Is bigger better? Analysis of efficacy and safety of lemborexant 5 versus 10 mg in insomnia patients.

OBJECTIVE: To assess the rationale for using lemborexant 10 mg as a starting dose or for those with partial response to 5 mg for the management of insomnia. METHODS: A review of MEDLINE, regulatory reports, and Canadian Agency for Drugs and Technologies in Health data was conducted. KEY FINDINGS: Lemborexant 10 mg as an initial dose offers minimal improvement in sleep metrics over 5 mg but increases somnolence risk. No evidence supports dose escalation for partial responders to 5 mg. CONCLUSION: Lemborexant 10 mg is generally not advisable as an initial or escalated dose due to higher harm

The efficacy and safety of duloxetine in treating refractory chronic cough: a randomized clinical trial.

BACKGROUND: Enhanced cough sensitivity is proposed as a potential cause for refractory chronic cough (RCC), and modulation of sensory nerve hyperresponsiveness is suggested as an effective treatment. However, the treatment of RCC has considerable potential for enhancement, particularly in terms of the targets and side effects. We aimed to investigate the efficacy and safety of duloxetine, a selective 5-HT and norepinephrine reuptake inhibitor, in patients with RCC. METHODS: This is a prospective, randomized, double-blind trial. RCC patients without mood disorders in the Tongji Hospital of Ton

Matsupexole: A novel nonergot dopamine receptor agonist with sustained efficacy in a rat model of Parkinson's disease and limited off-target activity.

BACKGROUND AND PURPOSE: Dopamine receptor agonists, particularly targeting the dopamine D2L receptor (D2LR), have been used to treat Parkinson's disease (PD). However, valvular heart disease and somnolence, mainly caused by activating the serotonin 5-HT2B receptor (5-HT2BR) and dopamine D3 receptor (D3R), respectively, currently challenge their clinical use. Here, we aimed to develop a novel dopamine receptor agonist with superior therapeutic efficacy for PD, while minimising adverse reactions. EXPERIMENTAL APPROACH: Matsupexole, a novel nonergot dopamine receptor agonist, was evaluated by ag

Severe respiratory syncytial virus bronchiolitis complicated by profound hyponatremia and seizures in an infant: a case report.

BACKGROUND: Respiratory syncytial virus (RSV) is the leading cause of bronchiolitis in infancy and the most frequent reason for hospitalization in the first months of life. While most infections follow a mild course, a proportion of infants develop severe disease requiring intensive care. Electrolyte disturbances, especially hyponatremia due to syndrome of inappropriate antidiuretic hormone secretion (SIADH), are a recognized complication and may trigger neurological symptoms. Severe hyponatremia with seizures is rarely documented in otherwise healthy term infants, which makes such cases clini
