Auditory radiations: Evidence for dorsal and ventral branches.

BACKGROUND: The acoustic radiations connect the medial geniculate nucleus (MGN) of the thalamus with the auditory cortex in Heschl's gyrus (HG). Classic histological dissections described a trajectory that courses through the optic radiations and beneath the posterior insula en route to HG. More recent histological work, however, along with probabilistic tractography, has reported a curved pathway composed of a genu, stem, and fan that is inconsistent with the classically described topography. Thus, the precise course of the acoustic radiations is not settled, motivating renewed anatomical cha

Differences in the time course of recovery of anosognosia: evidence from a longitudinal study of two cases.

Following right brain damage, some patients experience anosognosia, which is a deficit of self-awareness affecting motor, somatosensory, or visual functions on the left side of the body. While domain dissociations in anosognosia have been studied extensively, the time course of recovery remains poorly understood. This study examines the longitudinal recovery time course of anosognosia in two patients presenting hemiplegia, hemianesthesia, and visual field defect after right brain damage. Anosognosia was assessed using the Bisiach four-point scale, a self-assessment of daily life activities, an

The clinical implications of anosognosia in Alzheimer's disease.

INTRODUCTION: Assessing awareness of patients with Alzheimer's disease (AD) is essential for diagnosis and treatment. Anosognosia, defined as impaired awareness of cognitive and functional deficits, is associated with poorer prognosis, reduced decision-making capacity, increased risk of accidents, and earlier institutionalization. AREAS COVERED: This narrative review examines 53 studies. After outlining the main theoretical models of awareness in dementia, the multidimensional nature of anosognosia and its impact on patients' autonomy and decision-making abilities are discussed. This review a

Domain-specific deficits in categorical perception unveil the neural architecture of auditory agnosia in dementia.

Auditory processing deficits are a prominent early feature of Alzheimer's disease (AD), yet whether they reflect generalized cognitive slowing or domain-specific neural degradation remains unclear. This study combined a categorical perception paradigm with high-temporal-resolution electroencephalography (EEG) to evaluate neural responses to distinct combinations of acoustic cues, rapid temporal dynamics (consonants) versus stable spectral configurations (lexical tones), across a continuum of healthy aging, mild cognitive impairment (MCI), and AD. Rather than a monolithic functional loss, behav

[Mechanisms of High-Level Human Visual Cognition and Associated Disorders].

Human visual cognition relies on two major streams that originate in the primary visual cortex: the ventral stream, which projects to the temporal lobe, and the dorsal stream, which projects to the parietal lobe. The ventral stream processes visual features such as shape, color, and texture, which are integrated to support object recognition. Within this stream, cortical regions are further organized by stimulus categories, including faces, places, words, and objects. The dorsal stream links vision with action and spatial behavior and can be subdivided into two functional substreams. The dorso

Vestibular perception, balance impairment, and fall risk in community-dwelling older adults.

BACKGROUND: Vestibular complaints are common in older adults and are linked to imbalance and falls. Some older adults show impaired vestibular perception despite preserved peripheral-reflex ['vestibular agnosia (VA)']. Yet it remains unclear if VA is independently linked to imbalance and falls in otherwise healthy older adults. We therefore investigated the prevalence of VA in community-dwelling older adults, and examined its association to balance and prospective falls. METHODS: Vestibular perceptual thresholds were measured during yaw-plane rotational chair testing. Postural sway and instru

Capturing Behavioral Symptoms in Huntington's Disease Using the Huntington's Disease-Behavioral Questionnaire.

BACKGROUND: The Huntington's Disease Behavioral Questionnaire (HD-BQ) captures behavioral symptoms arising from cognitive, psychiatric, and functional domains. Recognizing the high prevalence of anosognosia in HD, the HD-BQ incorporates patient- and companion-reported versions. OBJECTIVE: Examine the utility of the HD-BQ in capturing behavioral symptoms in HD. METHODS: The HD-BQ was administered to 71 manifest HD patients and their companions, plus 71 healthy controls (HC). Differences in HD-BQ scores were examined using Mann-Whitney U and Wilcoxon signed-ranks tests. RESULTS: HD patients r

Case Report: Epilepsy-related pure word deafness in the elderly: an underreported presentation?

We describe a rare case of epilepsy-related PWD in the elderly. A 61-year-old right-handed man presented with status epilepticus. Although electroencephalogram revealed no epileptiform discharges, brain magnetic resonance imaging (MRI) showed hyperintensity in the left temporoparietal lobe and thalamic pulvinar on diffusion-weighted imaging (DWI). Cerebrospinal fluid revealed no pleocytosis and negative PCR results for herpes simplex virus and varicella-zoster virus. Following the recovery of consciousness, he exhibited a profound dissociation between auditory and speech/language modalities: h

Late-Onset Depression Mimicking a Primary Psychiatric Disorder: Diagnostic Pitfalls in Sporadic Creutzfeldt-Jakob Disease.

Human prion diseases are rare neurodegenerative disorders that typically present with rapidly progressive neurological decline. Sporadic Creutzfeldt-Jakob disease (sCJD) is a fatal neurodegenerative disorder characterized by rapidly progressive encephalopathy. Although classically presenting with cognitive decline and myoclonus, early manifestations may be predominantly psychiatric, particularly in older adults, leading to diagnostic delay. We report the case of a 75-year-old woman with no prior psychiatric history who initially developed late-onset depressive symptoms accompanied by behaviora

Balance recovery and its link to vestibular agnosia in traumatic brain injury: a longitudinal behavioural and neuro-imaging study.

BACKGROUND: Vestibular dysfunction causing imbalance affects c. 80% of acute hospitalized traumatic brain injury (TBI) cases. Poor balance recovery is linked to worse return-to-work rates and reduced longevity. We previously showed that white matter network disruption, particularly of right inferior longitudinal fasciculus, mediates the overlap between imbalance and impaired vestibular perception of self-motion (i.e., vestibular agnosia) in acute hospitalized TBI. However, there are no prior reports tracking the acute-longitudinal trajectory of objectively measured vestibular function for hosp

Brain connectivity for constructing new face representations in typical adults versus a prosopagnosic patient.

Will our brains get to know a new face better if we look at its external features first? Here we offer neurophysiological evidence of the relevance of external versus internal facial features for constructing new face representations, by contrasting successful face processing with a prototypical case of face agnosia. A woman with acquired prosopagnosia (E.C.) and 14 age-matched typical participants (7 women) were exposed to a face-feature matching task. External (E), internal (I) features, and whole target faces of unknown individuals (from an IdentiKit gallery) were displayed according to two

Permanent tactile sensory loss reduces neuronal activity in the amygdala and ventral hippocampus and alters anxiety-like behaviors.

Tactile information from the whiskers (vibrissae) travels through the somatosensory cortex to the entorhinal cortex and the hippocampus, influencing development and psychological well-being. The lack of whiskers affects cognitive functions, spatial memory, neuronal firing, spatial mapping, and neurogenesis in the dorsal hippocampus. Recent studies underline the importance of tactile experiences in emotional health, noting that while tactile stimuli modulate the dorsal hippocampus, the effects of tactile deprivation on anxiety-like behaviors and neural activity in regions like the ventral hippo

Auditory agnosia for environmental sounds in Alzheimer's disease: Effects on daily life.

BACKGROUND: Auditory agnosia for environmental sounds is a type of agnosia attributed to central auditory dysfunction. It is common in Alzheimer's disease, and is associated with peripheral hearing loss, although independent of it, and presumed independent of language deficits. The effects of this type of agnosia on daily life in Alzheimer's disease are unknown. OBJECTIVE: We aimed to assess the impact of auditory agnosia for environmental sounds in people with Alzheimer's disease while also exploring the role of unrecognized hearing loss. METHODS: We tested 34 home-dwelling people with Alzh

Adverse events in the nervous system associated with blinatumomab: a real-world study.

BACKGROUND: Nervous system toxicity (NST) is a frequent and serious adverse event (AE) associated with blinatumomab, the first bispecific antibody drug targeting CD19 and CD3. Real-world data are needed to better understand the incidence and characteristics of NST in clinical practice. METHODS: Data were obtained from the FDA Adverse Event Reporting System (FAERS). The reporting odds ratio (ROR), proportional reporting ratio (PRR), Bayesian confidence interval progressive neural network (BCPNN), and multi-item gamma Poisson shrinker (MGPS) algorithms were utilized for data mining. RESULTS: A

Cognitive function instrument-based anosognosia to predict amyloid status.

Anosognosia, a lack of self-awareness regarding cognitive dysfunction, often accompanies the progression of Alzheimer's disease (AD) pathology. This study explored the relationship between AD pathology and anosognosia measured by discrepancies in Cognitive Function Instrument (CFI) scores, as rated by participants and their study partners (SP). Using mixed-effects models on non-demented participant data, the results revealed that lower self-reported CFI score compared to SP ratings was significantly associated with positive amyloid PET results (odds ratio 1.081 per-1 decrease in ΔCFI). Our fin

Akinetopsia: a systematic review on visual motion blindness.

BACKGROUND: Akinetopsia, or visual motion blindness, is a perceptual distortion characteristic of Alice in Wonderland syndrome in which people see moving objects as disjointed 'jumps' or 'freeze frames.' Despite its profound impact, the condition remains poorly understood beyond the established centrality of cortical area V5/MT in visual motion perception. METHODS: We carried out a systematic review of case descriptions on motion blindness, including those where additional symptoms were described. RESULTS: Our search yielded data on 25 clinical and 27 experimental cases, the latter induced b

Vestibular Agnosia: Toward a Better Understanding of Its Mechanisms.

Background/Objectives: Vestibular agnosia is characterized by a reduced or absent self-motion perception while demonstrating the presence of normal peripheral vestibular function following stimulation. This condition has previously been reported by previous authors in different populations and more recently in traumatic brain injury patients. However, the underlying mechanisms responsible for vestibular agnosia remain a matter of debate. The objective of this manuscript is to review and compare the behavioral and neuroanatomical findings in populations where vestibular agnosia has been demonst

Differential affection of the visual information sub-streams in a patient with visual agnosia.

INTRODUCTION: Visual agnosia is a deficit of object recognition addressed to the damage of the ventral stream (VS). The dorsal stream (DS) is usually intact in these patients, and it can be derived from well-preserved reaching and grasping of visually presented objects. In this study, we presented a new case of a visual agnosic patient (AC) with an extensive lesion of the secondary visual cortex. METHODS: We examined the kinematics of his grasping behavior towards common day-to-day objects compared to a healthy control group. Both colored and color-masked objects were presented, and participa

Cerebral asymmetry: Historical introduction.

Cortical localization and hemispheric asymmetry of function are key concepts for analyzing the relationship between the brain, mind, and behavior in clinical neurology, neuropsychology, cognitive neuroscience, and modern philosophy. This chapter presents the history of asymmetries in the human central nervous system within the context of right-left asymmetries of bodily structure, as can be detected in individuals of virtually all living species. It has been known since antiquity that behavioral deficits on one side of the body could be caused by brain lesions on the opposite side. Yet, the id
