Charles bonnet syndrome with subsequent depression in acute angle-closure glaucoma: a case report.

Charles Bonnet syndrome (CBS) presents with complex visual hallucinations in visually impaired individuals with preserved insight. While most commonly associated with macular degeneration, it may occur in glaucoma. CBS is often under-reported due to stigma and may progress to psychiatric morbidity. A 63-year-old man with bilateral visual loss following acute angle-closure glaucoma experienced vivid hallucinations of animals and snakes. Initially concealed due to embarrassment, the symptoms were revealed during psychiatric assessment. Ophthalmic evaluation confirmed glaucomatous changes; MRI br

Altered EEG microstate transition patterns and visual hallucinations in Parkinson's Disease.

BACKGROUND: Visual hallucinations (VH) in Parkinson's disease (PD) have been linked to alterations in functional brain networks. EEG microstate analysis enables the examination of large-scale network dynamics at the millisecond scale. This study aimed to investigate whether specific EEG microstate transition patterns differ between PD patients with and without VH, and to determine their predictive value. METHODS: A total of 38 PD patients (18 VH+, 20 VH-) underwent 5-min, eyes-closed resting-state EEG recordings. Microstate segmentation was performed using a modified k-means clustering algori

Focal Cerebral Arteriopathy in a Young Adult.

Focal cerebral arteriopathy (FCA) is a rare, monophasic stenosis of the distal internal carotid artery (ICA) or proximal middle cerebral artery that primarily affects children but is occasionally seen in adults. It is unclear what causes FCA in adults, which complicates diagnosis and management. The case presented highlights diagnostic challenges using multimodal imaging, supporting conservative management, and the need for adult-specific treatment criteria. A 38-year-old woman with a history of breast cancer, migraines, and prior pulmonary embolism presented with a sudden-onset thunderclap he

Mapping of Subjective Accounts into Interpreted Clusters (MOSAIC): Topic Modelling and LLM applied to Stroboscopic Phenomenology.

Stroboscopic light stimulation (SLS) on closed eyes typically induces simple visual hallucinations, characterized by vivid, geometric, and colourful patterns. A dataset of 898 sentences, extracted from 407 open subjective reports, was recently compiled as part of the Dreamachine programme (https://dreamachine.world/) (Collective Act, 2022), an immersive multisensory experience that combines SLS and spatial sound in a collective setting. Although open reports extend the range of reportable phenomenology, their analysis presents significant challenges, particularly in systematically identifying

Visual Hallucinations and Illusions as Missed Diagnostic Clues in Autoimmune Disease: A Patient Perspective.

Non-psychotic visual hallucinations and illusions (referred to here as NVHIs), such as visual snow, can be indicators of serious underlying pathologies, including Systemic Autoimmune Rheumatic Diseases. In this Patient Perspective article, I recount some of the impacts of NVHIs, and their attribution/misattribution, on my autoimmune diagnostic journeys. I then draw on these experiences, and the literature, to suggest improvements to policies and practice. It is hoped that these types of changes could contribute to NVHI-related diagnostic opportunities being less often missed.

Modeling the hallucinatory effects of classical psychedelics in terms of replay-dependent plasticity mechanisms.

Classical psychedelics induce complex visual hallucinations in humans, generating percepts that are coherent at a low level, but which have surreal, dream-like qualities at a high level. While there are many hypotheses as to how classical psychedelics could induce these effects, there are no concrete mechanistic models that capture the variety of observed effects in humans, while remaining consistent with the known pharmacological effects of classical psychedelics on neural circuits. In this work, we propose the 'oneirogen hypothesis,' which posits that the perceptual effects of classical psyc

Cytotoxic lesion of the corpus callosum after gamma hydroxybutyrate withdrawal: a case report.

BACKGROUND: Gamma-hydroxybutyrate (GHB) is frequently misused for its sedative and euphoric effects. Abrupt cessation may result in severe withdrawal; however, neuroimaging correlates of withdrawal-related neurotoxicity remain poorly defined. CASE PRESENTATION: A 33-year-old male developed acute delirium, visual hallucinations, and autonomic instability after abruptly discontinuing frequent and high-dose GHB use. Routine toxicology screening was negative. Brain MRI revealed a focal splenial cytotoxic lesion of the corpus callosum (CLOCCs). He was clinically diagnosed with sedative, hypnotic,

A Case of Lisinopril-Induced Hallucinations.

BACKGROUND: Lisinopril is generally well tolerated, but rare neuropsychiatric adverse effects have been reported. CASE: We describe an 89-year-old woman who developed vivid nocturnal auditory and visual hallucinations approximately 3 months after starting lisinopril for hypertension. Neurologic examination, imaging, and laboratory evaluation were unremarkable. Hallucinations resolved completely within 2 weeks of drug discontinuation, with no recurrence on follow-up. CONCLUSION: Older adults may be particularly susceptible to medication-induced neuropsychiatric effects. Recognition of lisinop

From dots to faces: individual differences in visual imagery capacity predict the content of Ganzflicker-induced hallucinations.

A rapidly alternating red and black display known as Ganzflicker induces visual hallucinations that reflect the generative capacity of the visual system. Individuals vary in their degree of visual imagery, ranging from absent to vivid imagery. Recent proposals suggest that differences in the visual system along this imagery spectrum should also influence the complexity of other internally generated visual experiences. Here, we used tools from natural language processing to analyze free-text descriptions of hallucinations from >4000 participants, asking whether people with different imagery phe

'Everywhere I turn, I'm blocked': a qualitative exploration of experiences of Charles Bonnet syndrome and its impact on physical activity and falls.

BACKGROUND: Charles Bonnet syndrome (CBS) refers to visual hallucinations that can occur following a decline in vision. Although vision loss increases fall risk and susceptibility to falls-related variables such as activity limitation, the potential contribution of CBS is unclear. The present study adopted a qualitative approach to explore experiences of CBS from different perspectives, including its impact on physical activity and falls. METHODS: Semistructured interviews were conducted with 17 older adults living with CBS (mean age = 76 years) and 12 community falls and sight loss professio

Hyperactivity and Euphoria Without Agitation Due to Hyoscyamus niger (Henbane) Poisoning in a Child.

Hyoscyamus niger (henbane) is a toxic plant containing tropane alkaloids that can cause anticholinergic toxicity. Pediatric presentations may differ from adults and may include atypical behavioral manifestations such as hyperactivity and euphoria without agitation. An 11-year-old previously healthy boy presented with sudden behavioral changes, hyperactivity, and unexplained euphoria. Approximately four hours earlier, he had ingested about one tablespoon of fresh Hyoscyamus niger seeds. Physical examination revealed minimal mydriasis and mild flushing. The patient was cooperative but disoriente

Comorbidity Between Anti-GAD65 Autoimmune Encephalitis and Behavioral Variant Frontotemporal Dementia: A Case Report.

Background and clinical significance: Autoimmune encephalitis (AE) is an inflammatory brain disorder that manifests through a diverse, unspecific range of neuropsychiatric symptoms. When AE occurs alongside a primary neurodegenerative disorder, the shared symptoms can create a mixed clinical profile, making diagnosis more difficult and potentially postponing effective management and treatment. Case presentation: We describe the case of a 58-year-old female with a one-year history of progressive behavioral and personality changes who presented a subacute confusional state, psychomotor retardati

Predictors of postoperative visual and auditory hallucinations in hypertensive vs. non-hypertensive cardiac surgery patients: insights from the prospective multicenter VAACS study.

BACKGROUND: Hallucinations are common yet under-recognized after cardiac surgery and contribute to postoperative morbidity. Hypertension (HTN), the most prevalent comorbidity in surgical candidates, may alter susceptibility through impaired cerebrovascular autoregulation. Whether HTN modifies predictors of postoperative visual and auditory hallucinations remains unclear. METHODS: In the prospective, multicenter VAACS study, 1,332 adults undergoing CABG or valve surgery were stratified by hypertension (HTN, n = 517; non-HTN, n = 815). Visual and auditory hallucinations within 7 days were asses

Understanding healthcare accessibility for military veterans living with Charles Bonnet Syndrome: co-production of a psychoeducational toolkit.

INTRODUCTION: Charles Bonnet Syndrome (CBS) is a condition characterized by visual hallucinations in individuals with visual impairments. Among military veterans, CBS can cause considerable distress, leading to health-related anxiety and delays in seeking help. Despite its prevalence, awareness of CBS remains limited across healthcare services, resulting in underdiagnosis and inadequate support. There is a need to better understand experiences of managing CBS and accessing healthcare services, from both healthcare and service user perspectives. Additionally, tailored resources are needed to ad

Visual Hallucinations and Cognitive Impairment as the Initial Presentation of Severe Hypothyroidism in an Elderly Patient: A Case Report.

Background: Hypothyroidism is a prevalent endocrine disorder capable of affecting multiple organ systems. While somatic symptoms are common, prominent neuropsychiatric manifestations such as visual hallucinations and cognitive deficits are exceptionally rare as an initial presentation. This atypical clinical picture frequently leads to misdiagnosis as primary psychiatric disorders or neurodegenerative conditions. Case Presentation: We report the case of a 75-year-old male who presented with a three-month history of complex visual hallucinations and progressive cognitive decline. The hallucinat
