Associations between household chaos and appetitive traits in preschoolers and preadolescents.

INTRODUCTION: Prior research suggests that household chaos may relate to greater childhood obesity risk, though mechanisms are unclear. This study aimed to evaluate the cross-sectional associations between household chaos and appetitive traits in 92 preschoolers (Study A) and 184 preadolescents (Study B). METHODS: Parents completed the Confusion, Hubbub, and Order Scale and the Child Eating Behavior Questionnaire. Eating in the absence of hunger (EAH) was assessed in the lab. Data were collected at baseline and at either 6-month (Study A) or 1-year (Study B) follow-up. Associations between ho

Frontline realities in South African disaster response: Social workers' perspectives on practice challenges.

UNLABELLED: Social workers increasingly integrate disaster work into their practice, recognising it as a developing field within social work. For years, social workers have provided promotive, preventative and treatment services during disasters to ensure community well-being. The increasing demand for social workers in disaster management is driven by a world that is becoming more disaster-prone, requiring effective responses to mitigate their impacts. However, various challenges hinder the effective delivery of disaster social work services. Based on the latter, this article explores the mul

Increasing the visibility of delirium as a hospital acquired complication: A consumer co-designed, nurse-led quality improvement program in a quaternary intensive care unit.

BACKGROUND: Delirium is common in critically ill patients but is frequently under-recognised, inconsistently documented, and poorly explained to families. In our 48-bed intensive care unit (ICU), baseline chart review identified probable delirium in 65% of sampled admissions, yet the term "delirium" was documented in only 23% of medical records. OBJECTIVES: The objective of this study was to increase the visibility of delirium as a hospital-acquired complication through reliable screening, explicit interdisciplinary documentation, and consumer co-designed family engagement. METHODS: We condu

Artificial Intelligence for Gadolinium-Sparing Detection of Active Multiple Sclerosis Lesions: A Structured Narrative Review.

INTRODUCTION: Gadolinium-enhanced T1-weighted magnetic resonance imaging (MRI; T1c+) remains the reference standard for identifying active inflammatory lesions in multiple sclerosis (MS). However, cumulative exposure raises concerns regarding tissue deposition, cost, scan duration, and patient burden. Artificial intelligence (AI) has emerged as a potential strategy to reduce reliance on contrast administration. METHODS: We conducted a structured narrative review of studies evaluating AI-based approaches for detecting active MS lesions without routine gadolinium use. Eligible studies included

Feasibility and candidate risk factors for postoperative delirium in older ward patients: a prospective pilot study.

BACKGROUND: Postoperative delirium (POD) is a frequent neuropsychiatric complication after surgery, characterized by acute disturbances in attention, awareness, and cognition. This pilot study assessed the feasibility of a multidimensional assessment protocol and investigated candidate risk factors for POD in older patients undergoing elective surgery. METHODS: This prospective, observational, non-interventional, single-center pilot study enrolled 150 patients aged ≥ 65 years undergoing elective surgery lasting ≥ 60 min. Patients with dementia, neurological disorders (documented diagnoses), o

Postoperative Delirium and the Association to Continuously Monitored Severe Deviations of Vital Signs at the General Ward After Major Abdominal Surgery.

BACKGROUND: Delirium is the most frequent neurological complication after surgery and is associated with increased postoperative morbidity and mortality. The underlying pathophysiology is not fully understood, but is suggested to be multifactorial, including hypoperfusion and hypoxia; however, the role of deviating vital signs prior to postoperative delirium (POD) remains unclear. This study aimed to examine the association between severe physiological deviations detected by continuous wireless monitoring and the occurrence of POD within the first three postoperative days after major abdominal

Shared molecular mechanisms between colorectal cancer and cardiovascular disease drive perioperative neurological vulnerability.

BACKGROUND: Colorectal cancer (CRC) and cardiovascular disease (CVD) share overlapping molecular pathogenic axes-including chronic systemic inflammation, immune dysregulation, metabolic reprogramming, and cellular senescence-that collectively amplify perioperative neurological vulnerability in elderly patients. Postoperative delirium (POD), the most prevalent acute neuropsychiatric complication following major oncological surgery, emerges at the intersection of these shared pathogenic pathways: tumor-driven neuroinflammation, anesthesia-induced cerebral hypoperfusion, cardio-metabolic derangem

When Stroke Is Not a Stroke: A Case Report of Crimean-Congo Hemorrhagic Fever Presenting With Focal Neurological Deficits.

Crimean-Congo hemorrhagic fever (CCHF) is a potentially fatal tick-borne viral zoonosis with a wide geographic distribution, a significant case-fatality rate, and a risk of healthcare-associated transmission. Although CCHF commonly presents with nonspecific febrile illness followed by hemorrhagic manifestations, neurological symptoms may occur and can mimic acute stroke, creating diagnostic challenges in the emergency department. We report the case of a 72-year-old male farmer in the United Arab Emirates who presented with acute slurred speech, confusion, and right-sided facial droop, promptin

Haemophagocytic lymphohistiocytosis triggered by Kikuchi-Fujimoto disease: a diagnostic challenge.

This report describes a gentleman, in his 40s, presenting with 3 weeks of fever, confusion, widespread erythematous crusted rash, weight loss and generalised lymphadenopathy. Secondary haemophagocytic lymphohistiocytosis (HLH) was diagnosed based on established criteria.Treatment with anakinra and dexamethasone (after lymph node biopsy) was initiated promptly. CT demonstrated extensive lymphadenopathy, raising concern of an underlying lymphoma. Autoimmune serology was positive but not diagnostic for a specific rheumatological disorder. Ultrasound-guided core needle lymph node biopsy ultimately

Land Use/Land Cover Classification and Its Variability Along the Jiangsu Coast Based on Pearson-SHAP-RFE Feature Selection from Landsat Imagery.

The Jiangsu coastal zone has experienced substantial land use/land cover (LULC) changes under intensified anthropogenic activities, while spectral confusion caused by land-sea interactions increases the difficulty of accurate LULC classification. This study investigated LULC dynamics in the Jiangsu coastal zone in 2000, 2008, 2016, and 2024 using Landsat imagery and the Google Earth Engine platform. A total of 33 features, including spectral bands, spectral indices, texture, topographic variables, and nighttime light data, were extracted. To improve classification accuracy and model generaliza

Beyond egalitarian societies: Egalitarianism as a human universal.

In the Target Article, we argued that "egalitarian societies" are real, but that outcome-oriented, society-level taxonomies have caused confusion and disagreement. After showing that inequalities exist in even the most egalitarian societies, we called for greater research emphasis on processes underlying egalitarianism under a range of ecological/economic conditions. Thirty commentaries discuss, extend or, in several cases, query elements of our thesis. In this response, we address each commentary in turn, focussing on four unifying themes: (1) gendered inequalities; (2) outcome vs process; (3

UHMS Position Statement: Standardization of Terminology in Undersea and Hyperbaric Medicine: Adoption of "Hyperbaric Oxygen Treatment".

Hyperbaric medicine terminology has historically lacked standardization, with multiple terms and abbreviations used interchangeably across clinical practice, scientific publications, educational materials, and public communications. Inconsistent nomenclature may contribute to confusion regarding the definition, regulation, clinical standards, and physician oversight associated with hyperbaric oxygen treatment. The Undersea and Hyperbaric Medical Society (UHMS) endorses the term hyperbaric oxygen treatment as the preferred terminology for clinically delivered hyperbaric oxygen under accepted UH

Perceived Barriers Among Midwives to Implementation of the Modified Partograph in a Public Hospital in Gauteng Province, South Africa.

INTRODUCTION: Partograph monitoring aims to assist midwives, obstetricians, and other healthcare professionals in a midwifery/obstetric unit to diagnose and manage labor to reduce the risk of prolonged labor, augmentation, cesarean sections, shoulder dystocia, and intrapartum stillbirths. The modified partograph is a new evidence-based labor monitoring tool to assist with assessment and decision-making during labor. Delayed implementation of the modified partograph was observed in the obstetric unit of the particular public hospital, while the previous partograph remained in use. It was not cl

Sequential Contralateral Large-Vessel Occlusion Following Intravenous Thrombolysis: A Case Report of Dynamic Bilateral Cerebral Arterial Thrombosis.

Acute large-vessel occlusion stroke can evolve rapidly during the hyperacute treatment period, occasionally resulting in a clinical and radiological picture that differs substantially from the initial presentation. We report an unusual case of sequential contralateral large-vessel occlusion occurring within approximately three hours. A patient presented with transient dysphasia that resolved rapidly. While awaiting stroke assessment, he developed confusion, left-sided weakness, and dysarthria. Initial imaging demonstrated a distal right middle cerebral artery (MCA) occlusion with a National In

Spontaneous resolution of cerebrospinal fluid obstruction due to intraventricular hemorrhage: A case report.

Obstructive hydrocephalus is a serious and potentially life-threatening condition caused by the temporary blockage of cerebrospinal fluid (CSF) flow. Most cases require urgent intervention as spontaneous resolution is very uncommon. We present the case and brain imaging of a 63-year-old man who presented to the emergency department with complaint of headache, nausea, confusion, and dysarthria. Initial imaging showed a left caudate bleed with intraventricular hemorrhage and mild hydrocephalus secondary to obstruction of CSF in the aqueduct of Sylvius. External ventricular drain (EVD) was deferr

Psychological experiences of midwives during their first involvement in care following intrauterine foetal death: A mixed methods study in Japan.

BACKGROUND: Providing care following intrauterine foetal death (IUFD) can be emotionally challenging for midwives, particularly during early professional practice. However, little is known about how midwives experience and cope with their first involvement in IUFD care. AIM: To examine midwives' retrospective accounts of their first IUFD care experience within the first 3 years of practice and explore their coping processes. METHODS: A convergent mixed methods design was used. Forty-one midwives in Japan completed questionnaires assessing emotional responses to their first IUFD care and prof

Immunosenescence in tumor progression: from synergistic mechanisms to therapeutic strategies.

Tumor-associated immunosenescence is not a decline in immune function, but rather a state of immune ecological imbalance. It drives tumor progression through multi-dimensional mechanisms, including decreased immune surveillance, chronic inflammation, accumulation of suppressive cells, remodeling of TME, metabolic disorders, and epigenetic regulation. The challenges in immunosenescence research include conceptual confusion, an imbalance in research perspectives, the limitations of preclinical models, and the translational hurdles. This review summarizes the latest research findings on the role

Marked Variability in Lipid Profile Reporting Among Australian Pathology Providers: A National Cross-Sectional Review of Low-Density Lipoprotein Cholesterol.

BACKGROUND: Lipid profiles facilitate cardiovascular risk stratification, therapeutic monitoring, and patient education. Variability in pathology reporting practices may generate confusion among clinicians and patients. While there is a central accreditation body for the provision of pathology in Australia, reporting formats are not regulated. This study aimed to characterise the variation in reporting of low-density lipoprotein cholesterol (LDL-C) levels among Australian pathology providers. METHOD: A cross-sectional national review was conducted in November 2024. Public and private patholog

Glioblastoma Masquerading as a Presumed Ischemic Stroke: A Case of Delusional Jealousy and Fluctuating Psychosis.

Psychiatric symptoms can be the presenting or dominant manifestation of an underlying brain tumor, and this phenomenon is frequently misattributed to a primary psychiatric disorder, particularly when a plausible alternative explanation, such as a recent stroke or a known alcohol use disorder, is already present. We report a case in which glioblastoma was ultimately identified as the cause of a subacute psychotic syndrome that had initially been attributed to a preceding cerebrovascular diagnosis. A 64-year-old man with a history of alcohol use disorder was admitted to Internal Medicine with a
