[Pain patients show a higher hindsight bias].

Research on pain-related cognitions has up to now predominantly relied upon introspective questionnaire data. Experimental cognitive psychology offers an alternative way of access to the cognitive aspects of chronical pain. Building on the assumption that information-processing is in part uncontrolled, automatic and pre-attentive, similar processes are also expected to be relevant for pain-relevant cognitions and to be involved in health-related convictions and in coping strategies that can be assessed with questionnaires. Cognitive-psychological research has established the "hindsight bias" a

Association of the Resident Assessment Instrument (RAI) with changes in function, cognition, and psychosocial status.

OBJECTIVE: To evaluate the impact of the Resident Assessment Instrument (RAI) on changes in nursing home residents' functional status, cognitive status, and psychosocial well-being. DESIGN: A quasi-experiment involving the collection of longitudinal data on two cohorts of nursing home residents. One cohort was assessed before the implementation of the RAI, and the other was assessed after the implementation of the new assessment process. SETTING AND PARTICIPANTS: Over 2000 nursing home residents in 267 nursing homes located in 10 geographic areas were assessed during the pre-RAI period. In t

[Participants of mother-child health resort treatments].

Mother-child health-resort treatments have for many years been a major component of rehabilitative and preventive health care for women. The rising significance of health-promotion measures on the one hand, and increased attention to women's health aspects in rehabilitation and prevention on the other, have made in-depth consideration of the issues involved in target-groups, participation in and conceptual potential of mother-child programmes necessary. In a questionnaire study of n = 103 women participating in a mother-child programme, data were collected concerning their life situation, life

[The PANSS (Positive And Negative Symptom) Scale].

The PANSS scale, first proposed by Kay et al. (1987) and translated into French is an evaluation scale of 30 disparate items scored from 1 to 7 for psychopathological symptoms observed in patients presenting psychotic syndromes, especially schizophrenic states. A semi-structured interview (translated into French by J.P. Lindenmayer) and precise definition of the different degrees of symptomatic, severity make it possible to obtain satisfactory between-assessor and test-retest fidelity. Three scores obtained with this evaluation tool are generally calculated for evaluating three dimensions of t

Genetic variants of the human obesity (OB) gene: association with body mass index in young women, psychiatric symptoms, and interaction with the dopamine D2 receptor (DRD2) gene.

To examine the possible role of genetic variants of the OB gene in obesity we examined alleles of a dinucleotide repeat polymorphism, D7S1875, close to the gene, in a group of adult, non-Hispanic Caucasians. There was a significant correlation with body mass index (BMI) at age 26-30 years for males and females combined (P = 0.04) and females only (P = 0.028). Because of the frequent association between obesity and psychiatric symptoms all subjects were screened with the Symptom List 90 (SCL-90). There was a significant increase in scores for anxiety (P = 0.0005), depression (P = 0.003), and ot

Ethical conflict resolution based on an ethics of relationships for brain injury rehabilitation.

An ethics of relationships for brain injury (BI) rehabilitation is described based on three principles; (1) human relationships are important; (2) human relationships are as important as individual survival; (3) human relationships are important enough to extend throughout the family of humankind. Within the context of this ethics of relationships, ethical conflict resolution (ECR) is offered as a process to address disagreements among those involved in BI rehabilitation. ECR provides a means to arrive at moral decisions in situations in which people disagree about the appropriate course of ac

Physiology of REM sleep, cataplexy, and sleep paralysis.

The main neural structures generating muscle atonia and other phenomena characteristic of REM sleep are present in dorsolateral portions of the pons in the brainstem. Occurrence of REM sleep and the NREM-REM sleep cycle are probably determined by a balance or interaction between the cholinergic and cholinoceptive REM sleep-on neuronal populations and the monoaminergic REM sleep-off neuronal population. Neural activities producing generalized muscle atonia in REM sleep originate mainly in dorsolateral portions of the pontine reticular formation, descend through the medulla and spinal cord, and

Contributions of anterior cingulate cortex to behaviour.

Assessments of anterior cingulate cortex in experimental animals and humans have led to unifying theories of its structural organization and contributions to mammalian behaviour. The anterior cingulate cortex forms a large region around the rostrum of the corpus callosum that is termed the anterior executive region. This region has numerous projections into motor systems, however, since these projections originate from different parts of anterior cingulate cortex and because functional studies have shown that it does not have a uniform contribution to brain functions, the anterior executive re

Fear-relevant illusory correlations: what types of associations promote judgmental bias?

A. J. Tomarken, S. Mineka, and M. Cook (1989) found that high-fear individuals markedly overestimated the covariation between fear-relevant stimuli and aversive outcomes. The authors assessed what features of stimulus-outcome associations promote illusory correlations. In Experiment 1, participants with high snake fear exhibited significant covariation bias for slides of snakes and shocks, but not for slides of damaged electric outlets (DEOs) and shocks. In Experiment 2, individuals with high and low snake fear rated DEOs and shocks as belonging together better than snakes and shocks. However,

Stimulation in the human somatosensory thalamus can reproduce both the affective and sensory dimensions of previously experienced pain.

Thalamic structures involved in the unpleasant emotional or affective aspect of pain are poorly understood. We now describe studies of the region of the thalamic principal somatosensory nucleus (Vc) performed before thalamotomy for tremor in a patient who also had panic disorder. Microstimulation in the region posterior to Vc evoked chest pain, including a strong affective dimension, almost identical to that occurring during his panic attacks, as measured using a questionnaire. Results in our other patients indicate that stimulation-associated pain with a strong affective dimension occurred on

Menstrual phase-independent retrieval deficit in women with PMS.

Memory and attention were evaluated in 19 women with prospectively documented premenstrual syndrome (PMS) and 17 women without PMS. Testing occurred during the late luteal and follicular phases of the menstrual cycle. Estrogen, progesterone, and FSH levels were obtained on testing days for 23 of the women, while mood measures were obtained for all of the women. Repeated measures analysis of variance yielded significant group differences on recall measures, with no differences noted on recognition indices. No significant session or interaction effects were observed. This pattern of results sugg

Rational use of benzodiazepines in the elderly.

In the 40 years since the introduction of benzodiazepines into clinical practice, considerable controversy has surrounded their use. While there is little evidence to suggest widespread abuse or long term use in most age groups, benzodiazepines continue to be widely prescribed to older adults in both community and long term care settings. Several studies have described an increased sensitivity to the clinical effects and toxicity of benzodiazepines in older adults. However, it is unclear whether these observations are attributable to age-related changes in benzodiazepine pharmacokinetics or ph

Cognitive functions and quality of life in patients with low-grade gliomas: the impact of radiotherapy.

The role of early radiotherapy in the treatment of low-grade gliomas is controversial. For this reason the impact of radiotherapy on quality of life was studied in long-term survivors of biopsy-proved low-grade gliomas without signs of tumor recurrence. Twenty patients (age range, 18-66 years) had been treated with early radiotherapy; the other 21 patients (age range, 19-65 years) had undergone surgery or biopsy only. The interval from diagnosis to testing ranged from 1 to 12 years (mean, 3.5 years). Nineteen patients with low-grade hematological malignancies, surviving 1 to 15 years without c

Pain and suffering: what is the unit?

Nonspecific low back pain presents several problems to health care providers. Although the number of patients who are registered as disabled due to low back pain has dramatically increased in recent years, its incidence and severity remain fairly constant. The relationship between perceived backache and subsequent disability is not linear, and other factors may influence a patient's decision to seek help. These factors include the patient's psychological and emotional state, especially his or her past experiences, extant mood and anxiety about anticipated consequences. Physicians should not re

Propofol and seizures.

It is now clear that "seizure activity", excitatory phenomena, and/or a disorder of muscle tone are potential complications of the use of propofol. Whether this "seizure activity" is primarily, secondarily, or not at all a cerebral cortical event is still to be elucidated. Clearly propofol does have anticonvulsant activity, and also clearly it can produce an involuntary movement disorder, in certain patients, under certain conditions. Propofol is not the first anaesthetic drug to be implicated in the causation of seizures or abnormal movements nor indeed the first to appear to have anti-convul

Suffering and its relationship to pain.

Pain is a complex, multidimensional perception with affective as well as sensory features. In part, it is a somatically focused negative emotion resembling perceived threat. Suffering refers to a perceived threat to the integrity of the self, helplessness in the face of that threat, and exhaustion of psychosocial and personal resources for coping. The concepts of pain and suffering therefore share negative emotion as a common ground. Examination of the central physiological mechanisms underlying pain, negative emotional arousal, and stress helps clarify the physiological basis of suffering and

Patient satisfaction and side effects with levonorgestrel implant (Norplant) use in adolescents 18 years of age or younger.

OBJECTIVES: Levonorgestrel implants (Norplant) have recently been approved for use in the United States. Adolescents may be especially suited for this method of contraception because of its long duration of action and lack of dependence on patient compliance for efficacy. To date, however, its use in young patients has not been investigated. This study was undertaken to compare side effects and satisfaction with this new method during the first 6 months of use among adolescents and adults. METHODS: Twenty-one adolescents 18 years of age or younger and 30 adults 19 years or older participated

Mirth, laughter and gelastic seizures.

Little is known about what pathways subserve mirth and its expression laughter. We present three patients with gelastic seizures and laughter elicited by electrical stimulation of the cortex who provide some insight into the mechanisms of laughter and its emotional concomitants. The first patient had seizures manifested by laughter without a subjective feeling of mirth. Magnetic resonance imaging showed a cavernous haemangioma in the left superior mesial frontal region. Ictal subdural electrode recording showed the seizure onset to be in the left anterior cingulate gyrus. Removal of the lesion

[Depressive symptomatology and sleep apnea syndrome].

The sleep apnea syndrome (SAS), which is defined by more than 5 apneas or hypopneas per hour of sleep (9), is quite a frequent affection which concerns 1.4 to 10% of general population (1.7). The major daytime complaints of the SAS are daytime sleepiness, memory and attention disorders, headaches and asthenia especially in the morning, and sexual impotence (9). The nocturnal manifestations are dominated by sonorous and generally long standing snoring, increased by dorsal decubitus and intake of alcohol, with repeated interruptions by respiratory arrests. These manifestations are always noted b

Cross-cultural attitudes toward speech disorders.

Speech-language pathologists serving multicultural populations may encounter unfamiliar beliefs about speech disorders among the members of different cultures. This study used a questionnaire to look at attitudes toward four disorders (cleft palate, dysfluency, hearing impairment, and misarticulations) among 166 university students representing English-speaking North American culture and several other cultures (e.g., Chinese, Southeast Asian, Hispanic). The results showed significant group differences on items involving the subjects' beliefs about the emotional health of persons with speech di
