Self-inflicted ocular mutilation.

Self-inflicted mutilation of the eyes is an uncommon but fairly distinct entity. Patients are typically young men; they often have criminal records and histories of drug abuse. Underlying schizophrenia is a common feature. The patients frequently suffer from castration fears, oedipal conflicts, repressed homosexual impulses, severe guilt, and a need for punishment. We treated two patients with this disorder. The first, a 26-year-old prison inmate convicted of murdering his grandfather, cut his eyes with a razor blade. He had previously attempted to commit suicide, suffered from delusions, and

Importance of schizophrenic symptoms in the diagnosis of mania.

The authors examined a sample of 111 consecutively admitted patients who satisfied inclusion criteria for mania and further characterized them as exhibiting none, one, or two or more of five clinical features often believed to be associated with a diagnosis of schizophrenia: formal thought disorder, first-rank symptoms, auditory hallucinations, persecutory delusions, and catatonia. The presence and number of such symptoms were unrelated to any of the major demographic, clinical, historical, laboratory, or familial variables studied. The authors conclude that schizophrenic symptoms do not play

Persistent auditory hallucinations: coping mechanisms and implications for management.

The strategies used by 40 chronic schizophrenic out-patients with persistent auditory hallucinations to cope with the intrusive phenomena were explored. Frequent coping mechanisms included changes in activity, interpersonal contact, manipulations of physiological arousal, and attentional control. A high risk of suicide was noted. Coping behaviour appeared somewhat independent of clinical and sociodemographic variables. Successful coping appeared to result from systematic application of widely used coping strategies. The implications for the clinical management of patients with persistent audit

Command Hallucinations and self-amputation of the penis and hand during a first psychotic break.

An unusual case is presented of a psychotic young man who experienced command auditory hallucinations which directed him to amputate his right hand and distal penis to atone for a "terrible sexual transgression." The existing literature on genital self-mutilation is reviewed as well as some recent work on factors that might be used to predict an episode of self-mutilation. In patients who are felt to be at extremely high risk, careful monitoring and rapid medication schedules might be effective in preventing such an incident.

A questionnaire study of hostility in persistent auditory hallucinators.

Twenty-six psychotic patients selected for persistent auditory hallucinations were tested on the HDHQ Questionnaire. Their total hostility scores were found to be significantly higher than those of the mixed schizophrenic normative sample, suggesting a link between aggressive tendencies and auditory hallucinations. The total sample was further subdivided on the basis of duration, location and quality of voices, and subgroup comparisons undertaken. These suggested that location and quality of voices were independent of each other and differentially linked with other measures. Several hypothesis

Reinforcement of vocal correlates of auditory hallucinations by auditory feedback: a case study.

Although auditory hallucinations in schizophrenic patients are usually thought to be private events, several early writers observed vocalizations concurrent with hallucinations. The content of such vocalizations corresponded to what the voices were reported to have said. A schizophrenic patient is described whose whispers were increased to an intelligible level by the use of auditory feedback. This has implications for the self-control of hallucinations, and for neurological theories of verbal hallucinations.

Auditory effects of chronic alcoholism.

Clinical reports of hearing loss, tinnitus, and auditory hallucinations, as well as empirical studies of auditory phenomena, have sparked interest in the auditory effects of chronic alcoholism. The auditory system, owing to its multi-synaptic course through the central nervous system and to the many techniques available for diagnostic evaluation, affords a special opportunity for examination of transmission breakdown in alcoholics. The present paper reviews several different approaches to the study of disrupted hearing: histologic evidence: threshold measurement, evoked potential data; acousti

The relationship of demographic, clinical, and outcome variables to neuroleptic treatment requirements.

A retrospective study of 61 acute schizophrenic patients examined whether any demographic, clinical, and outcome characteristics distinguished patients who improved with placebo or low dosages of antipsychotics (PLD patients) from patients who required high conventional dosages of antipsychotics (HCD patients). Patients in the PLD group (n = 30) and HCD group (n = 31) were similar in overall level of psychopathology at admission. Prominent excitement and certain somatic and auditory hallucinations were significantly more frequent in the HCD patients. PLD patients were more likely to be female,

[Central deafness without aphasic disorders due to bilateral temporal lesion].

The authors report the case of a 68 years-old man with two strokes with an interval of 12 years in each hemisphere. In the last one, the patient presented total and persistent central deafness without any aphasic disorders, except verbal comprehension interference due to the deafness. During the first weeks he also had complex auditory hallucinations. Computerized tomography showed alterations suggesting old bilateral temporal lobe infarctions.

Platelet monoamine oxidase activity in subgroups of schizophrenic disorders.

This article summarizes findings from a series of studies that examined platelet monoamine oxidase (MAO) activity in patients with nonaffective schizophrenic disorders and schizophrenia-related depressions. The findings indicate that mean platelet MAO activity was not different from control values in the subgroup of nonaffective schizophrenic disorders without auditory hallucinations (that is, the S-1 subgroup). However, mean platelet MAO activity was reduced in the subgroup of nonaffective schizophrenic disorders characterized by the presence of auditory hallucinations often occurring in conj

Platelet MAO activity: relationships to clinical and psychometric variables.

Platelet monoamine oxidase (MAO) activity was measured in a group of hospitalized male psychiatric patients. The findings of this study confirm previous reports of: (1) reduced platelet MAO activity in patients with auditory hallucinations, (2) reduced platelet MAO activity in patients with alcoholism, and (3) a positive correlation between platelet MAO activity and a psychometric index of social introversion.

Auditory hallucinations and the Verbal Transformation Effect.

In an attempt to replicate findings reported by Slade (1976), 12 hallucinating and 12 non-hallucinating shcizophrenic subjects were compared on tests of verbal ability, personality and mental imagery variables and the Verbal Transformation Effect. No significant differences between the groups was demonstrated. When the data from both groups of schizophrenics were combined, a significant correlation was found between 2 measures of the Verbal Transformation Effect and the P-score of the Eysenck Personality Questionnaire.

Effect of hemodialysis on hallucinations.

A patient with chronic schizophrenia had two hemodialyses and two sham dialyses in a single-blind design. There was no change in the patient's affect after either procedure, but her auditory hallucinations disappeared after both hemodialysis and sham dialysis, with hemodialysis inducing a much longer partial remission.

Cimetidine-induced mental status changes: case report and literature review.

An apparent case of psychosis induced by cimetidine is reported. A 61-year-old Caucasian woman was hospitalized with chief complaints of left-sided paresthesias, headache, and vertigo. She had a history of hyperparathyroidism, thyroid insufficiency, and chronic but stable renal insufficiency. Admitting laboratory data indicated possible hepatic compromise. On day 16 of hospitalization, cimetidine (300 mg q 6 hr p.o.) was begun because of a falling hematocrit and guaiac-positive stools. Within 24 hours after cimetidine therapy was begun, the patient was confused, tearful, and disoriented. This

Schizophrenia: the nature of the psychological disturbance and its possible neurochemcial basis.

The diagnosis of schizophrenia is established principally by the presence of certain psychological symptoms which although subjective can be reliably assessed by standardized interviewing procedures. The most characteristic symptoms (Schneider's first-rank symptoms) fall into three groups: (a) auditory hallucinations of particular types, (b) 'ego-boundary disturbances', including intrusions into the stream of consciousness attributed to external agencies, and (c) delusional perception. Symptoms closely resembling those seen in schizophrenia can be induced in non-schizophrenic individuals by am

Psychomotor epilepsy and psychosis. III. Social and psychological correlates.

In a controlled investigation of paranoid hallucinatory chronic or episodic epileptic psychoses, the social data, psychopathological symptoms and the psychological test findings (WAIS, Rorschach, object-sorting, proverb-interpretation and the Stroop test) were analyzed in order to delineate the characteristics of the psychotic syndrome and to evaluate the significance of organic etiological factors. The study comprised 45 patients with complex partial epilepsy followed by psychosis after median 23 years, and 34 control patients with the same type of epilepsy of median 30 years' duration. Among

Platelet monoamine oxidase activity in schizophrenia. Relationship to disease, treatment, institutionalization and outcome.

A study was made of platelet monoamine oxidase (MAO) activity in non-medicated, newly-admitted schizophrenics and institutionalized chronic schizophrenics both on and off medication. These patients were compared to two control groups: normal subjects and brain-damaged institutionalized patients. No relationship was found between platelet MAO activity and the severity or duration of illness, duration of psychotropic medication, presence of auditory hallucinations or institutionalization. Mean platelet MAO activity did not differ significantly between the schizophrenic subgroups and control grou

Cross-cultural differences in indicators of improvement from psychosis: the case of betel nut chewing.

This paper uses a case from the Micronesian Island of Yap to illustrate that indicators of improvement from an episode of schizophrenic psychosis can vary tremendously from culture to culture. At times, the clinician must search diligently for such indicators since the patient will not verbalize any perception of improvement. A 39-year-old Yapese woman was evaluated by a psychiatrist and was found to have been psychotic for several years, suffering from gross delusions, and auditory hallucinations, flattened affect, and social isolation. In addition, she had discontinued chewing betel nut that
