Lithium is the most common and effective mood stabiliser in the management of bipolar disorder. Lithium-induced nephrogenic diabetes insipidus (NDI) is a well-recognised complication of long-term mood stabiliser therapy. However, management becomes exponentially more complex when therapeutic options, such as thiazides or amiloride, are contraindicated due to advanced chronic kidney disease (CKD) or electrolyte instability. We are reporting the case of a man in his 60s with a 40-year history of lithium use who presented with acute-on-chronic kidney injury (AKI) and severe hypernatraemia (164 mm

BACKGROUND: Absence of the posterior pituitary bright spot (PPBS) on T1-weighted MRI is often observed in chronic post-traumatic arginine vasopressin deficiency (AVP-D), but its association with permanent AVP-D remains unclear.
CASE PRESENTATION: We report five patients with chronic-phase post-traumatic AVP-D (156-965 days post-injury) who were transferred to our rehabilitation center. All patients had received long-term desmopressin therapy and exhibited persistent absence of the PPBS on T1-weighted MRI at admission. Despite these findings, desmopressin therapy was successfully tapered and d

Non-parathyroid hormone (PTH)-mediated hypercalcemia has diverse etiologies, including granulomatous disorders such as sarcoidosis, in which extrarenal 1-α-hydroxylase activity leads to excess production of 1,25-dihydroxyvitamin D (1,25(OH)₂D). We report a rare case of sarcoidosis presenting as isolated hypercalcemia without pulmonary involvement, complicated by initially normal 1,25(OH)₂D levels and a false-negative core biopsy. A 44-year-old man with hypertension and type 2 diabetes presented with malaise, polyuria, and polydipsia. Laboratory evaluation showed severe hypercalcemia with suppr

BACKGROUND: Neuroblastoma is the most common extracranial solid tumours in children, but catecholamine-secreting variants are rare. Unlike pheochromocytoma, routine preoperative α-blockade and extended perioperative monitoring are not recommended for asymptomatic neuroblastoma, as clinically significant catecholamine secretion is considered exceptional. We present a fatal case of hemodynamic collapse during general anaesthesia for magnetic resonance imaging in an infant with undiagnosed catecholamine-secreting neuroblastoma. Subtle preoperative symptoms were present, the risk was underestimate

Drug reaction with eosinophilia and systemic symptoms (DRESS), also known as drug-induced hypersensitivity syndrome (DIHS), is a severe, multiorgan, delayed hypersensitivity reaction with significant morbidity. Diagnosis is often delayed due to its mimicry of other conditions. We present the case of a 69-year-old Hispanic male with hypertension and stage two chronic kidney disease who developed trigeminal neuralgia and was started on carbamazepine. Approximately seven weeks later, he presented with a generalized rash, angioedema, and mucosal swelling, initially attributed to his concurrent lis

Central diabetes insipidus (CDI) is a rare condition characterized by arginine vasopressin deficiency, leading to persistent polyuria and polydipsia. Although it can present at any age, idiopathic CDI in young adults is uncommon and diagnostically challenging. Inflammatory conditions such as infundibuloneurohypophysitis must be considered, particularly when magnetic resonance imaging (MRI) reveals a thickened pituitary stalk and absence of the posterior pituitary bright spot. Early recognition and differentiation from diabetes mellitus, nephrogenic diabetes insipidus, and primary polydipsia ar

A preterm male neonate born at 28 + 2 weeks gestation, with a birth weight of 1520 g (large for gestational age) and antenatal polyhydramnios, presented with prematurity, early-onset sepsis, and septic shock. Between days 4 and 9 of life, the neonate developed marked polyuria, hyponatremia, and compensated metabolic acidosis with normal to transiently elevated serum potassium levels, initially obscuring the diagnosis of a tubular disorder. Subsequently, the biochemical profile evolved to hypokalemia, metabolic alkalosis, and hypercalciuria, consistent with Bartter syndrome. The clinical course

BACKGROUND: Pembrolizumab-induced type 1 diabetes mellitus (T1DM) is an uncommon but potentially life-threatening immune-related adverse event. Its clinical course is often abrupt, and evidence regarding its presentation, management, and long-term outcome remains largely derived from isolated case reports.
METHODS: Published case reports and case series describing pembrolizumab-induced T1DM were retrieved from PubMed, Embase, Web of Science, CNKI, and Wanfang Data from database inception to June 30, 2026, and retrospectively reviewed. Patient demographics, pembrolizumab exposure, clinical man

INTRODUCTION: Autism Spectrum Disorder (ASD) is a neurodevelopmental condition marked by social and behavioural impairments. While behavioural therapies have progressed, pharmacological options for core symptoms remain inadequate. Bumetanide, an NKCC1 antagonist, has shown potential benefits in ASD. This systematic review and meta-analysis evaluated its efficacy and safety in children and adolescents.
METHODS: We searched PubMed, ScienceDirect, Scopus, and ClinicalTrials.gov until April 2024 for RCTs and observational studies involving patients with ASD aged ≤18 years. Primary outcome measure

BACKGROUND: Gitelman syndrome (GS), an autosomal-recessive salt-losing tubulopathy caused by biallelic SLC12A3 variants, classically presents with normo- or hypotension. Hypertension is therefore considered exceptional and may delay diagnosis.
CASE DESCRIPTION: A 19-year-old man was admitted for progressive polyuria, constipation, proximal muscle weakness and fatigue lasting 6 weeks. Laboratory tests showed hypokalaemia (K+ 2.4 mmol/L), hypomagnesaemia (Mg2+ 0.48 mmol/L), hypochloraemia (Cl- 96 mmol/L), hypocalciuria (Ca2+/Cr 0.07 mmol/mmoL) and hyper-reninaemia (plasma renin 52 ng/mL/h; aldo

INTRODUCTION: Pheochromocytoma is a well-established cause of secondary hypertension and can lead to dysglycemia. However, its initial presentation as atypical diabetes, characterized by severe glycemic instability, is uncommon and poses a significant diagnostic challenge.
CASE PRESENTATION: A 49-year-old non-obese Chinese woman presented with a 15-month history of progressive polyuria and polydipsia. Initially diagnosed with type 2 diabetes, she developed significant glycemic variability with recurrent hypoglycemia and profound hyperglycemia, alongside a rapid decline in β-cell function. Dur

BACKGROUND: Nocturnal polyuria (NP), characterized by excessive nighttime urine production, is a common non-motor symptom in synucleinopathies, particularly in the presence of cardiovascular autonomic failure (AF). NP contributes to sleep fragmentation, increased fall risk, and exacerbation of morning orthostatic hypotension. Diagnosis requires a comprehensive assessment of lower urinary tract symptoms, comorbidities and medication use.
METHODS: We conducted a scoping review of the existing literature on PubMed up to December 2024 regarding NP associated with synucleinopathies and AF.

BACKGROUND: Hypercalcemic crisis from a functioning parathyroid adenoma (PA) is exceptionally rare in prepubertal children and typically presents with a non-specific complaint such as polyuria or lethargy. We report a 7-year-old girl who was urgently referred for acute, colicky abdominal pain and repeated vomiting, which clinically mimicked acute appendicitis. During the work-up, extensive metabolic tests incidentally revealed severe hypercalcemia and elevated intact parathyroid hormone (PTH), which led to the diagnosis of PA-induced hypercalcemic crisis. Her initial manifestation was an acute

Introduction Type 2 diabetes mellitus (T2DM) often requires combination therapy when metformin alone fails to achieve adequate glycemic control. Dapagliflozin, a sodium-glucose cotransporter-2 (SGLT2) inhibitor, and glimepiride, a sulfonylurea, are commonly prescribed as add-on therapies. This study compared their efficacy and safety in patients with inadequately controlled T2DM. Methods A nonrandomized prospective observational comparative study was conducted at a tertiary care teaching hospital between April and November 2024. A total of 170 adults with inadequately controlled T2DM receiving

INTRODUCTION: Surgical revascularization represents an effective treatment for moyamoya disease (MMD); however, it is associated with postoperative complications such as stroke and hyperperfusion syndrome. Intraoperative polyuria and electrolyte disturbances resembling diabetes insipidus (DI) are frequently observed, yet their clinical significance remains unclear. This study aimed to identify potential risk factors for postoperative complications and to evaluate the predictive value of intraoperative DI in individuals undergoing revascularization for MMD.
METHODS: A retrospective analysis wa

BACKGROUND: Postoperative diabetes insipidus (DI) is rare, involving the hypothalamic-pituitary axis following cervical spine surgery.
CASE DESCRIPTION: A 51-year-old male underwent an elective C3-C6 decompressive laminectomy for cervical spinal canal stenosis. Three hours postoperatively, he developed abrupt massive polyuria (1300-1800 mL/h) with polydipsia (specific gravity 1.002), consistent with DI. Treatment with fluid replacement, desmopressin, and vasopressin resulted in clinical improvement. The brain MR revealed a normal pituitary gland and stalk, whereas histology from the laminecto

Antineutrophil cytoplasmic autoantibody (ANCA)-associated vasculitis (AAV) is a relatively uncommon autoimmune disease, predominantly causing kidney or lung injury. Pituitary dysfunction is exceedingly rare in microscopic polyangiitis (MPA). Rupture or hemorrhage of renal artery aneurysms is also scarcely reported in AAV, especially in MPA. In this study, we describe a 68-year-old woman who presented with shortness of breath at rest, polydipsia, and polyuria. In addition to an increased p-ANCA titer (1:10) and the presence of anti-myeloperoxidase antibodies, T1-weighted magnetic resonance imag

Langerhans cell histiocytosis (LCH) is a rare clonal myeloid disorder with a broad clinical spectrum ranging from isolated lesions to multisystem disease. We report the case of a two-year-five-month-old boy who presented with a six-month history of treatment-refractory bilateral otorrhea, progressive mandibular swelling with premature tooth loss, seborrheic skin lesions, and weight loss. Further evaluation revealed severe polyuria-polydipsia suggestive of central diabetes insipidus. Imaging demonstrated multifocal craniofacial osteolytic lesions with hypothalamic-pituitary involvement. Histopa

Lithium remains a cornerstone mood stabilizer for bipolar disorder, with strong evidence for relapse prevention and a unique association with reduced suicide risk. Its benefit is counterbalanced by renal adverse effects, ranging from impaired urinary concentrating capacity and nephrogenic diabetes insipidus (NDI) to chronic tubulointerstitial nephropathy with progressive loss of glomerular filtration rate in a minority of long-term users. Mechanistically, lithium enters collecting duct principal cells via the epithelial sodium channel, disrupts vasopressin-regulated water handling by downregul
