Neuropsychiatric systemic lupus erythematosus (NPSLE) represents one of the most complex and heterogeneous manifestations of systemic lupus erythematosus (SLE). Psychosis is an uncommon but severe feature of NPSLE and usually occurs in the setting of established multisystem disease. Acute psychotic presentation as the sole initial manifestation of SLE is exceptionally rare and poses considerable diagnostic challenges. We report the case of a 24-year-old woman with no prior psychiatric or medical history who developed abrupt-onset psychosis characterized by insomnia, aggression, persecutory del

BACKGROUND: The pons, a critical structure located in the brainstem, plays a multifaceted role in various neurological functions, and any disruption to its integrity can lead to profound and diverse clinical manifestations. Recent reports have highlighted the association between pontine lesions and conditions such as emotional incontinence, rage, auditory hallucinations, and visual disturbances. We refer to a novel case that causes false hallucinations. We discuss a patient with a tumor in the pons exhibiting a rare constellation of symptoms that included unstable auditory, visual, and percept

UNLABELLED: Objective Condyloma acuminatum (CA), a common sexually transmitted disease caused by human papillomavirus (HPV) infection, is closely associated with the abnormal proliferation and differentiation of host keratinocytes. Brusatol (BRU), a quassinoid compound derived from the medicinal plant Brucea javanica, has gained attention for its ability to regulate multiple biological processes through the phosphoinositide 3-kinase/Protein Kinase B (PI3K/AKT) signaling pathway. It may also reduce Nuclear factor erythroid 2-related factor 2 (Nrf2) expression to induce ferroptosis in cancer cel

BACKGROUND: Obsessive-compulsive disorder (OCD) is a chronic, debilitating condition typically treated with cognitive-behavioural therapy (CBT) and exposure and response prevention (ERP). While effective, these approaches are often met with hesitation, as many patients struggle to engage in exposure-based treatment, and relapse remains a significant challenge. Virtual Reality-assisted Therapy (VRT), initially developed for auditory hallucinations, offers a novel, immersive approach that may enhance insight by helping patients externalize their OCD symptoms. This perspective shift may increase

BACKGROUND: Narcolepsy is a chronic neurological disorder treated with medications that promote wakefulness. Pitolisant, a histamine H3 receptor inverse agonist, is used to reduce excessive daytime sleepiness. Although pitolisant clinical trials describe mainly mild adverse effects, severe psychiatric reactions have been reported, yet detailed clinical descriptions remain limited. This case describes a first-onset manic episode with psychotic features shortly after pitolisant initiation in an adult without any psychiatric history, highlighting a rare but clinically significant potential reacti

A significant minority of patients who present with psychosis have an underlying medical ("organic") cause. Some of these secondary causes are reversible; therefore, early detection is critical. Psychopathology may be informative during initial assessment to determine which patients are at an increased risk of having an underlying medical cause and should be prioritised for enhanced investigation. Through a pre-registered (CRD42024511546) systematic review and meta-analysis, we compared the psychopathology of patients with psychosis secondary to a medical cause compared to patients with primar

Auditory hallucinations (AH)-the perception of sound in the absence of any external auditory stimulus-are among the most clinically significant and personally distressing symptoms encountered in psychiatry and neurology. Although AH is canonically associated with schizophrenia spectrum disorders, where it affects 60-80% of patients at some point in the illness course, it also emerges in major depressive disorder with psychotic features, bipolar disorder, post-traumatic stress disorder, borderline personality disorder, substance-induced psychoses, and a range of neurological conditions includin

Triple X (47,XXX) syndrome is a sex chromosome aneuploidy associated with learning disabilities, attentional deficits, and social impairments. Its psychiatric manifestations, particularly psychosis, remain underrecognized. This case report describes a 13-year-old girl with 47,XXX syndrome who developed two recurrent psychotic episodes during adolescence and ultimately achieved sustained remission under clozapine, illustrating the need for individualized, genetically informed care. The patient initially presented with persecutory delusions, auditory hallucinations, behavioral changes, and funct

A 21-year-old female patient with schizophrenia, whose primary symptom was refractory auditory hallucinations, developed intracranial hypertension syndrome following her first session of modified electroconvulsive therapy (MECT), presenting with severe headache, projectile vomiting, diplopia and meningeal signs. She subsequently received 1 Hz repetitive transcranial magnetic stimulation (rTMS) applied to the left temporo-parietal cortex with each session delivering 1,500 pulses at 74% of the resting motor threshold. Her auditory hallucinations diminished markedly 3 days after completing rTMS a

Background/Objective: Auditory and vestibular (AV) symptoms can be considered functional neurological disorders (FND) when they do not arise from structural abnormalities. These symptoms can arise as expressions of underlying neuropsychological or psychiatric conditions, yet they may also play a role in precipitating or maintaining such disorders. This systematic review aimed at exploring the prevalence of AV symptoms in post-traumatic stress disorders (PTSD) as well as to understand if they correlate with each other. Methods: We conducted a systematic review of the literature including PubMed

Transcranial direct and alternating current simulations (tDCS and tACS) are non-invasive neuromodulators with emerging evidence in neuropsychiatric disorders. This review compares their efficacy and tolerability in head-on trials. Following PRISMA guidelines, 955 studies were identified from Scopus, MEDLINE, and trial registries. After screening and reviewing abstracts and full texts by 2 independent authors, 11 studies comparing tACS and tDCS in neuropsychiatric disorders were included. Four studies in schizophrenia, 1 each in depression, mild cognitive impairment (MCI), and ataxia; 2 in tinn

BACKGROUND: Postpartum depressive episodes are common. Patients with bipolar disorder (BD) may also experience manic or mixed episodes postpartum, but cases of delirious mania (DM)-characterized by rapidly evolving delirium, mania, and may involve psychotic symptoms-are rarely reported. Given its severity, frequent underdiagnosis, and life-threatening risks, a thorough exploration of its clinical features, potential risk factors, and treatment strategies can aid clinicians in early recognition and intervention, thereby improving patient outcomes.

Background The co-occurrence of schizophrenia and obsessive-compulsive disorder has attracted increasing clinical and research attention, yet it is not recognised as a distinct diagnostic entity in current psychiatric classification systems. This overlap is associated with greater symptom severity, increased suicide risk, and poorer treatment outcomes. Despite its clinical relevance, reports from South Asia remain scarce. This case contributes regional data and supports the conceptualisation of a schizo-obsessive subtype, challenging existing nosological boundaries. Case Presentation We report

Adolescent first-episode psychosis may combine schizophrenia-spectrum, affective, trauma-related, and developmental features, and Capgras syndrome is uncommon in this age group. We report a confidentially de-identified case of a 16-year-old female high-school student with no previous psychiatric admission or known substance use who developed severe insomnia, reduced self-care, external auditory hallucinations, paranoid and referential ideation, Capgras-type misidentification of her father, and a multithemed bizarre delusional system after cumulative psychosocial stressors including earthquake-

INTRODUCTION: Mitochondria generate energy in the form of adenosine triphosphate (ATP), integral in regulating cellular activities. Pathogenic variants in mitochondrial DNA occur when excess free radicals are produced during abnormal electron transport, causing cumulative oxidative stress and decline in mitochondrial function. The brain is particularly susceptible to oxidative damage due to its high oxygen consumption and low levels of antioxidant enzymes. This predisposes to neuropsychiatric manifestations, which may be exacerbated by psychotropics that further impair mitochondrial function.

It is widely acknowledged that trauma is related to auditory hallucinations in psychotic disorders. Yet, this relationship has not been well explored in hallucinations across other sensory modalities. The current study investigated whether hallucination modality groups (auditory, visual, olfactory, tactile, multisensory) were significantly related to physical, sexual, or emotional trauma. The age at which the trauma was experienced (16 or below, 17 and above) was also considered. Multivariate linear models, analysis of variance, and regression analyses for sixty-six participants with a psychot

Atrophic gastritis is an autoimmune gastritis that is characterised by the production of antibodies to parietal cells and intrinsic factor of the stomach. It is uncommon in paediatrics, as a result, is not usually in the differential diagnosis. A boy in middle childhood presented with a history of auditory hallucinations and fatigue, his workup only showed iron deficiency anaemia which responded to oral iron replacement but rebounded off therapy. There was no underlying cause found until years later, when he presented with Raynaud's phenomenon leading to an autoimmune workup being done which s

BACKGROUND: Trigeminal neuralgia (TN) is a severe neuropathic facial pain syndrome that mainly affects older adults. Carbamazepine and oxcarbazepine are standard therapies, but their tolerability is often limited in the elderly. Lacosamide is a potential alternative through its modulation of slow sodium channel inactivation, yet its neuropsychiatric adverse effects remain poorly documented.
CASE REPORT: A 71-year-old man with right-sided TN refractory to carbamazepine and intolerant to oxcarbazepine was started on lacosamide. Pain decreased 30% to 40% at 150 mg twice daily, but daytime visual

PURPOSE: Recurrent brief vestibular attacks are often evaluated as peripheral disorders, but some reflect epileptic vestibular seizures requiring electroencephalographic (EEG) assessment. We sought to identify clinical clues distinguishing isolated epileptic vertigo/dizziness (iEVD) from vestibular paroxysmia (VP).
METHODS: We retrospectively compared 50 patients with iEVD and 50 with VP at a tertiary centre. Vestibular symptom descriptions were summarised according to the predominant motion percept documented in the initial clinical account. Clinical features, associated symptoms, EEG findin

BACKGROUND: The co-occurrence of seizure-associated psychosis and catatonia in epilepsy is diagnostically challenging, particularly when complex pre-existing psychiatric comorbidities complicate attribution. Few reports describe this triad in the context of an antidepressant switch.
CASE PRESENTATION: We report a 44-year-old woman with a 27-year history of epilepsy, maintained on carbamazepine and topiramate, and a longstanding psychiatric history of major depressive disorder, obsessive-compulsive symptoms, and chronic nihilistic delusions and intermittent hallucinations documented at baselin
