It is frequently stated that extravasation of urine after intrarenal surgery is deleterious to renal wound healing causing fibrosis or granulation tissue. A study using dogs was designed to test the hypothesis that the contact of urine with incised renal parenchyma may cause altered wound healing. This study showed that exposure of the renal parenchyma to extravasated urine from open collecting structures did not alter renal healing.

Perirenal urinomas may develop when obstruction to urinary flow creates sufficient back pressure to produce extravasation of urine. Such fluid collections have been described as mainly sonolucent. However, in the last 2 years, we have imaged five neonates with multiple, marked septations of the urinoma, four with posterior urethral valves and one with a ureteropelvic junction obstruction. Diagnostic confusion may arise when the kidney is not identified, when the septated nature of the urinoma is not recognized, and when urinary tract obstruction is not seen.

Mandatory surgical exploration of penetrating renal injuries has been advocated for adequate assessment and repair of renal and retroperitoneal injuries. We evaluated 27 consecutive patients with deep stab wounds of the flank and back who were thought to be at risk for renal injury. In the newborn, intraperitoneal extravasation of urine from a urachal laceration may follow an attempt at umbilical artery catheterization. Clinical results of this injury are neonatal ascites, anuria, and azotemia. Herein, we describe a case of iatrogenic bladder injury with successful management following late detection. The anatomy of the urachus and bladder in the newborn are reviewed as well as clues to the diagnosis of neonatal urinary ascites.

The authors report a case of rhabdomyosarcoma of posterior urethra they observed in a 10 year-old boy who was referred for an acute urinary retention. A case of unilateral perinephric pseudocyst formation as a result of extravasation of urine is reported. The main presenting signs and the initial clinical examination did not directly indicate severe renal involvement. The subsequent development of pseudocysts as a result of chronic extravasation of urine was positively diagnosed by laboratory investigations and ancillary procedures. Treatment consisted of surgical removal of the affected kidney.

Umbilical artery catheterization in the newborn can lead to perforation of the bladder or urachal remnant with extravasation of urine into the peritoneal cavity. We present a case of such an injury that led to uroascites and respiratory distress. This injury usually requires prompt surgical repair for resolution.

A parallel incision, unstented extravesical ureteroneocystostomy was used in 203 human kidney transplants. The reoperation rate was 1 per cent. Spontaneous nontraumatic perirenal extravasation of urine is an unusual phenomenon, and the majority of the reported case were caused by acute obstruction with passage of a ureteric calculus. Extravasation due to obstruction of more gradual onset occurs less frequently. We report four cases, three caused by tumor obstruction of the ureter, and one thought to be obstructed by the stricture due to ureteral inflammation. We discuss the diagnosis and treatment of spontaneous urinary extravasation, especially due to chronic ureteral obstruction.

We report a case of perirenal urinoma secondary to benign prostatic hypertrophy. This rare complication of bladder outlet obstruction was due to subsequent hydronephrosis and increased pressure in the renal cavities, leading to rupture of caliceal fornix and allowing perirenal extravasation of urine. Complete resolution of the urinoma was obtained with bladder decompression and conservative management.
