Psychometric structure and score interpretation of the SCL-90 among Chinese first-year undergraduates: an analysis of an open dataset.

INTRODUCTION: The Symptom Checklist-90 (SCL-90) provides total and domain scores whose shared and distinct information remains debated. METHODS: We analyzed an open dataset of 14,371 Chinese first-year undergraduates in calibration and validation halves. Four item-level structures (unidimensional, nine correlated factors, second-order, and orthogonal bifactor) were compared on the same 83 domain-scored items using robust maximum likelihood (MLR) in both halves and mean- and variance-adjusted weighted least squares (WLSMV) for ordered items in validation. A separate single-factor model include

Development and Validation of a Perioperative Risk Update Model for Postoperative Delirium in Patients Aged ≥ 80 Years Undergoing Hip Fracture Surgery: A Retrospective Cohort Study.

BACKGROUND: Postoperative delirium (POD) remains a frequent and serious complication after hip fracture surgery in very old patients. A clinically usable risk model could facilitate perioperative risk updating and targeted prevention in this vulnerable population. METHODS: We retrospectively included consecutive patients aged ≥ 80 years undergoing hip fracture surgery between January 2021 and December 2023 (n = 666). POD during hospitalization was the primary outcome. Candidate predictors covering demographics, comorbidities, perioperative variables, and routine laboratory tests were prespeci

Acute Delirium Management in Hospice and Palliative Care.

Acute delirium is a serious neurocognitive disorder that is highly prevalent among individuals receiving hospice and palliative care. Delirium causes significant distress for patients, families, and healthcare providers while contributing to prolonged hospitalization, increased healthcare utilization and costs, a greater likelihood of institutionalization, shorter survival, and higher mortality. It is characterized by acute disturbances in attention and awareness that often fluctuate throughout the day and may present as hypoactive, hyperactive, or mixed psychomotor subtypes. Delirium can be c

Development and external validation of an XGBoost-based prognostic model for small cell lung cancer with brain metastases.

BACKGROUND: Patients with small cell lung cancer (SCLC) and brain metastases (BM) have poor and heterogeneous survival outcomes, while existing prognostic tools have shown limited discriminatory performance. We aimed to develop an extreme gradient boosting (XGBoost) based prognostic model and perform preliminary external validation. METHODS: Patients with small cell lung cancer and brain metastases (SCLCBM) diagnosed between 2010 and 2020 were identified from the Surveillance, Epidemiology, and End Results (SEER) database for model development and internal testing. External validation was per

Network analysis of kinesiophobia and fear of progression and their association with cardiac rehabilitation adherence in post-CABG patients.

BACKGROUND: Coronary heart disease is the major disease burden of cardiovascular diseases in China, and coronary artery bypass grafting (CABG) is the preferred surgical procedure for complex coronary artery lesions. Postoperative cardiac rehabilitation can improve patient outcomes; however, kinesiophobia and fear of progression are associated with lower adherence to rehabilitation. This study aimed to identify core symptoms within the fear network, explore hypothesized directional relationships among symptoms, and analyze the associations between the two types of fear symptoms and adherence to

A modular risk management framework for integrating cybersecurity and patient safety in medical device software development.

Medical device software operates in increasingly connected environments where cybersecurity incidents can directly compromise patient safety. While established safety risk management standards such as ISO 14971 provide mature frameworks for managing unintended hazards, they implicitly assume non-malicious causes and that risks are amenable to probability estimation. Cybersecurity risk management, by contrast, addresses adversarial threats, exploitability, and inherent uncertainty, requiring fundamentally different vocabularies, risk models, and acceptance criteria. In practice, organisations o

The role of contextual factors, school-related attitudes and behavioural traits on lifetime antisocial behaviours among adolescents.

INTRODUCTION: Antisocial behaviour is a result of a multifactorial process that involves both social-level and individual-level factors. The present study aims to investigate the association of contextual factors with the odds of engaging in antisocial behaviours, and to explore the role of school-related attitudes and behavioural traits in the association between contextual factors and antisocial behaviour among 12- to 14-year-old adolescents. METHODS: The analytical sample included 1847 students of 29 secondary schools in Italy who participated in the baseline survey of the "GAPUnplugged" s

Revising the measurement of Eriksonian ego identity in contemporary Italian adolescents: the EPSI-IT.

INTRODUCTION: The Identity subscale of the Erikson Psychosocial Stage Inventory (EPSI) is the most widely used brief measure of Eriksonian ego identity, but it carries a historically situated gender item and has no published Italian validation. Instruments inherit the assumptions of the era that produced them, and an item can lose validity as those assumptions shift. METHODS: In two waves of an Italian school cohort in Rome (2025, N = 3,068; 2026, N = 2,896), we examined the original EPSI and developed the revised EPSI-IT. RESULTS: Among the models compared, a bifactor solution fit best in b

Neuropathic Pain: From Mechanisms to Therapeutic Interventions.

Neuropathic pain is caused by injury or disease of the somatosensory system. It involves peripheral nerve injury, central sensitization, transmitter imbalance, glial activation, and large-scale network remodeling. Despite substantial advances in pain biology, clinical assessment continues to rely predominantly on patient-reported symptoms, sensory examination, and conventional neurophysiological testing, approaches that often fail to delineate the predominant biological mechanisms underlying pain in individual patients. Recent advances in sensory phenotyping and multimodal imaging have enabled

Bladder dysfunction in parkinson's disease: An updated review and guide for the urologist.

PURPOSE: Bladder dysfunction(BD) is a common and debilitating manifestation of Parkinson's Disease(PD). However, guidelines for its management are lacking. We aim to address key bladder dysfunction issues that a urologist faces when managing PD patients. METHODS: BD issues in PD patients were identified by expert opinion. We discussed the available literature and the recommended management for each of the identified issue. RESULTS: Anticholinergics are traditionally first-line for overactive bladder (OAB) management; however, beta-3 agonists should instead be considered first-line therapy to

Comparing two case definitions for including severe acute respiratory infection patients in influenza and COVID-19 vaccine effectiveness studies among older adults using the test-negative design: VEBIS hospital network, seven European countries, 2023/24.

INTRODUCTION: Two severe acute respiratory infection (SARI) case definitions (CDs) are often used in test-negative design (TND) hospital-based vaccine effectiveness (VE) studies. SARI-CD1 requires ≥1 symptom (cough, fever, shortness of breath, anosmia, ageusia, or dysgeusia); SARI-CD2 requires both fever and cough. Using VEBIS hospital network end-of-season 2023/24 data, we compared the effect of these CDs on influenza and COVID-19 VE estimates in Europe. METHODS: Using the TND, we restricted analysis to seven countries recruiting patients using SARI-CD1; we created a subset of SARI patients

Postoperative Wound Care in Mohs Micrographic Surgery: Tradition, Variation, and the Evidence.

Postoperative wound care following Mohs micrographic surgery (MMS) has high variability and surprisingly limited real-world evidence. Despite over one million MMS procedures performed annually in the United States, recommendations for topical agents, pressure dressings, wound cleansing, dressing change frequency, activity restrictions, and wound check intervals vary substantially across practices and are frequently based on training tradition rather than clinical data. This narrative review critically evaluates 15 common post-Mohs wound care practices, characterizing the evidence quality for e
