Atrophy progression in frontotemporal lobar degeneration-TDP-C with primary progressive aphasia.

Clinicopathological correlations in neurodegenerative diseases have led to new insights on the neurobiology of selective vulnerability and the anatomy of cognitive networks. The neuropathological entity of frontotemporal lobar degeneration with abnormal precipitates of the transactive response DNA binding protein TDP-43 (FTLD-TDP) of type C (TDP-C) is one of the most distinctive examples. TDP-C invariably starts with neurodegeneration (atrophy) confined to the temporopolar regions. The process is usually asymmetric, causing behavioural abnormalities and associative agnosia when predominantly r

Diagnostic challenges in Landau-Kleffner syndrome.

A boy in his early childhood was brought by his parents to the Laboratory of Investigation of Neurological and Genetic Disorders of the School Clinic of the University with a suspicion of Autism Spectrum Disorder (ASD) and hearing loss. The parents' complaint was that their child had language regression, did not respond to auditory stimuli and was presenting ASD behaviours. The parents were searching for information and a possible diagnosis and treatment for their child. Considering the child's history, an electroencephalogram (EEG) and a CT exam, as well as audiological and language assessmen

Clinical Problem Solving: A 68-Year-Old Woman With Acute Apraxic Agraphia, Constructional Apraxia, and Finger Agnosia.

A 68-year-old left-handed woman with a history of hypertension presented with writing difficulties initially affecting numbers and then progressing to include letters. She described an inability to form the shape of numbers and found her hand drifting towards incorrect keys on her phone despite knowing what she wanted to select. Neurological examination revealed fluent but mildly slowed speech without paraphasic errors, intact comprehension and naming, and mild temporal disorientation, where she often stated the wrong year. Her writing was characterized by hesitation, poor spatial planning, an

The forgotten piece of a complex jigsaw: Vestibular assessment is a blind spot for England's NHS falls services.

BackgroundFalls remain a major healthcare burden from an individual to societal level. Vestibular disorders are recognised as a significant risk factor for falls in older adults, but identification and assessment of such conditions has only recently been included in national and international guidance. The extent to which National Health Service (NHS) falls services in England are including vestibular assessment is not well known.MethodA freedom-of-information request was sent to 117 Falls Services run by NHS Trusts in England to identify if they included vestibular assessment/management, if s

Disrupted brain topological network and its association with clinical features in posterior cortical atrophy.

The characteristics of structural brain topological network alterations and their correlation with clinical features in posterior cortical atrophy (PCA) remain elusive. This study aims to explore the structural topological network alterations and their correlation with clinical features in PCA. Thirty-four patients with PCA and 34 healthy controls were enrolled in this cross-sectional study and underwent diffusion tensor imaging, structural MRI and neuropsychological assessment. The graph theory method was applied to capture the individual structural properties of the network. Partial correlat

Anosognosia for aphasia: impact of statistical biases and aphasia severity.

OBJECTIVE: Anosognosia for aphasia is a condition in which patients may show reduced awareness of their language deficits. Statistical biases that may arise when people with very mild or very severe aphasia are included in group studies have raised concerns about the validity of self-report measures. Given these concerns, the interplay of cognitive biases such as the Dunning-Kruger effect (DKE) should be considered when evaluating awareness of language deficits. METHOD: A sample of 66 individuals with a unilateral left-hemisphere brain lesion completed a standardized aphasia assessment (the A

[Vestibular cortex and vestibular agnosia].

This review analyzes recent literature on the structure of the vestibular cortex. Researchers have identified at least ten brain regions that respond to vestibular stimulation. The vestibular regions identified by caloric, galvanic, and auditory vestibular stimulation are mainly located deep in the lateral sulcus and in the perisylvian cortex around the sylvian fissure. They are also found in the primary somatosensory, posterior parietal, frontal, extrastriate cortex, the cingulate gyrus, and the hippocampus. Together, these regions form the vestibular cortex, a broad network of multisensory r

Gerstmann Syndrome as the Sole Clinical Manifestation of Ischemic Stroke Secondary to Atrial Fibrillation: A Case Report.

Gerstmann syndrome (GS) is a rare neurological disorder characterized by the presence of agraphia, acalculia, right-left disorientation, and finger agnosia. In older adults, the isolated presentation of this syndrome may represent an early sign of an ischemic cerebrovascular event, particularly in patients who have cardioembolic risk factors such as atrial fibrillation (AF). We report the case of a 72-year-old male with a history of hypertension, type 2 diabetes, AF without anticoagulant treatment, and chronic alcohol use who presented with a five-day history of behavioral changes and difficul

Error unawareness as a driver of cognitive decline in individuals at risk for Alzheimer's disease.

Anosognosia, or unawareness of one's deficits, predicts faster clinical progression in Alzheimer's disease (AD), though its mechanisms remain unclear. We investigated five-year trajectories of response awareness and error-monitoring efficiency using second-order judgments from an annual word recognition memory task in cognitively unimpaired individuals at risk for AD from the INSIGHT-preAD cohort. Participants were divided into three groups according to amyloid status at baseline (positive, A + ; negative, A-) and clinical progression (controls, CTRL A+ or A-; and progressors, PROG). Task accu

The cortical electrophysiological changes evoked by natural vestibular stimulation in healthy and bilateral vestibulopathy.

Peripheral vestibular stimulation elicits brainstem reflex responses manifesting as a vestibular-ocular reflex nystagmus ("VOR") and evokes a perception of self-motion or "vertigo." Although VOR responses are objectively measured via eye movements, no such objective measure exists for assessing the cortical vestibular processing of self-motion. Understanding how the brain encodes vestibular-mediated self-motion could provide electrophysiological markers for clinical syndromes where reflex and perception are uncoupled, e.g., in vestibular agnosia. Hence, here we investigated the brain's encodin

Posterior cortical atrophy: A seven-year single case study of neurocognition and adaptation.

Objective: This paper describes the progression of the neurocognitive abilities and patient-centered adaptation strategies involved in different stages of Posterior Cortical Atrophy (PCA). Methods: The neurocognitive profile, functionality and adaptation strategies of a man (DG) who suffered from PCA are documented over 7 years. Results: DG experienced early and fast progressing visual attention and recognition difficulties (starting from alexia and evolving to prosopagnosia and object agnosia). He later developed visuo-spatial impairments and by the end of his illness, could no longer express

Posterior Cortical Atrophy Phenocopy Associated with Antiphospholipid Syndrome: A Case Report.

A 37-year-old woman with a history of antiphospholipid syndrome (APS) developed a progressive visuospatial impairment, including difficulty tying shoelaces and inserting keys. A neuropsychological assessment revealed simultagnosia, optic ataxia, dressing apraxia, alexia, agraphia, and left hemispatial neglect, mimicking the symptoms of posterior cortical atrophy (PCA). Brain MRI revealed occipital-parietal atrophy and FLAIR hyperintensities in the watershed areas, suggestive of chronic ischemia. Cerebral blood flow SPECT revealed bilateral hypoperfusion in the same regions. We report that chro

The Neuropsychiatric Phenotype of Klüver-Bucy Syndrome: A Systematic Review of Reported Cases.

Klüver-Bucy syndrome (KBS) is a neuropsychiatric condition characterized by hypersexuality, hyperorality, hyperphagia, visual agnosia, hypermetamorphosis, and emotional changes such as placidity and loss of fear. Distinct diagnostic criteria are used to define both complete and partial KBS, but clinicians often face challenges due to symptom variability and unvalidated criteria. In this systematic review, the authors searched the PubMed, Embase, PsycNet, Latin America and the Caribbean Literature on Health Sciences, and OpenGrey literature databases from their inception to January 24, 2025, fo

Subjective cognitive complaints in cognitively unimpaired Parkinson's disease: Associations with affective burden, motor severity, and awareness-related processes.

BACKGROUND: Subjective cognitive complaints (SCC) are frequent in Parkinson's disease (PD), but their significance in cognitively unimpaired individuals remains uncertain. OBJECTIVES: To investigate the frequency of SCC in cognitively unimpaired PD patients and examine associations with objective cognition, mood symptoms, and anosognosia. METHODS: In this cross-sectional study, 74 PD patients with preserved cognitive performance based on the Addenbrooke's Cognitive Examination-Revised (ACE-R) were included. A case of autotopagnosia in Alzheimer's disease: Mechanistic insights into body-part localization.

Autotopagnosia is a rare neuropsychological syndrome characterized by disordered localization of body parts. Reports remain scarce and the cognitive mechanisms underlying this syndrome are not fully understood. We describe the case of a 65-year-old Japanese man whose clinical and neuropsychological profiles and neuroimaging findings supported a diagnosis of probable Alzheimer's disease. Notably, he had episodic memory deficits and visuospatial impairment without aphasia, along with Gerstmann syndrome and marked autotopagnosia. Neuropsychological examinations assessed body-part knowledge and pr

Auditory radiations: Evidence for dorsal and ventral branches.

BACKGROUND: The acoustic radiations connect the medial geniculate nucleus (MGN) of the thalamus with the auditory cortex in Heschl's gyrus (HG). Classic histological dissections described a trajectory that courses through the optic radiations and beneath the posterior insula en route to HG. More recent histological work, however, along with probabilistic tractography, has reported a curved pathway composed of a genu, stem, and fan that is inconsistent with the classically described topography. Thus, the precise course of the acoustic radiations is not settled, motivating renewed anatomical cha
