Potentiation of opioid analgesia by psychostimulant drugs: a review.

Recent research has investigated drug combinations that enhance the analgesic effectiveness of their component substances. Many studies have examined the combination of opioids and psychostimulant drugs, such as amphetamine and methylphenidate. Despite the positive results reported in the literature, this combination is rarely used in clinical practice. The purpose of this paper is to review the literature on the opioid-amphetamine combination. Experiments with animal and human subjects provide convincing evidence that d-amphetamine or methylphenidate potentiate the analgesic effects of morphi

[Treatment of cancer pain: the role of co-analgesics].

Opioid analgesics are widely acknowledged as the most important drugs for the treatment of chronic cancer pain. Although these drugs can in most cases control severe pain, even when they are used appropriately, they may produce new symptoms or exacerbate preexisting symptoms, most notably nausea and somnolence. The combination of severe pain, anorexia, chronic nausea, asthenia, and somnolence is a frequent finding in patient with advanced cancer. An adjuvant drug should meet at least one of the following criteria: 1) to increase the analgesic effect of opioids; 2) to decrease their toxicity; 3

[Neurosomnology: in the cutting-edge of neurology].

The surnames that honor the title of this work belong to individuals fascinated by the function of sleep and its integration in neurological illness. Sleep is a function of the brain exhibiting changes with age that are predictable. There are two major manifestations of sleep alteration: somnolence and insomnia. A rich variety of sleep alterations is associated with numerous neurological disorders. Some occur acutely like in stroke whereas others appear gradually like those associated with Parkinson's disease and the dementias. The pathology of sleep is a new area of involvement in the scope o

[Excessive daytime somnolence in the adult population of Warsaw].

We studied prevalence of excessive daytime somnolence (EDS) in a representative cohort of people living in Warsaw, aged 38-67y. Forty eight items questionnaire concerning sleep habits and symptoms including Epworth Sleepiness Scale assessing EDS was mailed twice to 1503 subjects. The response rate was 79%. The average sleep time on working days was 7.1 +/- 1.1h and 8.1 +/- 1.3h on week-ends. Moderate DS was found in 21% and severe DS in 7.5% of the studied group. 0.4% of studied subjects admitted falling asleep while driving a car. These figures are comparable to that reported in recent litera

A large open-label study of venlafaxine in depressed outpatients by community-based physicians.

BACKGROUND: Studies to date suggest that venlafaxine is effective, well tolerated, and safe in a broad spectrum of patients. We examined the clinical utility and tolerability of venlafaxine in patients treated by community-based psychiatrists and family physicians in a naturalistic clinical setting. METHOD: Nineteen physicians each recruited 10 to 20 physicians to enroll 5 patients each maximum, diagnosed with DSM-IV major depression or dysthymia. The patients were at least moderately ill (Clinical Global Impressions) with a score of at least 32 on the Zung Self-Rating Depression Scale. After

Treating repetitive seizures with a rectal diazepam formulation: a randomized study. The North American Diastat Study Group.

OBJECTIVE: To evaluate the effectiveness and safety of a single-dose treatment for acute repetitive seizure (ARS) episodes (e.g., clusters) administered in a nonmedical setting by caregivers. BACKGROUND: Patients with epilepsy may experience ARS episodes despite optimal anticonvulsant treatment. Such episodes require rapid treatment as medical emergencies. Typically, the patient is treated in an emergency medical setting with i.v. medication by trained medical personnel. METHODS: The authors undertook a multicenter, randomized, parallel, double-blind study of a single administration of Diast

Transient extreme spindles in a case of subacute Mycoplasma pneumoniae encephalitis.

We describe a 4-year-old boy with delirium including somnolence, irritability, agitation and visual hallucination, associated with Mycoplasma pneumoniae (MP) encephalitis. The MP encephalitis was diagnosed from increased MP-antibody (> 1:1280). Electroencephalography during sleep revealed continuous 9 Hz fast wave activity over the bilateral frontocentral regions, which was thought to represent extreme spindles. These extreme spindles were in parallel with the degree of delirium. Transient extreme spindles have not previously been reported in MP or other encephalitis.

Daytime sleepiness: when is it normal? When to refer?

Most disorders that cause daytime sleepiness can and should be identified and treated. Physicians should recognize that excessive daytime sleepiness is a symptom with serious consequences, including higher risk of accidents and, in the case of obstructive sleep apnea, hypertension, stroke, myocardial infarction, and death. An algorithm for office-based evaluation, indications for further testing, and sleep lab testing methods are described.

[Hypersomnia. Sleep apnea syndrome and somnolence].

A RECENTLY DEFINED SYNDROME: Sleep apnea syndrome is defined as 5 episodes of apnea or 10 episodes of hyopnea per hour of sleep; the cardinal symptom is excessive diurnal somnolence. Two factors could explain sleep apnea syndrome: hypoxemia and fragmented sleep. ROLE OF HYPOXEMIA: Early studies focused on hypoxemia, but few took into account the effect of subjective complaints of somnolence. Further investigations based on objective measurements introduced a certain degree of complexity distinguishing between sleepiness and the capacity for voluntary vigilance. FRAGMENTARY SLEEP: More recent w

[Hypersomnia. Investigational strategy for evaluating hypersomnia].

CLINIC AND HISTORY: Expression of hypersomnia can take on several forms: long period of nocturnal sleep, excessive diurnal somnolence or both. History taking is essential. Useful tools include standardized questionnaires and a diary of sleeping habits. Not only do they provide evidence of the hypersomnia, but also information on possible eccentric origin(s). A pathological hypothesis can then be put forward: sleep apnea syndrome, narcolepsy, idiopathic hypersomnia, or periodic movements during sleep. POLYGRAPHY: Different recording protocols have been designed to determine sleep patterns, ofte

Promoting alertness with a short nap during a night shift.

The use of a short (< 1 h) nap in improving alertness during the early morning hours in the first night shift was examined under laboratory conditions. The study contained four experimental, non-consecutive night shifts with a nap of either 50 or 30 min at 01.00 or 04.00 hours. An experimental night shift without a nap served as a control condition. Each experimental shift was followed by daytime sleep. Fourteen experienced male shift workers went through all of the experimental conditions. The results showed that the naps improved the ability to respond to visual signals during the second hal

[Sleep-arousal dissociation in a case of hypersomnia].

INTRODUCTION: Dissociated sleep-arousal states are clinical and experimental phenomena which represent mixed forms of the three stages of this cycle (arousal, NREM sleep and REM sleep). CLINICAL CASE: We describe the case of a man who presented with a history of excessive diurnal somnolence for the previous 5 years. He also had symptoms of sleep paralysis, hypnagogic hallucinations, automatic behavior and excessive movements during sleep. He had had no episodes of loss of muscle tone; MR was normal; HL DR2 and DQwl antigens were negative; two polisomnographic and a Multiple Latency Sleep test

A feasibility study of roquinimex (Linomide) and alpha interferon in patients with advanced malignant melanoma or renal carcinoma.

Thirty-one patients with advanced renal carcinoma or malignant melanoma were treated in the first feasibility study of alpha-interferon (Roferon) and the new oral immunomodulating agent, Linomide. Linomide 5 mg or 10 mg p.o. daily was given for 2 weeks; alpha-interferon was then added at 3 MU s.c. three times weekly, escalating in each patient by 3 MU per week, if tolerable, up to 12 MJ. The combination was poorly tolerated with nausea, vomiting, somnolence and myalgia commonly reported. Adverse events accounted for treatment withdrawal in ten patients and contributed to withdrawal in four oth

Comparison of the tolerability and efficacy of citalopram and amitriptyline in elderly depressed patients treated in general practice.

The enhanced sensitivity of the elderly to the side effects produced by tricyclic antidepressants (TCAs), and the frequency and type of adverse events, have made the treatment of depression in this group difficult. The selective serotonin reuptake inhibitors (SSRIs) have been reported to produce significantly fewer undesirable side effects and display better tolerance than TCAs. We compared the therapeutic actions and side effects produced by citalopram, the most selective SSRI available, with amitriptyline in a group of elderly patients (aged 65 and older) diagnosed with major depression.
