Myeloid sarcoma of the septum pellucidum causing hallucinations and personality changes in an elderly woman: a case report.

Myeloid sarcoma is an extramedullary tumor composed of myeloid blasts, most commonly involving the skin, lymph nodes, and soft tissues. We report a rare case of myeloid sarcoma of the central nervous system (CNS) causing neuropsychiatric symptoms in an elderly woman with a history of JAK2-positive polycythemia vera and acute myeloid leukemia (AML) in long-term remission. The patient presented with a one-month history of complex visual hallucinations, short-term memory loss, and personality changes. Magnetic resonance imaging revealed a 2.4-cm enhancing mass in the septum pellucidum extending b

Abnormal periodic and aperiodic resting-state electroencephalographic markers in Lewy body and Alzheimer's diseases with cognitive decline.

Lewy body disease (LBD) and Alzheimer's disease (AD) are the most common causes of cognitive decline and dementia and are associated with characteristic alterations in resting-state electroencephalographic (rsEEG) activity. This multicenter exploratory study investigated periodic and aperiodic rsEEG features in patients with cognitive decline due to Lewy body disease (LBCD) and Alzheimer's disease (ADCD), compared with cognitively unimpaired older adults (Nold), and examined the clinical relevance of these markers in LBCD. A total of 140 LBCD, 135 ADCD, and 118 Nold datasets from the PDWAVES a

Nursing care of a pediatric patient with anti-NMDAR encephalitis complicated by secondary epilepsy: A case report.

RATIONALE: Autoimmune encephalitis in children often presents with neuropsychiatric symptoms and seizures and may progress to secondary epilepsy with prolonged functional impairment. Immunotherapy is the principal disease-modifying treatment, whereas comprehensive nursing care supports safety, complication prevention, rehabilitation, and continuity of care. PATIENT CONCERNS: A 13-year-old girl presented with a 2-month history of intermittent dizziness and poor appetite, followed by abnormal behavior, mood disturbance, visual hallucinations, and seizures. DIAGNOSES: Based on characteristic ne

Seven tesla MRI reveals amygdala and hippocampal subfield atrophy in dementia with Lewy bodies.

The 7T-DLB study investigated whether atrophy of individual hippocampal and amygdala subfields is present in dementia with Lewy bodies (DLB) as well as in Alzheimer's disease (AD) in which it has previously been established. MRI at standard clinical and research field strengths may have insufficient resolution to reveal the presence of atrophy in these regions in DLB; seven tesla MRI is ideally placed to examine subtle volumetric differences in these brain areas due to increased signal to noise ratio and tissue contrast. In this prospective, cross-sectional, seven tesla imaging study, 20 partic

Anatomo-clinical correlations in patients with lateral occipital seizures: A systematic review.

OBJECTIVE: Epilepsies presenting with lateral occipital seizures (LOS) pose diagnostic challenges due to their rarity and association with complex visual symptoms. We performed a systematic review to characterize the ictal semiology, electroencephalographic, imaging, and surgical features of LOS. METHODS: A comprehensive literature search was conducted between 1990 and March 2024 to identify patient-level studies reporting focal seizures originating from the occipital lobe, confirmed whenever possible by invasive EEG, MRI lesion, and postoperative outcome. Cases with occipital semiology due t

Pediatric occipital lobe epilepsy: A modern review of etiological classification, management, and outcomes.

Pediatric occipital lobe epilepsy (OLE) comprises syndromes with seizures originating from the posterior cerebral cortex. Previously "benign", recent International League Against Epilepsy (ILAE) updates reclassified these as self-limited or structural focal epilepsies, reflecting their complex causes and morbidity potential. The 2017 ILAE guidelines shifted from syndromic recognition to an etiology-driven approach. High-resolution magnetic resonance imaging is crucial to differentiate self-limited genetic syndromes from structural OLE (e.g., focal cortical dysplasia), as the latter often requi

Network based statistics associates increased connectivity to REM sleep disorder and hallucinations in early DLB.

Dementia with Lewy Bodies (DLB) is a heterogeneous disease characterized by four core clinical features: visual hallucinations, REM sleep behaviour disorder (RBD), cognitive fluctuations and Parkinsonism. We study the relationship between these features and functional connectivity from high-density electroencephalographic data of 33 subjects with early-stage DLB and 21 healthy controls. We used two connectivity metrics averaged on the individual theta and alpha bands, defined by determining the individual theta-to-alpha transition frequency through the package transfreq. After showing that DLB

Toxic encephalopathy due to self-administration of over-the-counter amantadine: a case report of a patient undergoing maintenance hemodialysis and safety alert.

This report describes a 49-year-old male patient with end-stage renal disease who developed acute mania, incoherent speech, and vivid visual hallucinations after self-administering an over-the-counter cold medication containing amantadine during regular hemodialysis. Laboratory toxicology analysis confirmed toxic encephalopathy caused by significantly elevated blood amantadine levels. Following five consecutive days of combined hemoperfusion and hemodialysis for intensive clearance, the patient achieved complete resolution of neuropsychiatric symptoms. This case highlights the unique high-risk

Trauma and hallucinations across the sensory modalities: the importance of considering experiences in childhood and adulthood.

It is widely acknowledged that trauma is related to auditory hallucinations in psychotic disorders. Yet, this relationship has not been well explored in hallucinations across other sensory modalities. The current study investigated whether hallucination modality groups (auditory, visual, olfactory, tactile, multisensory) were significantly related to physical, sexual, or emotional trauma. The age at which the trauma was experienced (16 or below, 17 and above) was also considered. Multivariate linear models, analysis of variance, and regression analyses for sixty-six participants with a psychot

A case of pure autonomic failure with a long-term-45-year-clinical course since the initial detection of abnormal blood pressure fluctuation.

This report describes the case of an 85-year-old male with a clinical phenotype consistent with pure autonomic failure (PAF) for more than four decades. Despite the presence of neurological manifestations, including bradykinesia, olfactory dysfunction, and visual hallucinations, phenoconversion to another α-synucleinopathy has not been confirmed based on established diagnostic criteria. Orthostatic and postprandial hypotension, low supine plasma norepinephrine levels, and reduced cardiac uptake on 123I-metaiodobenzylguanidine myocardial scintigraphy were observed, consistent with the postgangl

Understanding the genetic imperfections of Lewy body dementia.

Lewy body dementia (LBD) is a clinically and pathologically complex neurodegenerative disease. Its etiology remains poorly understood and, as such, it is likely underdiagnosed in clinical practice. Clinically, LBD is defined by fluctuations in cognition, visual hallucinations, rapid eye movement (REM) sleep behavior disorder (RBD), and motor dysfunction. The disease is pathologically defined by the presence of Lewy bodies in the brain, which are protein aggregates made up of misfolded α-synuclein; however, co-pathologies are often present and greatly complicate both symptom presentation and re

Benzodiazepine Withdrawal Masked by Clonidine: An Emergency Presentation With a Diagnostic Challenge.

BACKGROUND: Benzodiazepines are central nervous system depressants that act on GABA-A receptors and enhance the activity of the inhibitory neurotransmitter gamma-aminobutyric acid (GABA). Prolonged use of benzodiazepines carries a high risk of physiological dependence and abrupt discontinuation following high-dose or long-term benzodiazepine use may precipitate withdrawal. Benzodiazepine withdrawal symptoms include autonomic hyperarousal (e.g., tachycardia, hypertension, sweating and tremors), anxiety, insomnia, confusion and rarely psychotic features including delusions and hallucinations. 4F-MDMB-BICA toxicity: a fatal case and literature review of synthetic cannabinoid fatalities.

We report the death of a 32-year-old woman with a history of tobacco smoking who was found dead in her car after being investigated 18-days prior in hospital following a road traffic accident. Postmortem examination showed no evidence of recent injury. The most significant finding on internal examination was severe single-vessel coronary atherosclerosis. The remainder of the autopsy showed smoking-associated lung changes. Toxicologic studies were negative for common medications, usual street drugs and alcohols, but specialized analytical testing, developed in-house, identified the indazole-bas

Transient Ischemic Attack Progressing to Top-of-the-Basilar Syndrome: A Case Report.

BACKGROUND Top-of-the-basilar syndrome (TOBS) is a rare but clinically heterogeneous posterior circulation disorder characterized by acute ischemia affecting 2 or more territories supplied by the distal basilar artery, including the thalami, midbrain, occipital lobes, medial temporal lobes, and cerebellum. Intravenous thrombolysis with recombinant tissue plasminogen activator (rt-PA) is an established treatment for eligible patients with TOBS, and reports of transient coma induced during rt-PA infusion remain exceedingly scarce. Distinguishing isolated epileptic vertigo/dizziness from vestibular paroxysmia: Clinical clues from a retrospective comparative study.

PURPOSE: Recurrent brief vestibular attacks are often evaluated as peripheral disorders, but some reflect epileptic vestibular seizures requiring electroencephalographic (EEG) assessment. We sought to identify clinical clues distinguishing isolated epileptic vertigo/dizziness (iEVD) from vestibular paroxysmia (VP). METHODS: We retrospectively compared 50 patients with iEVD and 50 with VP at a tertiary centre. Vestibular symptom descriptions were summarised according to the predominant motion percept documented in the initial clinical account. Clinical features, associated symptoms, EEG findin

Resting-state functional connectivity alterations in dementia with Lewy bodies: a systematic review and activation likelihood estimation meta-analysis.

Dementia with Lewy Bodies is a neurodegenerative disease characterized by fluctuating cognition, visual hallucinations and parkinsonian features. Resting-state functional MRI has been increasingly used to investigate functional connectivity alterations in this disease, though findings have been inconsistent. This systematic review and meta-analysis aim to comprehensively synthesize existing evidence on connectivity changes in this condition to clarify underlying networks dysfunctions and provide future research directions. We analyzed studies from EMBASE, PubMed and Scopus. Thirteen eligible s

When headache meets visual disturbance: An evidence-based review of common and critical entities.

Concurrent headache and visual symptoms pose a diagnostic challenge across specialties, ranging from benign to life-threatening etiologies. Overlapping manifestations and terminological confusion often lead to misdiagnosis and delayed care. This narrative review searched eight databases from inception to May 5th, 2026, for articles on headache and visual symptoms. Extracted data were synthesized into a structured diagnostic framework. Key benchmarks were clarified across seven categories. Retinal migraine and migraine with aura are distinguished by monocular/binocular involvement, duration, an
