Transcranial focused ultrasound stimulation in schizophrenia: current evidence, mechanisms, and future directions.

BACKGROUND: Schizophrenia disrupts large-scale brain networks, especially circuits linking cortical and deep subcortical structures. Transcranial focused ultrasound stimulation (tFUS) is an emerging non-invasive neuromodulation technique with the capacity to target deep brain regions with high spatial precision. However, clinical evidence for tFUS in schizophrenia remains limited. OBJECTIVE: This narrative review synthesizes available preclinical and clinical evidence, with particular attention to mechanisms, safety, stimulation targets, methodological limitations, and translational challenge

Perception, assessment, and coaching: a systematic review and taxonomy of computer vision-based physical rehabilitation techniques.

The digital transformation of rehabilitation training has become a public health imperative driven by a global demand that outstrips professional medical resources and is compounded by a deficit in public rehabilitation literacy. As traditional hospital-centric models reach their scalability limits, there is a critical necessity for accessible home-based care solutions to ensure patients do not miss optimal recovery windows. To evaluate computer vision as a potential solution, this paper conducts a systematic review following the PRISMA 2020 reporting framework. We identify that its clinical m

Increasing access to CBT for psychosis patients: a randomized controlled trial evaluating a brief, targeted CBT-informed intervention for distressing voices delivered by Assistant Psychologists (the GiVE3 study).

Background Cognitive Behaviour Therapy for psychosis (CBTp) is recommended for treating psychosis. However, access to CBTp is extremely poor for patients internationally due to its intensive nature and delivery by highly trained therapists. This study evaluated the clinical and cost effectiveness of the GiVE intervention (Guided self-help CBT for distressing voices) delivered by briefly trained therapists (Assistant Psychologists). Methods This study was a pragmatic, two-arm, parallel group, superiority randomised controlled trial, comparing the GiVE intervention (delivered by Assistant Psycho

Reducing mental health stigmatisation among healthcare workers and students using a virtual reality protocol: the VIRTUS protocol, a randomised controlled trial.

INTRODUCTION: Schizophrenia is a chronic psychiatric disorder, frequently associated with stigma, including within healthcare settings that significantly impairs quality of life and may exacerbate symptoms. Virtual reality (VR), as an immersive tool, may allow healthy individuals to experience the first-person perspective of a patient experiencing psychotic symptoms. VR exposure may facilitate perspective-taking, foster empathy and help reduce stigma. However, the findings remain incomplete, with considerable variation between protocols and no data on implicit stigma. Here, we present a study

Efficacy and safety of rTMS for global symptom severity, negative symptoms, and auditory hallucinations in treatment-resistant schizophrenia: a systematic review and meta-analysis.

BACKGROUND: Treatment-resistant schizophrenia (TRS) affects up to one-third of individuals with schizophrenia and remains a major clinical challenge despite optimized pharmacotherapy. Repetitive transcranial magnetic stimulation (rTMS) has been proposed as a neuro-modulatory intervention for refractory symptoms, yet evidence specific to TRS remains limited and inconsistent. OBJECTIVES: To evaluate the efficacy and safety of rTMS for reducing global symptom severity, negative symptoms, and auditory hallucinations in adults with treatment-resistant schizophrenia, and to examine the influence of

Case Report: Anti-N-methyl-D-aspartate receptor encephalitis associated with bilateral mature ovarian teratomas in early pregnancy.

Anti-N-methyl-D-aspartate receptor (NMDAR) encephalitis is an autoimmune encephalitis that may present with acute psychiatric symptoms, seizures, movement disorders, impaired consciousness and autonomic dysfunction. A 30-year-old previously healthy woman at 10-11 weeks of gestation was admitted to the intensive care unit after an approximately 2-3-week history, reported by relatives, of insomnia, behavioral change, auditory and visual hallucinations, psychomotor agitation, aggressive behavior, and a generalized tonic-clonic seizure before admission. On admission, the patient had severe encepha

Prognostic value of the C-reactive protein-albumin-lymphocyte index for postoperative visual hallucinations following cardiac surgery.

BACKGROUND: Visual hallucinations (VH) are distressing and clinically relevant after cardiac surgery. C-reactive protein-albumin-lymphocyte (CALLY) index reflects systemic inflammation, nutrition, and immune competence. This study evaluated whether preoperative CALLY predicts 7-day postoperative VH. METHODS: In a prospective multicenter cohort (10 cardiac centers; September 2022 to May 2025), adults undergoing coronary or valve surgery were followed through day 7. Preoperative CALLY was calculated from routine laboratory measurements. VH discrimination was assessed using receiver operating ch

A case report of meningeal carcinomatosis presenting as rapidly progressive dementia: cytological diagnosis with ancillary immunohistochemistry of cerebrobrospinal fluid cells.

INTRODUCTION: Meningeal carcinomatosis (MC) is a severe complication of systemic cancers. Although typical presentations involve meningeal irritation or cranial nerve deficits, its manifestation as rapidly progressive dementia (RPD) in the absence of classic signs is exceedingly rare. This complication poses a substantial diagnostic challenge, especially when the primary tumor remains occult. METHODS: We present the case of a 56-year-old man with a 4-month history of progressive cognitive decline, behavioral changes, and visual hallucinations culminating in stupor. The diagnostic evaluation c

Use of Electroconvulsive Therapy for Treatment-Resistant Schizophrenia in a Patient Unable to Tolerate Clozapine Due to Clozapine-Associated Hyponatremia: A Case Report.

Clozapine is regarded as the most effective treatment for patients with treatment-resistant schizophrenia (TRS). However, serious adverse effects such as hyponatremia may necessitate discontinuation, leaving limited therapeutic alternatives. This case report describes a middle-aged male with chronic schizophrenia who developed clinically significant hyponatremia during clozapine titration, leading to its discontinuation prior to achievement of therapeutic serum concentrations. Despite prior antipsychotic trials with haloperidol, asenapine, olanzapine, and chlorpromazine, the patient had persis

[A case of anti-N-methyl-D-aspartate (NMDA) receptor encephalitis presenting with characteristic symptoms such as altered sense of time, visual distortion and hallucinations].

A previously healthy 20-year-old woman was admitted to our hospital because of psychobehavioral alterations, generalized seizure, and post-ictal drowsiness. Six weeks before this admission, depression developed, followed by dysgeusia, anorexia, hearing loss, auditory hallucinations, logoclonia, and generalized seizures, leading to the first hospitalization. On admission, the temperature was 37°C. She was awake, well oriented, but had logoclonia. Brain MRI and electroencephalography (EEG) were both unremarkable, but cerebrospinal fluid (CSF) analysis revealed 7 cells/μl. After admission, she be

'I just want an explanation': A qualitative investigation of how people with psychosis make sense of their visual hallucinations.

OBJECTIVES: Visual hallucinations (visions) in people with psychosis are common and often distressing. Cognitive theory emphasises a central role of appraisals in determining distress. We explored how people made sense of their visions to inform the development of targeted cognitive treatments for visions. DESIGN: Semi-structured interviews were conducted with 12 participants with psychosis recently reporting visions. METHODS: Reflexive thematic analysis was utilised. RESULTS: There was an overarching theme that participants lacked convincing and acceptable explanations as to why they saw v

Tacrolimus-Induced Atypical Posterior Reversible Encephalopathy Syndrome (PRES) in a Kidney Transplant Recipient: A Case Report.

We report a 36-year-old woman who presented with acute visual hallucinations, delusions, and dizziness 2 months after uncomplicated kidney transplantation. Brain magnetic resonance imaging (MRI) revealed T2/FLAIR hyperintensities in the dorsal pons and bilateral temporal cortices, atypical yet consistent with PRES. Tacrolimus-induced PRES was diagnosed in the absence of severe hypertension or other precipitants. Tacrolimus was discontinued and replaced with cyclosporine, with complete clinical resolution and normalization of MRI findings within 8 weeks. This case underscores the importance of

Physical Therapies and Neuromodulation for Treatment-Resistant Schizophrenia: A Review of Potential Mechanisms, Efficacy, and Safety.

Antipsychotic medications remain the conventional treatment for treatment-resistant schizophrenia (TRS), yet some patients exhibit poor responses. Due to the limitations of pharmacological interventions, physical therapies are increasingly recognized as essential supplements in managing TRS. This article systematically examines the current clinical applications and research advances of physical therapeutic techniques for TRS. Existing evidence suggests that electroconvulsive therapy (ECT), particularly when administered as an adjunct to antipsychotic medication, may improve positive symptoms a

Psychedelic 4-AcO-DMT attenuated looming-induced freezing behaviors by activating PrL CaMKIIα neurons in mice.

Psychedelics modulate emotion and cognition, yet the underlying mechanisms remain unclear. Given the ability of psychedelics to induce visual hallucinations, we employed the looming model-a visual threat assay that combines visual stimulation with innate fear-to investigate the effects of psychedelics. Unexpectedly, 4-acetoxy-N,N-dimethyltryptamine (4-AcO-DMT), a psychedelic compound that acts as a prodrug of psilocin and exhibits comparable central nervous system effects to psilocybin, significantly reduced looming-induced freezing behavior compared to R/S-MDMA, without eliciting overt halluc

Neurophysiological Evidence of Enhanced Cognitive Control Following Auditory Control Enhancement (ACE) Therapy: A Pilot RCT in Schizophrenia-Spectrum Disorders with Persistent Auditory Hallucinations.

Schizophrenia (Sz) involves positive symptoms such as auditory hallucinations, paranoia, and delusions, and are commonly linked to deficits in cognitive control. These deficits appear in tasks like the A-X version of the continuous performance test (AX-CPT), which reflects reduced proactive control, and in diminished auditory steady-state responses (ASSR) during selective attention. Notably, in Sz, ASSR to sounds presented during diverted attention increases with hallucination severity. We report results from a single-blind randomized controlled trial evaluating Auditory Control Enhancement (A

The neural mechanisms of Charles Bonnet syndrome.

Charles Bonnet Syndrome (CBS) describes the experience of visual hallucinations that cannot be explained by any other cause in individuals with visual impairment. CBS remains underrecognized and is poorly understood, with limited knowledge of its underlying mechanisms. This review examines the neurobiological basis of CBS, discussing neuroimaging, neurophysiological, structural, and biochemical perspectives. The review identifies evidence on the roles of neural compensation and cortical hyperexcitability in the emergence of visual hallucinations following visual deprivation. Despite methodolog

Semiology of Opercular Epilepsies.

In this systematic review, we aimed to characterize the semiology of 'pure' opercular seizures by identifying cases in which the epileptogenic zone was restricted to the operculum. Seventy patients from 24 articles were included and categorized into four anatomic groups: 11 frontal, 27 central, 14 parietal, and 18 temporal operculum cases. Across these four anatomic regions, we identified the following semiological patterns: Frontal opercular seizures were characterized by a lower frequency of auras, most commonly nonpainful somatosensory sensations affecting the limbs or trunk and never exclu

Auditory Hallucination Simulation and the Development of Empathy Among Undergraduate Nursing Students: An Integrative Review.

BACKGROUND: Auditory hallucinations (AHs) are perceptual experiences that can impact communication, daily functioning, and safety. Empathy is essential for effective therapeutic engagement. Nursing students often report feeling unprepared in dealing with individuals who experience AHs. Auditory hallucination simulation (AHSim) has been used in undergraduate nursing education as an experiential learning strategy to enhance empathy and insight. However, evidence regarding its impact on empathy among undergraduate nursing students remains limited. AIMS: This integrative review aimed to synthesiz

Complex auditory hallucinations associated with hearing impairment in elderly adults.

Auditory hallucinations are a common yet underexplored phenomenon in individuals with hearing impairment, particularly in the elderly. Oliver Sacks syndrome, often seen in patients with sensory deprivation, is typically characterized by musical hallucinations but can also present with nonmusical auditory experiences. This case series presents three elderly patients with hearing impairment who experienced a range of auditory hallucinations, including voices, instrumental music, and environmental sounds, all localized to the ear with greater hearing loss. Despite these auditory experiences, pati
