Background Polyuria-polydipsia syndrome encompasses a heterogeneous group of disorders, including central diabetes insipidus (CDI), nephrogenic diabetes insipidus (NDI), and primary polydipsia. Despite presenting with similar clinical manifestations, these conditions differ markedly in their underlying pathophysiology, prognosis, and management. Distinguishing between them remains clinically challenging, particularly in patients with partial disease and in resource-limited settings where newer diagnostic biomarkers such as copeptin are not routinely available. This study highlights the diagnos

Arginine vasopressin (AVP) deficiency, previously termed central diabetes insipidus, arises from impaired AVP synthesis or secretion by the hypothalamus and/or the posterior pituitary gland and presents with hypotonic polyuria and polydipsia. To differentiate AVP deficiency from AVP resistance and primary polydipsia, a stepwise diagnostic workup is required. In recent years, copeptin, as a reliable surrogate marker of AVP secretion, has been incorporated into diagnostic algorithms, and copeptin-based stimulation tests have substantially improved diagnostic accuracy. Once AVP deficiency has bee

Introduction: the sequential transition from central diabetes insipidus (CDI) to the syndrome of inappropriate antidiuretic hormone secretion (SIADH) is an exceptionally rare condition and represents a major diagnostic and therapeutic challenge. Case report: we report the case of a 47-year-old man with Burkitt lymphoma involving the hypothalamic-pituitary region who initially presented with polyuria, polydipsia, and hypernatremia. CDI was confirmed by biochemical findings and a positive response to desmopressin. Following desmopressin treatment and intensive chemotherapy, rapid tumor regressio

BACKGROUND: Nephrogenic diabetes insipidus (NDI) is characterized by impaired renal responsiveness to arginine vasopressin, causing defective urinary concentration and large volumes of dilute urine. Long-term lithium exposure is an important acquired cause. Perioperative fasting, restricted free-water intake, and ongoing fluid losses may unmask compensated lithium-associated urinary concentrating dysfunction and precipitate severe hypernatremia.
CASE PRESENTATION: A 61-year-old man underwent laparoscopic resection of a pancreatic tail lesion. His history included schizophrenia, type 2 diabete

Small cell lung carcinoma (SCLC) is an aggressive neuroendocrine malignancy commonly associated with paraneoplastic syndromes, most often the syndrome of inappropriate antidiuretic hormone secretion (SIADH). Central diabetes insipidus (CDI) is a rare manifestation and may result from hypothalamic-pituitary involvement by metastatic disease. A 53-year-old man with a significant smoking history presented with abdominal pain and was found to have a right perihilar lung mass with extensive metastatic disease. Liver biopsy confirmed SCLC. During hospitalization, he developed acute polyuria with wor

INTRODUCTION: Arginine vasopressin resistance (AVP-R), previously called nephrogenic diabetes insipidus is a rare inherited disorder characterized by excessive urine output (often over 10 l/d) and a risk of life-threatening dehydration, resulting from kidney collecting duct insensitivity to arginine vasopressin. Currently, no curative treatment exists. Preclinical studies suggest that repurposing of anti-fungal fluconazole enhances AQP2-mediated water transport. Here, we present results from a clinical intervention study evaluating the effect of fluconazole on 24-hour urine output and urine co

BACKGROUND: Nephrogenic diabetes insipidus (NDI), one of the common complications of chronic administration of lithium, is characterized by severe urine concentrating defects. It is associated with aberrant regulation of aquaporin-2 (AQP2) proteins as well as endoplasmic reticulum (ER) stress. Therefore, we evaluated renoprotective effects of Maresin-1 (MaR-1) and its associated molecular mechanisms in Li-induced NDI.
METHODS: forty-eight adult male Wistar albino rats were randomly distributed into four equal groups: control, MaR-1 control (40 ug/kg/day intraperitoneally), NDI (LiCl; 40 mmol/

Hypernatremia due to polyuria following cardiac surgery is rare. We report a case that required to identify the cause of massive polyuria. A 50-year-old male underwent aortic arch replacement for acute aortic dissection. Early postoperatively, he developed marked polyuria [over 3,000 ml within 6 hours after intensive care unit (ICU) admission, over 15,000 ml within 24 hours] and hypernatremia (peak value 187 mEq/l). We first assumed vasopressin deficiency due to cardiopulmonary bypass, and desmopressin nasal spray was somewhat effective. Continuous vasopressin infusion combined with massive 5

A 7-year-old, 3.44-kg, spayed female Poodle was referred for acute kidney injury following firocoxib administration. Initial diagnostics revealed severe uraemia, oliguria and elevated C-reactive protein levels. Despite fluid resuscitation and diuretic therapy, oliguria persisted, necessitating haemodialysis. On Day 2, post-haemodialysis, the patient developed progressive pulmonary infiltration. The acute onset of tachypnoea, in the absence of cardiac disease or fluid overload, suggested noncardiogenic pulmonary oedema. Differential diagnoses included uraemic pneumonitis, acute respiratory dist

DLG4-related synaptopathy is a rare neurodevelopmental condition caused by pathogenic variants in the DLG4 gene, which encodes the postsynaptic density protein 95 (PSD-95). The disorder is characterized by a broad clinical spectrum that may include intellectual disability, autism spectrum disorder (ASD), epilepsy, hypotonia, sleep disturbances, and marfanoid manifestations. Most reported cases arise from de novo variants, although autosomal dominant inheritance with variable phenotypic expressivity has also been described. This report describes a 21-year-old man presenting with chronic musculo

BACKGROUND: Urinary incontinence (UI) affects up to one-third of men aged ≥65 years, yet evidence to guide the selection of targets for self-management interventions remains limited. Although many contributing risk factors are potentially modifiable, little is known about which are considered clinically important by experts and are also viewed by older men as feasible and worthwhile to address. This study combined expert consensus with older men's perspectives to identify candidate priorities for future self-management intervention development.
METHODS: We conducted a sequential multi-methods

Apparent mineralocorticoid excess (AME) is a rare autosomal recessive disorder caused by loss-of-function mutations in HSD11B2, leading to impaired conversion of cortisol to cortisone and inappropriate mineralocorticoid receptor activation. Early recognition is critical, as untreated disease results in progressive cardiovascular and renal complications. We describe a 2-year-old boy with severe early-onset hypertension, polyuria, polydipsia, abdominal distension, and growth retardation. Biochemistry revealed hypokalemic metabolic alkalosis with suppressed renin and low aldosterone. Renal ultras

Euglycemic diabetic ketoacidosis (euDKA) is an increasingly recognized complication associated with SGLT2 inhibitors. Because serum glucose levels may remain normal or only mildly elevated, diagnosis can be delayed, potentially increasing morbidity. We present the case of a 51-year-old male with long-standing poorly controlled type 2 diabetes mellitus who developed euDKA shortly after initiation of empagliflozin/metformin therapy. The patient presented with generalized weakness, gastrointestinal symptoms, reduced oral intake, polyuria, polydipsia, and weight loss. Initial laboratory evaluation

UNLABELLED: Dexmedetomidine is a highly selective α2-adrenergic agonist widely used for sedation in the intensive care unit (ICU) and is generally considered well tolerated, with hypotension and bradycardia being the most commonly reported adverse effects. Rarely, dexmedetomidine has been associated with polyuria resembling a diabetes insipidus-like syndrome, particularly during prolonged infusions. We report the case of a 53-year-old male admitted to the ICU for severe alcohol withdrawal who developed profound polyuria during a 5-day dexmedetomidine infusion. Urine output progressively increa

BACKGROUND Rabson-Mendenhall syndrome (RMS) is an extremely rare autosomal recessive disorder caused by pathogenic variants in the insulin receptor gene, leading to severe insulin resistance and compensatory hyperinsulinemia. Classical features include acanthosis nigricans, non-obese or underweight body habitus, hirsutism, dental abnormalities, dysmorphic features, and variable growth abnormalities. Early recognition may be difficult when the initial presentation is dominated by non-specific symptoms rather than classical metabolic complaints. CASE REPORT A 10-year-old Saudi girl presented to

Diabetes insipidus (DI) is a heterogeneous disorder characterised by polyuria and polydipsia due to impaired arginine vasopressin (AVP) secretion or action. We describe a 36-year-old pregnant woman who presented at 28 weeks' gestation with weakness, severe hypokalaemia from renal potassium wasting, hypernatraemia and polyuria with dilute urine. Vasopressinase-mediated DI was suspected; there was no response to AVP, but desmopressin (dDAVP) produced a rapid clinical improvement, confirming the diagnosis. However, postpartum recurrence of symptoms prompted further evaluation, with imaging sugges

The post-covid-syndrome (PCS) is characterized by severe persisting fatigue and other long haul symptoms following COVID-19 infection. Disease-driving mechanisms are elusive, biomarkers are missing, treatment options few, and yet millions of people worldwide are affected. We hypothesized that the common complaint of polydipsia/polyuria and its attendant effect on osmolality can be quantified and identified as an unrecognized clinical feature of the PCS. We conducted a clinical case-series of 10 consecutive patients with PCS (5 females, 5 males; mean age 44 ± 14 y) who completed questionnaires

Type 1 diabetes (T1D) is an autoimmune disease characterized by pancreatic β-cell destruction and absolute insulin deficiency. In pediatric patients, T1D typically presents with classic symptoms such as polydipsia, polyuria, weight loss, or diabetic ketoacidosis (DKA). Although gastrointestinal symptoms may occur at disease onset, paralytic ileus as an initial manifestation of T1D, particularly in the absence of DKA, is extremely uncommon. A 15-year-old boy presented with severe abdominal pain and vomiting and was diagnosed with paralytic ileus based on abdominal radiographic findings. Mild hy
