Exoscopic Resection of an Olfactory Groove Meningioma Through an Anterior Interhemispheric Approach.

Olfactory groove meningiomas are challenging lesions due to their proximity to major vascular structures and frequent frontal lobe edema or hyperostosis. Various transcranial and endoscopic approaches have been described, each with specific advantages and limitations. We present the case of a 70-year-old woman with longstanding anosmia who recently developed apathy and irritability. An olfactory groove meningioma was resected through an anterior interhemispheric approach, chosen to minimize subfrontal parenchymal traction (Video 1). Exoscopic visualization provided excellent illumination and m

Genetic Basis and Heterogeneity of Congenital Hypogonadotropic Hypogonadism.

Congenital hypogonadotropic hypogonadism (CHH) is a rare genetic disorder characterized by absent or incomplete pubertal development due to impaired production, secretion, or action of gonadotropin-releasing hormone (GnRH). When associated with anosmia or hyposmia, it is termed Kallmann syndrome. CHH exhibits striking clinical and genetic heterogeneity, encompassing either sporadic or familial cases, with inheritance patterns that include X-linked, autosomal dominant, and autosomal recessive transmission. Over the past few decades, major advances uncovered the molecular basis of CHH, shedding

FAK inhibition following acute olfactory epithelial inflammation promotes neurogenesis and functional recovery through stem cell CNTF.

The sense of smell is maintained by regenerating olfactory sensory neurons (OSNs) from basal stem cells in the olfactory epithelium (OE). Acute inflammation destroys OSNs, causing hyposmia and anosmia, but activates basal cells. Manipulation of signaling pathways to promote basal cell proliferation and neuroregeneration would reveal novel therapeutic targets for smell deficits. We found that ciliary neurotrophic factor (CNTF) from horizontal basal cells (HBCs, quiescent stem cells) promotes neuroregeneration and functional recovery following methimazole-induced acute injury. Moreover, inhibiti

Association of IL4 and IL17A genetic polymorphisms with COVID-19 severity in a southern Brazilian population.

COVID-19 has demonstrated marked differences in clinical manifestations and outcomes, with severe cases presenting excessive inflammatory responses known as cytokine storms. Polymorphisms in cytokine genes, such as interleukin-4 (IL4) and interleukin-17A (IL17A), may influence immune responses and contribute to disease severity. This study investigated the association between the IL4 -34C/T and IL17A -197G/A polymorphisms and COVID-19 severity in a population from Southern Brazil. A total of 528 individuals were included, with 157 severe and 371 mild cases. DNA was extracted from buccal swabs

Olfactory Groove Meningioma Presenting With Visual Decline and Gait Disturbance: A Case Report.

Olfactory groove meningiomas (OGMs) are typically indolent neoplasms arising from the anterior skull base and frequently attain a considerable size prior to detection because early symptoms are often vague and nonspecific. Common clinical features include personality or behavioral alterations, diminished or absent olfaction, and gradually worsening visual impairment, which are often incorrectly attributed to psychiatric conditions or neurodegenerative processes, leading to delays in diagnosis. We report the case of a 55-year-old female who presented with a seven-month history of progressive bi

Prevalence of post-COVID symptoms across variants of concern and follow-up periods: A systematic review and meta-analysis.

OBJECTIVES: The interaction between SARS-CoV-2 variants of concern (VoC) and post-COVID symptom duration remains unexplored. This is the first study to evaluate post-COVID prevalence stratified by VoC and follow-up periods. METHODS: Six databases were searched (12/2019-12/2024) for studies of adults with laboratory-confirmed SARS-CoV-2 and symptoms lasting ≥3 months. Data were stratified by VoC (Alpha through Omicron) and follow-up (<6 vs ≥6 months) to estimate pooled prevalence using random-effects models. RESULTS: Pooled prevalence across 35 studies (n = 159,000) was 28.5% (95% CI: 21.6-36

Anterior Skull Base Metastasis From Papillary Thyroid Carcinoma: A Case Report.

Distant metastasis occurs in 1% to 4% of patients with papillary thyroid carcinoma (PTC) and most commonly involves the lungs and bones, with skull base involvement being exceedingly rare. We present a case of a 58-year-old male patient who experienced skull-base metastasis of tall-cell variant PTC 4 years after multimodal treatment of aggressive disease. The patient presented to the otolaryngology clinic with left-sided postnasal drip, anosmia, ptosis, and proptosis. Magnetic resonance imaging of head showed a 1.9 × 2.7 × 4.4 cm mass with extensive invasion of the sinonasal cavities, cribrifo

Neural correlates of olfactory dysfunction: A systematic review.

Olfactory dysfunction affects over 20% of the population. Despite progress in understanding its neural pathophysiology, research remains fragmented. This systematic review synthesizes evidence of brain structural and functional measures, and their association with clinical characteristics (e.g., etiology, duration) in patients with olfactory dysfunction. This may help to identify neural correlates and potential neuroimaging biomarkers of olfactory dysfunction's severity and progression. Following PRISMA guidelines, we screened 2374 papers and included 164 studies. Structural MRI studies consis

Senescence-like cells recruit γδ T cells to drive prolonged hyposmia after SARS-CoV-2 infection in mice.

Persistent hyposmia is a hallmark of post COVID-19 conditions, yet the mechanisms sustaining olfactory dysfunction after viral clearance remain poorly understood. Here, using mouse models of SARS-CoV-2 infection, we show that virus-induced senescence-like changes in uninfected olfactory mucosal fibroblasts persist long after viral clearance and drive prolonged olfactory dysfunction. These senescence-like cells secrete SASP factors, including IFNγ, CXCL9, and CXCL11, thereby recruiting γδ T cells to the olfactory mucosa. The accumulated γδ T cells produce excessive IL-17A, which acts on IL-17 r

Generation and Differentiation of Human Olfactory Epithelial Organoids from Adult Stem Cells.

The human olfactory epithelium possesses remarkable neurogenic capacity, sustained by a resident population of basal stem cells; however, establishing a dynamic in vitro model that faithfully recapitulates this regenerative process has been proven challenging. Here, we present an established protocol for generating olfactory organoids from progenitor cells isolated from adult human superior turbinate tissue. These organoids maintain stemness, as evidenced by SOX2 expression, and display a spatially organized differentiation pattern, characterized by uniform expression of the immature neuronal

Olfactory dysfunction as a marker of neurodegenerative disease in older adults.

BACKGROUND: Early identification of neurodegenerative disorders in older adults remains a major challenge in geriatric clinical practice. Olfactory dysfunction is a common feature of several neurodegenerative disorders, particularly synucleinopathies, where it often precedes motor and cognitive symptoms, whereas in primary tauopathies olfactory function is usually preserved or only mildly impaired. AIMS: This study aimed to evaluate the diagnostic utility of the 12-item Sniffin' Sticks Test (SST-12) for identifying neurodegenerative disease in older adults. METHODS: A total of 120 individual

Evaluating methods for quantitative olfactory assessment: a comparative longitudinal analysis of Sniffin' Sticks and alternative tools.

Anosmia and hyposmia, referring to total or partial smell loss, respectively, affect 3% to 20% of people. The Sniffin' Sticks Extended Test, assessing odor threshold, discrimination, and identification (TDI), is a well-validated tool widely used in European and US clinics. However, its time and resource demands limit routine use. During the COVID-19 pandemic, several rapid smell tests were developed, yet their classification performance and agreement with established psychophysical measures remain underexplored. We compared 4 alternatives-the Visual Analog Scale for self-rated smell ability (V
