Euglycemic Diabetic Ketoacidosis-Like State During the Treatment of Diabetic Ketoacidosis in Fulminant Type 1 Diabetes: A Case Report.

Diabetic ketoacidosis (DKA) is a life-threatening acute metabolic complication of diabetes characterized by hyperglycemia, metabolic acidosis, and ketosis. Appropriate insulin therapy and glucose supplementation during treatment are essential to prevent persistent ketoacidosis. A 53-year-old man presented with a one-week history of polyuria. Urinalysis revealed 3+ ketonuria and 3+ glucosuria. Laboratory tests showed a hemoglobin A1c (HbA1c) level of 8.0%, plasma glucose at 720 mg/dL, arterial pH of 7.23, and bicarbonate (HCO₃⁻) at 15.9 mEq/L. He was diagnosed with DKA and was immediately admit

Measuring the Impact of tolvaptan-induced polyuria on quality of life in patients with autosomal dominant polycystic kidney disease.

INTRODUCTION: Tolvaptan, a vasopressin V2 receptor antagonist (V2RA), is the only proven renoprotective therapy for patients with autosomal dominant polycystic kidney disease (ADPKD), but aquaretic side-effects limit its tolerability. Currently, no patient-reported outcome measures (PROMs) specifically assess the impact of tolvaptan on health-related quality of life (HRQoL) in ADPKD. METHODS: We developed the Tolvaptan Impact of Polyuria Scale (TIPS) questionnaire to evaluate tolvaptan-related treatment burden, with higher scores indicating greater burden. The TIPS, alongside the ADPKD-Urinar

[Analysis of stimulating factors for serum copeptin based on liquid chromatography-tandem mass spectrometry].

Copeptin, as a stable surrogate biomarker for arginine vasopressin (AVP), plays an important role in the differential diagnosis of polyuria-polydipsia syndromes. Current guidelines recommend dynamic monitoring of copeptin levels during stimulation tests to assist in differentiating AVP deficiency (AVP-D) from primary polydipsia. Although these methods are well-established, they have certain limitations. Therefore, safer and more feasible stimulants with reliable effects are of clinical interest. Additionally, the currently available copeptin assay, which is based on time-resolved immunofluores

Case Report: Treatment of hypersomatotropism in a diabetic dog with transsphenoidal hypophysectomy.

BACKGROUND: Pituitary somatotroph adenoma is rare in dogs and may cause hypersomatotropism (HS) leading to insulin resistance and diabetes mellitus (DM). CASE DESCRIPTION: A 10-year-5-month-old neutered male Staffordshire Bull Terrier presented with polyuria, polydipsia, progressive inspiratory stridor, and poorly controlled DM with hyperinsulinemia and insulin resistance. Serum insulin-like growth factor (IGF-1) was markedly elevated (1,214 ng/mL; reference interval, 42-449 ng/mL) and suggested HS which was further supported by a somatostatin suppression test. Magnetic resonance and computed

Unlocking the puzzle of primary monosymptomatic nocturnal enuresis: a comprehensive study on the management of Thai children through alarm therapy and medication.

BACKGROUND: Primary monosymptomatic nocturnal enuresis (PMNE) is a common pediatric condition that can improve with age but can profoundly impact self-esteem and quality of life. Despite the availability of treatment options in Thailand, there are no standardized guidelines. This study aimed to evaluate the efficacy of three approaches: enuresis alarm, pharmacological intervention, and their combination, to provide evidence-based recommendations for culturally relevant clinical guidelines for PMNE and explore factors influencing treatment efficacy. METHODS: A 5-year retrospective study was co

Fulminant type 1 diabetes with high-titer anti-glutamic acid decarboxylase antibodies: Likely rapid progression from stage 2 to 3.

A 61-year-old man visited our hospital with a sudden onset of polydipsia and polyuria occurring 5 days prior, accompanied by a 5-kg weight loss. A month prior, his glycated hemoglobin level was 6.2%. Precisely 8 days before the first visit, he had a fever, and hyperglycemic symptoms began shortly thereafter. At the first visit, he had a blood glucose level of 465 mg/dL, a glycated hemoglobin level of 7.7%, and ketosis. He was diagnosed with fulminant type 1 diabetes mellitus. The antiglutamic acid decarboxylase (GAD) antibodies were >2000 IU/mL, without other pancreatic islet-related autoantib

Nationwide Trends in Desmopressin Prescribing for Nocturnal Polyuria in Japan: A Population-Based Analysis (2020-2023).

OBJECTIVES: Desmopressin is an established treatment for nocturia due to nocturnal polyuria. In Japan, low-dose formulations (25 and 50 μg) were approved only for men in 2019. However, age- and dose-specific prescribing patterns in real-world practice remain unclear. Using nationwide aggregated claims data, we analyzed age-stratified prescribing trends and dose selection patterns. METHODS: Publicly available NDB Open Data (fiscal years 2020-2023) were used. Outpatient prescriptions for desmopressin orally disintegrating tablets (25 and 50 μg) were extracted by sex and 5-year age categories. P

Feature reduction using swarm optimization and random forest classifiers for early diabetes risk prediction.

Diabetes is a chronic metabolic disorder caused by excessive blood sugar levels, which leads to severe damage to other organs. Type 2 diabetes is, more often than others, a long-term metabolic disorder in which the body resists insulin or does not produce enough of it. Early diabetes detection with fewer features reduces patient burden, and machine learning makes the process more time-efficient. This study proposes three machine learning approaches that achieve both high performance and effective feature reduction using the Early Stage Diabetes Risk Prediction dataset. Multiple research works

The conundrum in diagnosing Maturity-Onset Diabetes of the Young (MODY) in a large German pedigree with early-onset diabetes and a novel HNF1A variant.

BACKGROUND: Genetic screening for maturity-onset diabetes of the young (MODY) involves sequencing of the coding regions of known disease-associated genes. We describe the complex and challenging diagnostic journey of a patient with early-onset diabetes with a novel, intronic HNF1A variant likely affecting a branching site. CASE PRESENTATION: The patient was diagnosed with diabetes at the age of 10 years after incidental hyperglycemia (HbA1c 8.1%, C-peptide 3.0 μg/dl), without polyuria, polydipsia, or weight loss. Type 1 diabetes associated autoantibodies were negative, but the patient had a st

Age and Nocturnal Polyuria in Nocturia: A Multicenter Voiding Diary Cohort Study.

INTRODUCTION: Although the prevalence of nocturnal polyuria (NP) increases with age, the mechanisms underlying age-related changes in voiding patterns and the daytime-nighttime distribution of urine volume remain unclear. Given that the severity of nocturia and bladder function may differ across age groups, age-specific voiding profiles require clarification to enable the development of appropriate management strategies. Therefore, in this multicenter cohort study, we aimed to analyze the urinary patterns and clinical factors associated with NP using a voiding diary. MATERIALS AND METHODS: Th

Cataracts as the presenting symptom of type 1 diabetes mellitus.

Type 1 diabetes mellitus (DM) usually presents in the pediatric population as weight loss, polyuria, polydipsia, and, occasionally, with abdominal pain, vomiting, and Kussmaul breathing consistent with diabetic ketoacidosis. In this case report, we describe a 17-year-old boy presenting with fatigue and bilateral blurry vision, found to have hyperglycemia and subsequent DM on a laboratory draw before cataract surgery commenced. This patient also had an added complication of pseudophakic cystic macular edema of both eyes after uncomplicated cataract removal, which has not been described in previ

GLUT9b- and ABCG2-mediated collecting duct urate transport uncovers a vasopressin-independent mechanism of renal water reabsorption.

Renal water reabsorption is classically regulated by vasopressin V2 receptor (V2R) signaling through cyclic AMP and protein kinase A, driving apical accumulation of aquaporin-2 (AQP2). However, collecting duct water handling is also modulated by vasopressin-independent mechanisms. Here, we examined intracellular soluble urate as a vasopressin-independent regulator of AQP2 trafficking. Intracellular urate accumulation in collecting duct cells was mediated by enhanced apical urate uptake via GLUT9b and reduced apical urate efflux through ABCG2, triggering phosphodiesterase-4 activation, reduced
