Association between visual hallucinations and cognitive performance in Lewy body dementia and Alzheimer's disease: A cross-sectional study.

BACKGROUND: Visual hallucinations (VH) are an important neuropsychiatric feature of dementia. The association between VH and cognition remains controversial. OBJECTIVE: To investigate the differences in clinical correlates of VH and explore the associations between VH and cognitive functional decline in individuals with dementia with Lewy bodies (DLB) and Alzheimer's disease (AD). METHODS: Outpatient medical records of 154 patients with DLB and 297 patients with AD between January 2017 and December 2023 were reviewed. We collected demographic characteristics and used neuropsychological asses

The involvement of the cholinergic system in Parkinson disease.

In Parkinson disease (PD), cholinergic dysfunction develops in the early stages of the neurodegenerative process and progresses over time. Basal forebrain cholinergic system dysfunction is historically linked to cognitive decline in the dementia spectrum, and its pathophysiologic role in PD-related cognitive impairment has now been well established. However, cholinergic system dysfunction is also linked to several other manifestations of PD, such as gait difficulties, REM sleep behavior disorder (RBD), neuropsychiatric manifestations such as depression and visual hallucinations, and olfactory

The cholinergic system in dementia with Lewy bodies.

Dementia with Lewy bodies (DLB) is a progressive neurodegenerative disorder pathologically characterized by the presence of neuronal intracytoplasmic inclusions known as Lewy bodies. Core clinical features include fluctuating cognitive impairment, recurrent visual hallucinations, REM sleep behavior disorder, and Parkinsonism. Cholinergic dysfunction is implicated in many of the symptoms of DLB, based on both pathologic and functional imaging studies, as well as the clear symptomatic response of cognitive and behavioral symptoms to drugs that modulate the cholinergic system. In this chapter, we

A case of dementia with Lewy bodies with psychosis induced by low-dose gabapentinoids.

BACKGROUND: Hypersensitivity to antipsychotic drugs is one of the supportive features of dementia with Lewy bodies, and side effects to drugs other than antipsychotics are also known to occur frequently. We experienced a case of dementia with Lewy bodies in which hallucinations and delusions repeatedly appeared and disappeared after administration and discontinuation of mirogabalin and pregabalin. CASE PRESENTATION: The patient, a woman in her late 70s, developed hallucinations and delusional misidentification of places and persons immediately after receiving a prescription of mirogabalin (15

Sex differences in prodromal dementia with Lewy bodies using the National Alzheimer's Coordinating Center data.

INTRODUCTION: Males are disproportionately represented in dementia with Lewy bodies (DLB). Little is known regarding sex differences in cognitive, neuropsychiatric, and motor features of the prodromal stage of DLB. METHODS: We used National Alzheimer's Coordinating Center longitudinal data to examine cognitive, neuropsychiatric, and motor features in 120 males and 23 females 2 years prior to a clinical diagnosis of DLB. RESULTS: Males were more likely than females to present with two or more core DLB clinical features 2 years prior to obtaining a diagnosis of dementia. Rapid eye movement sle

Cyberdelics: Virtual reality hallucinations modulate cognitive-affective processes.

INTRODUCTION: Psychedelics were explored for their potential in the mental health field. However, research was delayed by concerns over short-term side effects and long-term consequences of substance use. Technological advances enabled the development of Hallucinatory Visual Virtual Experiences (HVVEs), namely psychedelic experiences simulations in immersive virtual reality. This study investigated HVVEs' impact on cognitive flexibility, affective response, and autonomic activity. METHODS: 50 healthy participants underwent assessments of cognitive flexibility, control inhibition, emotional re

Awareness of Charles Bonnet syndrome among ophthalmologists: a survey study.

PURPOSE: Charles Bonnet Syndrome (CBS) is a condition characterized by complex visual hallucinations in individuals with significant visual impairment. Despite its prevalence, CBS remains under-recognized, leading to delayed diagnoses and unnecessary distress for patients and their families. Awareness among ophthalmologists is critical for early identification and management. This study aims to evaluate the awareness and understanding of CBS among ophthalmologists and identify gaps in knowledge, diagnosis, and management practices. METHODS: A cross-sectional survey of 322 ophthalmologists was

Anti-NMDA Receptor Encephalitis: A Narrative Review.

Antibodies against the NR1 or NR2 subunits of the NMDA receptor are linked to anti-N-methyl-D-aspartate (NMDA) receptor encephalitis, a type of encephalitis that mainly affects women. Clinicians who treat patients of all ages should be aware of this type of encephalitis since it may be a treatable differential for symptoms and indicators observed in neurology and psychiatric clinics. Auditory and visual hallucinations, delusions, altered behavior (often accompanied by agitation), reduced consciousness, motor disruption (from dyskinesia to catatonia), seizures, and autonomic dysfunction are typ

Minor Visual Phenomena in Lewy Body Disease: A Systematic Review.

Minor visual phenomena (MVP), such as visual illusions, pareidolias, feeling of presence, and passage hallucinations, are often experienced by patients with Lewy Body Disease (LBD), in addition to complex visual hallucinations (VH), even in the early stages of the disease. This systematic review aimed to provide an up-to-date literature review of the occurrence and prevalence of MVP in LBD and to assess their potential associations both with VH and visuoperceptual and visuospatial deficits. A systematic literature search was carried out in PubMed, Web of Science, APA PsycInfo, Scopus, and Coch

Multi-modal connectivity of the brain underlying visual hallucinations evoked by high-frequency intracranial stimulations.

BACKGROUND: Visual hallucinations are a common symptom in patients with epilepsy. To the best of our knowledge, there are no studies evidencing the functional and effective networks responsible for different characteristics of these manifestations. OBJECTIVE/HYPOTHESIS: In this study we aim to perform a multi-modal characterization of the networks underlying visual hallucinations that could be used for clinical purposes. METHODS: Patients suffering from drug-resistant epilepsy in whom visual hallucinations were elicited by alternating polarity high frequency stimulation (AP-HFS) during stere

Disrupted functional brain network associated with presence of hallucinations in Parkinson's disease.

Hallucinations negatively impact quality of life in Parkinson's disease, yet their neural mechanisms remain poorly understood, particularly in early disease stages. This study aimed to identify functional connectivity differences associated with visual hallucinations in early Parkinson's disease and to validate these findings across independent datasets. Resting-state functional MRI data from two independent studies were used: the 'Parkinson's Progression Markers Initiative' dataset was used as a discovery cohort (N = 25 hallucinators, N = 56 non-hallucinators) and the 'Incidence of Cognitive

Challenges in the diagnosis and management of depressive and cognitive disorders in older adults: a case report.

Dementia with Lewy Bodies and depression are two common conditions in older adults that can share clinical similarities or coexist. We aim to highlight the diagnostic and therapeutic challenges of these two pathologies through the case of Mrs. S., a 67-year-old patient admitted to the psychiatric department for agitation and insomnia. Her symptoms initially presented as depression, followed by severe agitation. In the admission interview, we observed an anxious patient with bradyphemia, primarily focused on well-detailed visual hallucinations related to death. Upon neurological examination, we

Visual hallucinations in neurosurgery: A systematic review and two case insights into Charles Bonnet Syndrome.

INTRODUCTION: Visual disturbances in brain tumor patients occur as a direct consequence of tumor location or as a complication after surgery. These can range from minor vision changes to severe impairments, including visual hallucinations. RESEARCH QUESTION: Charles Bonnet Syndrome (CBS) is an underrecognized cause of complex visual hallucinations in patients with visual system disorders, particularly following neurosurgical interventions. We present two cases of CBS following neurosurgical procedures, recruited from our department. MATERIALS AND METHODS: Detailed patient histories, neurolog

Severe kava withdrawal managed with phenobarbital.

Piper methysticum, known as "kava", is a plant endemic to and historically consumed in the Pacific islands. The roots contain psychoactive kavalactones with sedating and anxiolytic effects. While often marketed for anxiety or as a safe alcohol alternative, dermopathy and more rarely hepatotoxicity are well described with heavy use. Reports of withdrawal are rare. The leaves of Mitragyna speciosa, also referred to as "kratom", contain psychoactive alkaloids with interactions at μ and δ-opioid receptors. Kratom is commonly used for its stimulant and opioid-like effects, for which dependence and

Progression of clinical diagnostic features and cognitive decline in mild cognitive impairment with Lewy bodies.

BACKGROUND: Mild cognitive impairment with Lewy bodies (MCI-LB) may be identified prospectively based on the presence of cognitive impairment and several core clinical features (visual hallucinations, cognitive fluctuations, parkinsonism, and REM sleep behavior disorder). MCI-LB may vary in its presenting features, which may reflect differences in underlying pathological pattern, severity, or comorbidity. We aimed to assess how clinical features of MCI-LB accumulate over time, and whether this is associated with the rate of cognitive decline. METHODS: In this cohort study, 74 individuals seen

Exploring frailty in dementia with Lewy bodies: A pilot, cross-sectional study.

We aimed to investigate frailty in dementia with Lewy bodies (DLB) and its association with its core features. We retrospectively reviewed the clinical charts of 48 DLB outpatients. Frailty Index (FI) was generated by computing 39 age-related, multidimensional clinical deficits. FI scores varied from 0.08 to 0.54, with a median value of 0.26. FI showed a correlation with the severity of parkinsonism and visuo-spatial impairment, suggesting that some DLB features might be more influenced by frailty, whereas others, such as visual hallucinations, fluctuations, REM-behavior-disorder, seem not. Expanding the Spectrum of Charles Bonnet Syndrome: Severe Psychiatric Manifestations Associated With Total Vision Loss.

Charles Bonnet syndrome (CBS) is typically characterized by complex, non-threatening visual hallucinations in patients with visual impairment who maintain insight that their perceptions are unreal. While primarily considered an ophthalmological phenomenon, recent evidence suggests that CBS may overlap with psychiatric disorders presenting with complex visual hallucinations and impaired insight, complicating both diagnosis and management. These interactions challenge the view of CBS as a benign and isolated condition, especially when symptoms become more entrenched, resemble psychosis, or co-oc
