How Do Mindfulness-Based and Cognitive Behavioral Therapies Help Adults With Opioid-Treated Chronic Low Back Pain? A Qualitative Study.

BACKGROUND: Chronic low back pain (CLBP) is common and often refractory to existing treatments, necessitating long-term opioid therapy, which has substantial risks. Mindfulness-based (MBT) and cognitive behavioral (CBT) therapies are safe and can be effective for CLBP. AIMS: To better understand how MBT and CBT may help improve opioid-treated CLBP: METHODS: This qualitative study involved a subgroup of participants with opioid-treated CLBP who completed a 12-month randomized clinical trial comparing the effects of MBT versus CBT. Participants who reported improvements at 12 months, compared t

Burnout Factors in Neurology: A Survey Assessing Key Factors Among US Academic Faculty.

OBJECTIVES: Burnout can impair clinical judgment and is prevalent across all medical subspecialties. Neurology, in particular, is associated with high rates of burnout and low satisfaction with work-life balance. METHODS: A standardized survey using the abbreviated Maslach Burnout Inventory was administered from December 2020 to March 2022. Faculty from 19 academic neurology departments in the United States participated. Demographic, career, compensation, equity, domestic circumstances, parental leave, and burnout data were collected as part of the Women in Neurology Collaborative Study. Stat

Digital cognitive behavioral therapy versus Education for chronic pain in adults with sickle cell disease: 12-month randomized trial results.

BACKGROUND: Chronic pain affects up to 40% of adults with sickle cell disease (SCD), yet treatment options remain limited. While cognitive behavioral therapy (CBT) is effective in other chronic pain conditions, it is underutilized for SCD pain, and its effectiveness remains unclear. PURPOSE: The Cognitive Behavioral Therapy and Real-time Pain Management Intervention for Sickle Cell via Mobile Applications (CaRISMA) compared digital CBT to Education for chronic SCD pain in adults. At 6 months, the primary outcome (pain interference) showed no between-group difference but both groups improved (

Re-examining the pain-depression vicious cycle in knee osteoarthritis: a random-intercept cross-lagged panel analysis.

BACKGROUND: Pain and depression frequently co-occur in knee osteoarthritis (OA), traditionally viewed as a vicious cycle in which each worsens the other over time. However, conventional cross-lagged models cannot separate stable differences between individuals from real year-to-year changes within a person. We therefore used a random-intercept cross-lagged panel model (RI-CLPM) in the Osteoarthritis Initiative (OAI). METHODS: Of 4,796 enrolled OAI participants with, or at risk of, knee OA, 4,638 contributed data across four annual waves from months 12 to 48. The RI-CLPM was applied to knee pa

Prospective Evaluation of the Associations Between Pre-Cochlear Implant Abilities and Cochlear Implant Quality of Life-35 Profile Domain Scores.

OBJECTIVES: Although most cochlear implant (CI) research has focused on speech recognition ability as the primary outcome measure, research on self-reported, real-world functional outcomes as measured using patient-reported outcome measures is more limited but emerging. The present study explores the association between several pre-CI psychophysical and self-reported functional measures and corresponding CI quality of life (CIQOL) domain scores after implantation. DESIGN: A longitudinal study design was used involving 45 participants who underwent cochlear implantation for their first CI for

Measurement of patients' acceptable symptom levels and priorities for symptom improvement in advanced prostate cancer.

PURPOSE: Limited research has evaluated the success criteria and priorities for symptom improvement of patients with cancer to inform patient-centered care. In this study, we adapted and tested a measure of these constructs, the Patient-Centered Outcomes Questionnaire (PCOQ), for patients with advanced prostate cancer. We compared acceptable symptom severity levels following symptom treatment across 10 symptoms and identified patient subgroups based on symptom importance. METHODS: Patients with advanced prostate cancer (N = 99) participated in a one-time survey, which included a modified vers

Felt Stigma in Youth With Disorders of Gut-Brain Interaction: Implications for Abdominal Pain, Sleep, and Mood.

Children with painful disorders of gut-brain interactions (DGBIs) endorse pain-related stigmatization. However, little is known about how stigma may be associated with pain intensity. Consequently, we conducted a cross-sectional study to identify a potential pathway between felt stigma and abdominal pain in youth with DGBIs. 120 youth completed measures of health-related stigma, depressive symptoms, sleep disturbances, and pain severity. As expected, greater felt stigma was significantly correlated with greater depressive symptoms, sleep-related impairments, and abdominal pain. Furthermore, a

Determinants of acute weight gain during regional conflict among individuals with overweight and obesity receiving targeted multidisciplinary care.

PURPOSE: People living with obesity are susceptible to weight gain during periods of extreme stress. Identifying modifiable risk factors is crucial in regions affected by frequent conflict. METHODS: We therefore sent a survey to patients followed in a weight management clinic shortly after the 2023 onset of the Israel-Hamas war. RESULTS: Responses (n = 285) were collected for analysis a median of 58 days (IQR 48-58) after the conflict erupted and compared between subjects who did and did not experience acute, significant weight gain (≥ 3%). Median age was 57 (IQR 46-67) and 66.5% were female

Unveiling clouded consciousness: Broad-band EEG slowing tracks recovery from post-traumatic confusional state.

BACKGROUND: Post-traumatic confusional state (PTCS) frequently occurs during the recovery from disorders of consciousness (DoC) following severe traumatic brain injury (TBI). Confusional symptoms span multiple domains influencing consciousness, including impairments in the access and integration of mental contents, distortions in perceptual and emotional experiences, vigilance fluctuations, and deficits in memory, orientation, and executive control. While the clinical presentation can be systematically characterized using the Confusion Assessment Protocol (CAP), the underlying neurophysiologic

Associations between breathlessness and individuals' satisfaction with sexual life: a nationally representative internet survey.

INTRODUCTION: Sexual life is important for many people and may be limited by breathlessness. We evaluated associations between breathlessness and individuals' perceived satisfaction with sexual lives and explored mediating factors in this relationship. METHODS: A cross-sectional, online, population-based survey of Australian adults with key demographics (age, sex, rurality, state/territory) reflected the 2016 national census. Assessments included: demographics, breathlessness (modified Medical Research Council (mMRC) scale); satisfaction with overall sexual life; whether breathlessness had af

Shared effects of the opioid antagonist naltrexone on first-hand and empathic pain.

Empathy allows us to infer the affective state of another individual and resonate with it. Accumulating evidence suggests that on a neural level, empathizing relies on "shared neural representations", i.e. patterns of neural activation recruited both during the first-hand and the empathic experience of a specific affective state. Studies employing placebo analgesia have shown consistent reductions in behavioral ratings and neural activity for both firsthand and empathic pain. The mechanistic interpretation of such effects, however, remains elusive, as placebo analgesia could exert its effects

Participant Engagement With a Digital Behavioral Health App for Chronic Pain: Descriptive Secondary Analysis of a Feasibility Randomized Controlled Trial.

BACKGROUND: Chronic pain is a widespread condition that impairs quality of life and is often managed primarily with medications. National guidelines now recommend nonpharmacologic, mind-and-body, and behavioral approaches as first-line or complementary treatments. However, access to these evidence-based options remains limited. Digital health technologies offer a scalable way to deliver integrative, self-care interventions that empower patients to live well with pain. OBJECTIVE: This study examined engagement with and perceived usefulness of a patient- and health care professional-informed mo

Chronic Mild Unpredictable Stress Selectively Downregulates Exon I Bdnf mRNA in the Rat Frontal Cortex Independently of Anhedonia.

Chronic stress is often a precursor to depression. Multiple brain mechanisms underlying the development of depression involve alterations in neurotrophin signaling. In the present study, male Wistar rats were exposed to chronic mild unpredictable stress (CMS), a well-known rodent model of depression, in order to investigate changes in BDNF content, BDNF mRNA expression, and BDNF receptor expression in different brain regions. After 8 weeks of CMS, anhedonia, a symptom of a depressive state, developed in 35% of stressed rats, while 65% remained resilient. The CMS treatment did not significantly

Obesity simulation suits in health professions education: a scoping review.

BACKGROUND: Weight-related stigma toward people living with obesity is widespread in healthcare settings and has been documented among students and professionals across health disciplines. Such stigma may negatively affect communication, clinical decision-making, and quality of care. Educational strategies aimed at addressing weight bias and fostering empathy during the training of health professionals are therefore of growing interest. In recent years, obesity simulation suits have been introduced as experiential learning tools intended to sensitize learners to physical and psychosocial chall

Web-Based Gamified Auditory-Cognitive Dual-Task Training for Older Adults With Age-Related Hearing Loss: Pilot Randomized Controlled Trial.

BACKGROUND: Everyday listening ability is essential for individual health and well-being. Age-related hearing loss (ARHL) is associated with reduced communication engagement, social isolation, loneliness, cognitive decline, and increased dementia risk. Interventions that simultaneously target auditory processing and cognitive function, particularly within engaging and ecologically valid contexts, may offer greater benefits than unimodal approaches. However, culturally adapted, web-based, gamified auditory-cognitive dual-task training (ACDT) tailored for older adults with ARHL remains underexpl

STUDY DESIGN: Secondary analysis. OBJECTIVE: To investigate the Fear-Avoidance Beliefs Questionnaire (FABQ) for its ability to predict 6-month outcomes for patients with low back pain (LBP) participating in physical therapy clinical trials. BACKGROUND: Consistent evidence suggests that fear-avoidance beliefs are predictive of short-term outcomes for patients with LBP. However, proposed cut-off scores have not been widely investigated for longer-term outcomes in samples of patients receiving physical therapy. METHODS AND MEASURES: Subjects (n = 160) were participants in 2 separate randomized

Abnormal Resting-State Functional Connectivity Between the Dorsal Anterior Cingulate Cortex and the Limbic System Contributes to Pain and Emotion Regulation Impairment in Fibromyalgia Patients.

OBJECTIVES: The subdivisions of the anterior cingulate cortex (ACC) are involved in distinct functions in the processing of chronic pain and regulation of emotions. However, the specific impact of each ACC subdivision on fibromyalgia (FM) remains unclear. This study aimed to systematically investigate the abnormal resting-state functional connectivity (rsFC) patterns between the ACC (and its subregions) and other chronic-pain-related limbic cortices and subcortical nuclei in patients with FM. METHODS: Resting-state functional magnetic resonance imaging (fMRI) was conducted in 31 patients diag

Dronabinol for chronic low back pain in Veterans: protocol for a randomized, double-blind, placebo-controlled, parallel-group trial.

Chronic low back pain is common, disabling, and often inadequately controlled with current pharmacologic options, underscoring the need for safer non-opioid treatments. Dronabinol, a synthetic formulation of Δ9-tetrahydrocannabinol, has analgesic potential but has not been rigorously evaluated for chronic low back pain. This article describes a phase IIa, single-site, randomized, double-blind, placebo-controlled, parallel-group trial in Veterans with chronic low back pain. Seventy-five participants will be randomized 2:1 to oral dronabinol or matching placebo for 8 weeks following a screening
