Visual hallucinations in Parkinson's disease: The critical role of intermediate nucleus basalis of Meynert.

BACKGROUND AND OBJECTIVE: Patients with Parkinson's disease (PD) often experience visual hallucinations (VH) and delusions. PD patients with VH reportedly have a higher incidence of dementia than PD patients without VH. The nucleus basalis of Meynert (nbM) comprises acetylcholine-releasing neurons that are critical for memory, attention, and arousal. Notably, the nbM also exhibits pathological α-synuclein accumulation, which is a hallmark of Lewy body diseases, including PD. In the present study, we investigated the distribution of abnormal α-synuclein accumulation in the nbM. METHODS: nbM se

Cross-Species Evidence for Psilocin-Induced Visual Distortions: Apparent Motion Is Perceived by Both Humans and Rats.

BACKGROUND: Psychedelics, particularly psilocin, are increasingly being studied for their mind-altering effects and potential therapeutic applications in psychiatry. Visual hallucinations, especially the illusion of motion in static images, are a hallmark of their action. Despite growing interest, the underlying mechanisms remain poorly understood, as their systematic evaluation in both humans and animals is challenging. METHODS: To investigate psilocin-induced visual distortions, we designed a 2-choice visual discrimination task. Human participants and male rats indicated whether an image ap

The Role of Spirituality and Religiosity in the Maintenance and Recovery of Psychosis: A Systematic Review.

OBJECTIVE: Many individuals with psychosis consider themselves religious or spiritual and report using religion as a means of coping with their illness. However, research exploring the impact of religiosity and spirituality on the experience of psychosis is sparse, with most studies focusing on delusions or hallucinations with religious content. METHODS: A systematic review examined the evidence regarding the role of religiosity/spirituality in the maintenance and recovery of psychosis. RESULTS: A total of 35 studies were eligible for inclusion. In terms of maintenance, religiosity and spiri

Coexistence of epilepsy with eyelid myoclonia, schizoaffective disorder, and cavum septi pellucidi et vergae: A case report.

We report the first case of a 14-year-old boy presenting with the rare co-occurrence of cavum septi pellucidi et vergae (CSPV), epilepsy with eyelid myoclonia (EEM), and schizoaffective disorder (SAD). The patient initially presented with a one-month history of abnormal eye movements, including continuous blinking and upward rolling of the eyeballs, which occurred predominantly at night. There was no alteration in consciousness. A detailed history revealed that the patient had experienced mood disturbances, delusional beliefs, auditory and visual hallucinations, and significant behavioral dysr

Psychedelic 5-HT2A agonist increases spontaneous and evoked 5-Hz oscillations in visual and retrosplenial cortex.

Visual perception appears largely stable in time. However, psychophysical studies have revealed that theta waves (4-8 Hz) can modulate perception and have been linked to various cognitive states and functions. Neither the contribution of theta waves to cortical activity nor their impact during aberrant brain states have been resolved at high spatiotemporal scales. Here, using cortex-wide population voltage imaging in awake mice, we found that bouts of 5-Hz oscillations in the visual cortex are accompanied by similar oscillations in the retrosplenial cortex, occurring both spontaneously and evo

Addressing chronic visual hallucination by multimodal retinal imaging: a CBS case.

Charles Bonnet syndrome (CBS) is characterised by visual hallucinations in individuals with significant visual impairment. This literature review and case report focus on the unique presentation of CBS in an 82-year-old woman with age-related macular degeneration, who experienced visual hallucinations for over 10 years. The aim is to raise awareness of CBS among healthcare professionals and the public, addressing the diagnostic challenges that contribute to its underdiagnosis and mismanagement. A literature review was conducted to assess the prevalence and diagnosis of CBS. Databases including

Delusion of Misidentifying of Parents as Infants as a Subtype of Intermetamorphosis Syndrome: A Case Report.

Delusional misidentification syndromes (DMSs) are a group of disorders, characterized by consistent misidentification of individuals, locations, objects, or events. Four primary subtypes are recognized within this classification: Capgras syndrome, Frégoli syndrome, intermetamorphosis syndrome, and the syndrome of subjective doubles. We report a case of a woman with schizophrenia who experienced a strange delusion that her parents were her babies. This delusion did not arise from visual hallucinations of infants or from prosopagnosia. Furthermore, she denied experiencing auditory hallucinations

Atypical presentation for a gastrointestinal basidiobolomycosis and rare side effect of Voriconazole: A case report and literature review.

Gastrointestinal basidiobolomycosis (GIB) is a rare fungal infection often misdiagnosed with other gastrointestinal disorders. We report a case of a 6-year-old boy from Saudi Arabia who presented with fever, abdominal pain, and weight loss, initially suspected to have inflammatory bowel disease. Despite antibiotics, symptoms persisted, leading to a GIB diagnosis with liver involvement. IV voriconazole improved his condition but caused visual hallucinations, managed by dose adjustments. This case underscores the need for early suspicion of GIB in endemic areas and highlights the importance of p

The relationship between paracingulate sulcus length and visual hallucinations in Parkinson's disease suggests a neurobiological predisposition.

Visual hallucinations (VH) are common in Parkinson's disease (PD), yet their mechanisms remain poorly understood. Several studies have investigated structural brain correlates of Parkinsonian VH, but critical gaps in knowledge persist. An inverse relationship between auditory hallucinations and paracingulate sulcus (PCS) length, associated with reality-monitoring mechanisms, has been reported. This study examines the relationship between PCS length and VH in PD patients. Sixty-five PD patients (aged 48-81 years) meeting diagnostic criteria were included. The University of Miami Parkinson's Dis

Stroboscopically induced visual hallucinations: historical, phenomenological, and neurobiological perspectives.

Exposure to rapid and bright stroboscopic light has long been reported to induce vivid visual hallucinations of colour and geometric formations. This phenomenon was first documented by Purkinje over 200 years ago. Since then, significant progress has been made in understanding the effects of stroboscopic light and the experiences it induces through multiple waves of interest from the scientific, therapeutic, and broader cultural communities. Despite these advances, fundamental questions remain unanswered, including comprehensive characterizations of its phenomenology, its precise physiological

Introducing the pictogram-based ocular motor and visual-perceptual symptom scale: a multinational, cross-cultural feasibility study.

BACKGROUND: Patients with vestibular and ocular motor disorders often perceive oscillopsia, diplopia or visual hallucinations as their chief complaint. However, they often struggle with verbalizing these subjective ocular motor and visual-perceptual signs precisely, which complicates a correct diagnostic classification of the suspected pathogenic mechanism. METHODS: In this multinational and cross-cultural feasibility study, a novel pictogram-based scale of 10 common ocular motor and visual-perceptual symptoms (called Pictogram Ocular Motor and Visual-Perceptual Symptom Scale, POVSS) was deve

Dynamic medial parietal and hippocampal deactivations under DMT relate to sympathetic output and altered sense of time, space, and the self.

N,N-Dimethyltryptamine (DMT) is a serotonergic psychedelic, known to rapidly induce short-lasting alterations in conscious experience, characterized by a profound and immersive sense of physical transcendence alongside rich and vivid auditory distortions and visual imagery. Multimodal neuroimaging data paired with dynamic analysis techniques offer a valuable approach for identifying unique signatures of brain activity-and linked autonomic physiology-naturally unfolding during the altered state of consciousness induced by DMT. We leveraged simultaneous fMRI and EKG data acquired in 14 healthy v

Combined Dysfunction of the Amygdala and Nucleus Basalis Underlies Visual Hallucinations in Parkinson's Disease.

BACKGROUND: Visual hallucinations (VHs) are a common feature of Parkinson's disease (PD) believed to arise from disruptions to the functional architecture supporting sensory integration and attentional control. Across synucleinopathies, increased pathological burden in the amygdala and deficits in cholinergic modulation have been linked to VHs. However, the interaction of these changes and their combined contribution to the neurobiological mechanisms underlying hallucinatory phenomena remain poorly understood. OBJECTIVES: To investigate the convergent impact of amygdala and cholinergic dysfun

Association of the number of minor hallucinations and well-structured visual hallucinations in Parkinson's disease.

Minor hallucinations (MH), comprising three subtypes (presence hallucinations, passage hallucinations, and visual illusions), typically emerge during the early stages of Parkinson's disease (PD) and precede the onset of well-structured visual hallucinations (VH). Whether the number of MH is associated with VH in PD patients remains unclear. We investigated the association between the number of MH and VH in 372 PD patients. Log-binomial regressions showed that individuals with multiple MH had a significantly higher prevalence of VH (RR = 2.08, 95% CI: 1.11-3.90 for 2 MH; RR = 6.20, 95% CI: 3.40
