Case Report of Acute Severe Hyponatremia Induced by Desmopressin Administration During Chemotherapy.

Patients undergoing chemotherapy are prone to developing hyponatremia due to insufficient oral intake because of nausea and vomiting, administration of large volumes of intravenous fluids, syndrome of inappropriate antidiuretic hormone secretion induced by chemotherapeutic agents, adrenal insufficiency, and drug interactions. Severe hyponatremia during chemotherapy interferes with continuation of cancer treatment. In addition, it may be difficult to distinguish hyponatremia from chemotherapy-related adverse effects because of symptom similarity. This report presented a case of desmopressin-ind

Sacubitril/valsartan improves nocturia in hypertensive patients: a pilot study.

BACKGROUND: Nocturia is associated with falls, reduced quality of life (QOL), and sleep disturbances, yet remains underdiagnosed and undertreated. Its multifactorial etiology includes reduced bladder capacity, nocturnal polyuria (NP), and comorbid conditions like hypertension. While antihypertensive therapies have been linked to nocturia, the impact of newer agents remains unclear. This study investigated whether sacubitril/valsartan (Sac/Val), a novel angiotensin receptor-neprilysin inhibitor used for refractory hypertension, could improve nocturia. Sac/Val's unique natriuretic and diuretic e

Case Report of Overlap of Diabetic Ketoacidosis and Hyperosmolar Hyperglycemic State in a 5-Year-Old with New-Onset Type 1 Diabetes Mellitus: Diagnostic and Management Considerations.

Background and Clinical Significance: Overlap of diabetic ketoacidosis (DKA) and hyperosmolar hyperglycemic state (HHS) in children is a rare but life-threatening metabolic emergency. The coexistence of hyperosmolality and ketoacidosis increases neurologic vulnerability and complicates fluid and insulin management. Early identification and osmolality-guided therapy are essential to prevent cerebral edema and other complications. This case describes a 5-year-old boy with new-onset type 1 diabetes mellitus (T1D) presenting with DKA/HHS overlap two weeks after influenza vaccination-an unusual tem

Dietary Caffeine, Cold Exposure, and the Estrogen-TRPM8 Axis: A Nutri-Environmental Model for Lower Urinary Tract Symptoms in the Menopause Transition: A Narrative Review.

Background/Objectives: Lower urinary tract symptoms (LUTSs), particularly nocturia and urgency, often intensify during the menopause transition and may worsen with caffeine intake and cold exposure. This review aims to synthesize evidence relevant to a hypothesized caffeine-cold interaction in transitional menopause, focusing on water homeostasis and the estrogen-transient receptor potential melastatin 8 (TRPM8) cold-sensory axis, and to propose potentially actionable, nutrition-centered intervention candidates for future testing. Methods: Structured narrative review of PubMed, Embase, Web of

Enhancing Primary Care Recognition of Type 1 Diabetes in Children: Diagnostic Challenges and Strategies to Prevent Diabetic Ketoacidosis.

Timely recognition of type 1 diabetes (T1D) in children and adolescents is crucial to prevent acute complications such as diabetic ketoacidosis (DKA). This narrative review examines the pathophysiology, clinical presentation, and diagnostic challenges of childhood T1D, including the young age of onset, clinician training gaps, and overlapping symptomatology between T1D and other common pediatric illnesses. Despite increased awareness, a significant proportion of children still present with DKA at diagnosis due to misinterpretation of symptoms, such as polydipsia, polyuria, and weight loss. Thi

DUSP1 Attenuates Renal Injury in Diabetic Nephropathy by Modulating Ferroptosis: Evidence From Animal Experiments.

OBJECTIVE: Emerging evidence suggests that ferroptosis contributes significantly to the progression of diabetic nephropathy (DN). This study aimed to explore the potential association between dual specificity phosphatase 1 (DUSP1) and ferroptosis in a streptozotocin-induced DN rat model. METHODS: We analyzed microarray datasets (GSE30122 and GSE96804) from the gene expression omnibus (GEO) database to identify ferroptosis-related differentially expressed genes (FDEGs), with particular focus on DUSP1. Experimental validation was performed using 45 specific pathogen-free Sprague-Dawley rats: 15

Mediterranean diet preserves renal mitochondrial homeostasis and attenuates early diabetic kidney injury in db/db mice.

BACKGROUND: Chronic hyperglycemia, oxidative stress, and mitochondrial dysfunction are central drivers of renal structural and functional alterations associated with metabolic disease and accelerated tissue aging. In type 2 diabetes mellitus (T2DM), these mechanisms contribute to early kidney injury and progressive decline in renal resilience. Dietary interventions, including the Mediterranean diet (MD), have been proposed as complementary strategies to counteract metabolic stress, yet mechanistic evidence at the renal mitochondrial level remains limited. METHODS: Male db/db mice, a model of

Age- and Severity-Stratified Associations Among Polysomnographic Parameters, Lower Urinary Tract Symptoms, and Hormonal Markers in Men with Obstructive Sleep Apnea: A Cross-Sectional Study.

BACKGROUND: Obstructive sleep apnea (OSA) is associated with lower urinary tract symptoms (LUTS), particularly nocturia, though mechanisms including hypoxia, intrathoracic pressure changes, and hormonal alterations. While age and severity may influence these associations, stratified analyses remain limited. This study examined polysomnographic (PSG) parameters, International Prostate Symptom Score (IPSS) components, and hormonal/electrolyte markers in age- and severity-stratified men with suspected OSA. METHODS: In this cross-sectional study, 104 men (mean age 60.8 ± 9.8 years) underwent PSG.

Two Distinct Endocrine Conditions in a Single Pediatric Patient: Congenital Adrenal Hyperplasia and Type 1 Diabetes Mellitus.

Non-classical congenital adrenal hyperplasia (NCCAH) is an autosomal recessive disorder most commonly caused by partial 21-hydroxylase deficiency, typically presenting with late-onset hyperandrogenism. Type 1 diabetes mellitus (T1DM) is an autoimmune disease characterized by the destruction of pancreatic beta cells, although its coexistence with NCCAH, as described in the literature, is rare. We report an adolescent girl previously diagnosed with NCCAH at six years of age, identified due to premature adrenarche, elevated 17-hydroxyprogesterone and androstenedione, genetically confirmed by a ho

Psychogenic polydipsia in dogs - a review of pathogenesis, diagnosis and treatment.

Polyuria and polydipsia represent a common clinical presentation in dogs and may result from numerous disorders affecting different body systems. Compulsive water consumption is characteristic of psychogenic polydipsia, a primary polydipsia disorder rooted in neurologic, behavioural or environmental factors. While psychogenic polydipsia is relatively common and well described in people, little published information on its pathogenesis, diagnosis and treatment exists in dogs. Most dogs diagnosed with psychogenic polydipsia are young and systemically healthy, have significant polyuria and polydi

LRBA organizes distinct vesicular trafficking systems in distal nephron segments for water and sodium conservation.

Lipopolysaccharide-responsive and beige-like anchor protein (LRBA) deficiency is a rare genetic disorder characterized by immune dysregulation. The immune checkpoint molecule cytotoxic T lymphocyte-associated antigen 4 (CTLA-4) fails to perform proper membrane trafficking in the absence of LRBA. In addition to immune cells, LRBA localizes to intracellular vesicles in various epithelial cells; however, its physiological roles have not been accurately deciphered. It was observed in this study that LRBA facilitates water and sodium transport by promoting vesicular trafficking of aquaporin-2 (AQP2

Circadian rhythm disturbances in nocturia and nocturnal polyuria: A systematic review.

BACKGROUND: Nocturia is a common symptom among adults and is often associated with disrupted sleep and diminished quality of life. In many cases, it is driven by nocturnal polyuria, an imbalance in nighttime urine production. Emerging evidence suggests that circadian rhythm disturbances, which regulate hormone secretion, renal function, and blood pressure, may play a role in this altered diuresis. This systematic review evaluated the contribution of circadian dysregulation to the pathophysiology of two distinct nocturnal voiding patterns: nocturia (defined by voiding frequency) and nocturnal p
