A Case of Potential Oral Desmopressin Malabsorption Secondary to Concurrent Tube Feed Administration in a Patient With Arginine Vasopressin Deficiency.

Arginine vasopressin deficiency (AVP-D) is an endocrine disorder characterized by dilute, non-osmotic polyuria and hypernatremia, caused by a deficiency of antidiuretic hormone (ADH). Drug resources indicate that food may reduce absorption of oral desmopressin (DDAVP). However, it reportedly does not affect pharmacodynamics; therefore, no precautions are currently advised regarding food intake or tube feeding (TF). We present a case in which severe polyuria and hypernatremia developed after TF was initiated in a patient receiving oral DDAVP for AVP-D, which was corrected upon switching to subc

Characterization of Nocturia and Urinary Voiding Indices Along the Heart Failure Continuum in US Male Veteran Patients.

Heart failure (HF) is a growing public health crisis that progresses from cardiovascular risk factors to structural/functional heart disease and eventual HF symptoms. Nocturia, long recognized as a HF symptom, has not been characterized along the HF continuum. To characterize nocturia across HF stages, we retrospectively analyzed voiding diaries and clinical data from US male veteran outpatients treated for lower urinary tract symptoms from 2008 to 2022. Patients were categorized into HF stages 0 (no risk factors), A (HF risk factors), B (stage A criteria + structural heart disease), and C (st

Familial hypomagnesemia with hypercalciuria and nephrocalcinosis (FHHNC) with ocular involvement due to a novel CLDN19 variant.

Claudin-16 (CLDN16) and claudin-19 (CLDN19) are essential tight junction proteins in the kidney that are critical for magnesium homeostasis. Mutations in CLDN16 and CLDN19 cause Familial Hypomagnesemia with Hypercalciuria and Nephrocalcinosis (FHHNC), a rare autosomal recessive tubular disorder. The disorder can progressively lead to severe complications, including end-stage renal disease. Affected individuals often present with polyuria, polydipsia, nephrolithiasis, hematuria, muscular tetany, seizures, and failure to thrive. Notably, CLDN19 mutations have also been associated with ocular abn

Remission of severe familial proteinuria in a Bracco Italiano dog.

The objective of this report was to document clinical remission of suspected familial proteinuria in a Bracco Italiano dog. A 4-year-old intact female Bracco Italiano with a familial history of amyloidosis was presented because of unilateral chemosis, polyuria, and polydipsia. Hypoalbuminemia and severe proteinuria were detected via initial diagnostics. Although no renal histopathologic assessment was done, there was a strong clinical suspicion for renal amyloidosis. The dog was treated with telmisartan and colchicine (in addition to other therapies) for proteinuria and achieved clinical remis

Tolerability of SGLT2 inhibitors in patients with Fabry disease: An observational study.

Cardiac involvement of Fabry disease (FD) includes heart failure (HF) treated according to general guideline recommendations. The retrospective study investigates HF medication in FD focusing on sodium glucose cotransporter 2 (SGLT2) inhibitors. HF medication, laboratory and echocardiographic measurements as well as side effects of SGLT2 inhibitors were analyzed at baseline and the last available follow-up. The analysis included 99 FD patients treated with angiotensin converting enzyme (ACE) inhibitors or angiotensin II type 1 receptor blocker (ARB) (50%), beta-blocker (31%), SGLT2 inhibitors

The ketogenic diet is not for everyone: contraindications, side effects, and drug interactions.

BACKGROUND: The ketogenic diet (KD), initially developed for the treatment of neurological disorders, has gained increasing attention for its potential role in the management of various metabolic diseases. Alongside its expanding clinical use, concerns have emerged regarding its safety, tolerability, and suitability in specific patient populations. This review summarises key contraindications, clinical situations requiring caution, relevant drug interactions, and commonly reported adverse effects associated with KD. DISCUSSION: Rare absolute contraindications include selected inborn errors of

[Pregnancy and thirst, a remarkable duo].

Establishing a differential diagnosis for polyuria and polydipsia during pregnancy can be challenging. We describe a pregnant woman with severe polyuria (urine output 9L/24h) and polydipsia without pre-existing symptoms. Gestational diabetes and electrolyte disturbances were excluded; HbA1c and daily glucose profiles were normal. Due to pregnancy, no water deprivation or arginine stimulation test was performed. Copeptin, a surrogate marker for AVP/ADH, was <2 pmol/L (reference: 0-9 pmol/L), ruling out AVP resistance. Because of disabling symptoms, a trial of oral desmopressin was initiated dur

Pregnancy-unmasked latent autoimmune arginine vasopressin deficiency with anti-rabphilin-3a antibody positivity.

Although pregnancy-associated arginine vasopressin (AVP) disorder is rare, it is often attributed to excessive placental vasopressinase activity. However, vasopressinase-independent mechanisms, including gestational unmasking of latent AVP deficiency (AVP-D), may occur. We report a 37-year-old primigravid female who developed severe polydipsia and polyuria in the third trimester. She had hypotonic polyuria with inappropriately low plasma AVP levels and loss of the posterior pituitary bright spot on T1-weighted magnetic resonance imaging (MRI). AVP-D was suspected; desmopressin promptly relieve

A newborn with type 1 Bartter syndrome: challenges in the treatment and development during 30 months follow-up-a case report.

Type I Bartter syndrome is a rare autosomal recessive tubulopathy resulting from mutations in the SLC12A1 gene, leading to defective sodium-chloride reabsorption in the thick ascending limb of the loop of Henle. Affected neonates typically present with profound fluid and electrolyte disturbances, polyuria, and metabolic derangements. Early diagnosis and individualized management are crucial to prevent life-threatening complications and support appropriate growth and development. We report the case of a male preterm infant born at 33 weeks' gestation following a pregnancy complicated by severe

Resolved GH excess and arginine vasopressin deficiency following pituitary apoplexy.

A 10-year, 8-month-old male presented with polyuria, polydipsia, and secondary nocturnal enuresis for 4 months following a severe headache. Initial fasting studies raised concern for partial arginine vasopressin deficiency (AVP-D). Brain magnetic resonance imaging revealed a possible pituitary adenoma vs artifact. His height was >99th percentile; he was prepubertal and growth charts demonstrated growth velocity >7.5 cm/year for the past 2 years. Additional testing showed markedly elevated IGF-1 and IGF-binding protein 3. He passed a water-deprivation test, ruling out AVP-D. He underwent a GH s

Ultrasonographic and CT Features of a Neoplasia-Associated Visceral Arteriovenous Fistula in a Cat.

An 11-year-old neutered male European Shorthair cat presented with progressive weight loss, polyuria/polydipsia, lethargy, and recurrent hemorrhagic cystitis. Abdominal ultrasound revealed tortuous and dilated vessels with spontaneous echo-contrast within the right renal parenchyma. Pulsed-wave Doppler demonstrated continuous flow with pulsatile components within the abnormal vessels, consistent with an arteriovenous communication. Computed tomography (CT) angiography confirmed marked intrarenal vascular dilation, strong arterial-phase enhancement, aneurysmal dilation of the right dorsal renal

Stable pentadecapeptide therapy counteracts gentamicin-induced nephrotoxicity via nitric oxide-system modulation: evidence from a triple nitric oxide-agent approach in rats.

In rats, stable gastric pentadecapeptide (BPC 157) therapy may resolve severe gentamicin nephrotoxicity (100 mg/kg ip/daily through 8 days) and the nitric oxide (NO)-system relation. We applied BPC 157 (10 µg/kg, 10 ng/kg, in drinking water or daily intraperitoneally) along with a triple NO-agent approach. Triple NO-agent approach captures NO-system, as a whole, simultaneously the inhibition (N(G)-nitro-L-arginine methylester (L-NAME) 5 mg/kg ip/daily), the overstimulation (L-arginine, 100 mg/kg ip/daily), and the immobilization (L-NAME + L-arginine/daily). BPC 157 therapy counteracted gentami
