Hippocampal ischaemia from accidental 5-Hydroxytryptophan (5-HTP) overdose case report.

We report severe reversible hippocampal ischaemia following an accidental 5-Hydroxytryptophan (5-HTP) overdose. Serious consequences from 5-HTP overdose have not been reported. A 44-year-old previously well man ingested ten times the recommended dose of 5-HTP powder. After four hours he developed marked antegrade and retrograde amnesia, disorientation and confusion in the absence of loss of consciousness, seizure activity or features of serotonin toxicity. Magnetic resonance imaging (MRI) of the brain on day two revealed extensive symmetrical restricted diffusion bilaterally in the hippocampi,

Randomly fluctuating neural connections may implement a consolidation mechanism that explains classic memory laws.

How can we reconcile the massive fluctuations in neural connections with a stable long-term memory? Two-photon microscopy studies have revealed that large portions of neural connections (spines, synapses) are unexpectedly active, changing unpredictably over time. This appears to invalidate the main assumption underlying the majority of memory models in cognitive neuroscience, which rely on stable connections that retain information over time. Here, we show that such random fluctuations may in fact implement a type of memory consolidation mechanism with a stable very long-term memory that offer

Deriving testable hypotheses through an analogy between individual and collective memory.

The focus of this highly interdisciplinary essay is on an analogy between the formation of memory in individual brains and in collectives of individuals, and on the use of that analogy to derive two hypotheses. The first hypothesis involves the application of understanding of retrograde amnesia in individual brains, to the phenomenon of amnesia in collectives. The second hypothesis involves the application of observations of competition in the formation of collective memory, to memory formation in individuals. Evidence in support of both hypotheses is presented. This analogical reasoning leads

The Retrograde Memory for News Events Test (RM-NET) and the relationship between news event memory and performance on standard neuropsychological tests.

Novel tests of semantic memory (SM)-for example, memory for news events (NE; news facts) or famous personalities-are useful for estimating the severity of retrograde amnesia. Individuals with mild cognitive impairment exhibit relatively intact SM/language on traditional neuropsychological tests but exhibit consistent impairment on novel tests of SM, suggesting novel SM tests are dissimilar from traditional SM tests. To identify the relationship between NE memory and traditional cognitive measures, older adults (N = 51) completed a traditional neuropsychological battery and the Retrograde Memor

Cognitive trajectories during and after electroconvulsive therapy in patients with MDE: Taking different perspectives.

Cognitive function during an ECT care pathway is mainly investigated at the group level by analyzing mean cognitive test scores over time. However, there are important inter-individual differences, with some patients experiencing residual invalidating cognitive deficits. This study provides a nuanced examination of cognitive functioning during and after ECT by combining three approaches for data analysis. A cognitive test battery was assessed in seventy-three ECT-treated patients with a Major Depressive Episode (MDE) at up to five time points (baseline, immediately prior to the third session a

A model of bi-directional interactions between complementary learning systems for memory consolidation of sequential experiences.

The standard theory of memory consolidation posits a dual-store memory system: a fast-learning fast-decaying hippocampus that transfers memories to slow-learning long-term cortical storage. Hippocampal lesions interrupt this transfer, so recent memories are more likely to be lost than more remote memories. Existing models of memory consolidation that simulate this temporally graded retrograde amnesia operate only on static patterns or unitary variables as memories and study only one-way interaction from the hippocampus to the cortex. However, the mechanisms underlying the consolidation of epis

Pathological Slow-Wave Activity and Impaired Working Memory Binding in Post-Traumatic Amnesia.

Associative binding is key to normal memory function and is transiently disrupted during periods of post-traumatic amnesia (PTA) following traumatic brain injury (TBI). Electrophysiological abnormalities, including low-frequency activity, are common following TBI. Here, we investigate associative memory binding during PTA and test the hypothesis that misbinding is caused by pathological slowing of brain activity disrupting cortical communication. Thirty acute moderate to severe TBI patients (25 males; 5 females) and 26 healthy controls (20 males; 6 females) were tested with a precision working

First Pediatric Case of Autoimmune Encephalitis Associated With COVID-19 in Costa Rica.

Very few COVID-19-associated autoimmune encephalitis cases have been documented in children. This case report focuses on a previously healthy four-year-old girl who presented to the emergency room of the National Children's Hospital in Costa Rica in a postictal state due to a tonic-clonic seizure that progressed to status epilepticus. She had no previous history of fever or associated trauma. She was considered severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) positive by epidemiological linkage four weeks prior to the event, and her immunoglobulin G (IgG) levels for SARS-CoV-2 were

Evolving the SCAT5 for Ruling Out Higher-Severity Traumatic Brain Injuries-Can Decision Rules Developed for Emergency Settings Help?

BACKGROUND: Decision rules (eg, Canadian computed tomography head rule [CCHR] for adults and Pediatric Emergency Care Applied Research Network [PECARN] rule for children/adolescents) are used in emergency settings (emergency room [ER] rules) to assess traumatic brain injuries (TBIs). The rules have a high-sensitivity and near-perfect negative predictive value that help to rule out more severe TBI. CLINICAL QUESTION: Which criteria should be added to the Sport Concussion Assessment Tool 5 (SCAT5) to reach the sensitivity of the ER rules and improve the utility of the SCAT5 for screening for hig

Sevoflurane-induced amnesia is associated with inhibition of hippocampal cell ensemble activity after learning.

General anesthesia could induce amnesia, however the mechanism remains unclear. We hypothesized that suppression of neuronal ensemble activity in the hippocampus by anesthesia during the post-learning period causes retrograde amnesia. To test this hypothesis, two experiments were conducted with sevoflurane anesthesia (2.5%, 30 min): a hippocampus-dependent memory task, the context pre-exposure facilitation effect (CPFE) procedure to measure memory function and in vivo calcium imaging to observe neural activity in hippocampal CA1 during context exploration and sevoflurane/home cage session. Recovering object-location memories after sleep deprivation-induced amnesia.

It is well established that sleep deprivation after learning impairs hippocampal memory processes and can cause amnesia. It is unknown, however, whether sleep deprivation leads to the loss of information or merely the suboptimal storage of information that is difficult to retrieve. Here, we show that hippocampal object-location memories formed under sleep deprivation conditions can be successfully retrieved multiple days following training, using optogenetic dentate gyrus (DG) memory engram activation or treatment with the clinically approved phosphodiesterase 4 (PDE4) inhibitor roflumilast. Study of effect of nimodipine and acetaminophen on postictal symptoms in depressed patients after electroconvulsive therapy (SYNAPSE).

BACKGROUND: Postictal phenomena as delirium, headache, nausea, myalgia, and anterograde and retrograde amnesia are common manifestations after seizures induced by electroconvulsive therapy (ECT). Comparable postictal phenomena also contribute to the burden of patients with epilepsy. The pathophysiology of postictal phenomena is poorly understood and effective treatments are not available. Recently, seizure-induced cyclooxygenase (COX)-mediated postictal vasoconstriction, accompanied by cerebral hypoperfusion and hypoxia, has been identified as a candidate mechanism in experimentally induced se

Transient Global Amnesia.

Transient global amnesia (TGA) is a clinical syndrome characterized by anterograde amnesia, mild retrograde amnesia, and confusion up to 24 hours. Most commonly seen in patients older than 50 years, TGA results from the temporary impairment of short-term memory formation. Clinically, patients have time disorientation and often ask repeated questions regarding the day's events. Vomiting, headache, blurry vision, dizziness, and nausea may be present. A physically or psychologically stressful precipitating event, such as emotional stress, significant physical exertion, exposure to extreme tempera

Transient global amnesia: Model, mechanism, hypothesis.

Transient global amnesia (TGA) is a rare but well-described neurological disorder characterised by an acute self-limited episode of dense anterograde amnesia with retrograde amnesia of variable extent. Although the clinical phenomenology, neuropsychology, neuroimaging, prognosis, and precipitating and predisposing factors of TGA are well described, the pathogenesis is not currently understood. Existing proposals - epileptic seizure, stroke or transient ischaemic attack, migraine aura - all have shortcomings. Based on a computational neural network model of mnemonic function, a new model of TGA

A Case of Transient Global Amnesia: A Rare Diagnosis.

Transient global amnesia (TGA) is a syndrome characterized by a loss of anterograde memory with a less prominent loss in retrograde episodic memory that resolves within 24 hours or less. In this report, we present a rare case of a 62-year-old male who presented to the emergency department with sudden onset confusion and memory loss. Prior to this, the patient had no significant medical or psychiatric history. Magnetic resonance imaging (MRI) and computerized tomography (CT) showed a normal presentation, and a neurology consultation ruled out any organic brain abnormalities. After ruling out al

[Memory Loss - a Case of Sudden Amnesia].

Memory Loss - a Case of Sudden Amnesia Abstract. Transient global amnesia (TGA) is a clinical diagnosis with typical signs of an anterograde and retrograde amnesia. The underlying mechanisms are yet unknown, different hypotheses are being discussed. Ultimately there is a temporary dysfunction of the hippocampi. Consistent with this, transient uni- or bilateral punctiform hyperintense lesions may be found on DWI-MRI sequences, usually without correlation on FLAIR-weighthed MR-images. Symptoms usually resolve within twenty-four hours. There is no need for a specific therapy. A prophylactic thera

Acute-onset amnesia: transient global amnesia and other causes.

Acute-onset amnesia is a dramatic neurological presentation that can cause considerable concern to both patient and clinician. The patient typically presents with an inability not only to retain new memories but also to access previously acquired memories, suggesting disturbance of hippocampal function. Transient global amnesia (TGA) is the most common cause of acute-onset amnesia, and is characterised by a profound anterograde and retrograde amnesia that typically lasts for up to 24 hours. Although TGA has a strikingly stereotypical presentation, it can be challenging to distinguish from othe

A Case of Transient Global Amnesia Triggered by Sexual Intercourse.

Transient global amnesia (TGA) is described as a sudden onset of anterograde and retrograde amnesia. In this case report, we present a case of a 52-year-old man who came to our hospital experiencing sudden-onset confusion and memory loss an hour after engaging in sexual activity with his wife. Before this, the patient had no previous medical history. Four hours following the event, a non-contrast computed tomography (CT) scan was performed and showed no signs of any intracranial pathology. In addition, T1-weighted and T2-weighted magnetic resonance imaging (MRI) was performed 13 hours followin

Behavioral, neurological, and psychiatric frailty of autobiographical memory.

Autobiographical-episodic memory is considered to be the most complex of the five long-term memory systems. It is autonoetic, which means, self-reflective, relies on emotional colorization, and needs the features of place and time; it allows mental time traveling. Compared to the other four long-term memory systems-procedural memory, priming, perceptual, and semantic memory-it develops the latest in phylogeny and ontogeny, and is the most vulnerable of the five systems, being easily impaired by brain damage and psychiatric disorders. Furthermore, it is characterized by its fragility and pronen
