Visual Perception and Fixation Patterns in an Individual with Ventral Simultanagnosia, Integrative Agnosia and Bilateral Visual Field Loss.

BACKGROUND/OBJECTIVES: As high-acuity vision is limited to a very small visual angle, examination of a scene requires multiple fixations. Simultanagnosia, a disorder wherein elements of a scene can be perceived correctly but cannot be integrated into a coherent whole, has been parsed into dorsal and ventral forms. In ventral simultanagnosia, limited visual integration is possible. This case study was the first to record gaze during the presentation of a series of visual stimuli, which required the processing of local and global elements. We hypothesised that gaze patterns would differ with suc

Landau-Kleffner Syndrome Can Herald the Diagnosis of GRIN2A Gene Mutation.

Landau-Kleffner syndrome is a rare age-related childhood epileptic syndrome of linguistic decline and neuropsychological abnormalities as main clinical symptoms. It is a functional language disorder of children, manifesting with auditory verbal agnosia and other predominantly linguistic deficits. Also, cognitive and neurophysiological-behavioural abnormalities might manifest. Clinical seizures have been reported in three-quarters of children. Recent advances in genomic studies have provided important insights into the understanding of neurodevelopmental disorders such as autistic spectrum diso

Lesion-network mapping in task-dependent frequencies uncovers remote consequences of focal damage.

The brain consists of a multiplicity of networks with massively interacting nodes. Disruption of a node following brain damage can result in both short- and long-distance functional abnormalities, affecting even intact brain regions remote from the site of lesion (termed 'diaschisis'). Diaschisis has been well described previously, and structural and functional connectivity have been related to clinical findings. However, the mechanistic and neurophysiological properties of this remote loss of function, its temporal and spectral dynamics, and its impact on the whole brain remain to be elucidat

Assessing Developmental Gerstmann's Syndrome in an adult: a case report.

OBJECTIVE: Developmental Gerstmann's Syndrome (DGS) is a proposed neurological disorder characterized by finger agnosia, acalculia, right-left disorientation, agraphia, and in some cases, constructional dyspraxia. Case studies of DGS are limited, particularly those reporting on assessments in adults. The present case study demonstrates the presence of DGS symptoms in a young female adult with an autoimmune disorder but no clear history of neurological damage. METHOD: This client sought academic accommodations for her undergraduate math classes. She was administered a comprehensive neuropsycho

Rapid Review of Interventions Designed to Enhance Personalised Care for People With Dementia When There Are Concerns About Reduced Awareness of Difficulties.

OBJECTIVES: Awareness of difficulties varies in people with dementia. Low awareness, also termed anosognosia, has been implicated in carer stress and safety concerns, and can be a barrier to effective clinical communication. Little is known about how to manage situations arising from low awareness. This review looked for evidence of existing interventions to enhance care in situations regarding low awareness, and considered their utility, feasibility and acceptability when delivering personalised care. METHODS: We used systematic review methodology, adapted for a rapid timeline, searching fiv

Computerized Open-Source Navon Test (COSNaT): Normative data for the assessment of global processing abilities and simultanagnosia in the Italian population.

Disorders in global processing abilities may be evident when posterior cortical regions are damaged due to a focal brain lesion or neurodegenerative diseases, such as posterior cortical atrophy (PCA). While in severe cases global processing deficits are evident as simultanagnosia, subtle impairments may be present early, as in patients with multi-domain mild cognitive impairment (MCI). The aim of the present study is to provide norms for the Computerized Open-Source Navon Test (COSNaT), which has already been validated in clinical populations. After a baseline that requires identifying large a

Gerstmann's Syndrome and Limb Apraxia: A Single Case Study.

OBJECTIVE: Gerstmann's syndrome (GS) represents a still debated clinical condition, in terms of both symptoms' evolution and neural correlates. In depth, repeated neuropsychological assessments along with advanced methods of lesion analysis can contribute to a better understanding of the syndrome and clinical diagnosis. METHOD: The study reports a patient suffering from GS and limb apraxia following a left hemisphere lesion. Two in-depth assessments, at two and four months from the lesion onset, in addition to video material, document the symptoms over time. An in-depth analysis of the grey a

Optineurin mutation-associated language variant frontotemporal dementia.

A right-handed woman in her early 50s presented with progressive language difficulties with visual and auditory hallucinations. Upper motor neuron signs were present without lower motor neuron signs, insufficient to make a diagnosis of amyotrophic lateral sclerosis (ALS). MRI showed bilateral superior temporal atrophy (right>left) and F-18 fluorodeoxyglucose-positron emmission tomography (FDG-PET) scan showed right temporal hypometabolism. Her clinical diagnosis was atypical or mixed language variant frontotemporal dementia (FTD) given the presence of agrammatism and phonemic intrusions. There

Foundations of prosopagnosia: The three classic Austro-German reports.

The term prosopagnosia was coined 78 years ago, in 1947. At that time few reports of a specific disorder of face recognition existed, the three most detailed being those of Wilbrand (1892), Hoff and Pötzl (1937), and Bodamer (1947). These laid the basis for much of our current thought about the manifestations of prosopagnosia, its associations, its structural basis, and theories about its functional mechanisms. However, since they were written in German and few current researchers are fluent in that language, these early works are relatively neglected today. In this review we discuss their hig

Image complexity-based fMRI-BOLD visual network categorization across visual datasets using topological descriptors and deep-hybrid learning.

This study proposes a new approach that investigates differences in topological characteristics of visual networks, which are constructed using fMRI BOLD time-series corresponding to visual datasets of COCO, ImageNet, and SUN. A publicly available BOLD5000 dataset is utilized that contains fMRI scans while viewing 5254 images of diverse complexities. The objective of this study is to examine how network topology differs in response to distinct visual stimuli from these visual datasets. To achieve this, 0- and 1-dimensional persistence diagrams are computed for each visual network representing

Examination of Discrepancies in the Assessment of Anosognosia for Spatial Neglect.

BACKGROUND: Unilateral spatial neglect (USN) has been rigorously studied because it significantly affects multiple aspects of life. Research on anosognosia for spatial neglect (ASN), however, is limited. OBJECTIVE: To investigate whether the frequency and severity of ASN vary based on task, using items from the Catherine Bergego Scale (CBS) and the Behavioral Inattention Test (BIT). METHODS: Forty-four individuals with initial right hemisphere damage and a positive result on the CBS or BIT were included in the current study. The frequency of ASN was calculated for each item, followed by an a

Visual agnosia in the era of behavioral and neural investigations.

Single case studies have long been used to provide insights into the mechanisms underlying normal cognition, including in the domains of memory, language and visuoperceptual function, and standardized testing has been a steadfast companion in such investigations. Experimental approaches designed to address specific hypotheses have also been conducted and analytic methods have been developed for the comparison of single subject data to a control group. However, a seismic shift has occurred in the last decade or two in which neuroimaging, primarily magnetic resonance imaging, has been added to t

Gerstmann Syndrome: A Rare Clinical Presentation of Focal Hyperglycemic Seizures.

Hyperglycemic hyperosmolar nonketotic syndrome (HHNS) is a complication of type 2 diabetes mellitus that can progress to coma and death if left untreated. Focal hyperglycemic seizures are still an uncommon but noteworthy association of HHNS and most commonly involve the occipital and parietal lobes. Gerstmann syndrome, also called angular gyrus syndrome, consists of a tetrad of finger agnosia, acalculia, left-right disorientation, and agraphia that is usually accompanied by aphasia and most commonly presents in parietal lobe pathology. Here we report a case of a 50-year-old right-handed male w

Improvement in Functional Outcomes in Patients with Benign Paroxysmal Positional Vertigo.

This case report describes outcomes of three cases with benign paroxysmal positional vertigo (BPPV) who presented with an inability to state the symptoms of BPPV. The diagnosis is driven by patient-reported symptoms during positional testing or movement changes. People with traumatic brain injuries (TBIs) can have BPPV but report no symptoms of spinning (vestibular agnosia). The case report demonstrates that functional improvements are made in patients with vestibular agnosia. All cases were seen in an inpatient rehabilitation unit. Case 1 presented with a bilateral TBI with a daily Agitated B

Association between anosognosia and neuropsychiatric symptoms in Alzheimer's disease dementia patients.

Anosognosia, the lack of awareness of memory decline, and Neuropsychiatric Symptoms (NPS) are prevalent and debilitating symptoms in Alzheimer's disease (AD) dementia. Understanding the coexistence of these symptoms may help guide clinical interventions and treatment strategies. This study aimed to compare NPS prevalence in patients with and without anosognosia at baseline and to assess the association between anosognosia and NPS over time. We examined patients with AD dementia enrolled in the Alzheimer's Disease Neuroimaging Initiative (ADNI). To be included in the current study, patients had

Static Visual Agnosia Following Awake Resection of a Left Frontal Low-Grade Glioma: A Case Report of Ventral Stream Network Disruption ("Astatopsia").

Background and Clinical Significance: Visual agnosia and speech production deficits are well-described sequelae of neurosurgical interventions, but their selective dissociation remains rare. This report presents an unusual combination of postoperative deficits following awake resection of a left frontal low-grade glioma. Case Presentation: We present the case of a right-handed female with left hemisphere language dominance who had a left frontal low-grade glioma. Preoperatively, she exhibited anomia and dysexecutive syndrome, including difficulty completing everyday goal-directed tasks such as

Dandy-Walker Malformation and Optic Nerve Hypoplasia: A Developmental Case Highlighting the Psychiatric Impact of Central Nervous System Malformation.

Dandy-Walker malformation (DWM) and optic nerve hypoplasia (ONH) are rare congenital anomalies associated with neuropsychiatric morbidity, but their co-occurrence in adults has not been described. We present an adult patient with both DWM and unilateral ONH who developed auditory hypersensitivity, visual agnosia, phonological dyslexia, dysgraphia, and longstanding academic challenges, later evolving into recurrent depression, anxiety, and insomnia. Neuroimaging revealed a Dandy-Walker variant with cerebellar atrophy and ONH, and her presentation was consistent with cerebellar cognitive affecti

Preservation of waves VI and VII of ABR in pediatric and adult patients with lesions in bilateral auditory cortices or radiations.

BACKGROUND: Auditory-evoked brainstem response (ABR) is widely used in various areas of clinical medicine. There is a common understanding regarding the origins of waves I to V in human ABR. However, little is known from where wave VI or VII originates in the central auditory pathway. AIMS/OBJECTIVES: The aim of our research is to elucidate the origins of waves VI and VII by studying ABR amplitude and latency and conducting magnetic resonance imaging (MRI). MATERIALS AND METHODS: Eleven patients diagnosed as having central auditory disorders participated in this study. They include five pedi

Lesion correlates of impaired acoustic-phonetic perception after unilateral left hemisphere stroke.

Acoustic-phonetic perception refers to the ability to perceive and discriminate between speech sounds. Acquired impairment of acoustic-phonetic perception is known historically as 'pure word deafness' and typically follows bilateral lesions of the cortical auditory system. The extent to which this deficit occurs after unilateral left hemisphere damage and the critical left hemisphere areas involved are not well defined. We tested acoustic-phonetic perception in 73 individuals with chronic left hemisphere stroke and performed multivariate lesion-symptom mapping incorporating controls for non-sp
