Loss of Consciousness After Naltrexone Implantation: A Case Report.

Naltrexone is an μ opioid receptor antagonist that is used in alcohol and opiate use disorder. Naltrexone does not constitute tolerance and dependence, and cessation of the drug does not cause withdrawal symptoms. Sustained release form of naltrexone has been developed due to patient compliance issues. There is currently only one sustainedrelease form available in Turkey, which is inserted subcutaneously. In this case report, we present, a probable serious side effect of sustained release naltrexone implant. A 36 years old male with alcohol use disorder, developed a sudden clouding of consciou

Drawing promotes memory retention in a patient with sleep-related anterograde amnesia.

Drawing is a powerful tool to enhance memory in healthy participants and patients with probable dementia. Here, we investigated whether the drawing effect could extend to patient CT, a young woman with severe anterograde amnesia. Following surgery for a midline tumor involving her septum pellucidium and extending down into her fornices bilaterally, CT experienced a severe case of sleep-related amnesia. She can remember information encountered throughout the day, but when waking up in the morning or following a nap she forgets information learned prior to sleep. Here, we tested CT and 21 age-ma

The role of executive functions in long-term memory: case report.

The role of executive functions in long-term memory has been studied. We describe a single-case study, consisting of a 45-year-old male patient, hospitalized for right frontal stroke. After the stroke, the patient had memory alterations in everyday activities. However, performance in short-term memory tests was not significantly altered. Long-term memory assessments included pre- and post-stroke episodic, semantic, and procedural memories. Specific skills involved in the acquisition of new learning (auditory-verbal and visual reproduction) were also evaluated, as well as executive functions. T

γ-Aminobutyric acid type A receptor β1 subunit gene polymorphisms are associated with the sedative and amnesic effects of midazolam.

Midazolam is widely used for intravenous sedation. However, wide interindividual variability is seen in the sensitivity to midazolam. The association between genetic factors and interindividual differences in midazolam sensitivity remains unclear. The present study explored the association between common genetic variants and sedative and amnesic effects of midazolam. This prospective study included patients who were scheduled to undergo dental procedures under intravenous sedation. The sedative effect was evaluated using the Ramsay sedation scale 5 min after midazolam (0.05 mg/kg) administrati

Analysis of Clinical Utility of Functional MRI in Neurosurgical Decision-Making in Focal Epilepsy.

BACKGROUND: Functional MRI (fMRI) has proven valuable in presurgical planning for people with brain tumors. However, it is underutilized for patients with epilepsy, likely due to less data on its added clinical value in this population. We reviewed clinical fMRI referrals at the QEII Health Sciences Center (Halifax, Nova Scotia) to determine the impact of fMRI on surgical planning for patients with epilepsy. We focused on reasons for fMRI referrals, findings and clinical decisions based on fMRI findings, as well as postoperative cognitive outcomes. METHODS: We conducted a retrospective chart

Hippocampus supports long-term maintenance of language representations: Evidence of impaired collocation knowledge in amnesia.

Traditional systems consolidation theories of memory suggest that the role of the hippocampus in maintaining memory representations diminishes over time, with learned information eventually becoming fully independent of the hippocampus. Knowledge of collocations in one's native (L1) language are acquired during development and are solidly acquired by adulthood. Remote semantic knowledge of collocations might therefore be expected to be resistant to hippocampal pathology. Patients with hippocampal damage and severe anterograde amnesia completed two tasks testing English collocation knowledge or

Human retrograde amnesia and memory consolidation.

This paper reports a reassessment of published literature on the question of whether retrograde amnesia data from patients with severe trauma supports the idea that there is ongoing consolidation of long-lasting memories. That is, memory consolidation continues for decades with older memories being increasingly consolidated, and, thus, more protected from forgetting. Our analysis was limited to patients with specific traumas rather than neurodegenerative conditions that can be complicated by the additional presence of significant anterograde amnesia. These constraints were used because trauma

Personalized high-definition transcranial direct current stimulation improves cognition following carbon monoxide poisoning induced amnesia: A case report.

OBJECTIVE: High-definition transcranial direct current stimulation (HD-tDCS) has the potential to improve cognitive functioning following neurological injury and in neurodegenerative conditions. In this case report, we present the first use of HD-tDCS in a person with severe anterograde amnesia following carbon monoxide poisoning. METHOD: The participant underwent two rounds of HD-tDCS that were separated by 3 months (Round 1 = 30 sessions; Round 2 = 31 sessions). We used finite element modeling of the participant's structural MRI to develop an individualized montage that targeted multiple br

Damaged Relay Station: EEG Neurofeedback Training in Isolated Bilateral Paramedian Thalamic Infarct.

Stroke is a major public health concern and leads to significant disability. Bilateral thalamic infarcts are rare and can result in severe and chronic cognitive and behavioral disturbances-apathy, personality change, executive dysfunctions, and anterograde amnesia. There is a paucity of literature on neuropsychological rehabilitation in patients with bilateral thalamic infarcts. Mr. M., a 51 years old, married male, a mechanical engineer, working as a supervisor was referred for neuropsychological assessment and rehabilitation with the diagnosis of bilateral paramedian thalamic infarct after s

Anterograde versus Retrograde Effects of Damage to Identified Learning and Memory Systems during Acquisition, Retention, and Re-Acquisition of an Instrumental Visual Discrimination Task: Dorsal Striatum, Perirhinal Cortex, and Hippocampus.

BACKGROUND: The goal of these experiments was to determine which learning and memory system(s) were necessary for the retention of visual discriminations and subsequent acquisition of a second problem. The dorsal striatum should be involved in the acquisition and expression of this task based on previous work implicating this region in instrumental learning and memory processes. The perirhinal cortex has been implicated in learning and memory processes associated with visual information like objects, and pictures and may also play a role in the acquisition and/or retention of visual discrimina

Anterograde amnesia recurrence in temporal lobe epilepsy with amygdala-enlargement.

Temporal lobe epilepsy (TLE) can cause different types of memory impairments. Here, we report a case of immediate improvement of memory impairment following antiepileptic drug (AED) treatment in a patient with TLE with amygdala enlargement (TLE-AE), who rapidly developed recurrence. The patient was a man in his 60s whose family members complained of his amnesia. Neuropsychological investigations detected obvious recent and remote memory loss and executive function impairments. Our examinations revealed evidence of TLE and bilateral amygdala enlargement without any results suggesting organic di

Acute amnestic syndrome in fornix lesions: a systematic review of reported cases with a focus on differential diagnosis.

INTRODUCTION: Acute amnestic syndrome is an uncommon clinical presentation of neurological disease. Differential diagnosis encompasses several syndromes including Wernicke-Korsakoff and transient global amnesia (TGA). Structural lesions of the fornix account for a minority of cases of acute amnestic syndromes. Etiology varies from iatrogenic injury to ischemic, inflammatory, or neoplastic lesions. A prompt diagnosis of the underlying pathology is essential but challenging. The aim of this review is to systematically review the existing literature regarding cases of acute amnestic syndrome asso

Transient global amnesia and hippocampal diffusion restriction: an overlooked radiological finding.

Transient global amnesia (TGA) is characterized by isolated sudden anterograde amnesia. Diffusion restriction can be observed in the hippocampus on DWI-MRI at varying rates in TGA patients. This study analyzes the prevalence and characteristics of the hippocampal diffusion restriction (HDR), its relationship with vascular risk factors, and the prevalence of lesions overlooked in routine reports. 91 patients diagnosed with TGA at a tertiary hospital between 2011 and 2022 were evaluated retrospectively. The mean age was 64.8 ± 7.3 years, and 63.7% of patients were female. 75.8% of the patients h

A Rare Case of Exercise-Induced Transient Global Amnesia as an Exclusive Presenting Symptom of Hippocampus Infarct.

Transient global amnesia (TGA) is a rare condition characterized by a temporary loss of the ability to form new memories. Retrograde episodic memory loss may also occur but to a lesser extent. Although TGA is generally benign, its sudden onset and similarity to more dangerous conditions like transient ischemic attack (TIA) or cerebral vascular accident (CVA) can be concerning. We present the case of a 70-year-old female who experienced confusion and general memory loss after a vigorous workout on her stationary exercise bike. After displaying considerable amnestic symptoms, she was admitted to

[Transient global amnesia - benign memory blackout].

Transient global amnesia (TGA) is a typical clinical syndrome characterized by acute, predominantly anterograde amnesia. New epidemiological data assume a significantly higher annual incidence than previously assumed, namely around 15 cases per 100,000 people. Those affected, usually over the age of 50, cannot remember new memory content for longer than 30-180 seconds and therefore ask repetitive questions about current events. All other cognitive functions are unimpaired, and everything previously learnt, e.g. driving or cooking, can be carried out. The episodes are self-limiting and by defin

A Dive Into Oblivion: A Case of Transient Global Amnesia.

Transient global amnesia (TGA) is an uncommon neurologic disorder that consists of a sudden and temporary loss of memory, both present and past. Its causes and risk factors are not well known. We describe a case of a 58-year-old woman who was brought to the emergency department (ED) with sudden onset loss of memory and disorientation after a dive in the ocean. She presented memory deficits with incapacity to retain new memories and amnesia for the previous 24 hours. All exams ordered were normal, including computed tomography of the brain and laboratory analysis. After six hours of close monit

Transient global amnesia after the right temporal epilepsy surgery: A case report.

Transient global amnesia (TGA) is characterized by sudden and temporary memory impairment, while transient epileptic amnesia (TEA) represents amnestic attacks as the main manifestation of focal epilepsy with presumed temporal origin. We present a 48-year-old patient who experienced transient amnesia 10 weeks after right selective amygdalo-hippocampectomy for right temporal lobe epilepsy. Despite TEA being a plausible explanation for amnesia in patients with temporal lobe epilepsy, no epileptiform discharges were found during the amnestic episode and key features indicative of TGA, including lo

A Case With Bilateral Hippocampal Infarction Resembling Transient Global Amnesia.

Transient global amnesia (TGA) is a benign and transient condition with a sudden short-term amnesia. One of the conditions resembling TGA is hippocampal infarction, which requires relapse prevention treatments. In this report, we present a case with bilateral hippocampal infarction in whom distinguishing these two conditions was difficult for up to 1 week from the onset. A 60-year-old female visited our hospital with sudden onset retrograde and anterograde amnesia. Thin-slice magnetic resonance imaging (MRI) with 2-mm thickness revealed hyperintense signals on diffusion-weighted imaging (DWI)

Transient global amnesia predisposed by rizatriptan and temazepam.

Transient global amnesia (TGA) is a perplexing neurological condition primarily affecting older adults, characterised by the acute onset of anterograde amnesia. Patients remain oriented to self and symptoms resolve within 24 hours. Before diagnosing TGA, conditions such as transient ischaemic attack and stroke must be excluded. We present a case of a woman in her 60's who experienced acute onset memory impairment and confusion after taking rizatriptan for migraine and temazepam for insomnia the previous night. Initially treated as an acute ischaemic stroke with tissue plasminogen activator, a
