Clinical and diagnostic complexities of adipsic arginine vasopressin deficiency.

Adipsic arginine vasopressin deficiency (AAVP-D) is a rare hypothalamic disorder, with inadequate vasopressin production and impaired thirst, resulting in sodium and water dysregulation. It poses multiple challenges, including increased morbidity and mortality. We present a 24-year-old female who developed confusion after neurosurgical debulking of a central neurocytoma. Investigations revealed hypernatremia alongside raised plasma osmolality, low urine osmolality, polyuria, and adipsia. Transient AAVP-D was suspected; it was successfully treated with intravenous desmopressin, which resolved p

Bardet-Biedl syndrome in two sibling pairs: a case series.

BACKGROUND: Bardet-Biedl syndrome is a rare multisystem ciliopathy defined by retinal dystrophy, obesity, polydactyly, hypogonadism, and renal anomalies, often accompanied by neurodevelopmental and behavioral issues. Diagnosis relies heavily on clinical features, particularly in resource-limited settings where genetic testing is unavailable. CASE PRESENTATION: We describe two sibling pairs, all of South Asian, Pakistani ethnicity, clinically diagnosed with Bardet-Biedl syndrome using contemporary diagnostic frameworks. The first pair, two male brothers aged 17 and 21 years, came from a consan

Lower Urinary Tract Symptoms as Diagnostic Clues in Parkinsonism: A Practical Framework for Neurologists.

BACKGROUND: Lower urinary tract symptoms (LUTS) are among the most prevalent nonmotor complaints across the parkinsonian spectrum, yet they remain underutilized as diagnostic and management signals in neurology practice. Although prior reviews have characterized disease-specific patterns of urinary dysfunction, and recent guidelines address neurogenic LUTS broadly, no prior framework has explicitly focused on how the practicing neurologist can interpret LUTS as part of the diagnostic workup and integrate them into disease-specific management across parkinsonian disorders. METHODS: The aim of

Multiple endocrine adverse reactions and pituitary axis dysfunction induced by PD-1 immunotherapy in an esophageal cancer patient: Case report.

RATIONALE: Immune checkpoint inhibitors, particularly programmed cell death protein 1 (PD-1) blockers, have transformed cancer therapy but can induce immune-related endocrine toxicities. Severe involvement of multiple endocrine axes, including dual pituitary dysfunction, is rare and challenging to diagnose due to nonspecific clinical features. Highlighting such cases can improve early recognition and management. PATIENT CONCERNS: A 56-year-old man with esophageal cancer developed fatigue, polyuria, polydipsia, dizziness, and severe hyponatremia after treatment with tislelizumab, a PD-1 inhibi

Perirenal lipoma in a 14-year-old spayed female dog: A rare diagnosis.

Adipose tumors are common in older dogs. Most of these tumors present as benign subcutaneous lipomas; however, intracavitary lipomas, particularly those within the retroperitoneal space, are rarely documented in veterinary medicine. This case report describes a 14-year-old spayed mixed-breed dog with a history of recurrent urinary tract infections, hind-limb weakness, and prior subcutaneous lipomas that was presented because of polyuria, polydipsia, and behavioral changes. Abdominal ultrasound and subsequent computed tomography revealed a left-sided perirenal mass with fat, fluid, and soft-tis

Diabetic Ketoacidosis Complicated by Cerebral and Pulmonary Oedema in an Adult.

Diabetic ketoacidosis (DKA) is a life-threatening metabolic emergency. While cerebral and pulmonary oedema are recognised complications in paediatric populations, they are exceptionally rare and poorly characterised in adults. This case report describes an adult patient with severe DKA who developed both cerebral and pulmonary oedema during treatment. A previously healthy 36-year-old man presented with a two-week history of polyuria and polydipsia and progressive dyspnoea. He was alert and tachycardic and exhibited Kussmaul respirations. Initial investigations confirmed severe DKA: serum gluco

Pediatric hemophagocytic lymphohistiocytosis with predominant CNS involvement: MRI features and differential diagnosis.

BACKGROUND: Hemophagocytic lymphohistiocytosis (HLH) is a rare, life-threatening hyperinflammatory disorder resulting from immune dysregulation that leads to uncontrolled macrophage activation. The diagnosis of HLH is often challenging, as its clinical manifestations can mimic various systemic conditions, including sepsis, autoimmune disorders, and malignancies. This overlap, combined with the rapid progression of the disease, may delay treatment and increase morbidity and mortality. Neurological involvement is particularly frequent in pediatric cases. Neuroimaging plays a critical role in rai

Pituitary Stalk Enlargement With Panhypopituitarism and Arginine Vasopressin Deficiency in Lung Adenocarcinoma: Metastatic Infundibular Disease or Paraneoplastic Hypophysitis?

Paraneoplastic endocrine syndromes are well-recognised complications of lung malignancies, specifically small-cell lung cancer. The majority of patients with panhypopituitarism associated with lung cancer result from pituitary metastasis. In those with an enlarged pituitary stalk without obvious pituitary metastasis presenting with a combination of panhypopituitarism and arginine vasopressin deficiency (AVP-D), metastatic pituitary stalk infiltration should be considered rather than a paraneoplastic immune-mediated hypophysitis. We report a case of a 67-year-old gentleman with newly diagnosed

Neurosarcoidosis presenting with panhypopituitarism: diagnostic and therapeutic challenges: a case report.

BACKGROUND: Neurosarcoidosis is a rare but serious manifestation of sarcoidosis involving the central nervous system. When it affects the hypothalamic-pituitary axis, it can result in panhypopituitarism and diabetes insipidus due to disruption of anterior and posterior pituitary hormone regulation. Early recognition is critical, as delayed diagnosis may lead to life-threatening complications. CASE PRESENTATION: We report the case of a 42-year-old White man with a 12-year history of sarcoidosis who presented with abdominal pain, hypotension, hypothermia, hypernatremia, polyuria, and polydipsia

Pituitary involvement in granulomatosis with polyangiitis: a diagnostic and therapeutic challenge.

Granulomatosis with polyangiitis (GPA) is a systemic necrotizing vasculitis associated with antineutrophil cytoplasmic antibodies. Although pituitary involvement is rare, it represents a clinically relevant manifestation that may present as a sellar mass mimicking a pituitary neuroendocrine tumour, leading to diagnostic delay and inappropriate management. We report the case of a male patient who initially presented at 24 years of age with recurrent epistaxis and nasal crusting, with histopathological evidence of chronic granulomatous inflammation of undetermined aetiology. Four years later, he

Metastatic large cell neuroendocrine carcinoma: a rare cause of hypopituitarism.

Pituitary metastases from pulmonary large cell neuroendocrine carcinoma (LCNEC) are exceedingly rare but should be considered in patients presenting with arginine vasopressin deficiency and panhypopituitarism in the context of known or suspected metastatic lung cancer. Early recognition and prompt initiation of hormonal replacement therapy are essential to prevent life-threatening endocrine crises and to improve symptom burden. We describe a case of a 43-year-old male who presented with dyspnea, weight loss, and weakness who was found to have metastatic LCNEC involving the liver. Endocrine eva

Peritoneal dialysis-associated peritonitis in two patients with Bartter syndrome and end-stage kidney disease: a case report.

Bartter syndrome is a rare inherited salt-wasting disorder affecting the thick ascending limb of the loop of Henle. It is characterized by hypokalemic metabolic alkalosis, polyuria, and secondary normotensive hyperaldosteronism. A minority of patients may progress to end-stage kidney disease while maintaining polyuria and normokalemia. Peritoneal dialysis may represent a suitable option for these patients when kidney transplantation is not available. However, due to its rarity, there are no data on PD-related outcomes. Here we report two cases of PD treatment with BS from two Italian referral

Intrasellar symptomatic salivary gland choristoma: a case report and literature review.

BACKGROUND: Symptomatic ectopic salivary tissue in the sellar region is extremely rare. A few cases were reported in the literature. Its radiological features overlap with those of other sellar lesions, making a definitive preoperative diagnosis difficult. This case report presents the youngest treated symptomatic patient who was diagnosed with ectopic salivary tissue in the sellar region. CASE DESCRIPTION: A 6-year-old symptomatic girl presented with chronic headache, polyuria, polydipsia, and short stature. The hormonal assay revealed hypopituitarism with low growth hormone. Successful response to cabergoline treatment in a non-diabetic cat with hypersomatotropism and pituitary macroadenoma.

CASE SUMMARY: A 12-year-old male neutered domestic shorthair cat was referred with severe polyphagia, polydipsia, polyuria, weight gain and signs of neurologic disease, including blindness, head tilt, and ataxia. Magnetic resonance imaging (MRI) identified a suprasellar mass consistent with a pituitary macroadenoma. Hypersomatotropism (HST) was diagnosed based on phenotypic changes compatible with acromegaly and an abnormally high serum insulin-like growth factor 1 (IGF-1) concentration (1253 ng/ml). Medical treatment with cabergoline monotherapy (10 μg/kg PO q24h) was prescribed. Progressive

Lupus‑Associated Hypophysitis: A Narrative Systematic Review of Clinical, Radiological and Endocrine Features.

BACKGROUND: Lupus-associated hypophysitis is a rare but clinically important manifestation of systemic lupus erythematosus (SLE), characterised by heterogeneous pituitary dysfunction and inflammatory sellar changes that may mimic other pituitary disorders. Evidence is limited to isolated reports. We aimed to summarise clinical, radiological, endocrine features, management, and outcomes. METHODS: A narrative systematic review of MEDLINE, Embase, Web of Science, and Google Scholar to 10 November 2025. Reports were included if accepted SLE criteria were met and hypophysitis or hypothalamic-pitui

Exploring Thyrotoxic Hypercalcaemia Through Two Cases: A Myriad of Uncertainties.

Hypercalcaemia is a recognised complication of thyrotoxicosis usually related to increased bone resorption. However, coexistent hyperphosphataemia is uncommon and may create diagnostic uncertainty. It is postulated that serum calcium and phosphate levels rise due to high bone turnover and increased renal reabsorption of phosphate in response to excess thyroxine. Hypercalcaemia may also alter the clinical presentation of thyrotoxicosis, making both diagnosis and management challenging. We report two patients with this unusual biochemical presentation. The first patient was a 69-year-old woman w

Mikulicz Disease Revealing IgG4-Related Tubulointerstitial Nephritis: A Case Report and Literature Review.

Background and Clinical Significance: IgG4-related disease (IgG4-RD) is a chronic fibroinflammatory, immune-mediated multisystem disorder that can mimic neoplastic, infectious, or autoimmune conditions. Among its head-and-neck manifestations, IgG4-related dacryoadenitis and sialadenitis, historically referred to as Mikulicz disease, should be distinguished from the classical Mikulicz syndrome, which describes secondary lacrimal and salivary gland enlargement due to other systemic disorders. Renal involvement, most commonly in the form of IgG4-related tubulointerstitial nephritis (IgG4-TIN), is

A Case of Dual Endocrinopathies Induced by Pembrolizumab Following Treatment for Non-small Cell Lung Cancer: Type 1 Diabetes and Hyperthyroidism/Autoimmune Thyroiditis.

Pembrolizumab, a programmed cell death protein 1 (PD-1) immune checkpoint inhibitor, is commonly used in the treatment of squamous cell carcinoma, offering improved survival and tumor response. However, it can rarely lead to immune-related adverse events (irAEs), some of which may be significant. We report a 62-year-old male with squamous cell carcinoma of the lung, a 40-pack-year smoking history, prediabetes, and coronary artery disease, who presented with polydipsia, polyuria, and weakness. He was diagnosed with sudden-onset diabetic ketoacidosis. Laboratory workup revealed negative glutamic
