The role of gut microbiota in obesity-related cancers.

Obesity is an established risk factor for at least thirteen cancer types, yet the mechanisms by which the gut microbiota participates in obesity-related tumorigenesis remain incompletely understood. In this review, we summarize the role of the gut microbiota in the four core oncogenic pathways of obesity including metabolic reprogramming, the obesity-related secretome, hormonal dysregulation, and immune modulation. Mechanistically, microbial activity shapes these processes primarily through the production of metabolites, disruption of the intestinal barrier, and stimulation of innate immunity

The activity of D2 dopamine receptors of the ventral tegmental area changed the induced- cognitive responses of the lateral hypothalamic cholinergic stimulation following neuropathic pain.

BACKGROUND: Dopaminergic neurons within the ventral tegmental area (VTA) is involved in the development of cognitive dysfunction during chronic pain. It is controlling cognitive behaviors via its various projection pathways to different areas in the brain including lateral hypothalamus (LH). The activation of LH induced analgesia during chronic pain. In the current study, we assessed whether D2 dopamine receptors activity within the VTA can change the induced-cognitive responses of LH following its cholinergic stimulation during neuropathic pain in rats. Aditionally, we assessed whether cholin

Tailoring of a chronic pain self-management intervention for individuals with opioid use disorder in office-based addiction treatment.

INTRODUCTION: Office-based opioid treatment with buprenorphine for opioid use disorder (OBOT-BUP) is most effective when patients are retained in treatment long-term, but average treatment duration is less than 6 months. Up to two thirds of individuals with opioid use disorder (OUD) have chronic pain. Treatment of chronic pain in OBOT-BUP settings could improve retention in OBOT-BUP programs. Therefore, this study qualitatively assessed how to tailor pain self-management (PSM) to people with co-occurring chronic pain and OUD in OBOT-BUP settings. METHODS: This study takes a Qualitative Descri

Beliefs of delusional intensity about body image or eating-related concerns in individuals with Feeding and Eating Disorders: a systematic review and meta-analysis.

OBJECTIVE: In Anorexia Nervosa (AN) distorted body image beliefs range from overvalued ideas to delusions, significantly influencing illness trajectory. However, these phenomena remain underexplored, particularly in other Feeding and Eating Disorders (FED). This systematic review and meta-analysis investigates the prevalence and clinical significance of delusional-like beliefs related to body image and eating concerns in FED. METHOD: Following PRISMA guidelines, PubMed, Web of Science, and the Cochrane Library were searched for quantitative studies (1960-January 2025) investigating delusional

A Clinico-Radiological Correlation Study in Children With Severe Community-Acquired Pneumonia: Does This Child Have Pneumonia?

BACKGROUND: Pneumonia remains a leading cause of morbidity and mortality among children under five years of age, particularly in developing countries. Although the World Health Organization (WHO) provides clinical criteria for diagnosing severe pneumonia, overlapping features with other respiratory illnesses may limit diagnostic accuracy. Radiological evaluation, especially chest radiography, plays a crucial role in confirming diagnosis, assessing severity, and identifying complications. MATERIALS AND METHODS: This analytical cross-sectional study included 300 consecutively enrolled children

Assessing respiratory muscle effort during noninvasive respiratory support in acute hypoxemic respiratory failure: A State-of-the Art Review from physiology to bedside.

OBJECTIVES: Respiratory muscle effort is an important determinant of pathophysiology and clinical outcomes in patients with acute hypoxemic respiratory failure receiving noninvasive respiratory support (NIRS). Excessive effort reflects an imbalance between ventilatory load and muscle capacity and has been associated with patient self-inflicted lung injury, treatment failure, and worse outcomes, particularly when escalation to invasive ventilation is delayed. This review provides a clinically focused synthesis of the physiological determinants, bedside assessment, and practical implications of

Hypothalamic syndrome and acquired hypothalamic obesity: from fragmented care to integrated multidisciplinary management.

Acquired hypothalamic obesity (AHO) is a complex neuroendocrine disorder characterized by rapid weight gain and multisystem dysfunction following hypothalamic damage. AHO represents the weight-related manifestation of the broader hypothalamic syndrome (HS), a heterogeneous spectrum of dysfunctions that can also encompass disturbances in pituitary function, energy expenditure, sleep, behavior, and thermoregulation, with obesity itself absent in some affected individuals. This narrative review provides an updated and integrative overview of AHO pathophysiology, clinical characterization, and cur

Parallels between bipolar disorder and ATP1A3-related diseases: a window into the investigation of lithium for alternating hemiplegia of childhood.

BACKGROUND: Alternating hemiplegia of childhood (AHC) shares many aspects with the psychiatric condition, bipolar disorder. BODY: AHC is a heterogeneous genetic disorder that can manifest in various fashions but will usually involve paroxysmal neuropsychiatric symptoms that are precipitated by environmental events or stressors, superimposed over developmental abnormalities of various severities. It occurs as a consequence of a variety of mutations of the alpha3 subunit of the sodium pump (ATP1A3). Treatment is generally symptomatic with flunarizine a nonselective calcium channel blocker, but o

Assistive technology for developmental conditions: A scientometric analysis.

Assistive technology plays a pivotal role in supporting individuals with developmental conditions by fostering independence and addressing needs across physical, cognitive, and communicative domains. Despite its benefits, widespread adoption remains limited due to high costs, usability challenges, and limited real-world applicability. This study offers a comprehensive overview of assistive technology's role in the context of developmental conditions by outlining key research areas and influential publications. Using a scientometric approach, we examined 1322 documents from Scopus and their 44,

Actigraphy-based sleep outcomes in substance use disorders: A systematic review and meta-analysis.

Sleep disturbances are prevalent in substance use disorders (SUDs). Objective (e.g., polysomnography) and subjective (e.g., Pittsburgh sleep quality index [PSQI]) assessments are commonly used, with polysomnography enabling sleep architecture analysis and diagnosis of sleep disorders, but its use limited by logistical constraints. Actigraphy offers a feasible alternative for longitudinal and naturalistic assessment. We aimed to synthesize actigraphy-based sleep outcomes in individuals with SUDs and compare them with subjective sleep measures. We conducted a meta-analysis (PROSPERO: CRD42025107

When Mania Outruns the Law: Managing Risk in the Liminal Zone.

Patients with mania are often acutely unwell yet do not meet criteria for involuntary hospitalization. This creates a clinical and ethical dilemma the authors refer to as the liminal zone. These patients refuse treatment despite clear foreseeable harms, but the legal threshold for certification is not met. This situation produces moral discomfort for clinicians, disrupts team consensus, and is often equally distressing for families and friends who recognize impending consequences but lack formal authority to intervene. This article outlines practical approaches for managing liminal-zone patien

Leveraging Monaural Exposures to Reveal Early Effects of Noise: Evidence from Police Radio Ear-Piece Use.

Research into the long-term effects of noise on hearing is often confounded by health and lifestyle differences between individuals. UK police radio ear-pieces are capable of emitting high sound levels and, crucially, are worn in one ear, allowing between-ear comparisons which control for individual-level confounding factors. Low volume-control settings are recommended to reduce risk to police hearing, yet actual usage patterns and auditory effects remain unexamined. This study used a large-scale survey (N = 4,498) to assess ear-piece noise exposure and the associated hearing health. Most part

Association between dietary inflammatory index and sarcopenia in overweight or obese adults: a cross-sectional study.

INTRODUCTION: Over the past few decades there has been a growing interest in the use of music, which has seen it used to achieve a diverse range of outcomes. While music as an intervention for hospital patients has subject to considerable evaluation, many of these studies are small and findings are therefore often contradictory. This systematic review was conducted to investigate the effectiveness of music as an intervention for hospital patients. METHOD: A comprehensive search was undertaken involving all major health care databases. For studies to be included in the review they must have in

Longitudinal associations between adolescent body dissatisfaction, eating disorder and depressive symptoms, and BMI: a UK twin cohort study.

BACKGROUND: Body dissatisfaction is associated with later mental health problems and weight gain in young people, but the causality of these associations is unclear. We aimed to investigate the longitudinal associations between body dissatisfaction at age 16 years and later eating disorder and depressive symptoms, and BMI in young adulthood, with rigorous control for potential confounders including shared genetic influences (in objectives 1 and 2). We then quantified the proportion of these associations explained by genetic and environmental factors (objective 3). METHODS: We used data from t

Phenotypes and endotypes of cough in asthma.

Asthma is a common chronic inflammatory airway disease with diverse clinical manifestations, among which cough is one of the most significant and complex symptoms. Cough not only impacts patients' quality of life but also significantly influences the diagnosis and management of asthma. In recent years, with advancements in molecular biology and immunology, researchers have progressively uncovered multiple phenotypes of cough in asthma and their corresponding endotypic mechanisms, encompassing the activation of immune cells, the release of inflammatory mediators, and the role of airway neural r

Validating an aphasia-accessible ecological momentary assessment for daily depressive affect: A preliminary investigation.

BACKGROUND: Up to 60% of individuals with aphasia have post-stroke depression, resulting in worse quality of life. However, extant measures of depression in aphasia are insufficient, and people with aphasia are often excluded from studies of depression due to a lack of valid assessment tools. Ecological momentary assessment (EMA) involves brief, repeated surveys that capture fluctuations in the real world, and may be adapted for individuals with communication impairments. We created a novel, aphasia-accessible EMA of affective depressive symptoms and evaluated its feasibility and usability in
