Two cases of spontaneous extravasation of urine in chronic ureteric obstruction are presented. In 1 case the long-standing partial ureteric obstruction was caused by enlarged lymphomatous glands, and extravasation was followed by a perinephric abscess. In the second case, believed to be unique, there was a moderate chronic ureteric obstruction post irradiation, and extravasation was induced by a sudden rise in intra-abdominal pressure due to vomiting.

Three cases of urinary ascites in infants secondary to extravasation of urine from the bladder are reported. The first reported case of intraperitoneal extravasation from a perforated bladder resulting from an umbilical cutdown and the second reported case of an intraperitoneal rupture of a congenital diverticulum of the bladder are included. Successful management of urinary ascites was achieved by immediate diversion. The diagnosis of a vesical perforation should be considered in an infant presenting with ascites.

Eighty patients with urinary incontinence were treated by colposuspension and bladder neck plication. Fourteen patients had difficulty in establishing normal micturition postoperatively. A controlled canine experimental study has been performed to test the application of a biodegradeable graft to vesical augmentation. The graft, after providing initial bladder enlargement, is progressively reabsorbed while serving as a foundation for the regeneration and healing of the layers of the bladder remnant. Emergency uroradiology is warranted in a number of clinical conditions in the nontraumatized patient in order to determine proper therapeutic management. Studies of the ureteral response to changes in intravesical pressure (IP) have been conducted by surgically altering the neural and hydrodynamic integrity of canine ureters. Ureteral function was assessed by acute and chronic recordings of ongoing electrical activity. In this 3-part study the distribution of the post-gonococcal stricture has been related to the disposition of mucous glands in paraurethral glands by mapping the distribution of these glands in the human male urethra. Experimental study in the rat has indicated the role of extravasation of urine in the pathogenesis of urethral stricture. Lastly, the ultrastructure of urethral stricture tissue has revealed features that suggest why some strictures are fibrous and others resilient.

Extravasation of urine was seen in 24 intravenous pyelograms taken for renal colic during the year 1973-74. A number of the patients presented with an acute abdomen. Extravasation took place of retained urine under pressure in the renal pelvis with leakage either around the clayces, from a fissure in the pelvis or even around the ureter. Extravasations of this type are in general rabsorbed without trace. In 5 cases, operation was necessary to drain a purulent collection which had developed in association with the extravasated urine.

Late results after primary repair in 49 recent urethral injuries of the urethra treated through a perineal approach by sutures of realignment with splinting, and suprapublic and perineal drainage were assessed by measuring urethral flow, urethrography, urine analysis and other examinations. In 15 straddle injuries the results were excellent. Of 34 injuries associated with pelvic fracture 50% had satisfactory results. 12% had marked, but tolerable difficulties in micturition. Prostatic infarction is a relatively frequent complication of adenoma of the bladder neck. Nevertheless its importance is minimal as compared with infarctions of vital organs like the brain, heart, lungs or kidneys. General and local factors may play a role in its pathogenesis. Besides other factors, it may contribute to the development of acute retention. Attention is called to the difficulties of differential diagnosis at both gross and microscopic examinations. A misdiagnosis is most frequently made in neoplastic diseases of the prostate.
