Recurrent complex visual hallucinations (RCVH), particularly of people, are common during delirium and in dementia, especially Lewy body disease, and acquired eye disease. Studies in those with acquired eye disease suggest that facial distortions, with exaggerated eyes and mouths, are a common feature. We report, and discuss the possible mechanism, of two examples of similar facial distortions in hallucinations occurring during delirium, including one where the patient was able to draw what he experienced. The hallucinations in this report are consistent with the Perception and Attention Defic

Dementia with Lewy bodies (DLB) is a neurodegenerative disorder characterized by cognitive decline, motor symptoms, and other neuropsychiatric manifestations, including visual hallucinations, delusions and disorganized thought processes that can configure a psychotic episode. Managing psychosis in DLB is challenging due to hypersensitivity to antipsychotics. This case report describes a 72-year-old female patient with DLB who presented with psychosis and Parkinsonian symptoms. Initial management with olanzapine (10 mg) was effective for psychosis but led to motor worsening. The inclusion of le

BACKGROUND: Clinical distinction between Alzheimer's disease (AD) and dementia with Lewy bodies (DLB) poses significant challenges due to pathological comorbidity. Similar ages of onset and overlapping cognitive and psychiatric symptoms can lead to diagnostic inaccuracy and inappropriate treatment recommendations.
OBJECTIVE: Identify the best combination of clinical and neuropsychological predictors of AD, DLB, and mixed DLB/AD neuropathology in dementia patients.
METHODS: Using the National Alzheimer's Coordinating Center dataset, we selected either pure AD (n = 189), DLB (n = 21), or mixed

OBJECTIVES: Charles Bonnet Syndrome (CBS) involves visually impaired yet mentally healthy individuals experiencing visual hallucinations (VH) while being aware of the hallucinations' unreality. Its prevalence varies across different conditions, with low vision, bilateral vision loss and advanced age being established as risk factors. This meta-analysis aims to comprehensively assess CBS literature with regard to CBS prevalence in ophthalmic patients.
METHODS: A comprehensive literature search was conducted on June 11th and June 13th 2023, using the databases PubMed/MEDLINE and Web of Science.

AIM: We aimed to evaluate the understanding of delirium among multidisciplinary medical staff and assess whether a brief e-learning course improved their knowledge on the topic.
METHODS: A total of 611 medical staff members participated in the study, including 32 physicians, 393 nurses, 33 nursing assistants, 21 pharmacists, and 132 other multidisciplinary staff, which included non-clinical personnel. A 20-question delirium knowledge test was administered both before and after a 40-min, on-demand e-learning course.
RESULTS: At baseline, there was limited understanding that delirium involves

In addition to the core clinical features of fluctuating cognition, visual hallucinations, and parkinsonism, individuals with dementia with Lewy bodies (DLB) frequently experience chronic and debilitating major depression. Treatment of depression in DLB is hampered by a lack of available effective therapies and standard serotonergic medication for major depressive disorder (MDD) is typically ineffective. Dysfunction of dopaminergic neurotransmission contributing to anhedonia and loss of motivation has been described in MDD. The subgenual anterior cingulate cortex (sgACC) is important in mood r

Ekbom Syndrome, also known as Delusional Parasitosis (DP), is considered a rare psychiatric condition. Based on diagnostic criteria, it is characterized by the strong belief of being infested with various parasites, as well as the presence of perceptual disturbances, usually tactile and/or visual hallucinations. The syndrome can manifest idiopathically or in connection with other medical conditions and substance use. Diagnosis is challenging, as patients tend to pursue dermatological care initially. This case report describes an 81-year-old female diagnosed with Ekbom Syndrome, presenting with

Aberrant thalamocortical connectivity occurs together with visual hallucinations in various pathologies and drug-induced states, highlighting the need to better understand how thalamocortical interactions may contribute to hallucinatory phenomena. Flicker light stimulation (FLS) at 10-Hz reliably and selectively induces transient visual hallucinations in healthy participants. Arrhythmic flicker elicits fewer hallucinatory effects while delivering equal amounts of visual stimulation, together facilitating a well-controlled experimental setup to investigate the neural correlates of visual halluc

STATEMENT OF THE CLINICAL PROBLEM ADDRESSED BY THE CASE: String hallucination, a complex multimodal hallucination in patients with advanced Parkinson disease (PD).
BRIEF DESCRIPTION OF CASE PRESENTATION: Two older patients with advanced PD and dementia presented with a peculiar type of hallucination, in the form of seeing and feeling threads on the surfaces or coming out from the fingers. They continued manipulating these imaginary threads before putting them aside. Quetiapine was helpful in both cases.
SUMMARY OF THE KEY TEACHING POINT IN THE CASE: Apart from the commonly described passage

INTRODUCTION: The misdiagnosis rate of Dementia with Lewy Bodies (DLB) remains high. Accurate diagnosis is paramount in the era of anti-amyloid therapy. The main objective of the current study is to elucidate early clinical features of pathologically confirmed DLB without parkinsonism (DLB-nP) that potentially resemble Alzheimer's disease (AD).
METHODS: We accessed the National Alzheimer's Coordinating Center database from 2005 to December 2022 data freeze and included 3159 Alzheimer's disease, 91 DLB-nP, and 307 DLB with parkinsonism (DLBP) cases. Diagnosis of AD and DLB were based on existi

Lewy body dementia (LBD) encompasses neurodegenerative dementias characterized by cognitive fluctuations, visual hallucinations, and parkinsonism. Clinical differentiation of LBD from Alzheimer's disease (AD) remains complex due to symptom overlap, yet approximately 25% of dementia cases are diagnosed as LBD postmortem, primarily identified by the presence of α-synuclein aggregates, tau tangles, and amyloid plaques. These pathological features position LBD as a comorbid condition of both Parkinson's disease (PD) and AD, with over 50% of LBD cases exhibiting co-pathologies. LBD's mixed patholog

Lewy body dementia is associated with abnormal eosinophilic A-synuclein neural inclusions (Lewy bodies) in the brain. It is a neurodegenerative illness-and the second most common type of dementia after Alzheimer's disease-that causes memory loss and severe problems in carrying out daily activities. In this report, we describe a case of Lewy body dementia that began with early psychotic symptoms with atypical features (transition from hallucinosis (hallucinatory insight) to true visual hallucinations) -without Parkinsonism. The patient exhibited sensitivity to first generation antipsychotic med

The intersection of literature and neuroscience provides a fascinating way to explore human behavior through fictional narratives. Brazilian literature, particularly the work of Machado de Assis, excels in portraying characters with neuropsychiatric conditions. This work aims to establish connections between the fictional representations of human behavior in Machado's classic works and neurological conditions described by contemporary neuroscience. In The Posthumous Memoirs of Brás Cubas and Quincas Borba, Machado's characters exhibit behaviors that align with modern neurological diagnoses. Fo

BACKGROUND: Reality monitoring allows the evaluation and monitoring of reality through the assignment of information to internal or external sources, which is crucial to differentiate real events from imaginary ones. In schizophrenics, monitoring seems to be related to an error in the allocation processes, giving rise to false perceptions such as visual hallucinations, which are associated with a poor prognosis. This error can appear almost imperceptibly at an early age in life, making carrying out predictive or evaluation tests with paper and pencil unattractive. The computerization of techni
