Zopiclone-associated hallucinations: a case report and database pharmacovigilance study.

AIMS: Zopiclone is widely prescribed for insomnia, but the pharmacovigilance reporting signals for hallucinations remain poorly characterized. This study combines a case report and pharmacovigilance data to evaluate the association between zopiclone and hallucinations, compare hallucination subtype signals, and examine factors associated with higher reporting odds. METHODS: We reported a clinical case assessed by the Naranjo scale and analyzed the Canadian Vigilance Adverse Reaction Database (1994-2025) for zopiclone-associated hallucinations. Three disproportionality methods (ROR, PRR, and B

The eyes and Parkinsonian syndromes.

Parkinsonian degenerative syndromes, including Parkinson disease (PD), progressive supranuclear palsy (PSP), multiple system atrophy (MSA), corticobasal degeneration (CBD), and dementia with Lewy bodies (DLB), are frequently associated with ophthalmic manifestations that contribute substantially to impaired quality of life, reduced functional independence, and increased falls risk. These visual disturbances remain under-recognised in routine neurological practice despite their diagnostic and therapeutic relevance. This review summarises the spectrum of ophthalmic manifestations across Parkinso

Reduced volume of neuromodulatory brainstem nuclei is associated with early hallucination-related vulnerability in Parkinson's disease.

Early hallucination-related phenomena, including minor visual hallucinations (mVHs), are clinically relevant features of Parkinson's disease (PD), but their neuroanatomical substrates remain unclear. Brainstem nuclei supporting sensory gating, together with cholinergic and noradrenergic dysregulation, are proposed modulators of hallucinations. The objective of the present study was to test whether PD participants with early hallucination-related phenomena show reduced volumes of brainstem nuclei with cholinergic and noradrenergic relevance, using cross-sectional analysis of baseline structural

Electroconvulsive Therapy in Cochlear Implant Users.

INTRODUCTION: Cochlear implant manufacturers currently contraindicate the use of electroconvulsive therapy (ECT) in CI users, citing theoretical evidence of potential harm to the patient or the implant despite a lack of clinical data. We report two uncomplicated cases of ECT in CI users, including the first reported case of bilateral ECT in a patient with bilateral CIs. CASE SERIES: The first case involves a 66-year-old visually impaired male with bilateral CIs. He suffered from major depressive disorder complicated by refusal of oral intake despite maximal pharmacological therapy. He underwe

Reassessing Asymmetry Reduction in Psychosis: Cingulate Folding and Gyrification Covariance in Patients with Auditory Hallucinations.

BACKGROUND AND HYPOTHESIS: Prior research links a shorter paracingulate sulcus (PCS) to hallucinations in schizophrenia, but its symmetry hemispheric specificity and relevance to bipolar disorders remain unclear. We hypothesized that reduced PCS asymmetry and interhemispheric gyrification covariance in salience and auditory networks are associated with lifetime auditory hallucinations (AH) in psychotic spectrum disorders. STUDY DESIGN: We compared patients with and without AH, and healthy controls, focusing on PCS asymmetry in five ordinal classes, sulcal length and depth, and interhemispheri

Psychiatric Nursing Intervention for Clozapine-Induced Dysphagia in Schizophrenia: A Case Report.

INTRODUCTION: Schizophrenia is a chronic mental disorder characterized by disruptions in thought processes, perceptions, emotional responsiveness, and social interactions. Clozapine, a second-generation antipsychotic, is commonly prescribed for treatment-resistant schizophrenia. However, its use is frequently associated with side effects, notably hypersalivation. This case report discusses the clinical consequences of clozapine-induced hypersalivation, emphasizing the importance of individualized nursing care planning. METHODS: The descriptive case report presents the clinical course of a 47-

Brain Structural and Functional Alteration Network Localization of Visual Hallucinations.

BACKGROUND AND HYPOTHESIS: Visual hallucinations (VH), a key symptom in neurodegenerative and psychiatric disorders, are associated with a more severe psychopathological profile and less favorable outcome. Neuroimaging research has revealed widespread brain regions involved in VH, echoing the updated notion that neuropsychiatric symptoms correspond more closely to interconnected brain networks than to single brain regions. However, there is still a dearth of studies examining brain network localization of VH. STUDY DESIGN: We initially identified brain structural and functional alterations sp

Bodily and Cognitive Experience in Patients With Sepsis and Delirium or Subsyndromal Delirium.

BACKGROUND: Sepsis often causes cognitive changes like delirium and subsyndromal delirium (SSD). Delirium involves acute cognitive dysfunction, while SSD presents partial symptoms. Both conditions impact prognosis and are linked to neuroinflammation, altered cerebral perfusion and neurotransmitter dysfunction. This study explores the embodied cognitive experience in sepsis, integrating enactivist perspectives on the dynamic interplay of body, environment and neural activity. AIM: To analyse the lived bodily and cognitive experiences of patients with sepsis who experienced delirium or SSD. ST

The distinctive psychopathology of NMDAR-antibody encephalitis compared with primary psychoses: an international, multicentre, retrospective phenotypic analysis.

BACKGROUND: N-methyl-D-aspartate receptor (NMDAR)-antibody encephalitis is a life-threatening neuropsychiatric disorder requiring prompt immunotherapy. The earliest features are mental-state changes, often mistaken for primary psychosis. Improved clinical differentiation could assist rational diagnostic investigation and expedite immunotherapy. Inspired by patients' and relatives' lived experience, we aimed to explore the psychiatric phenotype of NMDAR-antibody encephalitis and the features common to and distinct from real-world episodes of psychosis. METHODS: In this international, multicent

Efficacy of non-invasive brain stimulation in treatment-resistant schizophrenia: A systematic review and network meta-analysis.

About one-third of patients with schizophrenia are classified as treatment-resistant schizophrenia (TRS) experiencing persistent symptoms despite adequate antipsychotic trials. Non-invasive brain stimulation (NIBS) emerged as a promising alternative treatment for TRS, yet its comparative efficacy remains unclear. We conducted a systematic review and network meta-analysis (NMA) to investigate the efficacy of NIBS for TRS patients (PROSPERO: CRD42023442563). Randomized trials comparing NIBS with sham or other active interventions in TRS patients were included. NMA with frequentist random-effects

Time-resolved Neural and Experience Dynamics of Medium- and High-dose N,N-Dimethyltryptamine (DMT).

N,N-dimethyltryptamine (DMT) is a fast-acting psychedelic drug that induces a radical reorganization of conscious contents and brain dynamics. However, our understanding of how brain dynamics support psychedelic-induced conscious states remains unclear. We therefore present a repeated-measures dose-dependent study of the subjective and neural dynamics induced through DMT under naturalistic conditions. Nineteen participants received either a 20-mg or a 40-mg dose of freebase DMT across two sessions in a blinded, counterbalanced order. Electroencephalography data and time-resolved measures of su

Substantia nigra pars reticulata involvement in auditory hallucinations of treatment-resistant schizophrenia: a deep brain stimulation case report.

One-third of schizophrenia patients exhibit treatment resistance, underscoring the need for mechanism-based interventions. We report that deep brain stimulation of the substantia nigra pars reticulata has acutely alleviated persistent auditory hallucinations by 64% in a treatment-resistant schizophrenia patient. Deep brain stimulation normalized hallucination-correlated elevation of intraoperative electrocortical theta-gamma phase-amplitude coupling at language-related cortical areas. These findings suggest aberrant cortical synchronization may be involved in the generation of verbal hallucina

Altered auditory seed-based functional connectivity in other specified schizophrenia spectrum and other psychotic disorder compared to schizophrenia spectrum disorders.

Few neuroimaging studies have examined other specified schizophrenia spectrum and other psychotic disorder (OSSO). We sought to identify features differentiating patients with OSSO from those with schizophrenia spectrum disorders (SSD) and healthy controls (HC) using auditory seed-based functional connectivity (FC) analysis. Patients with OSSO (n = 88), patients with SSD (n = 81), and HC (n = 85), matched for age, sex, and education, underwent resting-state functional magnetic resonance imaging (rs-fMRI) and clinical evaluation. To reduce heterogeneity of OSSO, individuals with specific subtyp

Virtual reality for visions (VRV): a proof-of-concept study examining the development of a new treatment for distressing visual hallucinations in people with psychosis.

INTRODUCTION: Visual Hallucinations (VHs) (seeing things that others do not, or visions) are a common feature of psychosis, causing significant distress and disability. Services rarely ask about these important experiences, and crucially there are no proven beneficial psychological treatments. There are at least two key challenges faced when treating VHs. First, people report not knowing why they see things others don't, which leads them to feel alone and different from others. Second, they feel they cannot trust their own eyes to tell what is real or not, which can lead to fears they will be

Psychedelic 5-HT2A agonist increases spontaneous and evoked 5-Hz oscillations in visual and retrosplenial cortex.

Visual perception appears largely stable in time. However, psychophysical studies have revealed that low frequency (0.5 - 7 Hz) oscillatory dynamics can modulate perception and have been linked to various cognitive states and functions. Neither the contribution of waves around 5 Hz (theta or alpha-like) to cortical activity nor their impact during aberrant brain states have been resolved at high spatiotemporal scales. Here, using cortex-wide population voltage imaging in awake mice, we found that bouts of 5-Hz oscillations in the visual cortex are accompanied by similar oscillations in the ret

"It Felt Good to Be Able to Say That Out Loud"-Therapeutic Alliance and Processes in AVATAR Therapy for People Who Hear Distressing Voices: Peer-Led Qualitative Study.

BACKGROUND: AVATAR therapy is a novel psychological therapy that aims to reduce distress associated with hearing voices. The approach involves a series of therapist-facilitated dialogues between a voice-hearer and a digital embodiment of their main distressing voice (the avatar), which aim to increase coping and self-empowerment. OBJECTIVE: This study explored therapeutic processes that are distinctive to AVATAR therapy, including direct early work with voice content and the role of the therapist in dialogue enactment. METHODS: People with lived experience relating to psychosis (peer researc

Effect on disease activity of ofatumumab in the treatment of glial fibrillary acidic protein astrocytopathy and 18F-DPA714 PET/MRI imaging assessment.

A 49-year-old male with Glial Fibrillary Acidic Protein Astrocytopathy (GFAP-A) presented with progressive cognitive decline, visual hallucinations, and bowel/bladder incontinence. Brain MRI revealed extensive white matter hyperintensities on T2-FLAIR sequences. After suboptimal response to first-line treatments including corticosteroids and intravenous immunoglobulin (IVIg), the patient was initiated on Ofatumumab (OFA) therapy. Treatment response was systematically monitored through validated clinical assessment tools (MMSE, MoCA, mRS, and CASE) and advanced neuroimaging, including conventio

Visual misperceptions in favour of living beings in Parkinson's disease: A psychophysical study.

BACKGROUND: Parkinson's disease (PD) patients often report seeing persons or animals rather than objects but this phenomenon remains poorly understood. Here, we use three experimental tasks to confirm such observation and to explore its cognitive mechanisms. METHOD: Fourteen PD patients with visual hallucinations (PD-VH), 14 PD patients without visual hallucinations (PD-NVH) and 14 controls with similar cognitive performance were tested using ambiguous stimuli. Ambiguous stimuli were morphs in which visual features from faces and flowers were melted together (Experiments 1 and 2) and a black
