Structural and mitochondrial dendritic degenerations in old hypoglossal motor neurons.

Hypoglossal motor neurons (MNs) within the medullary hypoglossal nucleus innervate the striated muscles of the intrinsic and extrinsic tongue. Dysfunction of the control of the tongue muscles may lead to problems such as dysphagia, dysphonia and the increased risk of aspiration pneumonia in the elderly. In the human and Fischer 344 (F344) rat motor systems, age-related muscle weakness and behavioural dysfunctions are contemporaneous to MN death. In other neurons, dendritic and mitochondrial degenerations are fundamental pathophysiological components preceding neuronal death. We aimed to determ

The Impact of Inspiratory Muscle Strength Training on Vocal Loudness in Individuals With Parkinson's Disease: A Pilot Study.

OBJECTIVE: People with Parkinson's disease (PD) typically develop hypokinetic dysarthria with associated hypophonia, which is among the most prevalent and bothersome dysarthric symptoms of patients and caregivers. Reduced respiratory support may be related to chest wall rigidity and/or reduced respiratory muscle strength, which can negatively impact vocal intensity. Therefore, respiratory muscle strength training has been the focus of multiple studies aiming to improve loudness in PD. Inspiratory muscle strength training (IMST) has been explored, with results suggesting modest increases in voc

The Effectiveness of Eclectic Voice Therapy on Multidimensional Voice Parameters in Patients with Primary Muscle Tension Dysphonia.

BACKGROUND: Primary muscle tension dysphonia (MTD) is a functional voice disorder characterized by excessive laryngeal muscle tension, resulting in various voice quality impairments and associated symptoms. While voice therapy is recognized as the primary treatment approach, there remains limited evidence regarding the multidimensional effects of eclectic therapeutic interventions combining multiple evidence-based techniques. AIM: To investigate the effectiveness of eclectic voice therapy on acoustic, aerodynamic, auditory-perceptual, and self-assessment voice parameters in patients diagnosed

Clinician Perceptions of Phonatory-Respiratory Kinematic Patterns in Primary Muscle Tension Dysphonia.

OBJECTIVES: Altered respiratory kinematics are thought to contribute to primary muscle tension dysphonia (pMTD) symptoms such as vocal effort, fatigue, and discomfort. Although speech-language pathologists (SLPs) often comment on respiratory patterns during voice evaluations, it remains unclear which visual-perceptual cues they rely on for clinical decision-making and how accurate these patterns are for identifying patients with pMTD. As such, this study aimed to (1) identify the respiratory patterns SLPs perceive in pMTD and (2) evaluate their diagnostic accuracy in distinguishing pMTD from v

Dysphonia Is Associated With Anxiety and Depression in the All of Us Research Program.

OBJECTIVES: Previous studies have examined the relationship between dysphonia and mental health, but often in small samples and not across age groups. In this large national epidemiological study, we examine if dysphonia is related to anxiety and depression generally and across cohorts of different ages. METHODS: This is a cross-sectional analysis of adults ≥ 18 years old in the All of Us dataset. The exposure was dysphonia (ICD-10 code R49.0), and the outcomes were anxiety (ICD-10 F41) and depression (ICD-10 F32-33). We performed multivariable logistic regression analyses, controlling for ag

Introducing ACT Into the Field of Voice Therapy.

Voice disorders are multidimensional phenomena involving anatomical and physiological features alongside emotional, behavioral, and psychological aspects. Although the literature acknowledges the relevance of psychological factors in voice disorders, the structured incorporation of psychological approaches into voice therapy remains limited. This article addresses that gap by presenting a preliminary framework for integrating acceptance and commitment therapy (ACT) principles into voice therapy. ACT is a well-established psychological approach with demonstrated adaptability across various prof

Children's Ability to Identify and Describe Dysphonia: A Multimethod Study.

BACKGROUND: Engaging children with voice therapy is a commonly reported clinical challenge. It is unknown whether this is because children cannot always perceive dysphonia, or whether clinicians do not explain it in age-appropriate terms. AIMS: This research aimed to analyse whether children can identify the presence of dysphonia in other children's voices and understand how they describe dysphonic and non-dysphonic voices. METHODS AND PROCEDURES: The research was carried out using a multimethod design combining quantitative and qualitative approaches. Thirty children aged 6;4-10;6 (17 male,

Analysis of VHI-10 in Patients with Comorbid Primary Muscle Tension Dysphonia and Emotional Disturbances.

OBJECTIVES/HYPOTHESIS: The goal of this study was to determine if coexisting emotional disturbance affects Voice Handicap Index-10 (VHI-10) scores in patients with primary muscle tension dysphonia (pMTD). We hypothesize that those with emotional disturbances will have a higher perceived voice handicap, and therefore higher VHI-10 scores. STUDY DESIGN: Retrospective chart review. METHODS: We reviewed the medical charts of patients diagnosed with pMTD at initial voice evaluation in 2019. We included patients in the emotional disturbances group if they self-reported depression, anxiety, or acut

Laryngopharyngeal reflux current developments and therapeutic strategies.

Laryngopharyngeal reflux (LPR) is reflux of gastric contents proximal to the upper esophageal sphincter and leading to a variety of symptoms including cough, dysphonia, globus, and sore throat. There is currently no diagnostic gold standard for LPR which often leads to over or under diagnosis and complicates therapeutic approach to this group of patients. This uncertainty has led to a high economic burden of the disease. The goal of this review was to present current recommendations in the diagnosis and treatment of LPR to aid in clinical decision making. There has been emerging research regar
