The Utility of Simple Cystometry.

IMPORTANCE: Despite its common use, there is limited contemporary evidence regarding the place of simple cystometry (SC) in the evaluation of urinary function. OBJECTIVES: Our objective was to assess the accuracy of SC with respect to complex urodynamic study (UDS), evaluate the correlation between them, and determine whether performing both tests on the same patient offers any additional benefit. STUDY DESIGN: In this retrospective cohort study, 609 consecutive women aged >18 years who underwent both UDS and SC within the same 6-month period between January 1, 2017, and December 31, 2022, w

Restoring cartilage-subchondral bone metabolism with biphasic magnesium microspheres ameliorate early osteoarthritis.

The spatiotemporal histopathological features of articular cartilage in osteoarthritis (OA) remain inadequately characterized, which impedes the advancement of strategies to halt irreversible joint deterioration. Herein a murine OA model, the initial phase (<10 days post-surgery) is characterized by pronounced mitochondrial dysfunction in chondrocytes and concurrent activation of subchondral osteoclasts. Beyond this period, irreversible cartilage degeneration ensues, marked by chondrocyte apoptosis and subchondral bone sclerosis. We further identified magnesium (Mg) as a key regulator of cellu

Rat Animal Models for Evaluation of the Effects of Sacral and Peripheral Nerve Stimulation on Bladder Function.

Neuromodulation, a therapeutic approach to the dysfunction of various organs, has proven effective in treating lower urinary tract dysfunctions. Despite its clinical success, the mechanisms underlying neuromodulation remain incompletely understood, emphasizing the need for standardized and reproducible animal models. Preclinical models of urinary bladder neuromodulation using sacral and peripheral nerve stimulation have been established for more than two decades; however, the existing literature provides limited methodological detail regarding the surgical procedures. This article offers a det

Selective elimination of mast cells via Siglec-6-targeted nanodelivery of drug payload.

BACKGROUND: Mast cell elimination through sialic acid immunoglobulin-like lectin 6 (Siglec-6) is a promising, yet minimally explored strategy for diseases characterized by mast cell overactivity or overproduction. OBJECTIVE: We combined nanotechnology with cell-free protein synthesis to develop a therapeutic strategy (anti-Sig6-NPmidoFab) for selectively eliminating mast cells. METHODS: Nanocarriers are loaded with the protein kinase inhibitor midostaurin and conjugated with cell-free protein synthesis-enabled clickable, synthetically dimerized anti-Siglec-6 antibody fragments to selectively

Managing the patient with hypertension and elevated heart rate in the routine care setting: expert opinion and narrative review.

Increased resting heart rate (RHR) due to overactivity of the sympathetic nervous system (SNS) increases the risk of adverse cardiovascular outcomes in hypertension by promoting multiple aspects of hypertension-mediated organ damage (HMOD) such as endothelial dysfunction, arterial stiffness, left ventricular hypertrophy and renal damage. A vicious cycle of SNS overactivity and adverse cardiovascular remodelling leads ultimately to increased risk of adverse clinical cardiovascular outcomes and heart failure with either reduced or preserved left ventricular ejection fraction. Epidemiological stu

Paradoxical reflex bradycardia in paroxysmal sympathetic hyperactivity following traumatic brain injury: a case report.

BACKGROUND: Paroxysmal sympathetic hyperactivity (PSH) is a devastating complication of traumatic brain injury (TBI), characterized by the simultaneous onset of various manifestations due to sympathetic overactivity. Paradoxical bradycardia, an uncommon manifestation of PSH, is poorly characterized, and there is limited evidence regarding its clinical features, pathophysiological mechanisms and targeted management. CASE PRESENTATION: A 25-years-old male sustained severe traumatic brain injury in a nighttime motorcycle collision with a parked vehicle and underwent multiple neurosurgical proced

Association between CTLA-4 gene polymorphisms and Type 1 diabetes in Kurdish patients.

Type 1 diabetes (T1D) is an increasingly complex disease influenced by both genetic and environmental triggers, leading to overactivity of the immune system. This preliminary study was conducted to investigate the association of two common cytotoxic T lymphocyte antigen-4 (CTLA-4) variants with T1D susceptibility in the Kurdish population of Iraq. Additionally, we evaluated the correlation between these polymorphisms and anti-GAD antibody positivity, as well as their possible influence on CTLA-4 gene expression. In this study, 52 patients (28 males and 24 females) with T1D and 21 healthy contr

Cardiometabolic Multimorbidity and Sympathetic Overactivity: Translational Rationale for Combined Hepatic-Renal Denervation.

Cardiometabolic multimorbidity, encompassing type 2 diabetes, hypertension, and metabolic dysfunction-associated steatotic liver disease, is closely linked to sympathetic nervous system overactivity and remains suboptimally controlled by pharmacotherapy alone. This state-of-the-art review evaluates the biological rationale, technical feasibility, preclinical evidence, and emerging clinical data supporting combined hepatic and renal denervation as an investigational neuromodulatory strategy for cardiometabolic disease. Preclinical models suggest that multiorgan denervation may modulate sympathe

The Role of Dermocosmetics in Acne Management: Implications for Clinical Practice in Brazil.

Acne is a prevalent skin condition worldwide, affecting diverse populations, including Brazil. Although acne guidelines are well established for topical and/or systemic medications, recommendations regarding dermocosmetics, which have become increasingly important in acne management, remain limited. This review examines the role of dermocosmetics within acne treatment protocols and provides practical guidance for their integration into skincare routines alongside conventional therapies. Six dermatology experts conducted structured PubMed literature searches (2012-2024) and discussed the findin

Cardiotropic adeno-associated virus-based gene therapy via intracoronary delivery for nonischemic heart failure.

Heart failure (HF) remains a leading cause of morbidity and mortality with significant residual risk despite major advances in medical therapies. Development of cardiotropic vectors have allowed gene therapy to be revisited to target the molecular roots of HF to potentially improve structural cardiac remodeling and cardiac contractility on top of functional improvement offered by current standard of care therapies. In a first-in-human phase 1 trial, a cardiotropic vector gene AB-1002 targets a phosphorylation pathway responsible for calcium handling in cardiomyocytes. AB-1002 stimulates inhibi

Hyperselective neurectomy versus tendo-achilles lengthening for pediatric spastic equinus: A comparative cohort study.

BACKGROUND: Spastic equinus is a common gait-limiting deformity in children with upper motor neuron disorders, particularly cerebral palsy. Surgical management has traditionally emphasized tendon-lengthening procedures such as tendo-Achilles lengthening (TAL), which correct structural shortening but do not address the neural mechanisms underlying spasticity. Hyperselective neurectomy (HSN) of tibial motor branches has emerged as a targeted approach to reduce focal spastic overactivity. PURPOSE: To describe two sequential treatment-era cohorts managed with TAL or HSN for pediatric spastic equi

Pre- and postoperative urodynamic risk stratification in infants and young children undergoing tethered cord release.

INTRODUCTION AND OBJECTIVES: Primary tethered cord syndrome (TCS) can lead to neurological and urologic dysfunction. While postoperative urodynamic studies (UDS) are standard for evaluating bladder function after spinal cord detethering (SCD), the role of preoperative UDS remains unclear, particularly in non-toilet trained (NTT) infants and children. This study assesses the utility of preoperative UDS by 1) comparing pre- and postoperative findings and risk stratification, and 2) examining correlations with long-term urologic outcomes. STUDY DESIGN: We retrospectively reviewed records of chil

Resistant hypertension and epileptic seizures: a possible modulatory role of renal denervation-a case report.

BACKGROUND: Renal denervation (RDN) is an emerging therapy for selected patients with resistant hypertension. Its broader autonomic effects may extend beyond blood pressure regulation. CASE SUMMARY: A 57-year-old man with resistant hypertension treated with six antihypertensive drugs and recurrent autonomic and generalized seizures underwent radiofrequency RDN. At 3 months, ambulatory blood pressure monitoring showed sustained improvement (24 h mean 139/87 mmHg), enabling therapy de-escalation. Generalized tonic-clonic seizures resolved, and autonomic/focal events markedly decreased without c

Circadian Systems and Neural Control of Blood Pressure.

PURPOSE OF REVIEW: Blood pressure (BP) follows a robust circadian rhythm and disruption of this rhythm is associated with hypertension and increased cardiovascular disease risk. This review discusses the current understanding of how circadian mechanisms regulate the neural control of BP, with a focus on autonomic brain circuits, local molecular clocks, and emerging neuroimmune mechanisms contributing to the regulation of BP. RECENT FINDINGS: Numerous studies show that autonomic control areas of the brainstem are important for circadian BP regulation. Regions such as the rostral ventrolateral
