Complex Visual Hallucinations in Post-stroke Epilepsy: A Case Report and Literature Review.

We herein report an 82-year-old man with post-stroke epilepsy (PSE) who presented with persistent complex visual hallucinations (CVH) involving past episodic scenes. Electroencephalography revealed epileptic discharges in the right posterior temporal and parietal regions. The symptoms resolved with levetiracetam and were maintained with zonisamide. A literature review suggested that CVH may occur when epileptic activity spreads to non-dominant temporal or parietal regions. In conclusion, CVH may reflect visual memory and retention and can be accompanied by psychiatric symptoms. Improved recogn

"Repetitive Transcranial Magnetic Stimulation for Auditory Hallucinations in Schizophrenia": Scoping Review.

BACKGROUND: Auditory verbal hallucinations (AVH) are among the most disabling symptoms of schizophrenia, often resistant to pharmacological treatment. Repetitive transcranial magnetic stimulation (rTMS) has emerged as a potential non-pharmacological intervention; however, existing evidence is fragmented across heterogeneous study designs. OBJECTIVE: This scoping review aimed to map the available evidence on the use of rTMS for AVH in patients with schizophrenia, focusing on stimulation parameters, clinical outcomes, neuroimaging findings, and gaps in current research. METHODS: A scoping revi

Delayed-Onset Acute Chorea Following Anesthesia.

ObjectiveWe aimed to present a patient who developed a severe chorea after orthopedic surgery, plausibly related to multiple anesthetic agents.CaseAn otherwise healthy 11-year-old girl developed choreiform movements and distressing behaviors (screaming, visual hallucinations) 24-36 hours after ankle-fracture surgery. With urgent stabilization needed and haloperidol not yet at an effective dose, continuous midazolam infusion provided prompt control. Other potential causes of chorea were excluded: brain magnetic resonance imaging and cerebrospinal fluid were normal; routine biochemistry and comp

Suicidal Ideation to Attempt Pathways and Experiences Among Adults with Schizophrenia Spectrum Disorders.

OBJECTIVE: There is an absence of investigations and knowledge of the pathways from suicidal ideation to attempt among individuals with schizophrenia spectrum disorders (SSDs). This study explored suicide attempt experiences and identified pathway steps and timeframes among adults with SSDs. METHODS: Quantitative and qualitative data were collected from adults with SSDs in a community mental health setting of the United States who had a history of suicide attempt and 6-month history of suicidal ideation (n = 19). The Pathway to Suicidal Action Interview (PSAI) was administered and data from q

Alien Hand Syndrome Secondary to Acute Anterior Corpus Callosum Infarction Despite Dual Antiplatelet Therapy: A Case Report.

Alien hand syndrome is an uncommon yet recognized complication of ischemic stroke. It is characterized by involuntary yet seemingly purposeful limb movements that occur without the patient's volitional control. The syndrome is most commonly associated with injury to the corpus callosum or adjacent medial frontal regions, where interhemispheric motor integration takes place. We report the case of a 50-year-old woman with multiple vascular risk factors who developed an acute infarction of the right corpus callosum. She presented with difficulty initiating movement, confusion, transient visual ha

Expanding the Phenotype of TLK2-Related Neurodevelopmental Disorder: Longitudinal Presentation in Two Young Adult Females.

The TLK2 gene encodes a serine/threonine kinase essential for chromatin assembly. Loss-of-function heterozygous variants were recently identified as a cause of a rare neurodevelopmental disorder (TLK2-NDD) characterized by a variable spectrum of developmental delay, autism, behavioral issues, and severe constipation. We describe two unrelated young adult females presenting with neurodevelopmental delay, neurobehavioral abnormalities, and constipation since infancy secondary to truncating TLK2 variants. Case 1 is a 26-year-old female who was referred to our clinic for extended assessment of new

Seeing without eyes: Relation between visual mental imagery and visual hallucinations.

Visual mental imagery (VMI) refers to the ability to "see with the mind's eye", in the absence of adequate sensory input, while visual hallucinations (VH) involve perceiving stimuli in their absence. This review investigates the relationship between VMI and VH across neurodegenerative and psychiatric conditions, focusing on Parkinson's disease (PD), Alzheimer's disease (AD), Dementia with Lewy bodies (DLB), and schizophrenia (SCZ). A relationship between VMI and VH was identified, with greater VMI vividness associated with increased VH occurrence in all conditions, except for DLB, for which no

Does hearing voices make you nervous? An ecological momentary assessment investigation of momentary emotional responses to the occurrence of psychotic symptoms.

BACKGROUND: Auditory hallucinations and delusions occur across neuropsychiatric conditions. As psychosis in bipolar disorder (BD) is often mood congruent, spikes in sad moods could precede occurrences of psychosis. The temporal association of episodic psychosis and negative affect (NA) is not understood in schizophrenia (SZ). We identified occurrences of episodic psychosis and temporally related them to NA with ecological momentary assessment (EMA). METHODS: Participants with SZ (N = 126) or BD (N = 114) were surveyed 3 times per day, 7 days a week, for 30 days. Surveys assessed location, soc

Charles Bonnet Syndrome: associations between psychosocial measures and visual hallucination characteristics in the visually impaired.

BACKGROUND/AIMS: The emotional response to Charles Bonnet Syndrome (CBS) (visual hallucinations in individuals with sight loss) is associated with negative affect, suggesting a link between psychological measures and hallucination characteristics. This study set out to investigate whether the association extends to a broader range of hallucination attributes and psychological measures, taking into account clinical factors likely to influence such associations. METHODS: 70 participants with self-reported CBS completed an online survey assessing hallucination attributes of frequency, duration,

[Associations between Self-reported Symptoms, Quality of Life, and Work Productivity Loss among Japanese Workers: A Cross-sectional Analysis of a Large-scale Questionnaire-based Person-reported Outcomes Database (PRO-DB)].

BACKGROUND: Japanese workers face long working hours, mental stress, and caregiving responsibilities, contributing to physical and psychological symptoms (e.g., fatigue, depressive mood, and sleep disturbances). These symptoms adversely affect the quality of life (QOL) and work productivity. Nationwide studies on symptom-QOL-work productivity associations across various occupations remain scarce. OBJECTIVE: In this study, we quantitatively assessed the associations between self-reported symptoms, QOL, and work productivity loss using a large-scale person-reported outcomes database (PRO-DB).

Anti-GAD65 Encephalitis: A Cryptic Case Requiring a Multidisciplinary Approach.

Anti-GAD65 encephalitis-like other autoimmune encephalitis conditions-poses a significant diagnostic challenge; its heterogeneous neuropsychiatric phenotypes mimic primary psychiatric disorders. The broad pathophysiological spectrum, ranging from limbic dysfunction to catatonia, underscores the need for systematic approaches that integrate immunotherapy with neuromodulation when standard interventions fail. We present a 78-year-old woman with no prior psychiatric history who developed acute-onset catatonia with visual hallucinations. Neurological evaluation revealed markedly elevated anti-GAD6

Trance and Possession Disorder With Dissociative-Psychotic Overlap: A Clinical Case Report.

Trance and possession disorder (TPD) is a complex and culturally influenced condition characterized by alterations in consciousness, identity, and behavior, often posing diagnostic challenges in clinical psychiatry. We report the case of a young woman who presented with a 12-hour history of visual and auditory hallucinations, disorganized speech, and aggressive behavior toward her partner, requiring emergency intervention and physical restraint. Symptoms began with the perception of unusual noises and external presences in her home, reportedly shared by her partner, prompting religious consult

Early Behavioral and Cognitive Changes in Patients With Pathologically Confirmed Alzheimer Disease, Lewy Body Dementia, and Mixed Dementia.

BACKGROUND AND OBJECTIVES: Alzheimer's disease pathology (ADP) and Lewy body pathology (LBP) are traditionally associated with distinct cognitive profiles. However, growing evidence highlights the role of behavioral and psychological symptoms of dementia (BPSD) in shaping clinical presentations. The combined influence of cognitive and behavioral symptoms across neuropathologically confirmed ADP, LBP, and mixed AD-LBP has not been systematically examined. This study aimed to identify clinically meaningful subtypes by jointly analyzing cognitive performance and BPSD profiles in individuals with

The role of anxiety in the relationships between psychotic experiences and suicidal ideation severity.

OBJECTIVE: The effects of anxiety on psychological pathways underpinning psychosis and suicidal thoughts are poorly understood. The overall goal of the present study was to rectify this by examining pathways linking emotional distress from psychotic symptoms, specifically, auditory hallucinations and delusions with suicide ideation severity, via anxiety, defeat, entrapment, and hopelessness. METHODS: Participants had psychosis and recent suicidal experiences and were under the care of mental health services. The design was cross-sectional. Two key mediated models were examined. Resolution of Visual Hallucinations in Parkinson's Disease After Right Occipitoparietal Subcortical Hemorrhage: A Case Report.

Visual hallucinations are common non-motor symptoms of Parkinson's disease (PD) that substantially impair quality of life, particularly in advanced stages. However, the neural substrates underlying visual hallucinations in PD remain incompletely understood. Here, we report a rare case of a 72-year-old right-handed man with PD whose visual hallucinations completely resolved after a right occipitoparietal subcortical hemorrhage. He developed PD at 58 years of age and later experienced wearing-off, dyskinesia, cognitive decline, visual hallucinations, and a false sense of presence. At 72 years of

AVATAR versus cognitive-behavioral therapy for medication-resistant auditory hallucination: a systematic review and network meta-analysis.

Auditory hallucinations (AH) frequently persist in schizophrenia spectrum disorder despite antipsychotic treatment. Cognitive behavioral therapy (CBT) remains an established psychological intervention, whereas AVATAR (Audio Visual Assisted Therapy Aid for Refractory auditory hallucinations) therapy has recently been introduced as a novel approach integrating interactive digital avatars. This meta-analysis compared the efficacy of AVATAR therapy with CBT for medication-resistant AH. A systematic search of five major databases up to June 1, 2025 identified randomized controlled trials (RCTs) tha

The Neuropsychiatric Symptom Profile of Mild Dementia With Lewy Bodies.

IntroductionNeuropsychiatric symptoms are often considered late manifestations of dementia and can be neglected during early clinical assessments. Besides visual hallucinations, less is known about other neuropsychiatric symptoms in mild dementia with Lewy bodies (DLB). We aimed to 1) describe neuropsychiatric symptoms and associated caregiver distress in mild DLB; 2) describe neuropsychiatric symptoms stratified by dementia stage within the included participants.MethodsIndividuals with probable DLB and their caregivers underwent clinical and neuropsychiatric assessments. Beyond Hyperexcitability: A Review of Neural Mechanisms in Charles Bonnet Syndrome.

Charles Bonnet syndrome (CBS) is characterized by complex visual hallucinations in visually impaired individuals who maintain intact cognitive function. Despite significant progress in understanding this condition, the precise neural mechanisms underlying CBS remain incompletely understood. This review synthesizes current evidence regarding the pathophysiology of CBS, with particular emphasis on emerging neurobiological models that extend beyond simple cortical hyperexcitability. Recent neuroimaging, neurophysiological, and computational modeling studies suggest that CBS hallucinations may ari
