The 20-item Noise Pareidolia Task: normative data and performance in MCI-LB and DLB.

INTRODUCTION: Visual perceptual difficulties occur in dementia with Lewy bodies (DLB). Normative data and cut scores for DLB and mild cognitive impairment with Lewy bodies (MCI-LB) are needed for the 20-item Noise Pareidolia Task. METHODS: Participants included 344 cognitively unimpaired (CU), 112 DLB, and 105 MCI-LB. Cumulative percentile ranking was used for normative classification. Receiver operating characteristic analyses, convergent validity, and associations with visual hallucinations (VH) were assessed. RESULTS: In CU participants, a perfect score was average, one error was low aver

Hyperactive Delirium Temporally Associated With Hydroxyzine in a Child With Succinic Semialdehyde Dehydrogenase Deficiency Presenting With Psychotic Symptoms: A Case Report.

BACKGROUND: Succinic semialdehyde dehydrogenase deficiency (SSADHD) is a rare autosomal recessive disorder of gamma-aminobutyric acid (GABA) metabolism caused by biallelic variants in ALDH5A1. The phenotype includes developmental delay, intellectual disability, seizures, sleep disturbance, anxiety, and other neurobehavioral symptoms. Psychotic symptoms are increasingly recognized in SSADHD but remain uncommon and incompletely characterized in children. In medically and neurodevelopmentally vulnerable patients, acute psychiatric decompensation may also be complicated by delirium, especially whe

Lamotrigine-Associated Visual and Tactile Hallucinations in a Patient With Bipolar Disorder: A Case Report.

Bipolar disorder is a chronic psychiatric illness characterized by recurrent episodes of mood disturbance and significant functional impairment. Bipolar depression is one of the most common and disabling manifestations of the disorder, contributing substantially to its overall disease burden. Lamotrigine is recommended as a first-line treatment for acute bipolar depression and for the prevention of depressive relapse. It is also recommended as an adjunctive strategy in combination with mood stabilizers for patients with persistent depressive symptoms. Despite its favorable tolerability profile

Hallucinations associated with bilastine in a teenager: case report and disproportionality analysis in VigiBase®.

Bilastine is a second-generation H1-antihistamine considered to have minimal central nervous system penetration and low anticholinergic activity. Neuropsychiatric adverse drug reactions (ADRs), including hallucinations, are not expected with bilastine, although such reactions have been reported with other second-generation antihistamines (sgAH). We report the case of a 13-year-old girl who developed visual hallucinations and panic attacks shortly after starting treatment with bilastine, after having experienced a recurrent sensation of presence while taking desloratadine. The patient's symptom

Adapting Falls Prevention Evidence for People with Charles Bonnet Syndrome Across Clinical and Community Settings.

Charles Bonnet Syndrome (CBS) refers to visual hallucinations associated with a decline in vision. Historically, the functional effects of CBS have focused on its psychological impacts, but recent work has highlighted that CBS may also lead to physical outcomes by causing and increasing susceptibility to falls. As awareness of CBS among sight loss and falls prevention specialists is low, its role in the sight loss and falls mechanism may be missed. A falls prevention approach for CBS-related falls spanning primary, secondary, and community services is indicated to ensure this potential contrib

Disseminated Nocardia otitidiscaviarum Presenting as Lung-Brain-Skin Syndrome in a Renal Transplant Recipient: A Case Report.

We present a case of disseminated Nocardia otitidiscaviarum infection in a 72-year-old male with a history of renal transplantation on chronic immunosuppressive therapy with prednisone, cyclosporine, and mycophenolate mofetil. He presented with pneumonia and was initially treated with ceftriaxone and azithromycin. Bronchoalveolar lavage (BAL) and cultures from a biopsied thigh lesion both grew N. otitidiscaviarum. His hospital course was complicated by visual hallucinations, and brain magnetic resonance imaging (MRI) revealed multiple intracranial abscesses. He initially received meropenem, tr

Zopiclone-associated hallucinations: a case report and database pharmacovigilance study.

AIMS: Zopiclone is widely prescribed for insomnia, but the pharmacovigilance reporting signals for hallucinations remain poorly characterized. This study combines a case report and pharmacovigilance data to evaluate the association between zopiclone and hallucinations, compare hallucination subtype signals, and examine factors associated with higher reporting odds. METHODS: We reported a clinical case assessed by the Naranjo scale and analyzed the Canadian Vigilance Adverse Reaction Database (1994-2025) for zopiclone-associated hallucinations. Three disproportionality methods (ROR, PRR, and B

The eyes and Parkinsonian syndromes.

Parkinsonian degenerative syndromes, including Parkinson disease (PD), progressive supranuclear palsy (PSP), multiple system atrophy (MSA), corticobasal degeneration (CBD), and dementia with Lewy bodies (DLB), are frequently associated with ophthalmic manifestations that contribute substantially to impaired quality of life, reduced functional independence, and increased falls risk. These visual disturbances remain under-recognised in routine neurological practice despite their diagnostic and therapeutic relevance. This review summarises the spectrum of ophthalmic manifestations across Parkinso

Reduced volume of neuromodulatory brainstem nuclei is associated with early hallucination-related vulnerability in Parkinson's disease.

Early hallucination-related phenomena, including minor visual hallucinations (mVHs), are clinically relevant features of Parkinson's disease (PD), but their neuroanatomical substrates remain unclear. Brainstem nuclei supporting sensory gating, together with cholinergic and noradrenergic dysregulation, are proposed modulators of hallucinations. The objective of the present study was to test whether PD participants with early hallucination-related phenomena show reduced volumes of brainstem nuclei with cholinergic and noradrenergic relevance, using cross-sectional analysis of baseline structural

Psychiatric Nursing Intervention for Clozapine-Induced Dysphagia in Schizophrenia: A Case Report.

INTRODUCTION: Schizophrenia is a chronic mental disorder characterized by disruptions in thought processes, perceptions, emotional responsiveness, and social interactions. Clozapine, a second-generation antipsychotic, is commonly prescribed for treatment-resistant schizophrenia. However, its use is frequently associated with side effects, notably hypersalivation. This case report discusses the clinical consequences of clozapine-induced hypersalivation, emphasizing the importance of individualized nursing care planning. METHODS: The descriptive case report presents the clinical course of a 47-

Brain Structural and Functional Alteration Network Localization of Visual Hallucinations.

BACKGROUND AND HYPOTHESIS: Visual hallucinations (VH), a key symptom in neurodegenerative and psychiatric disorders, are associated with a more severe psychopathological profile and less favorable outcome. Neuroimaging research has revealed widespread brain regions involved in VH, echoing the updated notion that neuropsychiatric symptoms correspond more closely to interconnected brain networks than to single brain regions. However, there is still a dearth of studies examining brain network localization of VH. STUDY DESIGN: We initially identified brain structural and functional alterations sp

Bodily and Cognitive Experience in Patients With Sepsis and Delirium or Subsyndromal Delirium.

BACKGROUND: Sepsis often causes cognitive changes like delirium and subsyndromal delirium (SSD). Delirium involves acute cognitive dysfunction, while SSD presents partial symptoms. Both conditions impact prognosis and are linked to neuroinflammation, altered cerebral perfusion and neurotransmitter dysfunction. This study explores the embodied cognitive experience in sepsis, integrating enactivist perspectives on the dynamic interplay of body, environment and neural activity. AIM: To analyse the lived bodily and cognitive experiences of patients with sepsis who experienced delirium or SSD. ST

The distinctive psychopathology of NMDAR-antibody encephalitis compared with primary psychoses: an international, multicentre, retrospective phenotypic analysis.

BACKGROUND: N-methyl-D-aspartate receptor (NMDAR)-antibody encephalitis is a life-threatening neuropsychiatric disorder requiring prompt immunotherapy. The earliest features are mental-state changes, often mistaken for primary psychosis. Improved clinical differentiation could assist rational diagnostic investigation and expedite immunotherapy. Inspired by patients' and relatives' lived experience, we aimed to explore the psychiatric phenotype of NMDAR-antibody encephalitis and the features common to and distinct from real-world episodes of psychosis. METHODS: In this international, multicent

Time-resolved Neural and Experience Dynamics of Medium- and High-dose N,N-Dimethyltryptamine (DMT).

N,N-dimethyltryptamine (DMT) is a fast-acting psychedelic drug that induces a radical reorganization of conscious contents and brain dynamics. However, our understanding of how brain dynamics support psychedelic-induced conscious states remains unclear. We therefore present a repeated-measures dose-dependent study of the subjective and neural dynamics induced through DMT under naturalistic conditions. Nineteen participants received either a 20-mg or a 40-mg dose of freebase DMT across two sessions in a blinded, counterbalanced order. Electroencephalography data and time-resolved measures of su

Virtual reality for visions (VRV): a proof-of-concept study examining the development of a new treatment for distressing visual hallucinations in people with psychosis.

INTRODUCTION: Visual Hallucinations (VHs) (seeing things that others do not, or visions) are a common feature of psychosis, causing significant distress and disability. Services rarely ask about these important experiences, and crucially there are no proven beneficial psychological treatments. There are at least two key challenges faced when treating VHs. First, people report not knowing why they see things others don't, which leads them to feel alone and different from others. Second, they feel they cannot trust their own eyes to tell what is real or not, which can lead to fears they will be

Psychedelic 5-HT2A agonist increases spontaneous and evoked 5-Hz oscillations in visual and retrosplenial cortex.

Visual perception appears largely stable in time. However, psychophysical studies have revealed that low frequency (0.5 - 7 Hz) oscillatory dynamics can modulate perception and have been linked to various cognitive states and functions. Neither the contribution of waves around 5 Hz (theta or alpha-like) to cortical activity nor their impact during aberrant brain states have been resolved at high spatiotemporal scales. Here, using cortex-wide population voltage imaging in awake mice, we found that bouts of 5-Hz oscillations in the visual cortex are accompanied by similar oscillations in the ret

Effect on disease activity of ofatumumab in the treatment of glial fibrillary acidic protein astrocytopathy and 18F-DPA714 PET/MRI imaging assessment.

A 49-year-old male with Glial Fibrillary Acidic Protein Astrocytopathy (GFAP-A) presented with progressive cognitive decline, visual hallucinations, and bowel/bladder incontinence. Brain MRI revealed extensive white matter hyperintensities on T2-FLAIR sequences. After suboptimal response to first-line treatments including corticosteroids and intravenous immunoglobulin (IVIg), the patient was initiated on Ofatumumab (OFA) therapy. Treatment response was systematically monitored through validated clinical assessment tools (MMSE, MoCA, mRS, and CASE) and advanced neuroimaging, including conventio

Visual misperceptions in favour of living beings in Parkinson's disease: A psychophysical study.

BACKGROUND: Parkinson's disease (PD) patients often report seeing persons or animals rather than objects but this phenomenon remains poorly understood. Here, we use three experimental tasks to confirm such observation and to explore its cognitive mechanisms. METHOD: Fourteen PD patients with visual hallucinations (PD-VH), 14 PD patients without visual hallucinations (PD-NVH) and 14 controls with similar cognitive performance were tested using ambiguous stimuli. Ambiguous stimuli were morphs in which visual features from faces and flowers were melted together (Experiments 1 and 2) and a black

EEG changes associated with hallucinations caused by Charles Bonnet Syndrome.

Charles Bonnet syndrome (CBS) is characterised by the presence of visual hallucinations following visual loss in many patients. The neuropathophysiology of CBS is poorly understood. We used electroencephalography (EEG) in individuals with frequent hallucinations in order to identify changes in neural activity that co-occur with hallucination onset. We found reduced α power in occipital electrodes at the onset of the hallucinations compared to offset (p < 0.05), suggesting that intermittent periods of low endogenous alpha may create the neural conditions for hallucination to emerge in CBS patie
