Neonatal diabetes mellitus (NDM) is a rare genetic condition characterized by persistent hyperglycemia presenting within the first six months of life. It is most commonly caused by mutations affecting pancreatic development or β-cell function. ZNF808 is a recently identified gene that plays an essential role in human pancreatic development, with biallelic pathogenic variants causing autosomal recessive pancreatic agenesis. We report the case of a seven-month-old Saudi male infant with a history of intrauterine growth restriction who presented at 2.5 months of age with severe hyperglycemia and

OBJECTIVES: To investigate the physiological validity of current diagnostic criteria for nocturnal polyuria and to derive and validate a symptom-anchored revision consistent with female aging.
METHODS: This retrospective cross-sectional study analyzed 3-day bladder diaries from 3,000 women with lower urinary tract symptoms. We assessed age-specific nocturnal polyuria prevalence using current International Continence Society definitions (nocturnal polyuria index ≥33% for women >66 y and ≥20% for women ≤65 y). To address discrepancies with aging physiology, a novel cutoff was derived using the

Nocturnal enuresis (NE) is a common and multifactorial pediatric condition characterized by abnormalities in bladder storage, renal water regulation, and central arousal mechanisms. According to the International Children's Continence Society, NE is defined as intermittent urinary incontinence that occurs during sleep in children aged 5 years or older, in the absence of congenital or acquired neurologic or urologic disease. Although often self-limiting, NE imposes a substantial psychosocial burden, causing embarrassment, low self-esteem, sleep disruption, and family stress. Advances in neuroph

Introduction Primary nocturnal enuresis is a common pediatric condition, classified into monosymptomatic (MPE) and non-monosymptomatic (NMPE) based on the presence of daytime lower urinary tract symptoms (LUTS). Distinguishing between these two forms is essential, as it directly influences management strategies. This study aimed to compare the clinical, functional, and psychological characteristics of children with MPE and NMPE. Materials and methods A comparative observational study was conducted over two years, including children aged five to 15 years who presented with primary nocturnal enu

Chronic kidney disease (CKD) is a progressive condition characterized by sustained loss of renal function, frequently accompanied by fibrosis and oxidative stress-mediated damage. In this study, adenine-induced CKD was used as an experimental model to assess the effects of pirfenidone. Rats fed a 0.75% adenine diet exhibited key features of renal dysfunction, including reduced body weight, kidney hypertrophy, elevated serum urea and creatinine, decreased creatinine clearance, polyuria, low urine osmolarity, increased urinary N-acetylglucosaminidase excretion, and reduced antioxidant activity.

BACKGROUND: In pediatric kidney transplant recipients, native nephrectomy is sometimes performed to manage conditions such as polyuria, nephrotic syndrome, and recurrent infections. To minimize surgical morbidity, our center adopted a technique from the adult transplant program involving ligation of native ureters at the time of transplantation as an alternative to native nephrectomy in selected patients.
METHODS: This retrospective study describes our center's experience with pediatric patients who underwent ureteral ligation at transplantation between December 2008 and December 2022. Patien

The association between Cushing's syndrome (CS) and diabetes mellitus (DM) in cats is rarely reported. Complications related to hypercortisolism and persistent hyperglycemia may occur, which may complicate the control of both endocrinopathies. This report describes the case of a 10-year-old Persian cat diagnosed with CS and DM. The patient presented with polyuria, polydipsia, chronic dry seborrhea on the dorsal region and flanks, and lack of hair regrowth on the abdomen. Through the dexamethasone suppression test (0.1 mg/kg) and the presence of persistent hyperglycemia with elevated fructosami

A 33-year-old male presented with "nasal obstruction," polyuria and polydipsia for a few years. His hormonal profile revealed panhypopituitarism, prompting pituitary imaging. Initial magnetic resonance imaging (MRI) findings in another institute diagnosed a craniopharyngioma with large cystic component. Subsequent follow-up MRI at our center revealed a large cystic sellar lesion that follows cerebrospinal fluid (CSF) intensity across all sequences. There was communication to the anterior recess of the third ventricle, and infrasellar extension via the sphenoid sinus into the nasopharynx, causi

We present the case of a 64-year-old Caucasian man with severe abdominal pain and vomiting preceded by recurrent epigastric pain, nausea, polyuria, polydipsia, constipation, and weight loss. The patient had significantly elevated calcium 3.84 mmol/L (2.20-2.60), advanced renal impairment with serum creatinine 253 μmol/L (59-104), serum urea 11.2 mmol/L(2.5-7.8), elevated serum lipase 1599 U/L (13-60). Hematological studies revealed normocytic normochromic anemia, Hb 96 g/L(130-170), adequately suppressed parathyroid hormone 1.3 pmol/L (1.8-7.9), indicating parathyroid independent hypercalcemia

OBJECTIVES: The current diagnostic methods for syndrome of inappropriate antidiuresis (SIAD) and cerebral salt wasting (CSW), are imprecise. Gold standard radioisotope methodologies are currently confined to research settings. Therefore, this study evaluated whether the change in copeptin levels could be an early diagnostic tool.
METHODS: A study was conducted on hyponatremic (≤ 130 meq/L) children. Subjects with endocrine disorders, systemic diseases, diarrhea, and shock were excluded. At baseline, copeptin and fractional excretion of uric acid (FEUA) were measured. Subjects then received ma

PURPOSE: Copeptin is an emerging biomarker for polyuria-polydipsia syndrome. Postoperative copeptin levels may predict acquired arginine vasopressin deficiency (AVP-D). This prospective study evaluated the diagnostic performance of serum copeptin for predicting postoperative AVP-D after transsphenoidal surgery (TSS). METHODS: Copeptin was measured preoperatively, immediately after surgery (periextubation, T0), upon first ward arrival (T1), daily until discharge, and at week 2. Delta copeptin was calculated as the percentage change from T0 to T1. RESULTS: Of 53 patients, 23 (43.4%) developed AV

Neonatal diabetes mellitus (NDM) is a rare metabolic disorder characterised by hyperglycemia within the first six months of life. While commonly monogenic, the MS4A6A gene, known for immune modulation and calcium signalling, has not previously been linked to NDM. We report a case of transient NDM (TNDM) potentially associated with a novel homozygous variant in the MS4A6A gene. A male infant born at 27 weeks of gestation (720 g) developed severe hyperglycemia (>200 mg/dL) and polyuria on day 14 of life, following the resolution of suspected meningitis. Investigations confirmed insulin-deficient

Diabetes mellitus (DM) is a chronic endocrine abnormality characterised by impaired insulin function and systemic hyperglycaemia, which alter fluid balance and hormonal regulation. The cardiac hormone atrial natriuretic peptide (ANP) modulates renal natriuresis and diuresis, and its expression is disrupted in DM. We evaluated the effects of oral aqueous garlic extract (GE) on physiological, biochemical, and molecular parameters in streptozotocin-induced type 1 diabetic male Sprague-Dawley rats, using metformin as a positive control. Parameters assessed included fasting blood glucose (FBG), bod

BACKGROUND: Hyponatremia due to the syndrome of inappropriate antidiuretic hormone (SIADH) occurs in approximately 25 % of patients with small cell lung cancer (SCLC). Tolvaptan has emerged as a promising option to treat SIADH in such patients. Here, we analyse the current literature on tolvaptan for managing paraneoplastic SIADH in SCLC.
METHODS: PubMed, EMBASE, Cochrane, and Web of Science were searched for studies that enrolled patients with paraneoplastic SIADH in SCLC and were treated with tolvaptan until December 2024.
RESULTS: Out of 155 studies identified by database searches, 29 stu

Granulomatous hypophysitis is a rare form of pituitary inflammation that closely mimics pituitary macroadenoma in clinical presentation as well as radiological appearance. Histopathology remains the gold standard diagnostic modality. Here, we present the case of a young female who presented with symptoms of anterior and posterior pituitary dysfunction, including secondary amenorrhea, cold intolerance, polyuria, and polydipsia. Magnetic resonance imaging revealed a sellar mass with suprasellar extension, suggesting a pituitary macroadenoma. Histopathological examination of the transsphenoidal b

CONTEXT: Gestational arginine vasopressin (AVP) disorder is classified into 3 categories: AVP deficiency (AVP-D), AVP resistance, and excess vasopressinase-induced AVP disorder. This is the first case report of gestational AVP disorder linked with magnesium sulfate administration.
CASE DESCRIPTION: A 26-year-old woman pregnant with twins was admitted to our hospital with threatened preterm labor (TPL). The day after continuous intravenous magnesium sulfate administration for TPL was initiated, the patient suddenly developed thirst, polydipsia, and polyuria. Because the plasma AVP level was ex

Sepsis is a life-threatening condition characterized by infection-induced organ dysfunction, with fluid imbalance and cardiovascular instability as cardinal features. Although circulating procalcitonin (PCT) is widely used as a diagnostic and prognostic marker in sepsis, its pathophysiological role remains poorly understood. Here, we identify a central neural circuit through which PCT directly disrupts fluid homeostasis: systemic PCT crosses the blood-brain barrier, activates calcitonin receptors, and depolarizes the Oprk1-expressing neurons in the ventromedial preoptic nucleus of the hypothal
