Anjiang formula inhibits PVN microglial activation and lowers blood pressure by targeting RhoA/ROCK2 pathway: a retrospective clinical and experimental study.

BACKGROUND: Neuroinflammation in the hypothalamic paraventricular nucleus (PVN) drives sympathetic overactivity in hypertension. The Anjiang Formula (AJ) shows clinical antihypertensive potential; however, the precise molecular targets mediating its central neuroprotective effects remain undefined. METHODS: In this translational study, we investigated the clinical efficacy of AJ and tested the hypothesis that it directly inhibits the central RhoA/ROCK2 signaling axis. We integrated a retrospective cohort analysis with mechanistic validation. Clinically, 85 elderly patients with Grade 1 essent

Molecular Docking and Dynamics-Based Repurposing of Thalidomide and Lenalidomide for GSK-3β Inhibition in AD.

INTRODUCTION: Glycogen synthase kinase-3 beta (GSK-3β) is a key serine/threonine protein kinase involved in several neural processes. Its overactivity contributes to the pathogenesis of Alzheimer's disease (AD) through tau protein hyperphosphorylation and amyloid-beta (Aβ) accumulation. Thalidomide and lenalidomide, originally developed as anticancer drugs, have shown potential inhibitory effects on GSK-3β in vitro. In this study, we repurposed these compounds to design novel GSK-3β inhibitors for AD therapy using computational methods. MATERIALS AND METHODS: The 3D structure of GSK-3β (PDB I

Arthrospira platensis prevents impairment of intestinal smooth muscle relaxation in diet-induced obesity via modulation of nitric oxide, prostanoid and oxidative stress pathways.

ETHNOPHARMACOLOGICAL RELEVANCE: Arthrospira platensis (Spirulina) has been traditionally consumed in Africa, Asia and Central America as a nutrient-rich restorative food associated with metabolic balance and gastrointestinal health. Its longstanding ethnopharmacological use suggests biological activities related to antioxidant and anti-inflammatory effects, which are relevant in conditions associated with metabolic disorders. AIM OF THE STUDY: This study investigated whether A. platensis supplementation prevents the reduction in ileal relaxant reactivity induced by a hypercaloric diet (HCD) i

An environmental enrichment facilitates the recovery of lower urinary tract function after spinal cord injury through downmodulation of neurogenic activity in female rats.

PURPOSE: Our objective was to determine whether exposure to an environmental enrichment (EE) promotes recovery of micturition following thoracic spinal cord injury (SCI) in rats. INTRODUCTION: Spinal cord injury (SCI) often disrupts bladder-sphincter coordination, resulting in neurogenic lower urinary tract dysfunction (NLUTD) that severely impacts quality of life. Environmental enrichment (EE) has been shown to enhance neuroplasticity and functional recovery after CNS injury, but its potential effects on urinary control remain poorly characterized. MATERIALS AND METHODS: Twenty-four adult fem

Enteric and Sympathetic Nervous System Pathways Mediate Early Life Stress Effects on Gut Motility and Pain: Mechanistic Findings With Human Correlation.

BACKGROUND & AIMS: Adverse experiences during early life can disrupt gut-brain axis development, but the mechanisms linking early life stress (ELS) to long-term gastrointestinal dysmotility and pain remain unclear. METHODS: We used a maternal separation (MS) mouse model of ELS and assessed visceral pain sensitivity, intestinal motility, and enteric nervous system (ENS) composition. Two large human pediatric population cohorts were also analyzed for associations between ELS and disorders of gut-brain interaction (DGBI) risk. RESULTS: MS in mice led to visceral hypersensitivity, sex-specific m

Persistent prostaglandin E2 upregulation and hormonal multi-resistance: A hypothesis for long COVID.

An explanation for long COVID (LC) and its many symptoms remains elusive, and no unifying upstream mediator of its pathophysiology has yet been identified. Viral infections, including COVID-19, upregulate Prostaglandin E2 (PGE2), - the principal lipid mediator of inflammation. PGE2 has broad paracrine effects, including modulation of autoantibodies, nervous systems, blood pressure, glucose levels, and apoptosis. Upregulated, PGE2 may drive multiple pathological processes through its four receptors: EP1-EP4. Through EP3, PGE2 stimulates characteristic viral sickness symptoms. Both severe and mi

Acid ceramidase overactivity drives ceramide loss, leading to atopic dry skin and Th2-skewed immune polarization.

Ceramide deficiency in the stratum corneum (SC) is a key etiological factor in atopic dermatitis (AD). To clarify the direct role of SC ceramide depletion in impairing SC barrier and water-holding functions and in initiating AD-like skin symptoms and disease-specific molecular alterations, we generated Tg mice overexpressing a mutant form of acid ceramidase (aCDase) under the control of the involucrin promoter, resulting in targeted expression in the upper epidermis. By 3 weeks of age, Tg mice developed noninflammatory, scaly skin characterized by severely compromised barrier integrity and wat

Children with enuresis - are they more constipated than others?

INTRODUCTION: The bowel-bladder interaction is well established, as is the fact that constipation can lead to daytime incontinence and/or recurrent urinary tract infections. However, the link between constipation and enuresis is less clear. The bowel's influence on enuresis is possibly important even though fecal disimpaction by itself will not make the child dry at night. Furthermore, parents and patients may be unaware of the child's constipation until the enuresis is evaluated. The same should apply to healthy children. The hypothesis that a full bowel affects bladder function appears to be

Obstructive Sleep Apnea and Hypertension: Is Aldosterone Blockade the Optimal Approach to Control Blood Pressure and Improve Oxygenation?

PURPOSE OF REVIEW: Obstructive sleep apnea (OSA) is common and strongly linked with systemic hypertension, including resistant and nocturnal forms. Traditional OSA therapies lead to modest blood pressure (BP) reductions, but optimal antihypertensive strategies in this population remain unclear. This review examines OSA driven hypertension mechanisms, differential responses to standard OSA therapies, and evaluates evidence for mineralocorticoid-targeted treatment - including blockade of aldosterone - as a tailored BP lowering and airway improving strategy. RECENT FINDINGS: OSA promotes hyperte

LGL-1 and the RhoGAP protein PAC-1 redundantly polarize the C. elegans embryonic epidermis.

Apical-basal polarity is essential for epithelial organization and function and is established by conserved cortical polarity proteins. However, the requirements for canonical polarity factors vary between tissues and organisms. For example, the basolateral protein lethal giant larvae (Lgl) is essential for epithelial polarity in Drosophila but dispensable in C. elegans. To better define the epithelial polarity program in C. elegans, we performed a genome-wide RNAi screen for synthetic lethality with lgl-1. Combined loss of LGL-1 and the RhoGAP PAC-1 caused embryonic lethality due to elongatio

Pharmacological Management of Stress Urinary Incontinence with Atypical Leakage Features, Symptom Response to Short-term Solifenacin Trial.

INTRODUCTION AND HYPOTHESIS: A subset of women with stress urinary incontinence (SUI) exhibit atypical features that may suggest hidden storage dysfunction or bladder overactivity. As urodynamic studies (UDS) are invasive and not always accessible, this study was aimed at examining whether a short solifenacin trial could improve symptoms and help treat women with SUI who present atypical leakage patterns. METHODS: Fifty women (39-60 years old) suffering from clinically diagnosed SUI with atypical features were treated with solifenacin 5 mg daily over the course of 4 weeks. The International C

How Can We Personalize the Delivery of Onabotulinumtoxin-A for Patients With Neurogenic Lower Urinary Tract Dysfunction?

BACKGROUND & AIMS: Neurogenic lower urinary tract dysfunction (NLUTD) can produce bothersome urinary symptoms, impact quality of life, and in some cases, lead to deterioration of upper urinary tract function. Intradetrusor injection of onabotulinumtoxin-A (BoNT-A) is approved for NLUTD in patients who have an inadequate response to or intolerance of oral pharmacologic therapy (with beta-3 agonists and/or antimuscarinics) for urinary frequency, urgency, urgency incontinence, and/or impaired compliance. Modifications of its use, including dose, injection location, number of injections, setting o

A Potential Urodynamic Classification of Voiding Patterns in Neurogenic Lower Urinary Tract Dysfunction Due to Lower-Level Spinal Cord Injury.

Objectives: To characterize urodynamic findings after lower-level spinal cord injury (LSCI) and to evaluate a new pressure-based classification framework-the bladder-sphincter dyscoordination syndrome (BSDS)-for describing voiding patterns. We also introduce a descriptive "neurogenic bladder outlet obstruction" (NBOO) phenotype for straining-dependent voiding difficulty. Methods: We retrospectively analyzed the first urodynamic studies (December 2020-August 2024) in 81 men with LSCI (injury at T10 or below). Key urodynamic measures included detrusor and intravesical pressures during filling an

Correction of Iatrogenic Facial Asymmetry Caused by Botulinum Toxin Using Contralateral Injection: A Case Report.

Facial symmetry is a key determinant of aesthetic harmony and psychosocial well-being. Iatrogenic asymmetry is a recognized complication of botulinum toxin injections and may cause both functional and emotional distress. Contralateral botulinum toxin adjustment is an accepted corrective strategy, but few case reports have documented its use for toxin-induced asymmetry with photographic evidence. A 44-year-old woman developed facial asymmetry after cosmetic botulinum toxin injections. She presented with upward deviation of the left oral commissure and midface both at rest and during speech, cau

Case Report: Hyperthyroidism in a patient with spotty skin pigmentation and atrial myxoma (Carney complex): coincidence, association or cause?

BACKGROUND: Carney complex (CNC) is a rare autosomal-dominant multiple neoplasia syndrome characterised by spotty skin pigmentation, cardiac myxomas and endocrine overactivity. Although thyroid follicular adenomas are common in CNC, the presence of CNC in combination with Graves' disease-related hyperthyroidism is rare, and the pathogenic association between the two conditions remains poorly understood. CASE PRESENTATION: An 18-year-old Chinese woman presented with palpitations and worsening fatigue over 2 months. She had a 2-year history of Graves' disease (treated with iodine-131 1 year pri
