Understanding Anti-Diuretic Hormone Dynamics in Off pump Coronary Artery Bypass Grafting (OPCABG) Patients: A Case Series.

OBJECTIVES: The conundrum of Polyuria witnessed post-cardiac surgery has been attributed to altered antidiuretic hormone (ADH) release secondary to both osmotic and non-osmotic stimulus. This transient DI-like state is more often seen in on-bypass cardiac surgeries but a few off-bypass occurrences have also been reported. A lot of conflicting evidence has accumulated trying to explain this altered water balance but a broader conceptual framework is lacking, which we have attempted to provide, with an objective of designing a future prospective study. DESIGN SETTING PARTICIPANTS INTERVENTIONS:

Catamenial diabetic ketoacidosis in a rural area of Kananga, Democratic Republic of the Congo: a case report.

Diabetic ketoacidosis (DKA) is a life-threatening acute complication of diabetes mellitus most commonly triggered by infection, metabolic stress, or inadequate insulin therapy. We report an unusual case of a young woman with type 1 diabetes who experienced recurrent episodes of ketoacidosis during the premenstrual period, suggesting a catamenial form of DKA. This case highlights the importance of considering hormonal fluctuations related to the menstrual cycle in diabetes management and in identifying factors of imbalance. The patient presented with abdominal pain, severe fatigue, and polyuria

[Clinical presentation and diagnostic procedures in canine hypoadrenocorticism].

ABSTRACT: Hypoadrenocorticism (HA) is a rare endocrinopathy in dogs that may occur in a primary form, characterized by a combined deficiency of glucocorticoids and mineralocorticoids or isolated glucocorticoid deficiency, or in a secondary form with an isolated glucocorticoid deficiency. In addition, iatrogenic HA may also occur. The primary form represents the most frequent type of the disease. Clinical signs are variable and often nonspecific, ranging from lethargy, inappetence, vomiting, and weight loss to polyuria/polydipsia or acute, life-threatening adrenal crises associated with hypovol

Primary hyperparathyroidism in a 4-month-old German Shepherd mixed breed dog.

A 4-month-old female-intact German Shepherd mixed breed dog was evaluated for polyuria, polydipsia, oral discomfort, tachypnea, and pyrexia. Blood testing documented total and ionized hypercalcemia with an increased parathyroid hormone concentration. Computed tomography identified bone abnormalities of the mandibule and mild, diffuse pulmonary changes. Ultrasonography of the neck documented one mildly enlarged parathyroid gland that was surgically removed. Histopathology indicated diffuse chief cell hyperplasia. The ionized calcium concentration normalized within 7 days postoperatively.

Hyperglycemic Hyperosmolar State As the Initial Presentation of Type 1 Diabetes Mellitus in an Adolescent: A Case Report.

Hyperglycemic hyperosmolar state (HHS) is a rare but life-threatening hyperglycemic emergency in children and adolescents. It is characterized by severe hyperglycemia, hyperosmolality, and dehydration, with absent or limited ketoacidosis. Although more commonly associated with type 2 diabetes mellitus, HHS may also occur as the initial presentation of type 1 diabetes mellitus in pediatric patients. We report the case of a previously healthy 13-year-old boy presenting with a three-week history of polydipsia and nocturnal polyuria. Initial evaluation revealed profound hyperglycemia (>1,000 mg/dL

A Case of Misdiagnosed Pediatric Congenital Nephrogenic Diabetes Insipidus due to AQP2 Variants.

BACKGROUND: Congenital nephrogenic diabetes insipidus (CNDI) is a rare disorder of renal water handling, often presenting in early infancy with severe polyuria and hypernatremia. Differentiating CNDI from central diabetes insipidus (CDI) remains challenging, and delayed identification may result in inappropriate therapy, leading to adverse growth and neurodevelopmental outcomes. CASE PRESENTATION: We present the case of an 8-month-old male infant with a history of suspected CDI since birth. He was treated with desmopressin (dDAVP) without clinical benefit. The absence of the posterior pituita

The Afferent Threshold Hypothesis: A Unified Neural Framework for Functional Bladder Capacity and Nocturia.

The Weiss nocturia indices have structured clinical thinking about nocturia for over 2 decades. By deriving simple ratios from the frequency-volume chart, they classify nocturia into nocturnal polyuria, reduced nocturnal bladder capacity, and mixed etiology. The arithmetic is sound. But the framework rests on an unexamined assumption: that the maximum voided volume, almost always the first morning void, represents "bladder capacity," and that the gap between this value and daytime voided volumes reflects normal physiology. We propose that what the frequency-volume chart measures during wakeful

Dorsal Pancreatic Agenesis Associated With New-Onset Diabetes Mellitus and Acute Pancreatitis in an 18-Year-Old Man: A Case Report.

Dorsal pancreatic agenesis (DPA) is a rare congenital disorder resulting from failure of development of the dorsal pancreatic bud. Complete DPA implies absence of the pancreatic neck, body, and tail and can be associated with both diabetes mellitus and pancreatitis. The patient was an 18-year-old man admitted with a history of epigastric pain, nausea, vomiting, polyuria, polydipsia, and unintentional weight loss of 4 kg over 8 weeks. The lab investigations included elevated serum glucose (412 mg/dL), urine ketones 3+, venous pH 7.31, bicarbonate 17.2 mEq/L, anion gap 14.8 mEq/L, lipase 486 U/L

Effect of Acupuncture on Urine Production and Storage in Prostate Cancer Survivors With Nocturia: Exploratory Analysis of a Randomized Clinical Trial.

BACKGROUND: Nocturia is multifactorial condition, but most treatments only target urine production or storage in isolation. Acupuncture has been shown to decrease nocturia, but its mechanisms remain unclear. This study explores the preliminary effects of acupuncture on urine production and storage to inform future mechanistic hypotheses. METHODS: We conducted a secondary analysis of 3-day frequency volume charts from a clinical trial in which prostate cancer survivors with nocturia were randomized to receive acupuncture or usual care (waitlist control [WLC]). Within-group posttreatment change

Disappearance of Posterior Pituitary Hyperintensity in Arginine Vasopressin Resistance: A Case Report.

The evaluation of arginine vasopressin resistance (AVP-R) and arginine vasopressin deficiency (AVP-D), respectively, previously referred to as nephrogenic and central diabetes insipidus, often includes brain magnetic resonance imaging (MRI). Its purpose is to assess pituitary structural changes and identify the physiological T1 hyperintensity of the posterior pituitary, present in most individuals. This hyperintensity reflects vasopressin storage, which shortens the T1 relaxation time. Its absence supports a diagnosis of AVP-D over AVP-R. A 13-year-old boy was referred to endocrinology in his

A standardized fraction of Cajanus scarabaeoides attenuates diabetic nephropathy via antiglycation, anti-inflammatory, and anti-fibrotic mechanisms.

Diabetic nephropathy is the critical microvascular complication of diabetes and a leading cause of end-stage renal disease. Its pathophysiology incriminates complex and interconnected mechanisms driven by hemodynamic and metabolic alterations, accompanied with glycation, inflammation, oxidative stress, and fibrosis. Targeting these cues through natural interventions may preserve kidney functions. The study aimed to investigate the renoprotective potential of a standardized ethyl acetate fraction of C. scarabaeoides (EAFCS), enriched with flavonoids including isoorientin, orientin, vitexin, iso

Diabetes Insipidus as a Rare Complication of Metastatic Breast Cancer With Pituitary Metastases.

Diabetes insipidus (DI) is a rare but significant and life-threatening complication of metastatic breast cancer, typically arising when cancer spreads to the pituitary gland. This condition results from impaired secretion or action of antidiuretic hormone (ADH) or arginine vasopressin (AVP), leading to symptoms including excessive urination and thirst. The posterior lobe of the pituitary and the infundibulum are most susceptible to metastatic involvement due to their direct arterial blood supply, making them more accessible to circulating tumor cells. This phenomenon explains why DI can be a m

Arginine vasopressin deficiency revealing a disseminated human chorionic gonadotropin-secreting germinoma.

Arginine vasopressin deficiency (AVP-D) is an uncommon disorder caused by deficient vasopressin production due to hypothalamic or pituitary gland dysfunction resulting in hypotonic polyuria, whose etiology oftentimes remains elusive. We present the case of a 20-year-old male patient evaluated for polyuria and polydipsia. Laboratory evaluation demonstrated hypernatremia, elevated serum osmolality, and inappropriately dilute urine with symptomatic improvement following initiation of desmopressin, consistent with AVP-D. Initial pituitary magnetic resonance imaging (MRI) showed a small enhancing l

Factors Affecting the Clinical Response to Desmopressin Treatment for Primary Monosymptomatic Nocturnal Enuresis in Children.

INTRODUCTION: Desmopressin is the first-line treatment option for primary monosymptomatic nocturnal enuresis (MNE), particularly when nocturnal polyuria is a key symptom. This study aimed to investigate the factors affecting clinical response to desmopressin and relapse status in children with primary MNE during follow-up. METHODS: Files of 576 paediatric patients presenting with primary MNE between September 2022 and March 2025, who commenced desmopressin treatment, were analysed retrospectively. The patients' demographic and clinical data were recorded. The clinical response to treatment wa

Clinical Determinants and Mortality Predictors of Acute Kidney Injury in Neonates: A Five-Year Retrospective Cohort Study.

OBJECTIVES: Acute kidney injury (AKI) is a frequent and clinically significant complication in critically ill neonates and is associated with increased morbidity and mortality. Although awareness of neonatal AKI has increased in recent years, data on mortality predictors and the trajectory of renal recovery remain limited, particularly in single-center cohorts. This study aimed to comprehensively evaluate the incidence, antenatal, perinatal, and postnatal risk factors, clinical characteristics, and short-term outcomes of neonates diagnosed with AKI in a neonatal intensive care unit, with a par

Inhibitory effects of honey-containing marinades on the production of heterocyclic amines (HCAs) in grilled meat: a review.

Achieving target blood pressure (BP) levels in clinical practice is one of the main challenges for physicians in the management of patients with hypertension. It is now recognised that the majority of patients will require at least two antihypertensive drugs to achieve optimal BP control; the use of combination therapy as first-line treatment is also increasing as BP goals of antihypertensive therapy become more ambitious. The fixed combination of zofenopril/hydrochlorothiazide (HCTZ) 30/12.5 mg/day is approved in Italy, France, Switzerland and Greece for the management of mild to moderate hyp
