Chorea-acanthocytosis masquerading as a progressive seizure disorder with apparent early immunotherapy responsiveness.

Chorea-acanthocytosis (ChAc) is a rare genetic disorder characterised by a hyperkinetic movement disorder, dystonia, cognitive and neuropsychiatric deficits and seizures. We report the case of a 30-year-old patient who presented with a decade of episodic neurological dysfunction and seizures. The condition was initially suspected to be an immunotherapy-responsive seronegative autoimmune encephalitis but progressed to oromandibular dystonia, raising suspicion of a neurodegenerative condition. Neuroimaging showed bilateral caudate atrophy and acanthocytes were seen on blood film microscopy in as

Long-Term Speech Outcomes in Moderate-to-Severe Childhood Speech Sound Disorder: A Systematic Review.

BACKGROUND: Parents of children with moderate-to-severe speech sound disorder presenting to clinic want to understand prognosis for their child; however, there is unclear evidence as to the specific long-term speech outcomes in this group. AIM: To review long-term speech outcomes in children with moderate-to-severe speech sound disorders. METHOD: A systematic review identified 3697 papers; 21 papers containing 15 unique studies (six had overlapping data) met inclusion criteria for moderate-to-severe speech sound disorder with at least one follow-up speech assessment unrelated to intervention

A Reply to Duffy et al. on Their Reactions to the Refined Operational Definitions for the Apraxia of Speech Rating Scale.

PURPOSE: This reply provides a response to the issues raised by Duffy et al. (2026) regarding our recent article. Despite the many points of agreement with the Apraxia of Speech Rating Scale (ASRS) authors that are clarified further in this response, some of the operational definitions we proposed are related to concerns that we think deserve discussion in the research community. Specifically, we argue that the item composition of the ASRS, a scale that has been developed and successfully used for progressive apraxia of speech, overlooks central features of apraxia of speech in stroke for reas

A Response to Mailend et al.: Refined Operational Definitions for the Apraxia of Speech Rating Scale.

PURPOSE: This letter addresses a recent paper by Mailend et al. (2025) that expressed concerns about the Apraxia of Speech Rating Scale (ASRS) and offered guidelines and definitions regarding its principles, scale labels, item definitions, and scoring procedures, including the addition of new items to the scale. The primary purpose of this letter is to clarify the uncertainty that other users of the scale may have about whether the current ASRS version (ASRS 3.5; Duffy et al., 2023) should be altered to include the numerous proposed revisions. It provides examples of proposed revisions that, i

Susceptibility-weighted imaging in patients with Corticobasal Syndrome or Progressive Supranuclear Palsy: distribution of iron deposits reflects clinical symptoms in 4-R tauopathies.

BACKGROUND AND PURPOSE: Patients with corticobasal syndrome (CBS) and progressive supranuclear palsy (PSP) often show the loss of the "swallow tail sign", which reflects degeneration of dopaminergic neurons and iron accumulation in the substantia nigra pars compacta, and peculiar neuroradiological signs which are, respectively, asymmetrical frontoparietal atrophy and midbrain atrophy. A T2* hypointense rim likely reflecting presence and distribution of neuroinflammation has been recently reported in the frontal cortex of CBS patients. Here we investigated (i) the frequency of conventional radi

Translation, Validation and Reliability Study of the Korean Version of Apraxia of Speech Rating Scale (K-ASRS) in Patients With Post-Stroke Aphasia.

Apraxia of speech (AOS) is a motor speech disorder that occurs despite the absence of paralysis, weakness, or incoordination of the speech muscles and is characterized by difficulty in planning, programming, and sequencing speech movements. Currently, the Korean language lacks sufficient measures for evaluating AOS, whereas the Apraxia of Speech Rating Scale (ASRS) has been widely used internationally to assess the presence and severity of AOS. This study aimed to translate the Apraxia of Speech Rating Scale into Korean (K-ASRS) and verify its reliability and validity. The translation process

Inner Speech Can Predict Naming Treatment Outcomes in Aphasia: A Pilot, Single-Subject Design Study.

PURPOSE: This single-subject design study examined whether self-reported inner speech (IS)-the subjective experience of retrieving a word internally-can predict item-level outcomes in a cueing-based naming treatment for individuals with poststroke aphasia. METHOD: Five adults with chronic, poststroke aphasia completed an IS self-report task and naming task to identify appropriate treatment stimuli in two categories: successful IS (sIS) and unsuccessful IS (uIS). Participants then completed 2 weeks of hierarchical, cueing-based naming treatment targeting both sIS and uIS items. Treatment effec

Causes and consequences of unawareness (anosognosia) of tool-action errors after left-hemisphere stroke.

Many individuals with limb apraxia after left-hemisphere stroke exhibit a lack of awareness of their tool-related action errors, i.e., unawareness of apraxia (UA; also called anosognosia of apraxia). Little is known about the prevalence of UA, the relationship between UA and apraxia severity, or its underlying mechanisms. Here, we assessed both the causes and consequences of UA. Based on a mechanistic model, we hypothesized that UA may arise because of deficits in representations signaling how tool-related movements should look and feel-a component of action knowledge-and that degradation of t

Melodic intonation therapy paired with video feedback: A potential method for bolstering production?

Melodic intonation therapy (MIT) is a behavioural speech-language intervention that uses pitch and rhythm (i.e. melody) to increase speech fluency and facilitate improved word retrieval. It is a well-known therapeutic option for individuals with non-fluent aphasia. Though MIT can enhance speech and language production, its clinical utility is limited when those engaged in MIT are unable to routinely use its facilitatory techniques independently. Video feedback is a tool that can enhance learning and has shown potential as a mechanism to bolster behavioural interventions. The present study aime

Hemispherotomy for atypical teratoid rhabdoid tumor-associated drug-resistant epilepsy: illustrative case.

BACKGROUND: Drug-resistant epilepsy (DRE) following treatment for atypical teratoid rhabdoid tumor (ATRT) is rare. The authors report a case in which hemispherotomy was performed to palliate new-onset DRE after ATRT therapy. OBSERVATIONS: A 6-year-old boy with a large left thalamic ATRT underwent subtotal resection, chemotherapy, and radiation therapy. His neurological baseline remained stable, with right hemiparesis, right hemifield neglect, and apraxia of speech. Two years after tumor recurrence and treatment, he developed frequent tonic seizures of left hemispheric origin. Despite trials o

Treatment and Long-Term Outcomes of Children with Co-Occurring Childhood Apraxia of Speech and Velopharyngeal Dysfunction.

A retrospective review of multidisciplinary velopharyngeal insufficiency (VPI) Clinic records from 2010 to 2024 was completed. Inclusion criteria for the review were diagnosis of childhood apraxia of speech (CAS) and documented evaluation for velopharyngeal dysfunction (VPD). Adequacy of velopharyngeal closure and perceptual judgment of resonance were identified as primary outcome measures. Surgical interventions and speech outcomes were also described. This review identified 74 children who were diagnosed with CAS and evaluated for VPD in the VPI Clinic. For the subset (n = 50) of children wh

A Rare Case Report of Neuroimaging, Electrophysiological, and Dynamic Laryngoscopy Studies in a Stroke Patient of Foix-Chavany-Marie Syndrome.

Foix-Chavany-Marie Syndrome (FCMS), also known as opercular syndrome, is a rare neurological disorder caused by an ischemic stroke that results in autonomic-voluntary dissociation. In this case report, we describe a patient who presented with an acute right frontal and parietal cortical lesion, in addition to a remote infarction in the left lateral paraventricular region. The patient exhibited a masked face, facial paralysis, dysarthria, dysphagia, and apraxia of eyelid closure while automatic pathways, such as crying and laughing, remained intact. MRI revealed an acute ischemic lesion in the

Prenatal diagnosis and genetic counselling of a rare de-novo 12p13.33p13.32 deletion and 15q26.2q26.3 duplication in a Chinese family.

BACKGROUND: Copy number variants are an important source of normal and pathogenic genome variations. Constitutional deletions involving the distal part of the short arm of chromosome 12(12p13.33p13.32) are very rare. These deletions are associated with an emerging condition associated with variable phenotype, including a specific speech delay sound disorder, labeled childhood apraxia of speech, intellectual disability, and neurobehavioral problems. Trisomy of distal chromosome 15q has rarely been reported. It is generally believed that the terminal duplication of 15q26.2q26.3 is related to ove

Toward a Diagnostic Template for Acquired Apraxia of Speech: Insights From an Academy of Neurologic Communication Disorders Review.

PURPOSE: Significant variation is observed in diagnostic assessments for individuals with acquired apraxia of speech (AOS). The lack of consistency across research related to AOS impacts the comparison of intervention outcomes. This extends to diagnostic criteria, assessment, and the inclusion of patient demographics. This viewpoint examines outcomes related to the consistency in diagnostic tool use and collection of demographic information and introduces a template to support researchers and clinicians conducting diagnostic evaluations. Materials include guidelines for a thorough case history

Neuropathological mechanisms of acquired apraxia of speech investigated with multimodal transcranial magnetic stimulation and electroencephalography.

Acquired apraxia of speech (AOS) is a debilitating motor speech disorder that frequently co-occurs with post-stroke aphasia. However, its distinct neurophysiological underpinnings remain poorly characterized. This study used combined transcranial magnetic stimulation and electroencephalography (TMS-EEG) to investigate cortical excitability and network dynamics in 30 patients with left-hemisphere stroke (14 with aphasia accompanied by AOS and 16 with aphasia alone) and 10 healthy controls. By stimulating the left inferior frontal region, we analyzed TMS-evoked potentials (TEPs), time-frequency

Non-fluent agrammatic variant of primary progressive aphasia in a bilingual Mandarin/English speaker: a case report.

OBJECTIVE: Non-fluent agrammatic variant of primary progressive aphasia (nfvPPA) is a neurodegenerative condition associated with effortful speech and agrammatism in language production. The neuropsychological assessment and diagnosis of nfvPPA in bilingual patients is not well characterized. This case report describes a 66-year-old bilingual Mandarin/English speaking male patient who experienced gradual progression of aphasia in both languages with initial preferential sparing of Mandarin. METHOD: The patient was seen for a comprehensive neuropsychological evaluation administered primarily i
