Extraneural cysticercosis mimicking soft-tissue tumors: a case report from a low-resource setting.

INTRODUCTION AND IMPORTANCE: Extraneural cysticercosis (ECC), caused by the larval stage of Taenia solium, is a major diagnostic challenge in endemic regions such as sub-Saharan Africa. ECC frequently presents as a nonspecific, solitary soft-tissue mass, easily mimicking benign tumors and leading to delayed management. This case series highlights the varied clinical spectrum of ECC in western Uganda, following the SCARE criteria. CASE PRESENTATION: We report two clinically distinct cases of ECC at demographic extremes: a 6-year-old male with an acute intramuscular mass in the rectus abdominis

Spatiotemporal trends of neglected tropical disease hospitalizations in Ecuador over 25-years from 2000 to 2024.

BACKGROUND: The World Health Organization identifies 21 neglected tropical diseases (NTDs) affecting millions globally. While their population burden is well recognised, less is known about the long-term trends in severe NTD-related morbidity requiring hospitalization. We analysed national trends and geographic patterns of hospitalizations attributed to NTDs in Ecuador between 2000 and 2024. METHODS: We analysed hospital discharge data from Ecuador's national registry over a 25-year period. Age-standardized hospitalization rates were estimated, and temporal trends were assessed using Joinpoin

Molecular mechanism of LRRC15 protein regulating Treg/Th17 cell imbalance through the PI3K/Akt/mTORC1 signaling pathway in Taenia solium cysticercus infection.

Taenia solium (T. solium) and its larval infections cause cysticercosis and taeniasis, posing serious threats to human health and livestock industry development. Our research group has, for the first time, successfully identified a leucine-rich repeat-containing protein 15 (LRRC15) from the (excretory secretory antigens, ESAs) of T. solium and demonstrated its ability to regulate the differentiation of host regulatory T cells (Tregs). This protein may be a critical factor in the pathogenicity of T. solium and its larvae. Our findings reveal that LRRC15 induces an immune differentiation between

A Multiphase Implementation of Ring Treatment: A Community-Engaged Control Program for T. Solium taeniasis/cysticercosis in Northern Peru.

Background Cysticercosis (CC) is a zoonotic disease transmitted between pigs and humans, caused by the larval stage of T. solium that leads to neurological morbidity in humans and economic losses in pig-farming communities in low-resource rural regions. Ring Treatment (RT) is an evidence-based intervention that integrates a community participatory approach to provide antiparasitic treatment to households within 100-meters of an infected pig. This study aimed to evaluate the multiphase implementation of RT as a public health program in northern Peru, with a focus on feasibility, adoption, and s

Responsibility sharing among children with Type 1 Diabetes and their caregivers: a cross-sectional survey from North India using the Diabetes Family Responsibility Questionnaire (DFRQ).

INTRODUCTION: Reconstruction has evolved long way from primary closure to flaps. As time evolved, better understanding of vascularity of flap has led to the development of innovative reconstructive techniques. These flaps can be raised from various parts of the body for reconstruction and have shown least donor site morbidity. We use one such peroneal artery perforator flap for tongue reconstruction with advantage of thin pliable flap, minimal donor site morbidity and hidden scar. MATERIALS AND METHODS: Our patient 57yrs old lady underwent wide local excision with selective neck dissection. P

The First Biological Validation of Taenia solium Risk Assessment Tools.

INTRODUCTION: Taenia solium places substantial burden on communities in low and middle-income countries, where neurocysticercosis is a leading cause of preventable epilepsy. Current diagnostic tests for T. solium are not suited for low resource settings or perform poorly. Several risk-assessment tools have been developed to support countries in identifying areas for further T. solium investigation and intervention. These tools are based on risk, rather than disease data, and have yet to be validated against biological results. METHODS: Two national quantitative risk-assessment tools (LISA and

The catalytic subunit of cAMP-dependent protein kinase in Taenia solium cysticerci (TsPKA-c): A promising molecular target for the development of novel anti-cysticercosis therapeutics.

Cysticercosis, caused by the larval stage of T. solium, remains a major neglected tropical disease with severe clinical and socioeconomic consequences in endemic regions. Although cyclic AMP-dependent protein kinase (PKA) signaling is fundamental to parasite development and survival, its therapeutic relevance in T. solium has not been comprehensively explored. This study provides an integrated molecular and functional characterization of the T. solium PKA catalytic subunit (TsPKA-c) and evaluates its potential as a novel therapeutic target for cysticercosis control. Recombinant TsPKA-c was suc

A Study of Frequency and Secular Trends of Seropositivity for Cysticercosis in Peru Over 20 Years.

INTRODUCTION: Neurocysticercosis is a leading cause of acquired epilepsy in endemic regions. Scarce longitudinal data sets exist to allow assessment of secular trends in prevalence. This study analysed temporal trends of cysticercosis seropositivity among suspected patients across Peru over two decades. MATERIALS AND METHODS: An ecological study was conducted using anonymized serological data from patients who underwent enzyme-linked immunoelectrotransfer blot (EITB) testing for the first time at a neurological referral centre in Lima, Peru. Seropositivity was defined as antibody reactivity t

Comparative assessment of immunochromatographic test kits using low-molecular-weight antigens from cyst fluids of two different genotypes of Taenia solium for serodiagnosis of human cysticercosis.

Human cysticercosis is a serious zoonosis caused by infection with larvae (cysticerci) of the pork tapeworm, Taenia solium. Infection can involve the nervous system, causing chronic headache and intracranial hypertension, focal neurological deficits, epileptic seizures, and paralysis. The disease is found in developing countries, where porcine cysticercosis is prevalent and undercooked pork is habitually consumed. This study aimed to develop immunochromatography-based test (ICT) kits, using low-molecular-weight antigens purified from cyst fluids of Latin American and Asian genotypes of T. soli

Comparison of Effectiveness Between Cysticidal and Surgical Treatments in Patients with Intraventricular Neurocysticercosis: A Single-Center Experience.

Neurocysticercosis (NCC) remains a major public health problem in endemic countries. Clinical manifestations and therapeutic strategies vary depending on the location of the parasite. While the benefits of cysticidal treatment are well established for parenchymal and subarachnoid NCC, the optimal management of intraventricular NCC (IVNCC) remains controversial. We conducted a retrospective study of 51 patients: 37 (72.54%) received cysticidal treatment as initial therapy and 14 (27.45%) underwent neurosurgical intervention. Although six months after treatment, the proportion of patients with i

Differential astrocyte activation by Taenia solium antigens: Specific induction of IL-10 and IL-1β by Excretory/Secretory (ES) products.

BACKGROUND: Neurocysticercosis (NCC) is a parasitic infection of the central nervous system (CNS) caused by the larval form of Taenia solium. This disease provokes an inflammatory response that intensifies when the parasite dies. This study aimed to assess the expression of the cytokines IL-10, IL-1β, and TGF-β in primary astrocyte cultures derived from rat brains at 5 and 14 days in vitro (DIV) following exposure to T. solium cysticercus antigens. METHODS: Primary astrocyte cultures obtained from 3-day-old postnatal rats were incubated for 24 h with either Excretory/Secretory (E/S) antigens

Cognitive sequelae associated with parenchymal and extraparenchymal neurocysticercosis.

BACKGROUND: While the incidence of neurocysticercosis (NC) with viable parasites has been declining in Mexico and some other countries, this trend is not observed globally, and the burden of patients who have previously suffered from NC remains considerable. Cognitive manifestations are often overlooked in the clinical management of NC. In routine practice, patients are generally considered cured once viable parasites are no longer detectable, despite the potential for persistent neurological sequelae. OBJECTIVES: To evaluate the frequency, characteristics, and severity of cognitive sequelae

Trajectories of Cognitive Decline in Patients with Calcified Neurocysticercosis: A Population-Based, Prospective, Longitudinal Study in a Highly Endemic Setting.

The long-term cognitive consequences of calcified neurocysticercosis (NCC) remain poorly understood. This population-based study investigates cognitive trajectories in individuals with calcified NCC in coastal Ecuador. Utilizing repeated Montreal Cognitive Assessment (MoCA) screenings, the study evaluates whether calcified NCC independently contributes to cognitive decline and the role of hippocampal atrophy (HA) in this context. Among 720 community dwellers ages 40 years old and older, 85 (12%) had calcified NCC. The mean interval between baseline and first follow-up MoCA was 3.27 ± 1.07 year

Seroprevalence and Risk Factors for Taenia solium Cysticercosis and Taeniasis among Children in Nigeria, 2018.

Children are vulnerable to Taenia solium, a parasitic tapeworm causing cysticercosis (a common cause of epilepsy) and taeniasis. We sought to better understand T. solium prevalence and associated risk factors in Nigeria using data from a 2018 nationally representative household survey estimating HIV incidence. We used a multiplex bead assay to determine the seroprevalence of T. solium IgG antibodies to T24H (cysticercosis) and rES33 (taeniasis) for 32,494 children aged <15 years. Positive antibodies against each antigen were detected in children from all states, with an overall antibody seropr

Etidronate May Reduce Residual Calcification in Porcine Neurocysticercosis.

Residual brain calcifications that occur after antiparasitic treatment in neurocysticercosis (NCC) can serve as foci of enduring neuroinflammation and associated seizures. Etidronate, a first-generation bisphosphonate, has exhibited efficacy in reducing the formation of ectopic brain calcification. In the present proof-of-concept study, its effects on post-treatment calcifications were evaluated in 12 pigs naturally infected with NCC, confirmed via magnetic resonance imaging. All animals received albendazole plus praziquantel for 5 days, either alone (n = 6) or with etidronate (20 mg/kg/day fo

Occurrence of autochthonous neurocysticercosis in Germany: a case report.

BACKGROUND: Neurocysticercosis [NCC] represents a severe manifestation of parasitic infection of the central nervous system [CNS] by larvae of Taenia (T.) solium, the pork tapeworm. Although it is one of the main causes of acquired epilepsy worldwide, endemic transmission in Europe has been largely eliminated, with only isolated autochthonous cases occurring. CASE PRESENTATION: A 43-year-old woman from a rural region in Saxony-Anhalt, Eastern Germany, with no history of travel abroad or contact with livestock or known tapeworm carriers presented with a first-time generalized epileptic seizure.
