Efficacy of Oral Melatonin Versus Oral Olanzapine in Improving Sleep in Cancer Patients with Insomnia: Prospective Comparative Study.

Sleep deprivation is a common concern for cancer patients. It has proven difficult to determine severity of sleep disturbances in a cancer group. Patients may fail to mention insomnia because they assume it is a common and temporary reaction to a cancer diagnosis or treatment, a condition that is frequently overlooked in cancer management. This prospective, double-blind, randomized comparative trial evaluated the efficacy and safety of oral melatonin (3 mg/day) versus oral olanzapine (5 mg/day) in 60 advanced cancer patients with insomnia over four weeks. Sleep quality was assessed using the P

Long-Term Safety and Maintenance of Response With Esketamine Nasal Spray in Participants With Treatment-Resistant Depression: Interim Results of the SUSTAIN-3 Study.

Patients with treatment-resistant depression (TRD) have higher rates of relapse and pronounced decreases in daily functioning and health-related quality of life compared to patients with major depressive disorder who are not treatment-resistant, underscoring the need for treatment choices with sustained efficacy and long-term tolerability. Adults with TRD who participated in ≥1 of 6 phase 3 "parent" studies could continue esketamine treatment, combined with an oral antidepressant, by enrolling in phase 3, open-label, long-term extension study, SUSTAIN-3. Based on their status at parent-study e

Profound vagal hyperreactivity unmasked after pacemaker implantation: a rare indication for cardioneuroablation-a case report.

BACKGROUND: Asystole caused by vagal activity with prolonged sinus arrest and absent escape rhythm is rare and may overlap with intrinsic conduction system vulnerability. Cardioneuroablation (CNA) is an emerging option for cardioinhibitory syncope, but its role after pacemaker implantation has been rarely reported. CASE SUMMARY: A 43-year-old man presented with recurrent syncope due to abrupt, prolonged sinus arrest of up to 25 s without escape rhythm. Cardiological evaluation, including tilt-table testing, echocardiography, Holter monitoring, and positron emission tomography, revealed no str

[The preclinical profiles and the clinical efficacy and safety of vornorexant (Vorzzz® tablets), a new orexin receptor antagonist].

Traditionally, benzodiazepines and non-benzodiazepines have been primary pharmacological treatments for insomnia. The introduction of orexin receptor antagonists has provided safer alternatives. Orexin is a neuropeptide involved in maintaining wakefulness, and blockade of its receptors OX1 and OX2 is considered to promote sleep with minimal disruption to the natural sleep. Vornorexant (Vorzzz® tablets 2.5 mg, 5 mg, 10 mg) is a dual orexin receptor antagonist developed in Japan and approved in August 2025 for insomnia. Designed with a short elimination half-life, vornorexant aims to minimize ne

Advances in Sodium Channel Modulation in Epilepsy Therapy: Focus on Eslicarbazepine, Lacosamide and Cenobamate.

Background/Objectives: Voltage-gated sodium channels (VGSCs) are among the most important molecular targets in epilepsy therapy. Unlike classical antiseizure medications (ASMs), newer sodium channel modulators selectively affect slow inactivation or persistent sodium currents, potentially improving seizure control while preserving physiological neuronal activity. This review summarizes the pharmacology, mechanisms of action, clinical efficacy, and therapeutic potential of eslicarbazepine acetate, lacosamide, and cenobamate. Methods: A narrative review of published clinical trials, meta-analyse

Treatment Preferences of Physicians of Pediatric Patients with Attention-Deficit/Hyperactivity Disorder in the United States and Canada: A Discrete Choice Experiment.

INTRODUCTION: Evidence on how treatment attributes, especially adverse events, influence preferences of physicians treating pediatric patients with attention-deficit/hyperactivity disorder (ADHD) is scant. This discrete choice experiment aimed to address this evidence gap. METHODS: A web-based survey was completed by physicians treating pediatric patients with ADHD in the United States (US) and Canada recruited from a market research firm's panel (12/2024-01/2025). Treatment attributes evaluated included efficacy (improvement in ADHD symptoms) and safety (irritability, upper abdominal pain, i

Reducing Daytime Sedation While Preserving Motor Function in Parkinson's Disease Psychosis: A Case Report of Psychotropic Timing and Deprescribing.

Psychosis and behavioral dysregulation in advanced Parkinson's disease (PD) are commonly managed with psychotropic medications that may worsen motor symptoms or contribute to excessive daytime sedation. We describe a 77-year-old male with advanced PD and psychosis admitted following agitation-related self-removal of a percutaneous endoscopic gastrostomy (PEG) tube. Collateral history revealed persistent morning somnolence that limited participation in physical therapy and daily care activities. His medication regimen included multiple agents with sedating properties, raising concern for cumula

Medications for Opioid Use Disorder, Opioid Withdrawal, and Opioid Overdose: A Review.

IMPORTANCE: Opioid use disorder (OUD) involves compulsive opioid use that causes substantial distress or impairment at work, school, or home. OUD, which is the third most prevalent substance use disorder worldwide, affected an estimated 3.7% of US adults (9 367 000) in 2022 and caused 53 774 deaths in the US in 2024. Medications for opioid use disorder (MOUD) reduce morbidity and mortality. Individuals with OUD also benefit from medications to treat opioid withdrawal and reverse acute opioid overdose. OBSERVATIONS: Methadone, buprenorphine, and naltrexone are medications approved by the US Fo

Periodic catatonia in the elderly: A case series highlighting the diagnostic challenge between delirium and catatonia.

Differentiating catatonia from delirium in elderly patients presents significant diagnostic challenges due to overlapping clinical features. This case series describes three elderly patients with periodic catatonia characterized by alternating states of stupor and agitation. Specific features, such as negativism and refusal to eat, were pivotal in distinguishing catatonia from delirium. Febrile episodes in these cases appeared to exacerbate symptoms, suggesting a potential role for systemic inflammation and cortico-striato-thalamo-cortical (CSTC) circuit dysfunction in the pathophysiology.
