevalence and clinical factors associated with UI in Saudi Arabian outpatient populations remain limited. METHOD: A cross-sectional study was conducted among 290 adults aged > = 60 years attending NGHA outpatient clinics in Riyadh and Dammam, Saudi Arabia. Participants were recruited consecutively during the study period. Urinary incontinence was assessed

Ectopic Ureteral Orifice: An Unusual Cause of Urinary Incontinence in an Adult.

Vaginal insertion of an ectopic ureter is a rare anomaly causing continuous incontinence despite normal voiding. We report a 30-year-old woman with lifelong urinary leakage that remained unrecognized until adulthood. Computed tomography urography revealed a left duplex collecting system with an ectopic ureter draining the upper renal pole into the vagina. She underwent successful ureterovesical reimplantation using the Politano-Leadbetter technique, achieving immediate and complete continence. This case emphasizes the need for meticulous evaluation in adults with chronic incontinence to allow

Pediatric Bladder Health Concerns: Does Where You Live Matter?

INTRODUCTION: The relationship between social determinants of health (SDOH) and bladder health (BH) concerns has not been explored. We evaluated the association between level of neighborhood disadvantage and resolution rates, loss to follow-up, and age at first visit for children with BH diagnoses. We hypothesized that children living in more disadvantaged areas would be older at first visit, more likely lost to follow-up, and less likely to experience symptom resolution at one y than children living in less disadvantaged areas. METHODS: We reviewed the records of all patients aged ≥3 y with
