BACKGROUND Carotid-cavernous fistulas (CCFs) are abnormal arteriovenous communications between the carotid artery and cavernous sinus. They may be direct or indirect and commonly present with both ocular and neurovascular symptoms. Endovascular embolization is the main treatment, with high rates of fistula obliteration and clinical improvement. Although generally safe, complications can occur, including cranial nerve dysfunction and vascular injury. Endocrine complications following carotid-cavernous fistula embolization are exceedingly rare. To our knowledge, only 1 case of syndrome of inappr

BACKGROUND: Tyrosinemia type I is a rare autosomal recessive inborn error of metabolism caused by deficiency of fumarylacetoacetate hydrolase (FAH), an enzyme essential for the final breakdown of tyrosine. The enzyme defect leads to accumulation of toxic metabolites, primarily affecting the liver, kidneys, and brain. Typically, diagnosed in infancy due to acute or chronic liver dysfunction, refractory rickets is an uncommon presentation. We report a child with tyrosinemia type I presenting with refractory rickets and hepatosplenomegaly but with preserved liver function. CASE PRESENTATION: A th

A 9-year-old neutered male miniature schnauzer was referred to our facility with polyuria, polydipsia, persistent hypercalcemia, hypophosphatemia, and elevated parathyroid hormone-related protein. Dual-energy computed tomography (DECT) imaging revealed a mass in the left anal gland (LAG) and enlargement of the left medial iliac lymph node (LML) and the left internal iliac lymph node (LIL). The mass was surgically removed, and the histopathological diagnosis was anal sac gland adenocarcinoma, with metastasis to the LML and LIL. The pathological findings were concordant with those obtained from

Obsessive-compulsive disorder (OCD) secondary to structural brain pathology is rarely described in pediatric populations. This single-case report describes a child with a pineal germinoma and obstructive hydrocephalus who developed prominent obsessive-compulsive symptoms, highlighting a potentially reversible neurobiological contributor to atypical pediatric OCD. The case illustrates the temporal association between symptom evolution, neurosurgical and oncologic management, and subsequent changes in obsessive-compulsive behaviors, contributing to understanding the role of cortico-striato-thala

Biological sex is an important determinant of kidney disease susceptibility and outcomes. The epigenetic modifier KDM6A is an X chromosome-expressed lysine demethylase and molecular scaffold that escapes X chromosome inactivation. Here, we compared the effects of deletion of KDM6A from kidney tubule epithelial cells in female and male mice (KDM6A TubKO ). Knockout of KDM6A from tubule cells aggravated kidney fibrosis caused by unilateral ureteral obstruction (UUO) in female mice, whereas male mice were unaffected by KDM6A absence. Unexpectedly, female (but not male) KDM6A TubKO mice developed

Arginine vasopressin deficiency (AVP-D) is an uncommon but clinically important cause of the polyuria-polydipsia syndrome. Establishing the diagnosis extends beyond confirming hypotonic polyuria and requires differentiation from primary polydipsia and arginine vasopressin resistance, together with identification of the underlying etiology. Unlike adults, children and adolescents with AVP-D frequently present with the earliest manifestation of an evolving neoplastic, infiltrative, inflammatory, congenital, autoimmune, or genetic disorder, making longitudinal clinical, endocrine, and magnetic re

Langerhans cell histiocytosis (LCH) is a rare clonal proliferative disorder of bone marrow-derived dendritic cells that is uncommon in adults. Its clinical presentation varies depending on the organ involved. Central nervous system (CNS) involvement poses a critical therapeutic challenge because of permanent neurological dysfunction. Here, we report two contrasting cases of adult-onset LCH with distinct clinical outcomes. Case 1 involved a 36-year-old woman presenting with a persistent headache. Neuroimaging revealed a solitary osteolytic lesion in the left parietal bone with dural thickening.

BACKGROUND: Postoperative central diabetes insipidus (CDI) is a well-documented but often transient complication following transsphenoidal surgery (TSS) for pituitary adenomas. CDI is characterized by excessive urine output due to impaired antidiuretic hormone (ADH) secretion following damage to the neurohypophyseal stalk or hypothalamus. While most cases resolve within a few days, severe cases with extremely high urine output are rare. This case is notable due to an exceptionally high urine output of 33.5 L in a single day, far exceeding typical postoperative presentations and highlighting th

Dexmedetomidine, propofol, and sevoflurane are widely used anesthetic agents, but reports of water metabolism disturbances-particularly drug-induced diabetes insipidus (DI)-associated with their use remain extremely rare. We report a case of a 51-year-old Chinese man who developed abrupt high-volume polyuria (> 600 mL/h; 24-h output of 8750 mL), hypotonic urine (specific gravity 1.003; osmolality 175 mOsm/kg), progressive hypernatremia (peak 156 mmol/L), hemoconcentration, lactic acidosis, and hypotension following transurethral seminal vesiculoscopy performed under general anesthesia. BACKGROUND: Central diabetes insipidus (CDI) is a frequent complication after craniopharyngioma (CP) surgery. Although many patients recover within 2 weeks, a substantial proportion develop persistent CDI that requires long-term management. Systemic inflammatory activation may aggravate hypothalamic-neurohypophyseal injury and delay functional recovery. However, reliable preoperative predictors of early recovery remain limited. This study aimed to develop a multivariable prediction model for persistent CDI at 2 weeks after CP surgery.
METHODS: This retrospective study included patients with p

Central diabetes insipidus, or arginine vasopressin deficiency (AVP-D), is a rare disorder caused by insufficient release of antidiuretic hormone from the pituitary gland. The disease is characterised by severe polyuria and polydipsia, which, in rare cases, can be caused by variants in the AVP gene. The aim of the present study was to identify the genetic cause of AVP-D in two families and compile and discuss an updated and complete catalogue of variants in AVP associated with AVP-D. Two Danish families with autosomal dominant inheritance of symptoms indicating AVP-D were included, and proband

CASE SUMMARY: A 2-year-old castrated male domestic shorthair cat presented with severe lethargy, hyporexia, dehydration and poor body condition. The incidence of polyuria/polydipsia was unknown because of the presence of multiple other cats in the household. Laboratory findings revealed severe hypernatraemia, isosthenuria, haemoconcentration and moderate azotaemia. MRI of the head demonstrated partial hypoplasia/aplasia of the hypothalamus and pituitary gland, confirming congenital central diabetes insipidus; additional endocrine testing excluded other causes. Initial fluid therapy improved th

Diabetic ketoacidosis (DKA) is a life-threatening complication of diabetes characterized by hyperglycemia, high-anion-gap metabolic acidosis, and ketonemia. Hypernatremia in DKA is uncommon and typically reflects disproportionate free water loss. Arginine vasopressin resistance (AVP-R), previously known as nephrogenic diabetes insipidus, may occur in the setting of hypokalemia, leading to polyuria, hypotonic urine, and worsening hypernatremia. We report the case of a 14-year-old previously healthy female who presented with new-onset severe DKA complicated by profound acidosis, hyperglycemia, h

BACKGROUND: Nephrogenic diabetes insipidus (NDI) is a rare, potentially life-threatening renal tubular disorder characterized by a decreased ability to concentrate urine due to antidiuretic hormone (ADH) resistance, leading to significant free water loss. The congenital form, most often caused by X-linked mutations in the AVPR2 gene, typically presents in early infancy. Due to its rarity and nonspecific clinical features in young children, such as failure to thrive, vomiting, and excessive fluid intake, diagnostic delays are common, especially in centers with limited prior experience in managi

Congenital nephrogenic diabetes insipidus (CNDI), also known as arginine vasopressin resistance, is a rare inherited disorder of water homeostasis in which the kidneys lose their ability to concentrate urine, leading to polyuria, polydipsia, and a risk of hypertonic dehydration. We report the case of a 3-month-old infant with symptoms present for approximately 2 months, characterized by episodes of fever, irritability, and poor growth. Upon admission, severe malnutrition and dehydration were noted. A diagnosis of CNDI was made based on vasopressin levels, plasma and urine osmolarity values, hy

A 10-year-old neutered female Maine Coon was presented with haematuria, weight loss, polyuria/polydipsia and progressive azotaemia. Diagnostic imaging revealed bilateral renal pelvic dilation, ureteral distension and ectopic ureters. Bacterial pyelonephritis was diagnosed, but the azotaemia progressed despite antibiotic therapy, and suspected ureteral obstruction prompted surgical ureteral reimplantation. The visualisation of omental nodules and thickening of the ectopic right ureterovesicular junction intraoperatively raised suspicion of malignancy, and histopathology revealed urothelial carc

Central diabetes insipidus (CDI) should be considered in neurologic patients with persistent polyuria and rising sodium concentrations despite appropriate fluid therapy. Earlier recognition, serial urine concentration monitoring, and desmopressin administration may help prevent severe hypernatremia, although outcomes may remain poor in patients with progressive intracranial disease.

BACKGROUND: Hypothalamic-pituitary involvement is common in neurosarcoidosis, but dedicated studies describing its impact are limited.
OBJECTIVE: To determine the clinicoradiographic characteristics, treatment responsiveness, and long-term outcomes of hypothalamic-pituitary neurosarcoidosis.
METHODS: Patients with definite or probable neurosarcoidosis were included if they exhibited radiographic involvement of the hypothalamus, infundibulum, and/or pituitary and were divided into symptomatic and asymptomatic groups. When available, radiographic features were directly reviewed by the authors.

BACKGROUND: Congenital nephrogenic diabetes insipidus (CNDI) is most frequently caused by mutations in the AVPR2 gene. Patients exhibit persistent polyuria due to renal insensitivity to antidiuretic hormone. Chronic high urine output predisposes to bladder dysfunction and upper urinary-tract dilatation, notably hydronephrosis. Although pharmacotherapy can partially reduce urine volume, its capacity to reverse established hydronephrosis is limited. Clean intermittent catheterization (CIC), a mainstay in managing neurogenic bladder, warrants investigation regarding its utility in CNDI-associated
